This place is horrific! Do not send your loved ones here. I had to call 911 on 3 different occasions for my father, he had pink eye and they didn't treat it, another a raging UTI with sepsis. They tore the skin on his left arm multiple times. His items went missing there. And there was no remorse or hope of them trying to find the items. His room was disgusting and I had to disinfect it myself. The man in the bed next to him has one arm and pees all over the floor in the bathroom. He also leaves food out so there is a ton of gnats and fruit flies in the room. I had a nurse give my dad pain medication when I told her not to. I said I'm his POA and I'm saying no. She did it anyway. Don't sent anyone you love to this facility. The management has too many bed and not enough staff to properly care for these people. It's like a horror movie every time I'd go there to see my dad. I'm grateful of his most recent hospital stay so my family was able to get him out of there. I'm going to pick up his items today that are still there. I'm interested to see what's missing. read more

Three patients in a room, about three feet apart, is not therapeutic and dangerous. Especially if one patient is seriously ill filling the air with her bronchial mucous. How does one get rest when someone is coughing all day and night. When complaining to nurses at nurse's station reply was she refuses treatment. Since when does a patient dictate treatment? Three to a room adds to your profits but detrimental to patients. read more

The people not the building are what is important. Everyone here was very helpful and caring. The cna have a lot of patients to care for but give each the attention they need. PT, OT, and Speech are motivating and really do make a big difference in recovery. My dad liked the food too! It is tough having 3 beds to a room, but all seem to have that now - but I think 2 beds to a room would make it easier to visit with your loved ones. They do have a nice area out front, a lobby and we joined dad while he ate in the dinning room too. read more

My friend was there from March - October 1, 2024 and was well taken care of by the nurses at the south station, also the CNAs. Fernando was very helpful in moving all of his things into my car. I got along with all of them too. My friend said they were always on time to help him with his needs! He was discharged and had to move to Los Angeles and we had a bittersweet goodbye. Hi my name is Lon Dillon and I spent 8 months at shoreline my experience was positive after a life threatening experience with sepsis in witch I was in a coma for 6 weeks and woke to amputated leg and severe muscle atrophy unable to do anything for myself like feed use the restroom walk or even move . For 13 years my mother was abused and neglected. She was in the memory care center. The other part of the nursing home is good. If your loved one has memory problems, do not bring them here! I cry every day knowing what happened to her. She was a good person, what a shame. read more

My wife of 40 years was chosen by our Lord God starting in 2010 on the path of dementia at the young age of 52. After my heart attack in 2015 from the pressure of worrying about her, I knew I needed help taking care of her health needs more than I could do at home and it became necessary for me and because of her Doctor's concern for my heath to place her in a Skilled Nursing Facility SNF. I had visited and interviewed all 15 facilities in Ventura County with my wife at my side to find the right place for my wife. All but one were too worried because she was extremely ambulatory and not in a wheel chair or laying on flat on her back in bed. These other facilities did not want to accept her because they thought they would not be able to keep up with her! This was not the case at Shoreline Care Center SNF. The admission's administrator at the time was Elizabeth Salazar and had a bed for my wife the same say in October 2016 and this year my wife will have been there for 7 years. I could feel the love and concern Elizabeth had for me and my wife. Over the years I visit the facility almost daily helping the nurses and CNAs where I can, I know most of the staff, nursing, certified nursing assistants (CNAs), one-on-ones, receptionists and even the kitchen staff all by first name. They are all loving, caring, courteous, professional, and attentive to the wife's needs. Everyone at the facility loves my wife and she is receiving excellent care. It's like they treat my wife as their own mother! When they observe a medical concern about a change in my wife's health they contact me 24/7 even if it is to give her an aspirin. I am involved before any change in her care is made, whether it is to send her to a hospital or any change in medications and much more. They do not proceed without my direction and permission. But now over the years, I have full trust in them because I know they love her. When I do not agree with a change which is less than 1% of the time, they communicate to the Medical Director and we work out a compromise. The staff at Shoreline are all extremely knowledgeable and professional. During COVID-19 my wife contracted the virus twice and the staff that I could see in the window only were physically crying because they did not think she would make it through. I want to point out, I have observed this level of love and care to every resident, not just for my wife. If I could get this place a zero star I would. They would take the food away before he got a chance to eat it because he was asleep and never woken for it and on top of that my mother showed up to find him so severely dehydrated he was on death's door and needed to be taken to the hospital immediately. Please do not take any of your family here if you really care about them they are the worst thing that ever happened to mine. My edit is to say he was a severe stroke patient that needed help eating and drinking and all the staff did was make sure he ate one bite before leaving. The staff at Shoreline Care Center has been taking care of my dad since January. I am impressed with their kindness and teamwork from the charge nurses to the CNA's and physical therapists. For example, I was struggling to get my dad in the facility after a walk and a CNA quickly came over to help even though it was not his patient. Don't let the outside of the dated facility fool you. It's what's inside that counts. It's not the building - it's the people! The Shoreline Staff create a professional, caring community that is attentive to the health care needs of each patient. From the Admissions Director, Social Worker, Nursing Staff and CNAs, and Dietician they all welcomed our Mother and our family and created an individualized Health Care Plan to insure ongoing quality care. I have continued to be impressed with how the team of administrators and staff greet patients by name and take the time to acknowledge them and ask if there is anything they need. Everyone works as a team to respond to patients' needs, regardless of job title. I am very grateful for the honest, direct proactive communication between the administration, nursing staff and NCAs and our family to insure we're all working collaboratively to provide the best care for our Mother. Our Mom is a fall risk but she needs to walk around to relieve the pain in her legs. This ican be challenging for the staff but they are so kind, patient and conscientious about watching over her and keeping her safe. Our family is very gtateful our mother is at Shoreline and know she is receiving professional, loving care 24/7. It is a huge relief to know Mom is in good hands. Thank you Shoreline!!! read more

Why hasn't there been any correction at all in over 10 years. I'm not sure if Yelp was around back in 2007 2008 but my grandfather was at this location and just like the reviews I saw from a couple weeks ago and years ago they are still short staffed and seem to not have great customer service. I am leaving this review 10 years later and still upset at the care that was given here. I understand caretaking is not for everyone but I wish they would be more considerate regarding staffing and the quality of workers. Another big thank you to Shoreline! Due to the pandemic, I've been unable to see mom face-to-face, but call the front desk and they are happy to put me through to the nurse's station, who give mom a portable phone so we can talk. Today is my mom's birthday. When I spoke with her today, she told me that three Shoreline staff members visited her room and sang Happy Birthday to her. I can look through the front door and see the entire staff wearing masks to protect the residents. Most everyone working there is cheerful and smiling, and some know me by name, as I visit almost every other day. As I may have said in prior reviews, some nurses will go out of their way to stop by, and give Mom a hug, or say a kind word. This is GENUINE caring that comes free from what must be a carefully selected staff. I've been in other skilled nursing facilities where the staff is short on eye contact and long on rushing by. Not so here! My mom can be abit snarky sometimes due to mild dementia, and I have watched staffers consistently exercise patience and understanding and let her comments roll off their backs. Reception was kind enough to deliver my stuffed animal and card for Mom today, and had it in her hands before I could reach her by phone. Top to bottom, I believe their customer service is terrific, and they are worthy of the name Shoreline CARE Center. read more

my name is durias taylor i've been a resident here for almost 2 yrs now. please if you read this do not send any of your loved ones here.the care taking here below sub-par they over medicate me you here they'll leave me you in soiled diapers for hours at a time the cna's hate to answer call lights here there is no physical activities here for us to be engaded in they steal your personal property here by both staff and patients here. there is absolutely no compassion here for us patients when ever the staff make daily constant mistakes most of witch causes us to stay get sicker the food is horrible we i cant go outside the facility on my own trapped like a prisoner nor do they have any programs for us to go out to the beach parks ect. the administration acts like he listens to u acts like hes going to change stuff around here but never does. this place is a straight fraud theres beeb multiple lawsuits filed on this place. just yesterday my mother and i had a christmas visit 12/25/2021was rudly interrupted by head nurse theadorea n was told we cant continue our visit because we were bothering a patient as a result of that my mother n i faught with eachother now she will not visit me here n we are not talking. but yet they boast about how much they promte familiy ties support yadda yadda yadda. thatz their christmas gift to my mother and i thank u shoreline!!!! lastly this place has no profesionalism its all personalism i know those arent words but you know what im saying. they need a whole 180 of this place please find another place as im doing the same thank u im in room 50c take care read more

The staff at Shoreline Care Center have taken care of my mom for the last several months and have shown her tenderness, gentleness, respect, dignity. I would recommend them again. Please know that a skilled nursing facility is not the same as an acute care hospital. I am grateful for the staff who showed up and didn't just do their job. They cared about my family member beyond a paycheck which says a lot about the kind of people who are dedicated to the care of others. Thank you Shoreline. This place is filthy and smells. They dont check on patients medical machines. Someone put my grandfathers oxygen machine on high and nobody noticed till we visited late at night and told the nurses. Pressing the call button is a 10 minute wait. Deserves less than a one star review read more

Last Friday, I received a call from a friend notifying me that my ex-husband had called him, telling him that Shoreline was releasing him to his car on the street with no shelter, nowhere to go. He would have been homeless, despite the fact that this is against the law. Shoreline, as with all nursing homes, is required to issue a "Safe Release Plan" which they did not follow. Despite the fact that the patient notified the admin that he would die if they released him to his car, they did not seem to care. Unfortunately, he suffered a coronary and was found unresponsive and pulseless in his room two days later. The worst part is all they needed to do was reach out to a dialysis center to get a simple letter on file, which I'm told would have satisfied the request of the insurance company, which had denied him -- this would have avoided a great deal of stress. In addition, (I'm adding to my original review) it's been 1 1/2 weeks since his passing and NO ONE from Shoreline has bothered to contact me or anyone in his family regarding his personal belongings (namely his driver's license which I need to process paperwork with the DMV). The head social worker NEVER returns phone calls. When I contacted her the Friday before my ex's passing to understand why they were throwing him out on the street, she (as well as her supervisor Francis) literally hung up on me! As everyone else has noted in the review, they absolutely don't care about their patients -- just getting their money. DO NOT TAKE YOUR LOVED ONES HERE! read more

I had the best experience with this facility. The staff was very helpful and getting my family member help with outside services that wasn't even in their scope as far as their job but they went out of their way to help us . In addition when my family member was discharged they called him the following day to make sure that he was OK and check on how he was feeling. In addition they provided additional help with getting him some help with placement for home health care. Thank you so much for helping us we really do appreciate it. read more

When I first came to the facility I was scared and not sure what to expect but I learned very quick that the staff that was very caring and loving. They made me feel like a part of their family. I only came in with a couple of things and people in administration who went out of their way to buy me clothes and things that I didn't have but needed. That alone speaks alot about the staff here. I have now been there twice and the second time I felt very welcomed. They remembered me from the first time and genuinely cared about me. I would definitely recommend Shoreline to anyone needing therapy. read more

This place is highly rated by the the compliance boards, which is critical. Sure, it's basic and not as tidy as it could be, but they are skilled nurses and meet the needs of their patients. I've been there a few times, so I definitely think it could use some more decorations, a paint job, and some landscaping, but they get the job done. Don't judge a book by its cover. read more

If I could rate 0 I would. MOSTLY All Of the stuff at shoreline are shitty people who have no heart. They basically killed one of their patients. They force feed him causing him to have food/water in his lungs which made him have blood clots. They also did not take him to the hospital when he had a fever which lasted 5 DAYS before they took him. There are other things that I've seen and heard about this place and let me tell you if you really want your loved one to die this is a great place to send them. read more

I found the staff at Shoreline to be friendly and helpful. The lobby was decorated for the season and welcoming. The nurses are very good at what they do and try very hard to assist patients as quickly and efficiently as possible. Parking is a challenge but they do have valet. Overall the facility is a good choice for people to transition from hospital to home. read more

This is my second comment in reference to Shoreline nursing facility. Food has improved 60% . My Mother is happy with the staff which is very important. I still have to clean her sleeping area eveytime I visit in reference to the floor. My mother is very independent, my concern is if she became incontinent they don't have sufficient staff to facilitate her needs. This has occurred in the past. Currently she's doing great thanks to the nurses who regulated her medication. read more

This is the worse place to stay. The rooms are filthy - curtains have buildup of dust. They have electrical cords running all over the wall (fire hazards). Beds have holes in them. The lady (Cindy) that runs it now, needs to learn how to run the place or get training from the lady that use to run it (Cindi Paulson), or pack up and leave. This one is running it to the ground along with her puppets Ruel (PT) , Theresa (OT). These people when caught in lies or busted doing stuff they are not suppose to do. They change records. The social Service lady Charito she don't help the people in there, she covers up for Shoreline. People make sure to have your cell phones recording and take lots of pictures. Oh NANNY Cam comes in handy!! The food is terrible and cold. Poor CNA's they have 15 -18 patients while most of the nurses treat the CNA's like slaves. Make sure family bring or order outside food. When does a dietitian (Lea) has a right to prescribe meds even vitamins. Oh if your diabetic she will make sure your diet includes starches and pastry's and cookies and then wonder why your sugar levels are so high!! Showers backed up with feces coming up out of the drain. Nothing gets done. I was in the shower 2 x's when that happen. I went and told maintenance Carlos, he said he never got a order. Carlos was great he went right away and cleaned it out. Roommates folley 4 days in a row ready to burst at the seams. The urine backing up into her bladder. She has been fighting UTI's , now you know why!!! Leaving a tourniquet on after putting a IV in her arm for 3 1/2 hrs. Arm swollen up. Then you have the asst. D.O.N blaming someone else. Oh no, it is both your faults. Take blame and correct the problem. Then the other older lady on weekends, I use to make sure she was alive. They would just give her crushed up meds. in a pill cup with ensure and leave. Then I would never hear the older lady so be crashed out Saturday and Sunday. I would look just to make sure she was breathing. Nobody would come in and check up on her or change her. The CNA's would drop her meals and leave, just to come back a hr later and take it away. I ask are you going to feed her, CNA would say she is sleeping. Yet they wake her to give her more meds in the pill cup with ensure. She was not getting feed. I would say something the next time they tell me to mind my own business. I would say, she be my business if you guys are treating her bad. I have a voice , she don't so I will speak up for her. Then right before the day shift would come in they would change her crappy, drenched diaper,sheets and blankets then wipe her down and spray her with some body spray. I would tell her kids when they came in. When taken showers they put you on a shower chair and so call you wrapped up. Your butt and privates are hanging out and parade you down the hall to the shower rooms. Don't let them do this to you. Get undress in the shower room. They will say "OH YOU REFUSING" make sure people have your recording on!! One of there forms you are not suppose to have cameras and recordings. They want you to sign. Which I NEVER SIGNED as Ciny said I did! I wanted to tape a so called meeting and she said I signed a paper. I told her to get it. She said NOW! Me: Yep get it. Cindy comes back with all excuses but no paper. Next day oh they wanted me to signed paper work, only to give me a page 2, 12, 8 out of 28 (example) . I ask where are all the sheets in between, was told oh you don't need that. I made sure I signed and wrote refused to give me all only signed the page I wrote. They do threaten you to sign. If not they say they will tell the DR that you are Incompatible. They treat you like your in a mental hospital here. This place can be ran better with the right people. this place is disgusting! other family stuck dad here without my knowledge. he hates it, staff are not attentive, he wears soiled clothing for days, when he rings them it takes 5 tries or more to get help. they Do Not clean the rooms. stinky place and my dad is getting out ASAP. I have seen enough neglect from the so called staff. UPDATE! My dad died here after 6 lousy days. He was deprived of all meds, never got to move or get out of the crappy bed, neglected and dirty most of the time. His oxygen was tampered with, his meals were disgusting, his dentures were broken and they did not care if he could eat. They tried to tell me he broke them while laying on his back. How? Impossible, liars! My dad had no pain but was force fed morphine? Apparently I had no say in the matter because my 2 ex sisters put him here to die, as they told me. They made sure to speed up his death too. He was drugged so much he could barely stay awake. I am heartbroken my dad had to go this way. I will never recommend this dump to anyone. Entering Shoreline Care Center is analogous to walking into the Psych Ward in the movie, "One Flew Over the Coo-Coo's Nest". People roaming the halls unattended, moaning, screaming, and asking for help. The smell of urine and fecal matter hits you immediately as walk in the facility. The nursing staff (almost exclusively Filipino) cluster around the nursing stations texting on there cell phone and talking to one an other, remaining largely indifferent to what is going on around them. The overriding concern of this facility is centered on collecting as much money as they can under Medicare billing guidelines or from the patient/patient's family. My mother, in a lapse of mental clarity signed over her sizable pension check ($4950/month) to Shoreline to share a room with two other people. With the help my brother, we are moving her to a new facility (Sunrise Senior Living) in Westlake Village, CA, which is infinitely nicer for less money each month. For $4,600 my mother will have a private room with her our private patio is a facility that is clean, modern, and comfortable. Avoid this despicable place at all cost. read more

My daughter has been at Shoreline Care Center for the last 2 weeks receiving physical therapy after a broken femur. She is much younger than the rest of the patients. Many of them are elderly and some with apparent dementia. From what I saw everyone was care for, no one was neglected. Activities were scheduled daily, all day. The doctors from the hospital thought my daughter would be there for a month and she will be home tomorrow. The physical therapist was excellent, the nurses always cheerful and helpful. The custodian installed cable in her room with her first request. Her bed was changed daily and her room was clean. The administration was helpful and considerate always. Some of the patients were somewhat difficult and they were treated with kindness and understanding. The building is older and not fancy, the garden was well maintained and pretty with flowers and vegetables growing, there is a smoking/vaping area for patients that is easily accessible, valet parking on the weekends, super efficient nursing staff and a few blocks from the beach. For the insurance she carries which is not accepted my many care centers, we were grateful to have found a resource for her therapy. Every time I would worry I would call her nurse and she always picked up the call and reassured me. read more
