This facility doesn't want to care for or help patients that may require extra support and more tailored therapies. The nursing staff acts in a way that is unbecoming of the nursing profession. They have laughed at, made fun of, and rolled their eyes when asked for help with my dad. I came in multiple times to find my dad sitting in his own filth in tears that nobody would help him. Sitting in wheelchairs that were too short so his legs were jammed into his chest, left there unable to move in excruciating pain. Having nurses laugh at patients, drag their feet to offer hands on care, and leaving patients filthy and unable to move directly violates ethics standards set out by the Nursing Practice Act. Professional conduct should uphold patients dignity and show compassion not demean people who have gone through life changing health issues. The therapist for the most part are kind and caring. Though there are a few who have no business working with people in this type of environment (I'm looking at you Jeff). The rehabilitation aspect is the only thing I trust this facility with which is why my dad had a family member there nearly 24:7. My sister stayed over night three nights a week. The one star is for them and nobody else! Case managers are not communicative and unavailable the majority of the time. Perhaps other people have better reviews and experiences because they are not seeing the type of care their loved ones are getting. all I can tell you is be present and do not trust that these people are properly caring for those you love. If your loved one had a stroke and can't speak clearly, walk, or take care of themselves do not leave them alone here. A shiny new facility is a great distraction for the patient neglect that is happening when you aren't looking.

I have never encountered such kindness, care, and attention as we did at SRH. From the moment he was admitted, the entire team made us feel supported and seen. Donna, the case manager, was a gift from heaven--she guided us through every step with so much compassion and clarity. Dr. Yabes and the other doctors were incredible--professional, attentive, and genuinely caring. Maria and all the nurses were beyond amazing--so gentle, patient, and dedicated. They treated my dad like family, and that meant everything during such a difficult time. Rick, the pharmacist, was also wonderful--thorough, caring, and great at explaining everything we needed to know. The Physical Therapists, Occupational Therapists, and Speech Therapists were the absolute best--encouraging, skillful, and deeply invested in helping my dad regain strength and independence. My dad stayed for about two weeks, and from beginning to end, our experience was outstanding. If you are considering this facility for your loved one, know that they will be in the best hands. Rehabilitation services are excellent. The nursing staff don't just do a job, they care about every resident. Sarah the assistant administrator helped me navigate paperwork and help with medical paperwork. She has been amazing and supportive as this is all new. Lets talk about there secret weapon and heavy hitter is Lucy Zepeda. Lucy has helped me navigate medical paperwork, my father's pension paperwork-website issues, medical insurance etc.... Lucy is absolutely a key player in this team. Not only can she handle my direct approach but is able to explain all the paperwork associated with all of the things that happened. Without her I would still be trying to tread water with it all. I can say she has taught me and absolutely been everything I needed. Thank you Lucy. The property is great for sitting outside with my dad.

This is my fifth night at Sherwood Healthcare center. I am receiving very good care and have…read moredeveloped a rapport with many of the RNs, LVNs, CNAs .Sometimes, for me, I'm very pleased with the type of care I receive. Sherwood is a very well established older facility. The rooms are spacious enough, it is clean, and of course the staff work very hard to keep things running smoothly. I am in room 25AB with a roommate. We seem to have the cooler and quieter area of facility.. sometimes I can hear the purr of the trains on the tracks that are nearby. Carol has been with Sherwood for over 30 years. She is amazing at her job, try hard to accommodate patient's needs. The Sien is in this facility are very good about providing fresh ice water in the pitchers for us. I feel it every time you ring the call button for something they try their best to accommodate our needs as soon as possible. They're very quick. The fear of Sherwood is a very professional and will run facility. The doctors and all the staff are very helpful. Nurse address issues and progress, expediently and strive to keep the patients any good rehabilitation environment. The PT department is very much involved in making sure that we have what we need in order to rehabilitate properly. My feelings are very positive about this facility.

There are more CNAs whose names I am forgetting that are excellent & wonderful caring people besides Sheetel & Nicole, but those 2 go above & beyond. I am a vegan & for the most part they were able to provide me with an excellent diet. They did a really good job on tofu preparation. One chef couldn't figure out what to do. Didn't realize that potatoes & rice are not meats, too. There are a couple of instances where I didn't receive pain medications in a timely manner. I had one nurse that I complained about & after that she made a point of waking me up on every shift to ask me if I wanted my pain medication. Which was better because it was almost always time for me to have it. The other instances were almost cruel. And I will tell you that your first night/day will be hellish because it was explained to me the orders for medications aren't received until the next day. I was so miserable & stuck that I wanted to escape and was willing to walk home- even though I wasn't exactly sure where I was in the city-and walk home in my hospital gown & shorts. My friend had taken my house keys & I still would've had to sit outside my apartment all night, but at least I would've been suffering at my home turf. I didn't & the next morning I got relief. For the most part, I recommend staying there for your recovery. They do want you to get better. Just be sure you have someone to advocate for you or you advocate for your loved one as always. One more thing, they never cleaned/changed my sheets while I was there. I didn't know if I had to request that. I had my friend bring my pillow from home & I recommend you bring a blanket, as well, because it's cold in there.

My mother-in-law was admitted here following an ER visit at Kaiser for recovery from a UTI, as she…read morewas experiencing instability with walking. The staff took excellent care of her--carefully monitoring her condition, administering medications, and supporting her recovery. Notably, she did not experience any falls during her stay, which was a significant relief given that she had fallen on her very first day at a previous skilled nursing facility. The reason I am giving four stars instead of five is due to a lack of communication after admission. Neither my husband nor I were contacted regarding her condition, diagnosis, care plan, goals, or discharge planning. It wasn't until I visited the facility in person four days later and requested to speak with someone that we received this information. My mother-in-law did mention that the care environment felt very constant, saying, "They never leave you alone--up at 5:30 for medications, bright lights, no privacy." However, I reassured her that the staff was being attentive and doing their job to support her recovery. By the fourth day, her infection had cleared, and she had regained independence in getting to and from the bathroom without incontinence issues. According to the discharge planner, she was also able to walk 175 feet with a walker under supervision. Special recognition goes to Sondra, the discharge planner, who was incredibly efficient. She coordinated the physician's discharge orders, medication reconciliation, and completed the necessary 602 forms for her return to assisted living--all by 2:55 PM on the same day I met her. We arranged private medical transportation, and my mother-in-law was comfortably settled back into her own bed by 6:15 PM that evening. Overall, we are grateful for the quality of care she received, with the main area for improvement being communication with family members.

I am fed up with doctors that think they can boss their patients around! When he works for Advanced he doesn't represent Sutter. He just happens to work for Sutter and Advanced. The real truth emerges. As a result of the fluid restrictions and simply awful food I lost a significant amount of weight. At Sutter Roseville Medical Center, the 2nd rehab facility I resided in and the assisted living facility I currently live at I was never subjected to fluid restrictions! On 7/10/26 I received an completely unwanted phone call from the administrator of this dump of a facility! That's the reason for the 1-star rating. Case closed forever.

It started off okay, about 2.5 to 3.0 stars and from there it went down hill FAST! I'm a type 1 diabetic and I would have to go without eating anything sometimes because all they would serve me was foods drowning in carbs and sugars. I begged and pleaded for a while then after that I had to start yelling and screaming and be the squeaky wheel to get anything done. Their social workers suck. If you don't keep on them they'll blow you off. As a matter of fact, Social Security told me that they did my forms wrong and I have to start all over again. So now I have to wait 200+days for my SSDI. What I was admitted for which was an infection in my leg was never taken care of and I was discharged in the same condition that I started. Most of the CNAs and Nurses are terrible. They barely speak English and they don't understand you most of the time. One time they pricked my pinky to test my blood sugar and it bled so bad it ran all the way down to my elbow. When I asked the nurse for a band aid she told me that she didn't have one! When I asked how in the HELL does a nurse in a Skilled Nursing Facility doesn't have a band aid she told me that they were locked up and only the MOD had the key and she was not going to be in until 8am. This happened at 6am. When I asked how come there isn't a first aid kit available she shrugged her shoulders and gave me a look of cluelessness. Totally deer in the headlights. They have an Asian resident that goes up and down the hall screaming and does nothing about it so don't plan on any peace and quiet. My roommate fell out of his bed once and was screaming bloody murder begging for help and I had to crawl out of my bed into my wheelchair and go scream at the nursing station to get him help. Their physicians suck. Lazy and dumb. I had to go to the emergency room twice in three months because they are that bad. It's pretty bad when you ride in the ambulance and the EMTs apologize for the shitty help your getting. The Emergency room says they're known as Shitty Creek Post Acute not City Creek Post Acute. I haven't even begun to scratch the surface of the nightmares I have had to endure. Now that I have been discharged 8 days early, they won't take my calls and I need them to transfer my prescriptions to my pharmacy and they said that they would pay the pro rated amount which I'm positive that is why they are dodging my calls. I'm out of certain medicine now and have no way of taking my insulin. I'm going to call my regular physician and see if he can get my prescriptions refilled for me and sent to my pharmacy. So much for getting the appropriate and professional help. The place needs a full audit and CHHSA needs to be notified and also the Ombudsman. This is no laughing matter. Zero out of 5 stars.
