    My 85 year old mom came to stay with you after falling and breaking her hip. She has…read moreAlzheimer's/dementia. I can guarantee you that if anything were to occur in the future, we will never step foot in your establishment again. Please see the letter I wrote to the head honcho, with absolutely no response whatsoever. This was also just the tip of the iceberg. Throughout this stay, there have been a multitude of issues. To be quite honest, it was very disconcerting. That being said, none of it was towards the actual care team. Everyone that I personally encountered were extremely kind and helpful. This is where we are going to get into the details. Your Director of Case Management, Karen Cook, is absolutely wretched at her job. I'm not going to mince words. I believe I have spoken to her, via phone, twice. Please keep in mind that one of these was during the care conference on September **th, two days after admission. The other was prior to admission. She wrote in an email that she tried to call me a few days later, and that she left a voicemail. Surprising, as I received no missed calls, or a voicemail in the time period that she said she called. Not once did she try to call back. As stated, the only time I received a phone call from this woman was within the first two days of my mom being there. The care conference included her, and at least one therapist. Since then, I have received ZERO updates from any therapist of any kind. Not only that, but some of the emails received from Karen came across as condescending and rude. So, that favor may have been returned. She is absolutely not clear about anything. Today, I picked my mom up to return her to *********, where she resides in the memory care facility. I didn't get a care conference of any kind. Was handed papers to sign by the nurse (who was very nice), and basically shoved out the door. I do have all emails from Karen, if you would like me to forward them. I am beyond appalled. NO updates on therapy!? Nothing?! Phenomenal. Now, I will say my mom is a hard headed broad that thinks she doesn't need sort of medical device to help her move around, when she absolutely does. That being said, I received 5-6 phone calls stating that she had fallen. Oon 3 or 4 of those, she was ok. Another one, a portable xray machine was called in. One was when she was transported to the ER for multiple tests, of which a diagnosis of a hematoma was figured out. On Tuesday, I came down to see mom, and curiosity got the best of me. I asked the nurse to verify how many times mom had fallen. She looked it up, and guess what.........only 2-3 (I cannot accurately remember how many she said, because I was so baffled), were actually documented!!!!!!!!!!!!! WHAT!? So she fell a multitude of times, but only thought to document a couple of them? That is asinine. So, to say the least, I am writing this so you can see what is going on in this business. This is absolutely deplorable. These are family members. These are loved ones. Would you be ok with this happening with a member of your family!? Especially an 85 year old woman with dementia/alzheimers!? Yeah, me either. Thank you for your time.

My doctor, affectionately called "Dr. E." is among a small handful of my very favorite physicians in my 80 years of life. The nurses and CNA's were all kind, helpful and quickly responsive to all of my call lights, requests for help and questions. My physical and occupational therapists were wonderful and enjoyable to work with. Far less important but still greatly appreciated is the physical environment. Bright and cheerful, lots of windows. If I ever need the services of another rehab center in the future I will ask I can go back there. I am sure there is no better facility. I recommend the Center at Northridge in Thornton Colorado to anyone that needs a skilled nursing facility to help them heal.

Brad identified we weren't making progress with my hip and suspected an injury that PT wouldn't help and referred me to an orthopedic specialist. Brad was right. I have a torn labrum (identified on MRI), which is what we had suspected. I truly appreciated his honestly, professionalism and desire to help me get the appropriate care. He didn't have me keep coming just to fill spots in his schedule or collect a co-pay. He had my best interest and recovery in mind. Brad has always been clinically excellent and professional and even though I'm not looking forward to surgery I know I will get excellent PT from him during my post-op and recovery phase. Thank you Brad!

It doesn't work...at all. Stays blinking constantly. Therefore the response time is long or nonexistent. The help said they have requested help three times from maintenance with no results. So my blinking room light gets ignored at the nurse call station. Shouldn't this be illegal in a rehab center? I'm not feeling like I can get help in a timely fashion to my needs.

My one complaint is that they didn't inform me that we could do a home evaluation soon enough to make a difference in my daily therapy. As for the CNAs, the afternoon and evening people were wonderful. On the morning shift, a few were excellent but others were rude that made some days very difficult. One male CNA would turn off the call button and on his way out say, "I'll get to you when I can." I'd turn the call button again after waiting an hour. And he would berate me with snide remarks when he did come in. I requested only female CNAs. This happened on morning shift. One of the first first female CNAs who came in to help me was angry at me for making more work for her. She said the male CNA was lazy. I felt she was quite rough with me. Then the CNAs on the morning shift only wouldn't respond to my call button for 1.5 to 2.5 hours. This would be only 2 times for those hours. When I asked for a supervisor, a nurse came in and told me that there were 3 other women requesting only female CNAs. She then started yelling that none of us were special and what was so important that I only wanted female CNAs. The discussion was out ofl control so I yelled for her to get out. I recommend that the company hire another female CNA for at least the first shift. There were only 2 CNAs for 30 patients requiring several levels of care. I would never recommend this facility to anyone.

But no one ever answered the calls to the billing department. I left messages, used the prompt to call me back and tried staying on the line - no response! Then I started emailing - still no response. Finally, I get a response from the front office - not even the billing department but still without a contact name and they tell me that my account is in collections!?!?

I also experienced shaking and ongoing discomfort that did not improve during my visit. These symptoms were concerning to me and were the reason I sought emergency care. During my visit, labs were drawn, and I was administered pain and nausea medication. However, I did not receive a clear explanation of my test results, what conditions were being ruled out, or why I was being discharged despite ongoing symptoms. I felt that my concerns were minimized and that I was rushed through the discharge process without adequate communication or reassurance. When I attempted to ask questions regarding my lab results and continued pain, the interaction became tense, and I left feeling dismissed rather than informed. I did not feel that my symptoms were fully addressed, nor did I feel confident that I understood my medical condition or next steps for care at the time of discharge. I am not submitting this grievance to assign blame, but to express concern about the lack of communication, explanation, and patient-centered care I experienced while I was in distress. As a patient, I believe it is reasonable to expect clear explanations, respectful communication, and reassurance--especially when presenting with ongoing pain and concerning symptoms. I appreciate the opportunity to have this reviewed and hope this feedback can help improve patient communication and care experiences in the emergency

The Emergency room was clean and not crowded. The staff made me feel very comfortable and was able to take care of me fairly quickly. Overall I was very happy with my visit, not looking to be back lol but I was happy how everything went.
