Had to make my way home with no car from Ventura, Ca to Los Angeles... in the middle of the night, approx 3 hours from home.. Very bad situation. I call every few months and still haven't gotten my money back, it has been 7 months. I am a very physically disabled person currently approaching major back surgery, and really need this money back. Pax seemed nice, and I gave him the money, but as soon as he went home I was left to deal with staff members who were not less than savage, as they kept my money. This is my first experience ever in any rehabilitation center. I am glad I can in when I did. I've met some great people here along my journey to support my decision to enroll here. If you're thinking about coming here and can. Do it. You won't retreat it. Top notch care & the food is above average & always fresh. Thank you Logan & staff for being a part of my journey. All the therapist are top line. Jake the gym trainer, the best. Super sweet gentleman. From the services they offer to the nurses. Top line. Not to mention for being form East LA native Mexican-America Chicano, I was treated with Class & respect. All walks of life come here that I lived with for 30 days. Community Memorial Hospital (CMH) released my grandmother here…read moreThursday afternoon and it was so creepy that we rearranged to go to another skilled nursing within an hour--I declined to sign any paperwork for admission into VPA. My main issues were: 1-STAFF: There was no staff to assist a new admit. After her transport drop off, I found my 90s grandma with a fractured pelvis sitting alone on an unmade bed, staring at a wall. There seemed to be various nurses, etc., but no one looked up/made eye contact with me, even when I'm sure they could hear me addressing my concerns to the creature they've hired to grunt at the front desk. 2-SECURITY: The woman at the front desk grunted for me to sign in at the electronic station and didn't look up. Also, the front doors are broken in that they are very difficult to open and the accessible button to auto-open doesn't work from the inside, and according to staff they've broken for awhile. Functioning doors is just basic for place that houses many people in wheelchairs. 3-BEST PRACTICES/LEGAL PRACTICES: The same woman at the front desk handed me a stack of paperwork on a clipboard and her only instruction was "Sign." A grunt-command is not a legal, informed consent and the paperwork required for skilled nursing is lengthy (medical and legal). I asked who I could speak with about any questions and who would be going over this with us, and she gestured to herself and said, "What question?" I asked to speak to admissions and/or the charge nurse and she gestured again to herself and said, "Sign." The alarm bells were ringing. Meanwhile, I checked in with my grandma and a CNA had set a dish in front of her that she's allergic to. Since there is no admissions process to speak of, they wouldn't know her allergies or literally anything about her. I have no idea if there are things such as sheets, a call button...no idea. Since they put a bib on her for no reason--she can feed herself--I'm assuming they are in possession of other sad textiles somewhere in the dankness. The CNAs were polite, but they are not decision-makers, and when I asked if this was unusual and who I should be talking to about this odd admissions process they were confused. I guess this is just how they treat people. Finally, the front desk creature drifted into the room, grunting for me to sign the admission paperwork. I declined and began the task of finding a different place where my grandma would be safe. CMH pushed back (it was an unpleasant exchange) until I told CMH that I was headed back to the ER with her. Within 5 minutes the case manager's manager called me back and they had found us a bed elsewhere. Interestingly, they warned me that it might be difficult to get the necessary paperwork back from VPA. PRIVACY- They're not concerned with patient privacy so it turned out that her paperwork was just sitting at the check-in desk and I took it. We waited for medical transport, and we left.

    My Mother with dementia was released to this facility with a broken pelvis. I was told they were…read morenot allowed to have bed alarms. I was very concerned she would try to walk without assistance due to the dementia. The care during her stay was adequate but the communication was horrible. My mother did try to get up and walk by herself and fell. I was not notified until an hour after she arrived at vcmc and was told she bumped her head and was taken to chm which she was not. The way I found out was the doctor from the ed called me. I didn't hear from vpa until 2 pm the next day asking if I knew where my mother was. I explained her situation and conditions and I have not heard from them since. I never received any communication regarding her progress and after numerous times trying to meet with the appropriate staff never did. My mother passed away from her injuries. I just want family members to know about their communication and how horrible it was.
