In October 2025, I lost access to my pain specialist after their office closed following an insurance change. I was instructed by my pain management office to see my PCP to help bridge the gap in my care while I waited to establish with a new pain specialist and to request another referral. I went to John Guerrero, FNP, because I trusted him to help me during this transition. I explained that I was a chronic pain patient and asked about continuing the medication that had been managing my pain while I waited to see another pain specialist. I was advised to discontinue the medication. I was also told that, according to him, opioid therapy in Imperial Valley was generally not continued except for cancer patients or elderly patients. I asked about the referral to a pain specialist because I was still experiencing chronic pain and needed someone to manage my care. I was told that I would be referred to UCSD and that they would contact me within approximately 2-3 weeks. I asked for the referral documentation, but I was told not to worry because they would call me. I subsequently attempted to manage my pain without the medication I had previously been taking. My pain became extremely difficult to control, and I ultimately experienced a medication-related medical emergency involving multiple medications and was evaluated in the Emergency Department. I was subsequently referred to a Suboxone clinic. I explained that I believed there had been a misunderstanding about my situation, because I was seeking appropriate treatment for my chronic pain and had not been seeking treatment for opioid abuse. I returned to Guerrero and explained what had happened. He became involved in my care and prescribed medication that I understood was intended to help me manage withdrawal symptoms. During my final visit, I attempted to advocate for myself and expressed my concerns about the care I had been receiving. I also stated that I felt pressured to take my concerns to the Medical Board because of what I had been experiencing. I was emotional, but I did not raise my voice or threaten the provider or staff. My recollection of that encounter is that Guerrero became upset, raised his voice, threw his hands up, and stated that he would not be threatened with the Medical Board. The office phones were ringing, and I also heard him yell to staff to answer the phones. The encounter ended abruptly, and I left the office upset and in tears. After that visit, I initially contacted the Medical Board regarding my concerns. The Medical Board subsequently informed me that, after reviewing the records sent by the physician, it concluded that no disciplinary action was necessary because it determined that the provider had acted appropriately. At the time of that initial complaint, I had not personally reviewed the records in the way I later did. After obtaining my medical records, I discovered statements that I believe are inaccurate and do not accurately reflect what occurred during my care. For example, I dispute statements portraying me as seeking a particular quantity of opioid medication, requesting amounts above prescribing guidelines, or being verbally aggressive or confrontational with the provider or staff. I also dispute other statements documented in the records regarding my interactions and requests. I have evidence supporting some of my concerns, and I believe some of the documentation does not accurately reflect what actually occurred. Because these additional concerns became known to me after the Medical Board's initial determination, I contacted the Medical Board a second time and submitted another complaint providing information regarding the disputed medical-record documentation and evidence supporting some of my concerns. As of the date of this review, I have not yet received a determination regarding my second complaint. I eventually found a new pain specialist and moved forward with my care. I did not return to Rambel because I wanted to continue an argument with the office. I returned because I discovered that the disputed medical records were being used in connection with my current medical care, and I am concerned about the effect inaccurate documentation could have on my ongoing treatment. I contacted the office on August 3 regarding my medical records and was told that the office manager would call me back. After waiting, I went to the office in person on August 10 to discuss the matter. I was told that the office manager needed to speak with the provider and would contact me with a response. I continued waiting for that response. I am sharing this review based on my personal experience and because I believe patients should know that they can ask questions, advocate for themselves.

I understand that, because many of the results of my injuries cannot be seen with the naked eye, I may appear to look normal. However, internally, I have bones that are misplaced, hardware in my neck, and nerve and muscle damage that causes severe pain. Pain Relief Solutions believed me when I told them how much pain I was experiencing, and that meant more to me than I can explain. At the time of my accident in 2019, I was told that I was a miracle because I shouldn't have survived my injuries. The doctors were unsure whether I would survive, and if I did, they believed I would likely be paralyzed. There were times when the pain was so horrible, even while taking strong prescribed medications, that I felt completely hopeless. As time went on, however, I continued pushing forward, and then I met Thomas Roger, PA. He gave me the encouragement and support I needed to keep moving forward. Today, I am truly grateful for the additional years of life I have been given and for the ability to continue living my life. I have good days and bad days. My baseline pain may range from 2-4/10, but with activities, long hours sitting, or studying, it can increase to 7-8/10 even while taking medication. Throughout my time at this office, the physician assistants were always respectful and extremely friendly. I never felt uncomfortable there. The office was always neat and clean, and the overall environment was calm and welcoming. Thomas Roger has been one of the best healthcare professionals I have ever dealt with. He listened to my concerns, understood my concerns about the risks of being on opioids, and never judged me. He knew that I wanted to eventually reach a point where I could manage my pain on my own because I am someone who does not like depending on anything or anyone. He also understood that I wanted to continue living my life. He knew I was studying in college, was aware of my extracurricular activities and changes in my workload, and knew how much I loved to dance. Because he helped manage my pain appropriately, I was able to continue having a life. I was able to be more present for my child instead of spending so much of my time suffering from pain. Throughout my time with him, he made adjustments whenever necessary, whether that meant increasing or decreasing my medication. I never felt overtreated or undertreated. He always worked to find the balance that was right for me. It was incredibly sad when I learned that insurance changes had caused major changes within the office and that I could no longer receive care there. The transition was extremely difficult and chaotic for me because I had built such a strong level of trust with my care team. Thank you so much, Roger, for everything you have done for me. I truly hope that one day I have the opportunity to see you again and tell you personally just how much you have helped me. You didn't just help manage my pain--you helped me continue living my life, being present for my child, pursuing my education, and doing the things I love. I will always be grateful for that.

Their staff is RUDE, & VERY unprofessional. I was scheduled for an appointment there at 10 am yesterday, November 11, Veterans day. I had called a few weeks back to verify that appointment was scheduled correctly because it was a holiday, a holiday I would have liked to participate in because I too, am a veteran. The entire conversation I got nothing but attitude from the FEMALE I had to deal with. Not only did I learn that I had to call the VA MYSELF (keyword) to RESCHEDULE my appointment, but "she" didn't even give me a number to call, instead she said I could look it up myself. When the conversation ended, I left it alone, & decided I'd just leave the appointment as scheduled. Yesterday, I showed up for my 10 am appointment at 9:45. At 10, they called me to the window & told me that my appointment had been canceled! Apparently, the FEMALE had taken it upon herself to call the VA & CANCELLED the appointment I was trying to RESCHEDULE a few weeks back. Much to my surprise, the FEMALE I had to deal with on the phone spoke up & tried to defend her actions when I asked (very loudly) why they had done that. I then redirected my attention to her & proceed to give her my 2 cents about her attitude on the phone & what she'd done to my appointment. Long story short, I've been living with back pain for over 10 years now. I waited for MONTHS to get some kind to treatment, I FINALLY had an appointment, & they went & cancelled it on me, without my permission, & to top it off, no one ever CALLED to let me know that it had been cancelled, NOT rescheduled! Then when I had asked for their names & lisence number's, they refused to give it to me because I'd told them I was goint to report them for what they had done. I finally had to leave because I was so livid my sentences weren't even making sense anymore, I was shaking & was about to jump through that window to attack that FEMALE for what she had done. I HOPE to GOD that if she happens to read this review, that she count's her blessings that I did choose to walk out instead of being dragged out in handcuffs. I would have given this place no stars but, unfortuniataly that wasn't an option.

Message went unanswered. Sent an email, finally got a call back and referred to yet another doctor who doesn't take my insurance. Called back and left a message that has so far gone unanswered. It's been about 2 months at this point. It's not like I am in pain or anything. Oh.. Ill just take some advil and use lidocane as recommended. Totally needed a doctors advice to take some OTC crap... like I didn't think of that. These people don't give a shit about you or your pain.

The pain would happen all timed of the day and night without warning. It was so bad that it would wake me up in the middle of the night. I was also experiencing headaches that were piercing and no joke!! I was referred to Dr Raiszadeh by Worker's Compensation who related he was the best in the business. I, of course was skeptical since it was Worker's Compensation saying so. Well, let me tell you something, I am 110% with Worker's Compensation!! He performed surgery to fuse C3 to C7. This was major spine reconstruction surgery!! A very complex procedure required extraordinary skill and patience. I was extremely fortunate to have Dr Raiszadeh. He was thorough from start to finish making very clear what needed to be done and he sprang into action knowing the significant risk to my health and safety this injury presented. I can't thank him enough for his challenging surgery. The after care, like anything else he does was phenomenal as well. I highly recommend him to anyone suffering the way I was to go and see him!! You'll find that he really cares about his patients...he'll take expert care of you. By the way, as complicated as my surgery was, I retained full range of motion! Also, the scar on my neck used to access my cervical spine is no longer visible!! What does that tell you!! Surgery happened in March 2018. I wrote this review in September 2018. Don't wait any longer, Call him now!!

I am writing this as a daughter who is watching her mother face the consequences of an incurable cancer diagnosis that was delayed for for a very long time!!! My mother repeatedly sought care from this provider for symptoms that should have raised concern and warranted a more thorough evaluation. Instead, the seriousness of what was happening was missed, and precious time passed. It is incredibly frustrating and heartbreaking to know that while my mother trusted her healthcare team, the signs were there and the opportunity to dig deeper was missed. A patient's symptoms are not an inconvenience. They are not something to be brushed aside or explained away without proper investigation. As healthcare professionals, we are taught to listen to our patients, recognize changes, look for patterns, and advocate when something does not fit. When that does not happen, the consequences are not just numbers in a chart, they are REAL people, with REAL families, and real lives affected. My mother cannot get those months back. I am sharing this because other patients need to understand the importance of being persistent. If you feel something is wrong and you are not being heard, keep pushing. Ask questions. Get another opinion. Your life is worth advocating for. Patients deserve to be taken seriously the first time. Dr. Robles I hope you read this, you know what you did!

Doctor pressures patients into getting things they do not need but wants to profit from the insurance. Dr.Robles followed with a negative remark "What if I don't want to be your doctor anymore?". This type of behavior is unacceptable and can jeopardized patients' health by causing elder people by getting them heated & so upset.
