They hire a lot of contract workers that seem to care not about call lights or patients. Always understaffed. My husband called for Ensure protein drink 7 times in one day because he never received it, even though they said, "I'll be right back." His kidney doctor had recommended it but apparently his opinion didn't matter. I went to King Sooper and bought some and put it in a cooler full of ice and took it to him. When we left to bring him home, an RN hooked him up to the portable oxygen tank...... and never turned the oxygen ON. He rode for half an hour home with zero oxygen. By the time we got home, he nearly collapsed in the garage with his walker, trying to get in the house. It isn't like this was just any old business this is supposed to be a HEALTH CARE facility. If you have a loved one in this facility, visit EVERY day and take and problems they are having seriously. read more

This place hires mainly contract employees who are not intersted in helping the patients or even being there. Very horrible and unattentive. No one seems to know what anyone else is doing. Patients are told that there concerns and questions will be answered but no one ever gets back to them or followed up on. The patients are given their food in their rooms. The dining hall is reserved for patients who need help eating. And the food is horrible. I would recommend anyone looking for a rehab facility to look elsewhere, this was not a good choice. read more

Based on my personal experience, I was very disappointed with the quality of care my mother received at The Center of Lincoln. In my view, the attention and compassion provided were far below what we expected when entrusting them with her care. I personally believe the facility's recordkeeping was not only inconsistent but also failed, in my opinion, to accurately represent what truly happened during her stay. It seemed to me that financial priorities were placed ahead of genuine concern for residents' well-being. When my mother's health began to noticeably decline, our family--not the facility's staff--had to strongly request that she be sent to the hospital. From what I understand, she was later found to be dehydrated and suffering from serious infections that required an extended hospital stay. This is only my personal perspective, but I would not feel comfortable returning my mother to this facility. I would encourage other families to be very cautious and to thoroughly investigate before making a decision about care here. read more

This review is based solely on my experience with them as part of an evaluation and tour process. Great energy. Bright rooms. Large therapy gym. All looks great but was told they had some rain damage from last week and a few rooms were under constructuin, so they were full. Being continuously full to me seems like a good sign. One thing I didn't like was all patient rooms were on the 2nd and 3rd floor and the therapy gym was in the first. Also the dining room(s) were closed so all meals were being served in room only. But other than that, looked clean and would definitely consider here in the future! read more

My dad used these guys 3-4 times. The first few times it was great. It has since gone downhill. The PT employees are FANTASTIC! Thus the 1 star. Our room was filthy. Smelled of urine so bad we had furniture moved out of the room - still was bad. Guests complained about it as well. Everything was dirty. Food was disgusting, poor a best - that is if it even came. TV was broke. I had to fix it myself. I will not even discuss the regular Dr on staff -phones it in. Support staff was snotty and basically nonexistent. Couple of good nurses, but the rest were there wishing they were home. Steer clear! Sad really, because 2 years ago they were a 4-5 star review!! read more

I was discharged after spending 10 days in rehab. Previously, I have been in other facilities over the years. This is the best rehab center I have ever been in. Everything from the CNA's, nurses, PT/OT therapists were wonderful. The food menu was both delicious with a wide variety to please anyone's taste. My husband is there and it's nothing short of elder abuse the way he is being treated. I do not recommend this place. Making him wait up to an hour or more for restroom privileges. It says they have a dining room, not true. They eat in their room and they won't help getting him in a position that works. Was very displeased about the way they treated my dad and his needs. And using the call button to go to the bathroom, they would make him wait for 10 minutes or longer where he would end up. Wetting his pants, they were just very rude. Nurses are rude called and complained to the executive manager, the director of nurses and nothing was done.They just wanted to usher him right out after that wouldn't send the loved ones that you care about there. read more

I don't think there are any 5-star rehab facilities. This was the nicest one my mom has been in, and she'd been in quite a few.

The therapists at this skilled nursing facility are excellent, but the nurses are hit and miss and sometimes very rude as it seems as though they don't want to help! This facility needs to have more empathy towards their patients, whether it's pain management, challenges doing things, or billing issues where the insurance did not pay what the signed contract indicated. In this latter situation, they should not just send a bill asking to pay, even though the contract states that patients are responsible if insurance doesn't pay what is expected. Instead, they should reach out to their patients and give them a heads up to show that they truly care about their patient's well-being. I also feel like the case manager assigned to us did not communicate well - only when he had to. We had to follow up numerous times on things after being released that should have been coordinated better by the case manager. Overall, Center at Lincoln provides the care they are in business do to, but it could be so much better if they just had more empathy. read more

I wish I could give zero stars to this facility. My dad stayed there twice a few years ago and the quality was good now it is excuse my French s***! My mom is there for rehab and she never was never changed they had her pooping in a trashcan her soiled clothes were on the floor in the shower never cleaned or picked up or even bagged her clothes. This not even acceptable for an animal. I discourage a family member having there family staying here. Stay with them if you can or check other facilities. Just because it is in a nice area doesn't mean your family member will be treated well or with respect. So discouraging and sad. read more

Do not send anyone you know or love to this place! I can't believe the hospital has this rehab facility on their recommended places to go. My mom was there for 2 nights and it was the worst 48 hours of her life. She broke her hip and was recovering from surgery. She never saw a doctor, when she needed help to go to the bathroom it would take 40 minutes for someone to show up and the food, I can't even describe what a joke it was. A 4 yr old could make a better grilled cheese, horrible! My sister I "rescued" our mom the early hours of New Years Day 2024. Fortunately, we were able to find a recommended rehab facility a couple days later that was amazing. When we were in the process of getting her into the new rehab facility The Center at Lincoln wouldn't provide her records or the release form she sign, they actually said they had no record of her being there. My former husband had a stroke in November and this was one of the only places accepted by his Kaiser insurance, Senior advantage ( which I highly recommend NOT getting). The staff were not friendly, acted like you were putting them out by asking for anything. I had to ask for a comb and toothbrush and basin , had to refill his water- all basic things that should have been taken care of at admission. ( I was there 3 hrs after he got there). They sent him to the ED 3 times for "aggressive behavior" and the third time refused to take him back. We had to ask about a case conference and the nursing manager said, "oh it was in your paperwork - it was scheduled for yesterday. " Tge case manager was useless and he was not let back on Christmas eve. - and scheduled for discharge on the 27th, Please do not send your loved one here. read more

One star is a compliment. I have never in my 20 plus years in Healthcare seen such a bunch of neglectful individuals in my life. Your LOVED ONES will NOT be safe, don't GO HERE! My mom suffered a STROKE and broke her hip and she is a heart patient. Nursing took it UPON THEMSELVES to override my mothers' Doctors' order for 24 hour OXYGEN. Doctors are never there, so they don't know much tbh... they wanted her heart checked out to see why her blood pressure was up??? FEAR, NEGLECT, and NO OXYGEN will do that to an 85 year old heart patient!! They couldn't even GET HER READY FOR AN APPOINTMENT. In the four weeks my mom was there, she was THOROUGHLY bathed ONCE. NOW, she has the beginnings of a PRESSURE SORE on her tailbone????? Dirt on the floor in my mom's room and filthy tile in her bathroom, never saw housekeeping... EVER. If this is all KAISER PERMANENTE can do for my Mother, WE WILL BE LOOKING FOR NEW INSURANCE!! Pics to come, and probably some litigious looking into as well. The blonde Nurse Angela is very rude and I heard her talking badly about several different patients, and other employees. She has a very poor attitude and can be heard complaining and talking badly about everyone around her. I think she should be fired due to her lack of professionalism and rudeness. This place went to Hell. They used to be great. They repeatedly left my 93 year old Father waiting for help to get to the Bathroom for over40 minutes. No lunch sent with him to Dialysis? Rude techs? Cold food? You ring the bell repeatedly after 8 PM, and no one answers to let you in? I waited a half hour till someone came out. Done. Busting him outta here in the morning. If I could give negative stars, I would. My sister was there for one week. She was treated horribly. She kept telling them she was not feeling well and was weak. They told me she was just depressed and worried. They got mad because she had diarrhea. They told her they "could not find" the Imodium and did not give it to her for in a timely manner. To top it off, her stools were dark and tarry. Even I know that indicates a GI bleed. They still waited 3 days before the doctor did something and finally had her transferred to the closest hospital emergency room. The ICU doctor from the hospital contacted me to say he did not expect her to make it through the night. Her blood count was extremely low (due to an internal bleed). She was in kidney failure. Her blood pressure was extremely low, and a multitude of other things. If she was not one of the toughest people I know, she would not have made it. The hospital saved her life. If the staff at The Center at Lincoln had waited much longer, she would have died. Not only will I not recommend this place to anyone, I would recommend changes be made from the top down; a clean sweep, or shut it down. People's lives are in danger when obvious signs of a GI bleed are not addressed immediately. What else are they missing or ignoring in the many other patient's in their care? It is frightening to consider. Steer clear of The Center at Lincoln. Do your homework and go elsewhere. I almost lost my sister, because I trusted them. read more

Even his case manager was very rude and had an attitude with my husband. The cna on job for 4 days didn't help. Dez the other nurse told my father in law don't shit in the diaper. Well listen if you and your nurses won't help then what else can he do. Absolutely horrible facility I don't recommend this place or would I recommend it. I was on the phone on set real occasions my father in law called the nurses and no one came. They gave him old food we asked Hem to throw away days earlier they don't care For the patients. I wish I could give a ZERO!!! Completely rude nurses, food was terrible. My mother would be left soaked in urine because they would NEVER check to see if she needed to get to the bathroom, she ended up with sores and a rash from being left for so long, even after we complained several times! The physical therapy times would be written on her board in her room, and I would be there during those times, and they would never come to do her therapy. All the nurses do is play on the computer or their phones. I would spend the entire day there and NEVER see anyone come to check on her. If they came to bring her food, I would ask for them to change her, and every time was told they weren't the main nurse. When I turned in the grievance form they gave me, the nurse said they would forward it, but never got a follow up. I'm sure it went straight in the trash. I could not wait to get her out of there. read more

After surgery at St Joe's Hospital, they sent me to Lincoln for rehabilitation. I did not want to go, nor did I think I needed to. The rehabilitation they sent me for was unrelated to the reason I was in the hospital. I have a pre-existing coordination problem and; though I tried to tell them that I have had to deal with it for years, it made them "uncomfortable" and they did not listen to me. As this all was in the middle of the COVID-19 outbreak and quarantine, I was unsure where else I could stay and I ended up being there for a number of weeks. So, They don't listen. Some of the staff are rude and obviously don't want to be there. read more

I would just like to express my gratitude for everything that the staff at "the Center at Lincoln" did for me during my 30 day stay at their facility. The nursing staff was professional, friendly and did their best to accommodate my needs while recovering from a broken femur. The inability to get out of bed made me totally dependent on their assistance to do even basic tasks such as daily hygiene and bathing. Everyone that came through my door had a smile and willingness to make me as comfortable as possible. Being able to maintain a positive attitude during that length of a stay can be a challenge but they did their best to help me keep my spirits up. I would also like to thank the physical therapy staff for helping me get back on my feet. Everyone that I worked with understood what was required to help me regain the use of my leg. Since I also have ALS it was critical that they also understand the limitations it presented to not over stress my muscles. Their encouragement and positive approach helped to make my recovery as rapid and enjoyable as possible. The facility is lovely and easy to navigate. There are many areas indoors and out that allow patients to visit with family and friends outside of the confines of their room. Activities are planned nearly daily and games, books and puzzles are available for use. My only complaint would be the food service. The menu was very diverse but I found most of the meals to be dry and lacking in flavor. Understand that my wife is an excellent cook and has probably spoiled me when it comes to my diet. I would certainly recommend "the Center at Lincoln" to anyone requiring a rehabilitation facility. read more

We are getting very poor communication about our family member at this facility. They don't return our calls or requests from the doctor's office to return calls. Staff requested we approve an ambulance when O2 level was low and finger was cold. Good medical practice dictates use of an alternative digit. Paramedics arrived and found absolutely no reason to transport. We have been unable to visit after the two week quarantine as we were not told we needed an appointment to do so. When we asked to make an appointment we were told person we needed to speak to "was busy" and it's been 7 hours and she has not called us back so we can make the appointment to visit. We are very concerned about the quality of care at the facility. read more

If you can prevent any loved ones from visiting please do! The nursing staff has constant verbal altercations at night keeping patients awake, limited PT and OT sessions even though that's their purpose AND they administer the COVID vaccine patients (1st dose) then won't allow patients who have been discharged to return for the required necessary 2nd dose. After hours on the phone for clarification between primary care, Walgreen's rep and Center at Lincoln, my mom can not receive the 2nd dose until offered to the general public, which could be months, even though they gave her the 1st dose. COMPLETELY IRREESPONSIBLE!!! A handful of competent kind staff but the majority shouldn't be dealing with the elderly. Sandra, Director of Nurses is either incompetent or just doesn't care how her staff performs their duties and Lucas Carroll, Executive Director is obviously completely out of touch with what is happening at this facility. Very sad for the amount of money they charge insurance per day. read more

My father was in for rehab for 21 days and he called every day begging us to come and get him out. He complained that the nurses didn't answer the call light. The nurses insisted on a sitter because he would try and get up on his own when they didn't answer. We were told he didn't have any regular clothes despite the fact he had them on when he was admitted. After my sister spoke heatedly to the nurse, they magically were able to find his clothes as well as his shoes, watch and shaver. They are not trained or equipped to handle elderly patients with cognitive issues. In fact, my father was worse cognitively here than in the hospital. Whatever gains he made physically, he regressed mentally. read more

Wonderful facility with wonderful staff. My stay was a daily pleasure; service was always with a smile. I cannot think of a better place in which to rehabilitate. The PT and OT department offered compassion and professional treatment at all times. The CNA staff was efficient and helpful, as were all of the nurses and LPN staff. I can't think of enough superlatives to praise sufficiently. I really don't want to ever have to come back but if life's events dictate that I must, I will know that I can look forward to the best possible experience. read more

Please, please, don't EVER bring your loved ones to this facility. My young (very independent) mother was discharged from the hospital and sent to this facility for a week while she waited for shoulder surgery. Upon arrival we were met with a meal and an absolutely wonderful nurse, but that would be the last positive moment for 4 days. Later that night they were SIX hours late getting her pain medication and when they finally gave it to her, they had increased her dose so significantly that she began hallucinating and was convinced she was having a heart attack. They offered to transport her to the ER, but said WE had to call 911. Once the paramedics arrived, they said she was not a medical emergency and was therefore not allowed back at the hospital AND we had to leave The Center. I had to BEG them to us take back the decision and allow her to stay. We were promised PT and OT twice a day in their training room, but we only saw one per day for about 5 minutes at a time in her room. She never left her room once!! The nurses refused to help her shower (going on day 5 after her accident), which led her to develop a bacterial infection all over her arms, tummy and legs. When we asked them to call a doctor to look at the infection, they said they couldn't reach them and didn't want to call again. She was neglected for so long she developed a blood clot in her leg. When she asked for a doctor, one of the assistants came back with LOTION and offered to give her a foot massage instead!! WHAT?!? None of their equipment ever worked and because of improper readings from the blood pressure machine they wanted to give her MORE meds!! After 4 days of abuse from the nurses, constantly fighting to keep her on a regular pain regimen and watching her infection and leg grow worse and worse, I finally called her surgeon and begged him to admit us back to the hospital. There were so many complications that could have been easily avoided had we never admitted her to this facility. Please keep in mind that these are only SOME of the horrible things that happened during my mom's stay. We are so horribly traumatized from this facility. read more

My mother-in-law was here for 2 months after a major surgery that diminished her vision and the RN's, CNA's and Rehab staff were all amazing. They accommodated her and even moved her room to include more counter space In the restroom so she could be more independent. The year that we came and the following year, this facility had great reviews from the state survey. All of the rehab staff were amazing and I felt the care and compassion she received was exceptional. My husband was admitted to this facility late on a Saturday afternoon following successful hip replacement surgery. The PT and OT at the hospital recommended that since he had a more complicated healing and recovery that it would benefit his recovery to go to a rehab place for more help so he could be stronger, heal and be ready to go home. We were told where his room was and not given any instructions or what to expect for him as he recovered. Staff seemed not qualified, many spoke broken English and then he was almost dropped while being moved from a wheelchair to the bed which really scared us. They never came with any medications for him and after he was told to order his night and next day meals, the meal arrived incomplete. I had to go to the desk on the floor to ask if someone would be coming to check on us and let us know what would be happening, I was told that they were understaffed on the weekends and two others came in at the same time and we would have to wait as they were completing paperwork. Staff seemed not caring or concerned and we wonder why they even agreed to take him if they were short staffed and not ready. Finally a head nurse came by after 3 hours and apologized and we told her we did not feel safe there and we wanted to go. We asked if she could help us find a company to pick him up and she said she was going off shift and the next nurse would help us. She never came. We found out that PT would not begin to Wednesday and would have to be there for 4 days before any one would come and tell us about the program. After trying several companies that transport patients, dialing 911 and also the fire department we were not able to get a ride home. We learned that the local policies of emergency departments can only transport patients back to the hospital or to another care facility not to your home. We could not get back into the hospital since he had been discharged and we did NOT want to take a chance on another facility. We were blessed by our church family who purchased a wheelchair for him and along with some friends helped us get him out of there and brought him home to recover! We did not get any of his papers as we left with meds that he should take since no one was at the desk and did not seem to care if we left. I`m adding to the large number of negative reviews. Communication was almost nonexistant. Signed papers laid around my room until I was dismissed 10 days after signing. Case Manager never came to pick them up. I laid in bed one Sunday morning until 11:45 because no one came to help me get on 2 boots and to the bathroom. The food caused me to lose some weight because it was mostly inedible. One morning I was rushed to a dr appointment which I did not require. My word was not respected. Arriving at the dr`s office I was told the appointment had been rescheduled by someone at Center at Lincoln. However no date had been set. The nurse "in charge" was on vacation in Mexico. Following my melt down the director of nursing and the Nurse Practicioner visited me. I complimented the low level CNAs and accepted that they were overworked. The DoNursing assured me they were fully staffed! Inquiring of the aids assured me they were very understaffed. This is not a good place for a person recovering from illness! I give it 0! read more

I want to let folks know about my experience at the Center at Lincoln, the nurses and CNA's were always friendly and very caring, Mia, Tracy, Amber and all the rest that took care of me. When it comes to the PT's I want to thank Brooke who made me feel comfortable during my rehab and always made me laugh, also would like to thank Shelby who always ask how I was doing with rehab. Now with OT I want to thank Chris, Kelly and Ann Marie who worked with my shoulder 3 times a day to make sure it would not lock up on me. They came to my house to make sure I would be safe when I came home. We are extremely pleased with how professional and caring my mother's nurse, Amanda is. She puts her heart in her work and is very experienced in assisting patients patiently and lovingly. When my mom had her stroke she acted immediately and within minutes had the paramedics take my mother to the emergency room.
