I figured they see the results of the surgery which put them in the driver's seat. I wound up requiring rotator cuff surgery. Had the surgery on a Wednesday and that Friday I was on the table getting tortured.....not really having been through the process in the past I was familiar with what to expect. The staff was great, from the front desk to the therapists. They did everything with the insurance company and kept the surgeon in the loop. In conversation with other people they were surprised that I started therapy so soon after my surgery. The first couple of visits were a little painful but with the guidance of the staff I was able to push through. Fast forward after my insurance cut me off I was thankful that they supplied me with a print out of the exercises which enabled me to maintain my progress. I was impressed with Profesional that I referred everyone that was looking for a physical therapist

    If you are considering placing a loved one here, please read!…read more My mother was admitted to The Enclave in April 2022 for short-term rehabilitation after a hospital stay. At the time, she was living at home with daily care, fully capable of self-feeding, writing, shifting in bed, managing her wheelchair and even giving herself injections. What happened over the next 11 months at The Enclave was a steady and devastating decline, caused by neglect, poor oversight and what appears to be financial manipulation of Medicare/Medicaid benefits. Staff repeatedly insisted that my mother's decline was due to her "chronic condition" rather than their care. We now know that was not true; since leaving The Enclave, she has regained some strength and improved at another facility. Basic Care Neglected and Alarming Conduct by Staff * Staff ignored obvious daily needs: showering, grooming, nail care, phone accessibility; room was filthy with a sticky floor. Personal items were constantly vanishing. * Medication was mishandled, with refrigerated meds left out and refills left to the family to arrange. * Staff were unfamiliar with her catheter, delaying proper care for months. * Food was unnecessarily pureed into oblivion despite her ability to chew and swallow - they just did not want to be bothered to assist her eating real food. * A staff member referred to my mother as "the devil," hid her personal belongings and used her charger (and actually snapped at me when I moved it during a visit) - complaints to the caseworker about this treatment were met with ambivalence. Medical Negligence * First wound doctor exam came two months after admission. By November, she had multiple stage 4 wounds down to the bone. * Notes repeatedly claimed she used a spirometer (to prevent pneumonia), but she never once received it. Records looked like copy/paste entries from previous entries. * She endured constant infections, dehydration and pneumonia. Staff failed to ensure she ate or drank; food trays were left untouched without help. * In December-January, she became frail, unresponsive, and malnourished, yet there was no plan of care for feeding assistance. I thought she was going to die. Shocking Withholding of Hospital Care * Despite worsening wounds and repeated infections, she was not sent to the hospital until I traveled to NY and insisted they call an ambulance. * Records show a lung mass was first noted in August 2022 but only addressed months later. She was finally transferred to Greenwich Hospital in February 2023, where doctors were horrified at her condition and said she should have been admitted far earlier. Questionable Medicare/Medicaid Billing * On 10/31/22, right after disclosing that her wounds were too severe for her to go home, The Enclave reengaged her Medicare benefit for "urgent care" with no explanation to the family. This conveniently locked in three full months of Medicare billing (Nov-Jan), while they also collected her Medicaid spend-down (~$7,000). * When she needed placement after Greenwich Hospital, almost no rehab centers would accept her because The Enclave had exhausted her Medicare coverage. Final Impression The Enclave did not treat my mother with dignity or compassion. They ignored obvious care needs, allowed her to deteriorate, misrepresented treatments in the records and seemed more focused on extracting Medicare money than sending her to the hospital when she needed it. When I picked up her belongings after less than 24 hours in the hospital, her room was already cleared as if she had never existed. I guess they needed a bed for their next "patient." Just look at the before-and-after pictures of my mother. She entered capable and independent. She left frail, malnourished, with stage 4 wounds and permanent damage after less than 11 months. I also included a photo of ONE of her terrible wounds and her urine bag from the Enclave vs. the Greenwich hospital so you can see how dehydrated she was at the Enclave. This facility should be investigated. Don't be fooled by their lobby fountain, this is a house of horrors.

Since Burke in White Plains didn't have any available appointments, I decided to return to Elite Physical Therapy in Purchase, NY--a clinic I had previously visited in March for a meniscus tear. During that earlier experience, I felt the care lacked follow-through and personal attention. When I returned after surgery, I specifically requested not to be paired with the same therapist, but unfortunately, that request was not honored. From the beginning, I made it clear that I needed a focused, one-on-one recovery experience. Despite expressing that, I encountered several issues that ultimately led me to seek care elsewhere: * Limited engagement during sessions; exercises were shown but not followed up on * No structured weekly progress reviews or therapy plan adjustments * The therapist seemed to be juggling too many clients at once, which left little room for individual attention * I mentioned having a preexisting back issue, but was still placed in a position that led to numbness in my leg * The clinic felt overcrowded and lacked the staffing needed for more attentive care * After 8 weeks of consistent therapy, I saw little to no improvement While the staff were polite and well-intentioned, the overall environment and structure did not meet the level of care I needed for a successful post-surgery recovery. Additionally, the facility itself feels too small for the volume of patients it handles and could benefit from a deeper focus on cleanliness and general upkeep. In the end, I chose to continue my therapy elsewhere--and have seen real progress since making that change. I hope this review is helpful to others considering Elite Physical Therapy in Purchase, especially those in need of individualized, attentive post-operative care.

After one visit with Dr Hefler figured it out. By this point I'd already had 6 MRIs from other doctors meds, injections to no avail. He called me with the news himself not a nurse. I couldn't be more impressed and grateful. I couldn't be more grateful to him after years of debilitating pain someone finally figured it out. He then referred me to Dr Greene for surgery another fantastic experience. All in all I will ONLY ago here for any orthopedic needs and send everyone else here. PT employees are fabulous, nurses and desk staff always made a pleasant experience. Highly recommend.

    Beware Dr. Paul Tomaszewski. He is fake all the way through but plays it close to the vest. Even…read morehis scheduler does not know that when telling a patient he will perform shoulder surgeon, he has no such intention but is too (pathologically) scared to reveal this to the patient and tells his well-meaning scheduler to schedule a high radiation CT scan. The only glimpse of the truth the scheduler gets is when asking the patient if Tomaszewski discussed the results of the test and learns he has not. T. strings his patients along when showing up for supposedly pre-surgical visits, maintaining his friendly facade, one of smiles to conceal ill-intent, not being at all specific about the procedure he supposedly is going to perform. When everything coalesces as irregular and the patient asks this provider if everything is okay, not a word is revealed that no surgery will be done. Nothing is okay when complicated shoulder surgery is involved. He comes up with one delay after another. Patient services tries to help the patient but he fakes out patient services too. When trying to make progress and ask if the provider's PA can be present in the appointment, the disingenuous response is that his PA sees his patients by himself. Patient relations called the PA to arrange an appointment with the PA who admitted that he does not meet with any patients independently. Dr. P.T does not want the PA to know that he never explains anything about the procedure or what to expect. When asking to buy equipment sold in company store, Tomaszewski says only he can bring the patient to the store and he sees patients and there's no assistant to fill in when he knows all along that there is an equipment rep that he is supposed to tell the patient about. The equip rep learned of his story and was perturbed. Tomaszewski has no role. He tells patient relations who the patient needs to contact to confirm the culminating pre-surgical visit will occur, because he won't return a patient's phone call, he tells patient relations to tell the patient they do not have to show up. The patient must to learn how to proceed and finally learn details about surgery. Once there, the provider alleges he did not return the call as he knew the patient would show up. He is evasive about his past experience with the procedure in question. In fact he's evasive about his background, having come from a factory-style NYC hospital to work in a holistic environment. He keeps saying surgery will happen so the patient doesn't make other plans but gets worse during the season-long wait until he sends a surgery order replacing visit notes which states to feel free to call before surgery if any questions and then when calling he admits the claims on the order are wrong and he is not performing surgery. The patient learns NOTHING from him. I invested much time making all of the arrangements for surgery, then having to apologize to all my helpers. The only way to talk to him is in an appt. He says he refuses to look at or respond to portal messages but his scheduler reveals he gives him his portal messages daily and he reads them. He won't respond. The secretary confirms he reads them every morning. Recent development: Someone new at ONS. Her name is Barbara Santoro. She's strictly administrative, also having worked in NYC typical impersonal style environment and unfamiliar with what distinguishes ONS in general, save for Tomaszewski and now her. In contravention of patient relations' genuine role of patient advocacy she believes when a provider engages in misconduct, the victim of it should be banned. She learned patient services is trying to intervene in recognition of a problem with this provider. Santoro, totally out of place at this office, acting to change its nature and undermine just moved this provider to a new location, rather than arrange his discharge. This worsens the problem. Now a new group of patients will be subject to his deceptions which are known and about which he has been spoken to. She discharges the victim who lost months, went along with fakery, showed nothing but respect knowing something is wrong. She ensured that he can victimize more because he brings in money at least for 1 appointment, never mind the right provider is there, and providers at the same place recommended others to me who can help. She canceled my appt with one to ensure I am denied help and the problem provider is protected, meaning patients are at risk. She ensures you're denied care, a 1st for this place, and that a problem provider is retained. Don't seek help when you discover any provider's irregularity here. With Dr. T it won't show on the initial visit. Just don't do a CT scan. You won't have surgery. Just see another. No matter how much harm this provider does, Sanoro's agenda is to protect him and remove the insightful patient. Don't be fooled by her asking you about the problem. It's not to help but undermine you.

I had sciatica that was worsened by a, unknown at the time, thyroid issue. First, I would see Dr. Chimes for half an hour and I swear he would deeply massage a sore spot for 15-20 seconds then take TWO MINUTES of notes after. Maybe this cycle would repeat 3-4 times. Then he'd put electrified acupuncture needles in my back and leave. Numerous times I asked him what he was writing and he said he took detailed notes. It was SO WEIRD. Second, the whole time he would treat you he would complain mercilessly about how hard his life is with his family and working with the NY Yankees. Third, as I wasn't getting any better, during my last two visits he outright yelled at me that there's nothing he could do. During my last visit, he was yelling at me while I was crying and then he put in the needles. He came back 10 minutes later apologizing profusely for yelling and that "I know we can get you better." Well, I went somewhere else after that where they don't yell and I got better. It was a solid 10 out of 10 on the crazy scale.
