He was sent to Trinity Hill Care Center for rehabilitation. Unfortunately, 2 days before Thanksgiving he was discharged. I say unfortunately because he should not have been discharged. I was told his discharge included a home health aid that would be visiting daily at his residence to assist him with his blood sugar, food etc. (He wasn't at all able to comprehend even how to check his own blood sugar). No home health professional came to my brothers home. Trinity apparently did NOT schedule this as part of his discharge, but it appears as sheduled on his discharge documents which I have. For 16 days he was alone at his residence, with no help. He could not even walk after 4-5 days. How does this happen? My sweet brother was found unconscious on December 12 and passed away on Christmas Eve. Low blood sugar caused irreparable brain damage. They have no idea how long he was lying unconscious before he was found. I spoke to the home health facility listed on the Trinity discharge docs and they have no record of a phone call, intake, referral, they have nothing from Trinity regarding my brother.They said to me they have no record at all of his name anywhere. This vital part of my brother's care apparently was never put in place. It could have saved him. Likely would have. What happened Trinity? Well I asked and: Trinity Admin named George promised to call me back 2 weeks ago,... nothing. Then again another promise to call me back 1 week ago. ....nothing. This is not care. This is apathy. And this is a dangerous facility.

They are too busy chatting it up among themselves or on their phones. My CNA's have NEVER made my bed, I have to practically beg for Ice Water, When I was sick, they ignored the call light for up to an hour. This place is the worst place I have ever seen and unfortunately had the misfortune of being placed here d/t Insurance.

    This is a great place for physical therapy. First and foremost, the physical therapists I've…read moreworked with really know their stuff. I've worked with Stephany mostly, but also had a session with Katie. Their training (MPT, DPT) and experience (24 years for Stephany) enable them to home in surprisingly quickly on causes of pain. My right shoulder has been really hurting after a fall. The usual culprits, muscle trauma and arthritis, are major sources of this pain. But Stephany soon realized that a pinched nerve in my cervical spine is also causing substantial pain. She's given me a menu of stretching and extension exercises that are simple and effective. And after she has worked on--manipulated--my shoulder for a while, I leave PTSMC feeling decidedly better. Katie worked with me one time when Stephany was unavailable. She had thoroughly discussed my problems with Stephany beforehand, and I got the same level of expert therapy guidance from Katie as I had experienced with Stephany. Stephany and Katie are pleasant and patient. Sidney and Sam at the front desk are pleasant and invariably helpful. The studio itself is bright with plenty of outside light streaming in through the windows, well laid out, and clean. After years of physical therapy that was annoying, but necessary, I consider myself very lucky to have come across PTSMC, where I actually look forward to my physical therapy sessions. In an "hour-long" appointment I was lucky to get 2 minutes of face time with the doctor, who didn't seem particularly interested, just wanted to move on to the next patient. To add to the profit-drive factory farm feel, OAH (and OASC, which, somehow, is a completely separate entity from OAH - although of course they don't explain that to you) have extraordinarily opaque billing practices: In the 2 months since my surgery I have received bills for hundreds of dollars from at least 4 separate "companies" and counting. Along with assorted smaller bills. Follow up care has been disappointing - again, essentially zero time with the doctor (Dr. Stephen Davis) and, instead of referring me to PT or giving me exercises to do, Dr. Davis told me to look up exercises on YouTube. Not really the medical expertise I am looking for, or paying for to be honest. The only good thing is that the Orthopedic Associates Surgery Center is a beautiful facility. The OAH offices (Plainville, etc.) are your typical doctors offices, nothing special, if anything a little more worn and dated than I expected. In all, would not recommend unless they are your only option.
