As an outside observer, I have been impressed by the staff at Signature Pointe's physical therapy department, particularly those who go above and beyond in patient care. I was sent to signature point after having four fusions in my lower back. I was there for a month but only remember one day they allowed me to become extremely septic and had to call 911 where Hospital had to save my life. I almost died. I wouldn't recommend this place to anyone. read more

Where to begin? Many of the caregivers at Signature Pointe are thieves stealing from the bedridden, most vulnerable residents. Stealing cell phones, chargers, expensive perfume, chair cushions, personal blankets, clothing, etc. No accountability from anyone. Caregivers and staff don't wear name tags, but a name is required in order to make a complaint. A conundrum, isn't it? The fact that they employ an abuse coordinator tells you they have widespread abuse which needs coordinating. One doesn't call a dogcatcher if there are no dogs. Hygiene is a word they can't spell. The food is inedible and sub-par. It breaks my heart that the elderly are treated in this manner. This is not a medicaid facility and the private pay cost to live here is exhorbitant. Patients are wandering the halls in their underwear dragging catheter poles. Visitors wearing hijabs are wandering the hallways asking for face masks. Staff DOES NOT respond to call buttons, but have time to be constantly on their cell phones accessing social media. This place is a thing of nightmares. I do not have one good thing to say about this place. Any good reviews you read likely come from a staff member, an employee of the facility or the owner. read more

Abandon all hope ye who enter here. My loved one is incontinent and requires diapers. She is not provided them and her bed is constantly wet and clothing is urine soaked when I visit. She is blind and has fallen multiple times requiring hospitalization which required staples to her scalp. Rude and non-caring staff. Deflection is their response to complaint. This is a private pay situation - not welfare and the price is exorbitant! Cleanliness is an abstract concept. Supplies are rationed. Food is inedible. DO NOT GO HERE UNDER ANY CIRCUMSTANCES! This is about the worse facility in Dallas, not the worst because that would go to Treemont. My uncle was registered there for rehabilitation after a fall. Everything was fine until we notice a tote bag we brought him was missing. Then today, Friday 2/07, he was transferred to a doctors appointment and left there. The doctors office attempted to contact Signature Pointe but no one was able to help. I had to call and get the 3rd party transport information to get him back to Signature Pointe. Who forgets to pick up a patient? Signature Pointe, that's who. To trust your loved one with ANYONE should not be taken lightly. If you cherish your loved one and want the absolute best care for them, do not allow them to be admitted to Signature Pointe. I cannot wait until he is released and back home where he will be safe. read more

I was admitted here for a month. The therapist were great! They came twice a day and I really got a lot stronger. They were kind and even printed out material to work on over the weekend. The negative: All the residents were much older-i was mistaken for an aide if I ate in the dining room. Requests like Change the TV, more coffee and even help back to their room. I ate all my meals in my room. I enjoyed the activities, bingo was fun and the weekly singers were great. The staff overall was good like most of us we all have good days and bad. If you are on pain medicine you have to request it they don't bring it with the rest of your pills. Very good with hygiene-bathing every other day. I had a lot of friends I met and will remember this Rehabilitation center. One final note when I left all the management and staff was there to say farewell. Lots of hugs and best wishes. read more

My mother left Signature Pointe in early January, and she was even sad to leave! She loved it there (I was quite astounded that anyone could love the hospital!! haha). They most certainly saved her life! My mother is now perfectly, finally well. The at home aftercare that SP recommended was absolutely excellent. I want to personally thank and commend all the staff on the 3rd floor especially Beth Gayton, Baruch, Melvin, Esther (night nurse), Kimuto, Chrissy Bushee, JoAnne (we truly love you JoAnne!), Sohini, Jenny (and all the excellent PT and OT therapists especially DEBBIE in OT whom we all adore!), and Charity. I've called the assistant director of nursing and left 3 messages and she hasn't called back. I've called the executive administrator and left a message and she hasn't called me back either. There are many assisted living facilities around the DFW metroplex and I've had personal experience with three - two for my aunt and one for my dad. Signature Pointe is definitely one of the better ones in Dallas. The ladies at the reception desk are friendly and welcoming to all guests and visitors when we arrive. Checking-in is time consuming, and kind of a pain since Covid, but they want to keep their residents and staff safe and healthy. The staff is also friendly and respectful of residents and their family. I visit my aunt weekly and they've always been friendly and considerate. My aunt has a nice room with a big window and her own thermostat. Occasionally residents clothes get mixed up but the staff is quick to fix any problem. And, I've never seen any resident in distress or uncomfortable. Now for the bad news: visitor parking is HORRIBLE. There aren't enough spaces and you may drive around looking for a place to park. There are lots of empty spaces for residents and employees and not enough for guests. If you're looking for an assisted living center in Dallas, definitely give them a call. I went to Signature Pointe after knee surgery because I was not able to walk as well as expected. The outside is very beautiful, but my husband said parking was very difficult. Inside, I think they are keeping the place up with coats of paint and it was fine. I was put in a bed and was diapered. Once a day on weekdays, someone would come and get me, transfer to a wheelchair and go to the rehab room which was very nice, for about an hour. The rehab therapists were good and if other conditions had not made me leave, I would have regained knee function pretty quickly. On weekends it was basically 24 hours a day in the bed. The problems for me was being flat on my back in bed with diapers for the majority of every day. Yes, they could leave me in the dining room in a wheel chair but that became uncomfortable for me after an hour or so. My bariatric wheelchair was too hard for me to handle, so I always had to ask an aide to move me. My bed would not come to a sitting position, and they agreed it was broken but didn't replace it. I should have been more demanding because being able to sit up would have helped me. The food was not very good. I think I could easily lose weight if I had stayed! The eggs were so bad it was a topic of discussion in the dining room. I had to laugh because I met a lady there as I was leaving, and she said her dog loved those eggs! The television mainly had old channels. At night there was a lot of noise, some of it people calling for help and some of it the staff. But the big, big problem was those diapers. There always seemed to be a wait because someone was in line before you for a diaper change, so a lot of time was spent being wet. I asked if they were understaffed but the reply was no. After a few days I realized I had a urinary tract infection. I asked them to test me and they did, but the results didn't come back quickly. I decided to leave and had my husband come and get me against the advice of the management. The next evening I was in a hospital emergency room getting help for the UTI. If you are able to walk around or manage a wheelchair, your stay might be better than mine was. read more

We selected this place when we needed a skilled nursing facility for our Mom. While they were able to provide me with a same day tour, I should have known that the tour guide as a no show should've been the 1st red flag. However, as the director agreed to give me a tour, I dismissed that. Cosmetically, Signature Pointe looks good., but when you look past that there's a lot of work to be done. 1. The response time for the aides is a minimum of 15 minutes, especially on the weekends. They will say it is less because in about 8 minutes, they come to see what you want, turn off the call light and say someone will be back. 2. On check-in day only the assistant director of nursing came to ask any questions. No one asked what the therapy goals were or had me complete any paperwork. 3. Mom was left alone while brushing her teeth and washing her face from the wheelchair. The aide left her to take a phone call. 10 minutes later he wasn't back and she passed out and hit her head. The nurse heard her yelling after she came to. She was the one to call me. I arrived within 20 minutes and 90 minutes later the doctor hadn't seen her. When she did, condescending was the best description of her demeanor. We went to the ER and luckily she suffered only a large knot on her forehead. 4. At the meeting I requested regarding the incident, I learned the aide had been fired and that there is a rule against aides using phones while in patient rooms. I know this isn't followed. At the end of the meeting I signed paperwork and was told this incident was proof Mom couldn't be left alone. 5. The director of physical therapy was No help. She never asked what Mom's goals were or how she got there. She assumed she would Never be able to transfer on her own. To me,that's nit their job to assume. 6. On the day we transferred her out, they reached an all time low. No one called to say what time the transport was arriving. Mom refused to leave until I was called. They refused to let her back in her room. Despite the fact Mom had suitcases and a dirty clothes bag, they tossed all of her things in garbage bags. 7. There are 2 employees who were very helpful. Donna, an OT, was so kind and helped her a lot. She was her bright spot. Chrissy, the social worker, is a Godsend. She truly cares about the patients. I know nursing homes are important, but places like these are the reason people hate them. read more

Severely understaffed. Many rude uncaring aides. New aides appear to receive no training. My mother is not mobile and is frequently soaked in urine when I arrive. Some nurses are very nice. Some are very rude to both residents and guests. My Mom has been here almost 2 years and her physical and mental condition have deteriorated considerably. If you are not mobile and mostly able to care for yourself, the skilled nursing facility here is NOT for YOU! The rooms are not cleaned well unless you do it yourself. On weekends they frequently run out of necessities...bathroom wipes, bed sheets, towels, etc. I think you get the picture. read more

If I could give a no star I surely would. This place is so under staffed and the staff they have is either rude, uncaring or dose not know what the patients needs are. My mother is here and her care has been horrible. Her doctor talked her into going here promising this was an excellent place. She has went to been on a few nights crying herself to sleep. The excuse I have been given is " I am so sorry we are so short staffed" She was told she didn't have pain meds on her orders, the nurse told her there was nothing she could do to help her, walked out her room and shut the door. She in fact had meds on her orders. The nurse said " oh I was off work two days and forgot what was on your med list" How is this right????? On December 17, 2023 I was admitted for extreme back pain I could not move at all and was given a nice room, no. 432. I enjoyed all the therapists that diligently worked with me twice a day as scheduled. A special thanks to Mary Ann as she wanted to do some things with me while I was on a mat but could not get me up as she tried for several minutes but my back hurt so much that I could not really move when another therapist came over and the two of them tugged me up. This therapist who I liked would get me down to the exercise room first thing in the morning. Was her name Meredith? Jessica was really good with me and so was Richard. Let Richard know that I can easily get in and out of bed now as I'm sure before I left to go to my son's home which was Friday, December 22 that he was doubtful how I would do the bed routine. For the most part the office experience I had was good in making sure I got my meds on time. YaLonda, the nurse, Assistant director of Nursing? was really outstanding. I liked the young woman that brought me my meals as I ate in my room and she assisted me with a shower. She was friendly to me. The meals were okay. I really liked the hot oatmeal. I enjoyed bingo and interacting with the various people I came in contact with. I don't recall the name something like Am Ten and the mind games she had for me. Moved my Parents into Assisted Living a few months ago. The staff has been exceptionally nice, knowledgeable and genuine.

My daughter was admitted to Signature Pointe on December 18th, 2020 for short term, inpatient physical therapy care, which will probably be for three weeks. The Patient Care Administrator, Sandi, should have communicated with her within 24 hours of admittance, but did not contact her until December 23rd. That call was made only because I complained repeatedly about my daughter having not heard from her. I have called Sandi several times to discuss various problems my daughter has experienced during her stay there, one of which was being verbally abused by a nurse practitioner named Sarah. Sarah also abused me verbally during a phone call. Sandi's response was "Sarah can come across that way." My response was " Every patient deserves to be treated with dignity and respect". Sandi told me I would receive verbal progress updates twice weekly regarding my daughter's physical therapy. During the two weeks my daughter has been there, I have received one update, only because I called Sandi. Attending physicians have not entered patient observation notes into my daughter's chart in a timely manner. In fact, no notes were entered into her chart for two weeks. I have left a message for the facility administrator to call me, but she never bothered to call me back. I live two time zones away from my daughter, so all of my communication is through phone calls. However, due to great concern over my daughter's health, both physically and mentally, I will be traveling to Dallas soon. read more

Initial review was in 2012. Update is February 2019. While being pleased with the Independent Living side from 2012 to 2018, we have a very different experience with the Assisted Living side of the house in 2019. The facility is aging. They are putting in new carpet and putting on a new coat of paint on everything. The instrastructure is not being maintained well. Repair requests take several weeks to be resolved. Repeated problems with air conditioning. Over the last few years there has been a lot of change in department heads and directors. The last director was always absent, and never returned calls or email. There is currently no fully qualified social worker, only a junior level one (associate level?). The new ownership (maybe a couple of years ago) is not as responsive as previous. They raised prices significantly on everything from guest meals, to guest rooms (for visiting family), to resident charges. At the same time, maintenance has decreased. Staff turnover is much higher, especially at the director and department head level. This is an organization in turmoil. The assisted living side is not and has not been nearly as well maintained as the independent side. Our parent had been skipping meals lately and staff failed to inform us of this. "It's not our job on independent living side," a family member was told. After hours, a family member went to Assisted Living kitchen, could not find clean dishes. We're out of here. read more

The incompetent staff of Signature Pointe caused the death of our father. The management then fabricated their own version of the horrific event, which seven family members witnessed. The TX Board of Nursing RULED that the Signature Pointe staff did indeed cause the death of our father. Additonaly, Signature Ponte management was found to have destroyed the original medical records and they created a Signature friendly version of the event, to cover up their actions. I am extremely disappointed with the care my elderly father received after being in rehab at Signature Pointe for 3 weeks this June and July. Although the actual physical therapy was very good, and the therapists kind and professional, the day-to-day care was poor. These are some of the things I observed (in 4-6 hour visits, over 5 days): his call light was out of his reach 4 different times; no one came in to re-position him (necessary for preventing bedsores); no one came in to encourage him to drink water (dehydration is one reason he had to be hospitalized); no one offered to get him into his wheelchair to go to the dining room for meals; and one of his long-term medications was simply forgotten, the entire time he was there. There were several times that there were items delivered with his meal (including thin liquids, which he could choke on) that he is not allowed to have--information which was right on the paper on his tray. He is also on supplemental oxygen; sometimes they remembered to put it on him, sometimes they didn't. Although the aides were kind, there definitely needs to be more of them, and they need to be better trained to cover these essential caretaking tasks. read more

My family is VERY pleased with the care given at Signature Pointe. We moved my parents there just over 1.5 years ago. They both started out on the Rehab floor. We eventually moved Mom to a Retirement apartment. The apartments are comfortable. We were not able to move Dad to Retirement, but he received very good care in Rehab, his short stay in Skill Nursing, and in Assisted Living. The staff doesn't seem to have a lot of turn over. This is very good because the residents receive consistent care. Family members and staff build a relationship to best care for the residents. The employees are compassionate and professional, from the doctors and administrators to housekeeping, wait staff and maintenance. I love that the facility has 5 levels of care: independent living/ retirement, assisted living, rehab, skilled nursing, and a memory care unit. I highly recommend family members check into Signature Pointe on the Lake to see if it will fit their needs. read more

I can't believe the state has not shut this facility down! I will be contacting them! I have had enough!, Very expensive but horrible facility!, Every time I go to visit my Mom I spend the entire time speaking to someone that is " in charge" in office. All they care about is getting NEW victims in that can't defend themselves!! No fresh water is provided daily and for the love of god, you have to at least have water to survive. They do not on a regular basis, either come and pick up my Moms to take her to the dinning room or bring her meals to her room. I was just there a few days ago and they promised to make sure she was fed her lunch...they completely skipped her lunch after I had 3 people including the so called head of facility, that day tell me she would be fed. I was a witness to them only finally bringing her dinner!! My Mom is sharp as a tack, which is unfortunate for the facility, as it is better for them to not have someone that can report and tell how they are mistreated. I spent 2 hours cleaning the filth in her room my last visit! No trash can liners, which is totally unsanitary, her wipes get stolen whether I bring her some or if they charge us and bring them to her room... I have put off this review off way to long to let them have a chance to do the right thing....but it continually gets worse rather than better.... it is also hard to get anyone to answer the phones there... rudness runs rapid at this facility and no one likes to think their Mom is being treated badly ... they have to wear diapers whether they are incontinet or not because when they need assistance to the bathroom so they won't fall, it Can be hours before there is a response after the cord is pulled time and time again! My mom is still there because she fell trying to get t the bathroom on her own! Also they have lost 3 pair of expensive hearing aids and even though we keeping replacing them, they don't bother to replace her batteries we have provided! Please take this review seriously! :( BTW... I work with review companies and the reviews you see with 5 stars are usually where someone from the facility is writing the reviews... DO NOT BE FOOLED PLEASE!! Read the language in the reviews.. Most real reviews don't talk about and list the levels of care such as 5 levels less they are employees from the facility. I mean come on! Please between the lines....you can tell if an employee is writing a review to promote their facility because they know bad reviews are costly. I left a one-star review one day ago. To be fair, would like to say there are some good employees here that really care about the residents. I won't name them because I am sure to leave someone out. I am sure the work for them can be rewarding but also back-breaking at times & emotionally draining. It takes a special type of person to work with these people, many who are very fragile and ill. The workers (aides) probably do not get paid well. I just wish the upper management or owners could put in place some changes to assure better safety and compassionate care. It also is true that many ALs and residential care facilities do get much negative attention which affects morale. All this being said, I wish older people could just stay with their loved ones as long as possible. Our society needs to honor our elders, disabled, broken and ill people in a more caring way. Do not take your loved one to this facility. My mother had to wait 30 minutes to one hour after ringing the call button for a nurse to respond. Many times the nurses were sitting and talking or talking on their phones. When she needed to be cleaned up after a bowel movement in bed, when the nurse finally came, she was told that they could not take care of it at this time. This is unacceptable! We have been able to move my mother to a better rehab facility, where, instead of being in bed most of the time and doing only leg exercises, she is walking-with help, standing up, and doing a variety of leg and arm exercises. She is in good spirits and I am grateful for family members who took her out of Signature Pointe and moved her to a location where the nurses care and are more responsive to her needs. read more

Unfortunately, Signature Pointe was bought in 2016 and many of the best directors left. Although the aides are still good, the management is not. I have seen 5 "new" directors in Memory Care Assisted Living in 2 years, and about the same amount of change in Activity Directors. Chaos seems to reign. Because SP is undergoing renovation, my mother was moved unwillingly to another room. It seems that thoughtful and careful choices for top personal and moves during this renovation are nonexistent. read more

Our family recently had a bad experience with one month of "skilled nursing" at this facility. The place looks and smells good, and we were fooled into placing our frail, very elderly parent there. We now know that there are currently serious management problems at Signature Pointe. No one seems to know what is going on, and it is very hard to get information about your patient. We went to visit our family member one afternoon, and the patient was gone! We found out that he had been sent to the emergency room the night before, and admitted to the hospital. No one ever called to tell us. It was shocking. This is just one example of the lack of follow-through and lack of professionalism that we experienced. OMG! If you love your mother or father do NOT take them here. My mother was transferred there after knee replacement surgery for after care. She was admitted Friday afternoon. She was not fed all weekend unless we brought it to someone's attention. They would look shocked that she had not been fed, then bring her a cold leftover meal. At no time was she given pain medications, ice, nor therapy. When we requested this, we were told that therapy was not done on the weekends. They would only bring her a Tylenol after repeated buzzing. And never received any ice packs. She could not get to the bathroom on several occasions (see no pain meds or icing) and had several accidents. When we complained, they gave her diapers! Monday through Fridays they have a full staff and if you visited then, it looked great. I complained to the nursing administrator and she promised changes. But, then the skeleton crew would come in for the weekends and the nightmare would begin again. One nurse was there three weekends in a row, for two shifts back to back and was the only nurse on the floor overnight. You would think it was over when we broke her out, but no, their tentacles of evil continued. They sent her home with a bagful of meds, some she was already taking, but also blood thinners and blood pressure medicine. She questioned the blood pressure medicine and was told "the doctor ordered it". She dutifully took them. Every 6 hours, as prescribed. When she went to the emergency room, a week later, she was admitted for blood pressure of 65 over 40. Blood was only feeding her major organs. It caused colitis and subsequent dehydration. They told her she should not have been given that nor the blood thinners past two weeks after her surgery. This is criminal and negligent. THEY SHOULD NOT BE IN BUSINESS. Save yourself, and your family grief and choose somewhere, anywhere else. read more

We love Signature Point! My Mom has been there for over a year, and my Dad lived his last year there next to a huge window with tons of birds and squirrels and people all around him!! The place has the best yoga teacher and performers coming every thursday and the staff is like family!! I live near by so I check in a bunch. My Mom is so busy now - I have to make appointments!! LOL My Dad was in two of the floors on the care side - and I was there a bunch with him during that time. It is very challenging and difficult with this stuff .... it can never be perfect - but I assure you - the staff does the best they can .... and dealing with people , that is all I want. My Mom treated like she matters and she has a life!!! read more

Choosing a memory care facility for my father was one of the toughest decisions we ever made. We looked at several places but when we met Adam Koll, Director of Memory Care at Signature Pointe, we knew we had found the right place. Adam and his team provided my father with outstanding care. The staff at Signature Pointe treats the patients with care, respect and dignity! As my father's health declined, we were able to move him to the "Skilled Nursing" section of the memory care floor. This was a blessing because we did not have to move him to another facility and he was still under the care of Adam and his team (be aware that most pure memory care facilities do NOT have skilled nursing services on site). my mom just checked out of the rehab facility and we were very disappointed. The staff always seemed very friendly and were initially very attentive. However, once my mom decided she was going to check out and head home, the difficulties began. On checkout day, we were told the doctor would come that morning to take out the gastric tube. Then promised he would come in at 4pm. At 6pm we finally located him leaving the facility in the parking lot, and he managed to come back in and remove it so she could go home. When we got home, the medicines were a mess and the paperwork was difficult to read. We called and spoke to the nurse at SP, but said she couldn't help us because they get rid of everything on patients when they check out - although it was less than 24 hours after. Medicines should be clearly walked through and explained and there was plenty of time for it. It is something that should be handled carefully and clearly. Hoping now we figured it out well enough to make it through the weekend and going to meet with her personal doctor to make sure we are on the right track with all of these prescriptions we were given..... No help there from Signature Pointe. read more

My mother lived at Signature Pointe On The Lake for many years. She lived in the independent care and then assisted living. When it became apparent that her dementia was progressing and she was no longer making safe choices we had to change her living environment. Not an easy task. She moved to the Memory Care on the second floor. A great decision although it was a change. It is difficult to see the ones you love lose their independence. The staff was kind and caring and respectful of my mother. My husband and I would drop in at all different times of the day and night and I always felt my mother was in good care, she was happy, her room was clean, she was clean and her clothes were clean. Adam Koll is wonderful as the Director of the Memory Care. The staff is caring and loving. If I had to make the same decision today I would not hesitate to choose the Memory Care at Signature Pointe on the Lake. read more
