Great experience! So thankful for all the staff there! I was only there overnight, but could not ask for better care!!

SIX HOURS attempting to reach the Center's doctor to check on my 84 year old mother's condition yielded ZERO response. After 5PM NO ONE was answering the phones. I tried all phone options. She is a heart, kidney, lung, and CLL patient and was experiencing painful swelling in her left hand, going up her arm. Finally had to call paramedics. This facility does not take medical conditions seriously! read more

My Dad is 90 and is staying at this facility. It is absolutely gorgeous, so you kind of get caught in the moment. Don't be fooled by aesthetics. It is week 3 for him at the moment. Week one I would have given it 1 star. It starts to grow on you. Medicare has limited choices. You need to be on top of things, or else things can go down quickly. This is true for most of these types of transition facilities. Keep on your toes especially if the patient is elderly. Keep notes and reach out to your case manager to make sure things are taken care of in a timely manner. Make sure you are on top of that. Truth be told in my opinion they need an extra CNA. Each wing has up to 16 patients.1 nurse and 1 CNA for all of these patients is a bit much. I thought they'd disagree, they don't. I asked , they were like "oh this is fine."They are also not on the receiving end. I have timed how long it can take for a response once the call button is pressed. In the day the worst was 11 minutes. In the evening it was 1:30 minutes, that was just once. It was enough for me. Mostly it was close to about 25 minutes. Well, 25 minutes, a person can pee and poop allover themselves. After 5 minutes if there was no response I had to get to the nurses station. I had to make many appearances. It wasn't received well, but when they figured out why I as upset, they got it. What can I say to know me is to love me :-) Changing shifts is also not great. So don't expect an introduction. You have to figure it out once they get into your room. I think this place needs structure, & loose ends need to be tied. That is the most challenging thing here. Annoying...very exhausting. On a good note, the CNA's especially Rosalie , Roberto are great. Most make immediate effort to answer your call. Physical therapy Stacy, & Mark & Roy are awesome. This they have on point. There is an RN on the floor everyday until 6-7pm. Also a weekend RN. An RN is very rare in these types of facilities. Most are LPN's and think they are RN's. I love hearing LPN and RN are the same??? That' s why there are designations. Anyhow there are a handful of good ones; maybe less (3rd floor). LOVE Kelley, my fav one on the floor, Denise is also very good. Marsha is thorough, but may seem not interested. You will find out that she does have the best intentions in mind. Pity I don't have more names. I will evaluate at the end of this stay. 2 years ago, my Dad was sent here by the Mayo Clinic, after a stay with them. He loved it and he was always happy upon my arrival. Recently, he had a medical emergency, that will require more rehabilitation. He asked if we could set him up there again, "Because they have an excellent Rehab Program there." Well, that settled our dilemma, as to where to send him next. Problem solved. Excellent!!! read more

I am recommending The Center of Arrowhead for rehab, My experience there was rewarding and most satisfied due to all the staff. I was treated with respect and compassion. I just had a total right knee replacement and straight from hospital. My rehab that I received was most satisfying, When I left I was way far on my complete recovery due to there expert knowledge. I question the level of integrity of the leadership team. They seem to be VERY judgmental on who they admit, even though their brochure says they don't discriminate. I experienced discrimination first hand. I am sure you will have no problems if you are a senior citizen, but I am in my 30's. read more

Please don't send anyone here. I was a patient for nine days and can honestly say that this was the worst experience of my life. The physical therapy department is good. I also had one caregiver/aide who was totally empathetic and caring. The food was good but the dietary trays were not always correct nor complete. Because I was at the end of a hallway my tray was always last to be served, resulting in lukewarm food. As for the rest; too many errors and more errors. The nursing staff do not talk to each other nor pay attention to anything else going on. I was given medicine no longer prescribed for me. I also supplied my own medicine for part of my stay. I was discharged without any prescriptions for home. I was overmedicated numerous times. One nurse in particular worked at least four double shifts. I asked three times for the soap dispenser at the sink in the bathroom to be refilled after my first two days in the facility and it was finally replaced after I left. The place had an odor, especially at night. The carpet tiles in the hallways are especially filthy and should be replaced in spots. It's too bad that such a nice facility does not live up to its potential. read more

I personally stayed when I had an accident to recover. It was like staying in a luxury hotel. Very beautiful, immaculately clean and the staff is professional yet friendly. Their chefs are good and unlike most facilities this one provides lots of options for you to chose from to suit your palette. Therapy department is all about getting you on your feet again. They are the neatest group of people. I could tell you who I think are the number one nurses and CNA's but that would spoil the fun as they are all amazing people!! read more

We brought my Grandma in last night 1-7-2019 she is 96 and unfortunately towards the end. The staff last night was great with the exception of a couple hiccups. The facility was very nice from what i can tell. She arrived at 6:30 p.m. so we didn't get to walk around and really check it out. I will be posting real updates so people can follow our timeline here and make a decision for your family members. Update: The staff were amazing... They were very attentive to my grandma and family. My only complaint would be that they run this large place on a very small crew. I don't think ANYONE can effectively treat Patients like that. I hope management reads this review and hires more staff to ease your current employees and keep them around as well as care for all the people there in the manner they deserve. I felt bad for the staff running their butts off and we were there around the clock for 4 days... Unfortunately my grandma passed. The employees were all wonderful with the process.. Beautiful facility, as it is brand new. My 90 year old neighbor fell and broke his R leg in 3 places, requiring Orthopedic surgery. This is not your avg 90 yr old, I should begin by telling you this. He is sharper than a tack and seems to bounce back from just about anything. As most older people are set in their ways, I knew from the get go that finding a place he would be 100% happy staying at wasn't going to be easy, if even possible. The first facility he went to in central Phx after being discharged from Arrowhead hospital he lasted only 2 days. He'd been there before, after breaking a hip seven years earlier. The day I picked him up to take him home he cried (literally) because he didn't want to say goodbye to the staff. Evidently a lot can change in seven years. So we moved him to the Center at Arrowhead and crossed our fingers. I first called admissions on a Sunday. I figured I'd leave a voicemail and expect a phone call back the following day, Monday. Much to my surprise I got a live person who was able to answer all my questions and listen to my concerns. Her name was Monica. If it hadn't been for the holdup of paperwork, that there was nobody in on a Sunday at the place we were trying to get him out of, Monica was ready to accept him as a new patient. On a Sunday! This facility is top notch when it comes to the equipment for physical therapy and the facility itself. Each patient gets his/her own private room. If you've ever stayed at an upscale Hilton or Hyatt, this is what I would have to compare it to. He had a LARGE flat screen tv, his own bathroom w huge walk in shower, a nice size closet to hang up his clothes, etc... The meals were prepared by a chef and he actually had menu choices. The few occasions I happened to be there and he had opted to eat in his room rather than go down to eat in dining area where he could socialize with the other patients, the presentation of the food was like what you would find at a 5 star restaurant. I was shocked. The other facility he was never given menu options that I recall. A few times a week the activity coordinator would hold ice cream socials downstairs, which the family/friends were welcome to attend free of charge. On Easter Sunday they had an Easter egg hunt off the patio on the main floor for any children visiting. I also noticed they were dying Easter eggs, which later that evening I noticed a few of them had been dropped off at our room. It was nice. If nothing else, it just added a little color to the room. Each room has a private phone line/phone number. I'm on the road a lot and could never seem to remember the direct phone number to his room, so I ended up calling multiple x daily to the main number/front reception the entire three weeks he was there. Nobody was ever anything but friendly to me each time I called, always a happy voice, eager to "connect me to room 320." Each floor has a "game room" / office also. The room is stocked with board games, magazines, local daily newspaper, books on tape and a puzzle table where there's always one in progress. I used to drop in there and find 3-4 pieces just to break the monotony of the day. It made for a nice break sometimes. There is also a computer with internet access and a printer for use! There is free tea, coffee and hot chocolate as well. My reason for not giving five stars? There seemed to be a bit of confusion or lack of communication between outside physicians and staff within the facility. But then I think you find this sort of thing at most places like this in the medical field especially. I'm not convinced it wasn't actually the Ortho surgeon who dropped the ball and was at fault for the communication issue. Overall, if you are seeking a facility that is easy to come and go as a visitor, has private, spacious patient rooms and is clean and smells nice ...look no further. Your search is over. Mother has been in this facility twice now and has been treated like a queen. They never let her slack. She's there to work. Her job is to get well and regain her independence. My heart was breaking for her at first when I saw how hard they pushed her. But, boy did it ever pay off in the end! CA really knows their stuff! read more

My father had his leg amputated was transported there for therapy several things: no help when needed to go to the bathroom female nurse refused to help response turn the light on male nurse will assist food is horrible patient get a bath twice a week my father smelled my father complained he could not breathe or sleep I complained several times to the nurses on duty their response I will text the Dr. their response your father is having anxiety ( needless to say we had him taken to the hospital by ambulance in ICU with fluid in his lungs) I also forgot to mention $140 from his jacket that money for copays for upcoming dr appointments no where to be found but Dr Shaba is at home trying to decide what he wants to do according to the nurses had to remind the nurses what needed to be put on his dressing (meds) nurses never communicated when I would ask for updates day shift never communicated with evening shift nurses where never on the same page for example one nurse said your father didn't get melatonin last night that's why he didn't sleep well, another nurse will say he did get melatonin so whose telling the truth I remember calling at times when my dad had issues and he wasn't getting any assistant I would call and I would go round and round in the phone queue I would have to hang up and call back a Tom answered who was very rude CNA's volunteers where sent home New Years Eve weekend due to discharge not enough work according to CNA on duty seems to me plenty of work available with the long waits Head Nurse over all the RN'S and CNA's assigned to the patients especially the one particularly to my father I would ask her questions regarding my fathers condition she never had an answer first response my husband and I got I was off for two days ok I will pardon that days later approach her again still no answer instead she would refer me to the RN on duty her title is the Head Nurse you would think she would know more about her patients she's over Its sad that some had to be reminded what is their job duties I had to clean his room on the weekends, change out his pillow cases he would poop his pants at night due to the long waits he fell out of bed due to the long waits trying to get in help to get into his chair and I myself witnessed the long waits I was on vacation the week of Dec 23- Jan 1,2018 and I spent a lot of time there. Facilities like these are so invaluable and so are the employees. I recently broke my ankle at work and needed a rehab to learn how to walk again with a boot. I am 52 so the thought of spending time in rehab was a difficult transition from the hospital. What a relief for me. I was welcomed and treated wonderfully. I ended up needing to stay for a month so it became a second home. Anything new and potentially uncomfortable was met with care, understanding and ease. The nurses, aides and PT/OT staff are the best in their field. They take the time to get to know you so can feel at home. Can't forget the kitchen staff, the food was amazing, a pleasant surprise and such a variety. Honestly, it felt like hotel room service as they cater to what you like. You can tell that everyone enjoys caring for people as it shows in how they do their job day in and day out. Clean, nice atmosphere, quick service. They score a perfect 5, every time you press your help button someone is right there!! Shout out to Rhonda, Melissa, Sophia, Stacy, Neal, Melinda and Trish. Thank you for taking such good care of me while I recouped and preparing me to feel comfortable once back at home. I will refer to all my friends and family if we ever need help again. read more

My mother was a patient at The Center at Arrowhead. During her stay, daytime staff was very good. The facility itself is beautiful and modern looking and the food is good. However, the night time staff leaves much to be desired. My mother needed help in the middle of the night because her blood sugar was going too low, and could not get a nurse to answer the call button even after shouting for help for nearly an hour. She called 911 to get help before a nurse came in. Two days later, when her blood sugar fell low again, she asked the night nurse to check her blood sugar. The nurse refused. They did not chart her low blood sugar. In addition, an LPN on staff incorrectly advised my mother that her short-acting Insulin was long-acting insulin. Finally, my mother spent the entire night unresponsive and disoriented with no one calling me. I found her in the morning essentially unconscious and was advised that she would be transferred to the hospital on a "non-emergent basis." When my mother got to the hospital, we found she had vomit on her that had been there since the day before. Please don't send anyone you love to this facility. They need to make some serious changes. read more

I was at the facility last summer for one month I am a retired nurse I was so impressed with the staff and the CNA's the care was fantastic food was at best marginal but they did try to accommodate me and meet my wishes. I was very appreciative. I would highly recommend this facility to anyone who needs rehab or even hospice care. read more

My father fractured his hip and after a every we chose The Center at Arrowhead because it was new and the close proximity to home. Sure wish I had read some of these Yelp reviews first. My brother had to throw a fit to get them to get Dad dressed each day and out if bed. They told us he'd eat 40-60% of his meal yet a 170 pound man left the center 20 pounds lighter. If you don't stay with your loved one, they tend to get ignored. They left my father in his room in his wheelchair and at some point he scooted himself out of the chair and landed on the floor. We have no idea how long he'd been on the floor. We named one of the nurses Nurse Ratchet because she was such a sour person. We sat and watched cna's go into a clean, empty room and not come out for 15-30 minutes, lights go unanswered. Things improved slightly when we demanded a meeting with the director and head of nursing and the social worker. I would never encourage anyone to take a loved one there. read more

I am suppose to get 720 hours of therapy per week and only got 600 this week. Most of the therapist are very, very good but a few want to deny you therapy; unfortunately they are the ones that seem to be running the organization on the weekends. Also this place takes hospice and critical ill (both mental and physical) patients whom are not even capable of receiving therapy. This ongoing situation puts a burden on those that really need therapy and are denied the time they need. Although their are super good therapist here, it appears they are over run by the negative attitudes of a few that outweigh the benefits of therapy for those that need it. I would not recommend this place for long term therapy read more

Don't let it's newness and beautiful exterior fool you. My mother was admitted to this facility after falling and breaking 3 ribs in January 2018. I live out of state and flew down to visit her over a weekend. I work in a rehab myself so I have an idea of how things should be. There are a number of outstanding issues that I noted right away. -First of all, most of the staff were not friendly to me. My mother was not in her room when I arrived and no one questioned my presence there. I stopped at the nurses station to ask where she might be and didn't find anyone particularly helpful. Staff were hanging out at the station talking to each other and seemed annoyed when I would ask them a question. The girls at the front desk and dining room were nice though, must give credit to them -Staff did not answer call lights. In fact, i saw them walk the other way time and time again. A patient in the room next to my mother was yelling her head off, scaring all of the other residents and no one bothered to address her needs until I went out and found a nurse to help her. - My mother's room was dirty. I never saw any housekeepers around tidying up the place -I walked in one morning and found mom laying across her bed sideways. She had obviously gotten out of her wheelchair (brakes likely not locked) and wanted to lie down. No one seemed to care when I pointed it out the nurses. -The communication from staff is terrible. Living out of state and being mom's POA (she has mild dementia), I rely on phone calls and expected to be updated on what's going on. Lakisha the discharge coordinator called me back only about 5% of the time I asked her to. I even told her in person that I hadn't received calls from her when she said she would. She apologized but didn't improve anything. There didn't seem to be any instructions for discharge. If there was, they didn't share it with me. -A few days before I arrived I scheduled a meeting to happen when I was there. The front desk had arranged it. They had no record of it when I arrived. -There is no one there to do laundry. Good luck figuring that one out if you don't have any one else in town to help -My mother arrived there and had no clothes. They did not bother to tell me this for the first couple of days -The rehab staff were "ok". I am not impressed with the care she received and they did not work on balance for picking up something off the floor. I would think that they should consider that for the elderly fall risk population, no matter what living situation the patient is in. -and after receiving such "great care" (thick with sarcasm there) , my mother fell the very next day and broke her hip. She is obviously going to need rehab again. She will not be returning to Arrowhead. -I would recommend you don't send your loved one there either. read more

My mother was recently here for rehab for an injury to her foot. The goal, so I've been told, was to get her ready to be able to go home and take care of herself. After being there almost 2 weeks, she was no more ready to go home then she was when first admitted. She tried several times to ask for COLD water to drink ( she had been slightly dehydrated), the staff either couldn't be bothered to accommodate her or they forgot, needless to say the water sometimes wasn't even changed in a day's time. This may seem a small issue to some, but this is one of many issues that wasn't addressed or wasn't important enough for someone to tend to. Also, when someone tells you they are having trouble having a BM, and they're very uncomfortable, wouldn't you think this could be investigated? I have been a visitor in other rehab facilities in other parts of this country, and the nurses or staff keep track of their patient's bathroom activities. Another point that should be addressed; if messages or directions are written on "a white board" and can't be seen by the patient from where they sit or from their bed, that patient should not be scolded from someone because they weren't ready to go down to PT. And then this brings me to the "Biggie"- a few nights ago, my mom was supposedly being assisted to get ready for bed, and by more than 2 people, I might add. When a patient expresses to said personal, "there's too many people in here, and too many feet on the floor, I'd feel better if there were less people involved", I would think that would be a clue to lessen the numbers of players. Consequently, my mom fell, along with one of the aids, and low and behold, she broke her other foot (not the one she originally was admitted for). The next maddening thing was, no one was notified of this incident until the next day. The staff told my mom they couldn't do anything about this until at least Monday. An x-ray was supposedly done, that showed a compound fracture, and they let her lay there all weekend. The next upsetting thing-the staff out and out lied about what happened, they tried using the excuse that my mom was confused about things. She wasn't allowed to see the incident report, nor was my brother allowed access when he asked about it. Bottom line....I would never recommend this facility for anyone for rehab. My brother and I are intending to report this to all agencies and resources we can find to let everyone know what kind of care the patients are NOT getting. I don't live near enough, or have vacation time I could use to come there and address these concerns in person, so I figured a review was the next best thing for now, so other people might see this before agreeing for their loved ones to be placed in this facility!! Oh, one more thing...my mom said the food SUCKED!!! read more

Wow just like my experience at the center being dismissed. Just because I am not in your computer system does not mean that I and my mother are not real people with real experiences at your facility. Those are some fancy numbers that don't mean anything. It just tells me that you care more about numbers and being behind a screen playing on yelp than about the people whom you are entrusted to care for. I feel bad for your employees if you blame your lack of leadership and care on a disgruntled employee. I've ran multiple companies and the reason why employees become disgruntled is lack of support and mistreatment from their superior. Next time try practicing some servant leadership and see where that gets you. Also try not to be lock up in some office behind a computer looking in your charts and actually look into the eyes of the people. Oh and by the way the phrase you are looking for is "run of the mill" not "meal". read more

I wish I would've taken the poor ratings and not trust that they've made improvements to their nursing staff and that they've resolved their issues. First of all, I'd like to thank Jeremy, the Young gentleman from admissions (from Iraq), Kimberly, Nichole, Sandy and Dr. Nassaur for being extra nice. But it takes more than just them to run this place... it lacks professionalism, nursing staff (they mainly have LPNs and CNAs, not RNs), and it always seemed like I was arguing with someone for not providing my mom with the care she needed. My mom came to the facility to get stronger so she can continue the rest of her chemo treatments, but she never got a chance... they couldn't manage her pain, she wasn't getting her meds on time (because they're short staffed). They didn't clean the room/bathroom unless we asked; no bedside manners for most of the nurses; they didn't have time to give my mom showers because they were short staffed. They did not go out of their way to care for my mom... until my mom was put on Hospice; very interesting how the same staff can be super attentive once they get paid more by Medicare once your loved one is on Hospice. Makes me sick to my stomach that they tend to your loved one more once they know they'll pass away and they get more money. A medical records staff approached my 17-yrs old daughter and literally crushed her by telling her that her grandma won't make it to her 50th year anniversary... who the F**K is she to come and do that? What happened to HIPPA... we never asked for her input, but yet she came and told my daughter that she reviewed her records and it doesn't look good. Please, I beg you... DO NOT take your loved one there if you truly love them. Don't let the nice facility fool you! My dearest mom passed away in that facility. read more

The CNAs, nurses, and therapists are great at this facility. They are very kind, caring and easy to work with. The place is modern and clean for the most part. HOWEVER, do not expect any help from the administrators. The case managers and main facility staff were awful at following up with patient needs when it came to patient care plans, insurance, coverage, process and procedures, etc. The case managers will rarely return any calls and they make it VERY clear that they are NOT social workers so they are "limited" to what they can assist with. There are no social workers on site, which is very unfortunate for the patients, especially the elderly who are under their care who need guidance. The only one who was very helpful was the kind lady who runs the Medical Records department. Other than that, don't expect to get any sort of assistance from the admin, especially if you're a Medicare patient. read more

I selected the Center at Arrowhead, rehabilitation hospital after my total hip replacement. I am 57. Upon arrival, I was greeted by name by several staff members. I felt, and really appreciated the welcome! I was impressed with the new, bright, clean, and inviting environment. I was thrilled over staying in a private room, with a large window and roomy restroom with private shower. I had several wonderful CNAs, and nurses who did a great job. I also had great experiences with the OT and PT staff. I even participated in the craft making and activities, such as the salsa tasting contest, which helped get your mind off of managing pain and rehabilitation. Meals were also really well prepared. Everyone was friendly, down to earth, and showed compassion. I would recommend Center at Arrowhead, and would stay here again. read more

My mother is currently in the facility and was undergoing physical therapy for a fall. She was cleared by PT on Friday April 6 and due to be discharged. Early in the morning on Saturday April 7th her feet became tangled with staff members feet while being transferred from a bedside commode to her bed. She ended up on the floor along with one of the staff members. She didn't have any pain at the time of the incident, but when she woke up Saturday morning she was having extreme pain and swelling in her left foot. An x-ray was ordered and the result was she has a fracture just above her left ankle. She asked me to get her a copy of the incident report, but the charge nurse refused saying "we can't print them here". I asked if I could look at it in her chart, and again I was refused. However, the charge nurse agreed to read the report to me. Federal law mandates if a patient requests copies of their medical record, the facility must provide it. An incident report is part of the medical record, and by not providing it they are breaking a federal law. I contacted my mother's attorney, and it was suggested I contact the AZ Department of Health and Human Services, the AZ State Medical Board, the state Inspector General, and the Joint Commission. He suggested I file complaints with each of these entities. read more

I just don't know where to begin as I have just too much to say. Our 92 yo mother was admitted here for therapy after having anterior hip replacement. After her driver placed her in her room, she sat in the wheelchair for 1 1/2 hrs before anyone came in the room. That was the first RED flag. Aside from the therapy she received which was good, it was all downhill from day 1. There is a different nurse every day, and there is NO communication between anyone. she would ring her buzzer for help needing a pain pill for example. 45 minutes to 1 hour she had to wait to receive her pill, I kid you not, I was there to observe. The nurse told me she had to finish her rounds before giving pain meds! She could have been having a heart attack and died before any one attended to her. We would order her meals for her and NEVER got what we ordered. We spoke with Director of Nursing about everything, she said she would take care of it. To the day upon discharge, nothing had changed. Aside from needing to hire more staff that is needed desperately, this place is barely a year old and no one knows their left from their right. Mom's surgeon recommended this place and you can bet he will hear about it! I know they were glad to see us go, (not as glad as we were to leave) because to get anything done I was the one who had to chase people down just to get what my mother needed. Her surgeon said she had to wear pressure stockings for 4 weeks/24 hrs a day.
