    Husband's bed he was laying in before going back to hospital

    Had to go to facility atv10pm at night Either no one answered the phone and was automatically hung up on 3 out 14 calls that thru to nurses desk were not answered either Had to knock on the door bc the lady at desk that came from the back would not acknowledge me Went to 4th floor and found my husband confused incoherent and bruised on his forehead. There was also poop on the wall after he had to get himself up after the fall Not notes of what happened in his record Told nurse to call 911 When was put on the gurney his bed was soaking wet They were suppose to make him strong not weaker Well the hospital said he was lucky I went to check on him High ammonia levels and pneumonia They saw that he was not ok To find him in his hospital gown, I guess they got him ready NOT He could have went into a coma I knew something was not right If this happens to you GO TO THAT FACILITY but not this one They should have called 911 themselves DO NOT PUT YOUR LOVED ONES HERE

When I called and spoke to Christine about the situation she immediately sprung into action and expedited the transfer of my mom to Villages. My mom is a 73 year old retired nurse and the nurses at villages treat her with admiration and respect. I have overheard them telling my mother "thank you for your service as a nurse" which truly warms my heart. Christine has kept in constant contact with me via text and phone calls that I feel like she is a friend who I can count on to make sure my mom is being cared for even when I can not visit for a couple days. Knowing that my mother is getting the care she deserves for her karmic retribution of being an exceptional nurse for over 40 yrs is such a peace of mind for me. The rooms are private and very well decorated to add to the list of wonderful things. They allow you to bring your leashed pets in for a visit too! We feel so lucky to be at villages of lake highlands!!!

They had to be moved several times due to his issues but the staff was always caring and pleasant. I learned so much what these nurses had to endure daily, I have a new respect for what they do at Walnut Place. The head nurses communicated very honestly with me and my brother on what steps would be best for both of them. It is always hard to move your loved ones to a nursing facility from their home. But I truly feel that Walnut Place was the best place for them.

    My dad was a resident at Walnut Place from July 2021 through December 2021. At the time, my dad was…read more85 years old and had been diagnosed with dementia for approximately 2 years. He was initially hospitalized at Presbyterian for a UTI in July and the doctors there recommended to my mom that he be transferred to Walnut Place for further treatment/rehab. I am unsure if there is an affiliation between Presbyterian and Walnut Place but it seems likely that there is. My mom took Presbyterian's recommendation at face value and had my dad moved to Walnut Place.  My dad lived on 3 different floors while he was a resident there. It was in his first room on the 2nd floor where he tried to get out of bed. This resulted in him falling and fracturing his wrist. To this day, it is unclear to me why he was in a bed without bars since this would have likely impeded this activity. The staff on this floor was fairly hit and miss. Shortly after this, my dad moved to the Rehab To Home program. These rooms are located on the 6th floor, and this was his best experience. The rooms were all clean with small kitchenettes, the staff was friendly, and my dad's needs were attended to, such as cutting up his food & feeding him, changing him regularly, clean bed linens etc. His last and final move to the 3rd floor is the primary reason for this rating. This floor always smelled like urine. As soon as you stepped off the elevator, it hit you in the face. I saw at least 3 roaches in my dad's room during his time there. Face masks, paper towels, and wet wipes were scarce if existent at all. We never found one of the foot pedals for his personal wheelchair after it mysteriously disappeared.  I would do my best to come to Dallas 2 weekends a month. It never failed that when I would arrive on a Friday or Saturday morning that I would find my dad with food all over his face and clothing. My dad could not cut up food or feed himself at this time and there were instructions for staff to assist with this. When I would ask staff about this, they would get short and defensive with me and tell me they did not have the time. Even more disturbing than the food was the fact that my dad was often soaked in urine and occasionally soiled with feces. My dad was still lucid at this point and he was very embarrassed about this. I can only speculate that my dad was in similar conditions on the mornings that I was not there.  My dad was moved to this floor because staff had determined that he required 2 staff members to assist him, and that was not a "service" available through the Rehab To Home program. Good luck finding 2 staff members on the 3rd floor who were not only available but also willing to help. I would track someone down and either get told, I'll be there soon (and not see this person again), or I'd be met with attitude about how busy they were, my dad's room wasn't in their section etc. One aide practically yelled her schedule in my face. I would offer to help and sometimes that would result in him getting cleaned up sooner but then there was no reason for him to be on this floor if the 2-person requirement couldn't be met with current staffing. It was $6000/month for this "service." Do your due diligence now, folks. If you have an aging parent or family member, start looking at facilities now. Have a list of questions. What is the turnover rate? How many staff are there during busy times such as meal services? Is there a daily checklist for each resident? Who maintains that checklist and ensures that each item is carried out? What accommodations do they provide and how does the facility ensure that these accommodations are implemented?  Ask for recommendations from trusted friends and family. Do not just take your loved one to the first facility that a hospital or doctor recommends. We were lucky that my mom could sit with my dad a lot of the day to ensure he received the care he needed. Not all residents were that fortunate.  There were some very kind staff who worked at Walnut Place, and I always made sure they knew how much we appreciated them. The arts & crafts ladies were so sweet and you could tell they loved their jobs. The front desk staff, the physical therapists, and the doctors were all knowledgeable and helpful. But the aides here, who are the backbone of this facility (and this industry, in general), were overall unreliable, unpleasant & not helpful. If they are indifferent and/or negligent, it is your loved ones who will suffer as a result.

We brought her home that weekend and the loving and caring staff not only got her comfortable but more importantly, she had a series of falls the last few months before this hospital visit, so she had not walked for almost 8 months. She was started on some muscle strengthening exercises, given some tender love and care, and not only did she make it through the weekend, but she walked about a month later for the first time in a long time, and lived almost 7 months after receiving such initial devastating news. We collectively know that it was Accord Palliative and Hospice Care that went above and beyond for our family. Thank you guys so much for caring for us in our time of need. -Davis Family

I visit 2 to 8 times a…read moremonth and always observe kindly, respectful treatment of the residents, even the ornery ones. The facility is clean, the food is satisfactory and adequate. Our family is grateful that our brother has a place there. He nearly lost his life due to careless medication management in a private assisted living facility in Hunt County where he stayed for 17 bleak months. That ended with an ambulance ride to the ER and 2 weeks in ICU due to bad medication management. In that process, our brother's cognitive faculties were so damaged that he qualified for Medicaid long term care. I'm sad about his medical status but the Meadows takes good care of him. I regret that Medicaid reimbursement rates are so stingy that life is not very diverting and interesting for Medicaid-dependent nursing home residents. Of course I have fears about the general trend in Medicaid reimbursement. But the Meadows does very well with the resources it has to work with. I'll pay special tribute to Mr.Bill, the loving heart and soul of the place! He's not in management nor is he a licensed professional therapist or anything, but he is all over that facility doing his best to ensure that spirits are high. He has been doing this for decades and I believe he truly is the house Angel.

Several outstanding employees quit or got fired when the facility was sold to owners. Several key employees who had worked for the previous ownership and now retain their jobs under the new ownership, are people I now regard as dangerous. As of late 2025, my rating of this nursing home bottomed out. I'm sorry to say the place is unacceptable. My brother WAS well and comfortable, but he has deteriorated after being abused and physically assaulted by a mentally ill roommate. The Facility lied to us about it. At 6 ft 2 in, my brother now weighs less than 130 pounds, and looks like he was just released from a Nazi concentration camp. The Director of Nursing looks me in the eye and declares he's fine.

The facilities are great. The residents rooms and bathrooms are private and spacious with high quality buildouts. The food is good. I've noticed generally high quality, tasty food getting served. The staff means well. I don't doubt the sincerity of the staff wanting to help improve the quality of life of the residents whether that mean providing activities in the common areas, get them medical care when needed, speak to them encouragingly when they need to be re-guided. The issues I ran into require a little bit of context. My father is averse to getting help on showers. The PVN nurse suggested we look into getting some meds to help on shower days and get some external extra help to provide showers for him. While I wasn't happy to pay for extra help, given the high cost of PVN I thought the request was presented in a professional manner. The issues come in with a lack of professional approach to medical management and follow-up. I consider these issues significant enough to ding PVN down to 2 stars. My father's doctor agreed that some medicines could help make shower days a little easier and that it was important for him to stay clean on a regular basis. The problem is that now, despite this being the PVN staff's idea, they are unable to get the medicines to my dad on the morning's of the shower days. They have missed this 3 out of the last 4 shower days, which then makes the task of giving showers a monumental task. It would seem that this would be easy to address given that they claim to give him his other medicines, but the one medicine that was introduced - effectively at their request - is something they seem unable or unwilling to give him. I have called numerous times and can get an answer about 50% of the time. I have sent 4 emails on this topic to the Head Nurse & her assistant and have a 0% response rate thus far. In my mind this boils down to poor leadership. I have no way of communicating with the senior leaders of PVN to get these issues addressed. The nurses ignore my emails, and worse, deny my father the access to medicines that will improve his and his fellow residents quality of life. it is my belief that PVN needs to fundamentally transform the culture and leadership of their organization. They are currently falling far short of their mission.

I appreciated her diligence and the effort she put into my case. Old review : I had total knee replacement surgery. This company contacted me initially and we set up times. The RN came and she was Amazing! The PT came and he was great. What I have major issues with is I was supposed to be getting 3 sessions a week for two weeks minimum.. On my about to be third session , the PT said oh we don't have pre authorization. I wondered about this so I called. The company would not call me back. I called twice. I waited 7 hours and no follow up call. I called a third time around 3 in the afternoon . I finally got someone and they said, " Oh we don't have pre authorization." I said , "Then how did you guys send people to my house in the first place?" They still have not answered that question. I still should be having PT sessions , it was knee surgery, the Dr. office did send that over (I checked) , and still no services and no calls from this company when I should be having therapy. Knee surgery is a difficult surgery mentally and emotionally, and to have to deal with this is unacceptable. I rated the company a 1 only because I really liked the PT and OT. Their management , scheduling , return calls needs to be improved.

They started out in IL (independent living) and 3 years ago we lost Dad and Mom moved into DL (dependent living). All buildings are connected so Mom gets to see old friends at Sunday Brunch. (She is 93 now.) She has made many new friends in the DL building who are charming and so nice to her. I'm writing this because over the last 5-6 months I've been joining Mom for Happy Hour and have gotten to know her friends. I can't tell you how many stories I hear them share about other facilities in Dallas (Austin and Houston too) about how bad the food is and how they themselves (or friends) are ignored and not cared for. They told be several times that when they tell people how awesome everything is at The Forum... their friends don't believe it!! LOL, LOL. Well... it's true! Yes, I confess I take Mom to Sunday Brunch so I can get one of the best omelets in Dallas (in the IL building) and the rest of the food is so nice. My sisters take guests and we always celebrate birthdays and holidays at Sunday Brunch. Enough about that. Let's get to the brass tacks. The staff is probably the best I've ever encountered. If I'm with Mon and she has an ache and needs some Tylenol, I call the front desk and the nurse brings it to her within 15 min or so. VERY RESPONSIVE!! (Abbey is so awesome!) Mom has a Help Watch on her arm and can press the button and help comes immediately. As well, the staff will call me with every incident (like sliding out of her chair, etc.) Major or minor - they update the family immediately. AWESOME! Ok. So you are looking for a place for your parents. Absolutely check out The Forum. Have frank and direct conversations with the staff who show you around (and see if you can pay to sample the Sunday Buffet!!) Our family has so much peace knowing even the cleaning people care about a quality job and if they observe anything that needs attention - they alert the right people. She is the most caring and most gentle person I have ever met. From the first day she treated me like we were old friends. I was comfortable around her from the first minute we met (which I feel is super important when it comes to someone in the medical field). She sympathizes with the fact I'm in pain and does everything in her power to make the cleaning of the wound as bearable as possible (if you've never had a bad burn, this is practically impossible).
