I couldn''t be more pleased with the excellent care I received while here for almost three weeks in December 2025. My doctor, affectionately called "Dr. E." is among a small handful of my very favorite physicians in my 80 years of life. The nurses and CNA's were all kind, helpful and quickly responsive to all of my call lights, requests for help and questions. My physical and occupational therapists were wonderful and enjoyable to work with. Far less important but still greatly appreciated is the physical environment. Bright and cheerful, lots of windows. If I ever need the services of another rehab center in the future I will ask I can go back there. I am sure there is no better facility. I recommend the Center at Northridge in Thornton Colorado to anyone that needs a skilled nursing facility to help them heal. read more

My dad has been for almost a month and now he has a bedsore. As far as his therapy, he can't do the therapy while he's in rehab because he needs a steroid shot from his primary care doctor. He can't get a steroid shot from his primary care doctor because he's in rehab. All they did was put the cart before the horse and now his discharge date is here and he's no better off than when started. read more

My 85 year old mom came to stay with you after falling and breaking her hip. She has Alzheimer's/dementia. I can guarantee you that if anything were to occur in the future, we will never step foot in your establishment again. Please see the letter I wrote to the head honcho, with absolutely no response whatsoever. This was also just the tip of the iceberg. Throughout this stay, there have been a multitude of issues. To be quite honest, it was very disconcerting. That being said, none of it was towards the actual care team. Everyone that I personally encountered were extremely kind and helpful. This is where we are going to get into the details. Your Director of Case Management, Karen Cook, is absolutely wretched at her job. I'm not going to mince words. I believe I have spoken to her, via phone, twice. Please keep in mind that one of these was during the care conference on September **th, two days after admission. The other was prior to admission. She wrote in an email that she tried to call me a few days later, and that she left a voicemail. Surprising, as I received no missed calls, or a voicemail in the time period that she said she called. Not once did she try to call back. As stated, the only time I received a phone call from this woman was within the first two days of my mom being there. The care conference included her, and at least one therapist. Since then, I have received ZERO updates from any therapist of any kind. Not only that, but some of the emails received from Karen came across as condescending and rude. So, that favor may have been returned. She is absolutely not clear about anything. Today, I picked my mom up to return her to *********, where she resides in the memory care facility. I didn't get a care conference of any kind. Was handed papers to sign by the nurse (who was very nice), and basically shoved out the door. I do have all emails from Karen, if you would like me to forward them. I am beyond appalled. NO updates on therapy!? Nothing?! Phenomenal. Now, I will say my mom is a hard headed broad that thinks she doesn't need sort of medical device to help her move around, when she absolutely does. That being said, I received 5-6 phone calls stating that she had fallen. Oon 3 or 4 of those, she was ok. Another one, a portable xray machine was called in. One was when she was transported to the ER for multiple tests, of which a diagnosis of a hematoma was figured out. On Tuesday, I came down to see mom, and curiosity got the best of me. I asked the nurse to verify how many times mom had fallen. She looked it up, and guess what.........only 2-3 (I cannot accurately remember how many she said, because I was so baffled), were actually documented!!!!!!!!!!!!! WHAT!? So she fell a multitude of times, but only thought to document a couple of them? That is asinine. So, to say the least, I am writing this so you can see what is going on in this business. This is absolutely deplorable. These are family members. These are loved ones. Would you be ok with this happening with a member of your family!? Especially an 85 year old woman with dementia/alzheimers!? Yeah, me either. Thank you for your time. read more

My father was sent to The Center of Northridge on Tuesday March 31st. When I showed up on Tuesday evening to see my father. I found him sitting in a chair in soiled clothes. When I went to find someone to help me. I was told they did not know he was even in the room. The staff said he had not been evaluated yet. I was told I could not stay because it was 8PM. When my mother went to the center on Wednesday morning to visit him, he was not there. The center had sent him back to the hospital during the night and NEVER notified anyone within the family. The hospital sent him back to the center on Friday April 3rd. Then again on Saturday April 4th in the middle of the night, the center transferred my father back to the hospital. Once again, no notification was provided to anyone within the family. We found out he was moved when we showed up on Easter morning and his room was empty. His personal belongings were still in the room unsecured. How can The Center of Northridge say they care about their patients when they do not care about the families. A simple phone or even a text message. I have reached to management about this situation and they are ignoring my request for additional information. read more

AWFUL care!!! Dad entered this facility able to walk the length of a hospital corridor. He got one hour of PT/week and the remainder he laid/sat hunched over in a bed with no rails and was scared to death he'd fall. After 5 days here, he couldn't stand on his own, lost 12 lbs., had greenish-grey skin tone and appeared drugged as he couldn't maintain a conversation. Was sent to the ER with a fever and infection, but hospital staff found neither. They were going to discharge him back to this hell-hole and I loaded him in the van, took him home and rehabbed him myself with no medical background whatsoever. I honestly thought, in another day, he'd be dead if I hadn't. If this is a 5-star facility, heaven help the people who get sent to the 1-4 star facilities. Unable to reach anyone by phone at the facility just auto-transferred to non-working extensions. Long, long, waits for help to the bathroom. Rude and dismissive staff. Truly unbelievable lack of care and concern. Director of the facility offers fantastic lip-service and nothing more - oh, sorry, he did offer that the last person they had there with my dad's condition ended up dying. read more

I have used other rehab centers and center at Northridge is head and shoulders above the others. Equipment and facilities are great, but the real difference is the staff. Not just therapist, but everyone. Friendly, happy and you feel they really have your best in mind. After 1 week, much further along than I thought I would be. read more

Given that most of the residents here are facing life or death recovery situations, experiences will vary. My elderly parent came here after injuries sustained in a fall. Recovery was successful. This isn't a place where one is coddled. Care is given, terms are described, it's very transparent. Residents here will face a Turing point here - they will progress in recovery and become well enough to continue recovering at home, or they will decline and move on to a long term facility to face the inevitable. The staff keep the residents clean and comfortable and free of all distractions, and they fed a steady diet of physical therapy. The turning point is clear, and all residents gone of these two ways. read more

Beautiful facility. Understaffed on weekends and holidays. Bare minimum care by floor nurses and CNA's especially on weekends and during holidays. They seemed to resent their jobs and were mostly unfriendly and seemed bothered by requests. Not a lot of dignity towards patients trying to recover from traumatic injuries . One of the most expensive Rehab Facilities. Each day costs the same whether it's a weekend or a holiday or week day and yet the care is lacking significantly on the weekends and holidays. Very Good and friendly therapists, just not enough therapy sessions. I would advise looking for a less fancy rehab center with friendly and interested staff. read more

Their care is the same level of mediocrity as many other rehab facilities and possibly some 7-Elevens. Caseworkers (in our case "Brandi") do not return phone calls or follow up IN ANY WAY. They'll even call with messages like "if you want to be involved in this conference call about care for this person, call us back" but then you can never reach anyone so you leave a message and guess what, don't reach out with the conference call information. On the other hand, the billing department is VERY engaged and will call you literally every day without fail when they need something. Clearly they need THESE PEOPLE to be the case workers. They don't provide ANY helpful information like what to bring for patients while they are there and when you make your best guess and try to drop stuff off, the woman at the front desk rolls her eyes and begrudgingly takes the bag of clothes from you (you can't visit and drop them off yourselves due to covid) and then WALKS AWAY while you're trying to provide additional information. UPDATE: boy what a load of fun. After leaving a voice mail for Bradi's supervisor Troy (who never calls back either) they sent my mother home with a wheelchair rx (after they told me I'd be on my own to arrange for one AND without her personal items (glasses, deodorant, shampoo, etc). And do you think anything was communicated? Of course not! Run, do not walk, from this place with your loved one (or yourself) if you find yourself here. read more

My surgeon's office made arrangements for me to stay 2 weeks after having back surgery where L4 & L5 were fused. I was to be able to walk one mile at the end of the period and take care of myself when I got home, even though I wouldn't be able to drive. After staying 5 days during which I did a few PT exercises, I was thrown out. The "justification" was that I had a score of 90% on a few PT exercises they'd given me vs a maximum of 77% Medicare allowed for me to stay. I never came anywhere near walking one mile - only going around the block, using a walker. Tell me: How the h--- can someone score 90% on PT 5 days after major surgery? The doctor, who made the determination, didn't even do me the courtesy of speaking to me. Needless to say, the appeals process was a total farce. My siblings, with their own physical disabilities, living in Texas and at high risk for Covid 19, were not able to travel here to take care of me. I wouldn't recommend this place; a more fragile person could have a major setback or early death. read more

My Father unfortunately had to undergo back surgery and his post care included acute Physical Therapy (PT). So as this place is conveniently located on Pecos St . about a mile North of 120th Ave ot is easy t find ws ot is a stand alone bldg. This place is a short term facility and is there to help the patients with thier after care in order to get them back on thier feet and home. The facility is nice inside and contains a gym and also a salon. The guests are also allowed to go outside and leave the facility upon giving the staff notice. During my Fathers stay he recieved speech therapy, physical therapy, and occupational therapy. The staff was pretty accommodating and asked my Father when and what times he would like the therapy sessions to take place. I was present when his meals arrived a few times and the meals looked pretty decent and were served 3x per day. The staff from what I saw were for the most gentle and seemed to enjoy thier job. I would recommend this place based on the treatment my Father recieved and the things I saw. read more

My husband was at center of Northridge for over a month He was well taken care of. The doctor- Dr Macharia- was beyond kind and caring and knowledgeable. She treated my husband, also a physician, with respect and dignity even in his confused state. Nurses and aids were very attentive and made sure he got his various meds. The vegetarian diet food he needed was good. The place is clean and well taken care of. PT and OT are very professional and caring, just wish there was more time with them. Would recommend Northridge for rehab. Very nice place read more

My Dad recently had a stay at Center at Northridge. I was fortunate enough to stay with him as I live out of state. I found everyone there enormous kind, helpful, courteous, compassionate, and answered all our questions. They took such great care of my Dad. He had pysical Therepy, they gave him all his pills on time , they took him by van to an outside dr. Appt., they helped him shower , they made sure WE had bath towels, they brought him hot meals, , they answered over and over a failing old man memory question without batting an eye. Even I began to question myself wondering how I was going to care for him once he was back at his home before I had to go back to my own but their example made me want to be better. All of their team work was AWESOME I can't thank them enough for getting him on a great start to being back to himself. I'm very happy that he was helped by CENTER AT NORTHRIDGE read more

I have been a patient here for 5 weeks for complications post surgery. I came here especially for physical therapy and speech swallowing therapy. They have excellent therapists in occupation therapy, physical therapy and speech therapy. The nurses and team are caring and kind. The rooms are big and nice and once I was able to eat again I was able to enjoy the tasty food from the kitchen. Sometimes they is a delay in answering the call button but that is because they are spending time with another patient that needs them. I am glad I choose to come here. read more

Very good service. The people are nice, the food is decent & I get a little freedom. Only thing I will diss them on is not enough therapy. I should be in the gym daily getting some workout in. Knowing they are busy, 7 day per week therapy/gym time would be beneficial. read more

We were so thankful to have my mother in law transferred here recently after a very bad experience with a different rehab facility in the area. Staff was very nice and she was placed on a set therapy schedule each day, which is what was needed to start healing. She is diabetic, managed by diet, and the menu offered many items (ordered in advance) that she was able to eat and enjoy. They were adequately staffed and there were not long delays in waiting for pain medications or needs to be met. She thrived once we moved her to this facility... it was like night and day from the previous place. She was able to return home after 1 1/2 weeks. It is also a beautiful facility and her room was immaculately cleaned each day. Again, we were so thankful to have our loved one moved to this facility... it made all the difference in the world. read more

I toured and met with staff at this gorgeous new facility, that you will feel really proud of yourself for dropping your family member off at. That being said, as a healthcare provider, I'm quite concerned at the low ratio of registered nurses (RN) to patients. I was told by the admissions representative there are 1-2 RNs (a charge nurse, and a nurse manager) for 48 patients, plus 8-10 certified nurse aides (CNAs). While that may be enough physical bodies for that many patients, I would not feel comfortable with my loved one being supervised there. I can't imagine how the nurse(s) can deliver and monitor medications on time and and in a safe manner with that many patients under their care. Other areas of concern was no onsite laundry, so family has to pick up and return clean clothes as often as their family member might need. Also they use a contracted transportation service (Limo Care) for getting patients to and from appointments. For such a large facility, they should absolutely have their own staff taking patients to and from important doctors appointments, not some limo service, which I'm guessing gets passed on to the consumer in their daily rates. read more

I just recently spent 5 1/2 weeks at The Center at Northridge under going antibiotic IV therapy after surgery for a serious bone infection. I had wonderful care while I was there by the nursing and CNA staff . I was on the third floor in wing 3A. My therapists were the best. I had therapy twice a day most days. My case manager and other support staff were easy to reach and talk to. The meals were great and they even have an alternative menu you can order off of if you simply want something different. They transported me by van to my many follow-up doctor appointments and the drivers were friendly and very careful. The whole facility is clean and the lobby looks like a fancy hotel. The rooms with attached bathroom were large and each had a window looking out on the city. I am very glad my family and I chose this center and when I have surgery again in 4-5 months I hope they have a room for me. read more

My 58-year-old wife survived five years with grade 4 brain cancer, a cancer that was in remission when she was admitted to the Center at Northridge. In less than two weeks she was dead. The standard of care here is appalling. Yes, there were exceptions but not many. The CNAs were slow and surly when she asked to be taken to the bathroom. They told her to wait or pee in the bed. She fell twice trying to get to the bathroom to avoid an accident. She called me at 3 a.m. crying because a CNA told her she didn't have time to take her to the toilet. I complained to the executive director and nursing supervisor. We all met and they agreed that someone would check on her every two hours. The exec. director gave me his cell phone number and said call anytime and he would respond. So they didn't check her every two hours - especially the abysmal night shift - and I called the guy. He never responded. I just happened to run into him at the facility and told him they weren't checking on her. He vowed to fix. Again, he didn't. One night while sitting with her she began coughing up blood. It became a torrent. The nurse and I put her on the floor and they called 911 - shouldn't a place like this have a doctor around or a code blue function? The paramedics tried to revive her for 45 minutes. They asked why her belly was distended. No one had noticed. She never regained consciousness. A nurse told me I was responsible for removing the body. It was 10 p.m. I had no idea what that even meant so I called funeral homes until I found someone who would pick up my wife of 37 years. She was laying inside on the ground, blood was all over the floor and the nurses went about their business without so much as a single word of sympathy. The next day the funeral director who removed her body said the facility was responsible to keeping the body for 24 hours and I should not have been put in the position of having her moved. I never heard from anyone again from this facility. Her death was caused by a GI bleed, a doctor who did not see her declared. She never had any issues in that area. Did it develop there? Was it a result of falling? Stress? Did it develop and nobody noticed because the care was so slipshod? Quite possibly. I went to this place because Kaiser, my insurer, sent us there. I am now haunted by the thought that my wife could be alive today if she had gone somewhere else. I will never get the image of her bleeding out of my head and I will never stop thinking that if the staff had been more caring and attentive they could have caught what killed her before it had a chance. Don't send anyone you love here. Its a nightmare and at the very least ruined the last days of my wife's life. I'll never forgive myself for not getting her out of there sooner. read more

Do Not send anyone here, there are a couple of people that work there who care, the other's don't my mom has been there for almost 90 days and i'm trying to move her ASAP. The staff will ignore the call lights and will take their time getting into the rooms that need help. they are supposed to have someone there until 800 pm answering the phone, but often times, that's not the case. no one will answer and when you call the executive director he will call and nothing changes. Very poor service for the money. Think twice before sending your loved one here. read more

So thankful for the Center at Northridge. We started at a different facility and that was horrible to say the least. We transferred to Northridge and we saw an immediate improvement! Before arriving at Northridge, my father had spent 4 days lying in a bed with no care, no rehab, no doctor, no case worker - the entire time. A waste of 4 rehab days. After arriving at Northridge, he had therapy twice a day, case meetings weekly, a private room, and clean facilities. He discharged from the Center in a little over 2 weeks. I am so grateful. I'm glad I started elsewhere so I can really see the differences between facilities. They have an assigned nurse and CNA to each hall, so somewhere between 12 - 14 rooms. But just because it's a more direct workload doesn't mean they don't have other patients. Starting elsewhere helped me adjust my perspective about my expectations about call times. It's not immediate like it is at the hospital, but it's acceptable. Knowing that we needed to be proactive and call BEFORE we had a problem to stay ahead of the pain was helpful for us, too. He wasn't always the most compliant patient and sometimes he can be a handful, but the staff was always patient, understanding, and consistent with their messaging with him. It helps to have that kind of advocacy. Having a loved one in rehab is very stressful on everyone. I'm so thankful to have had the staff at Center at Northridge caring for my dad. They kept me informed when I was away, gave me daily updates when I'd check in, and bent over backwards to meet his needs. My highest rating. So thankful and relieved! read more

This skilled nursing rehab is amazing . They have worked wonders for my husband. He had a lengthy hospital stay and had to start from the beginning, standing up from the bed was a learning experience. In a little over 2 weeks he was walking across the gym...without a walker! Take a tour of this facility, anytime during physical , occupational and speech therapy, I would be shocked if you didn't see many patients being worked with, in the halls and in the gym. I havent met a staff member who hasnt been happy, gracious, competent and compassionate. Of course there has been, on very few occasions, where the call light went unanswered for longer than what we are used to, but I find it would be overstaffed to try and plan for the days when more than one patient may need more personal attention. This facility has a 5 star hotel feeling ambiance but don't kid your self, they take their jobs seriously and their job is to get your loved ones home safely and as healthy as possible. read more

My father stayed in this rehab center for just under 3 weeks. He showed absolutely no improvement during his time there. He was very weak from brain surgery and needed help to sit up, to stand, to use the bathroom and bathe. His left arm especially was extremely weak, with loss of function and control. This place is understaffed to say the least. They would insist he get out of bed, but would put him in a wheelchair (without putting a cushion under him) rather than seat him in the recliner in the room. And then they'd leave. A skinny man with no upper body strength to hold himself up. In a wheelchair with a low back. They would leave him for an hour or more. Then give excuses as to why they couldn't get anyone in to help him to bed. I would stand outside the room and accost staff until he wa kos back in bed because simply asking didn't work. Now he has a bed sore from that damn wheelchair. One of his hearing aids was lost. The family had to notify the staff that he was not sleeping, that he was not eating, that we suspected a UTI and that we thought he was depressed. The FAMILY, who are not medical professionals and were not with him 24 hours a day. The UTI was not addressed--we had to ask three (3!) times for a test to be run. While they agreed to run a test each time we brought it up, and we even requested one from the PA, they did not actually perform the test for more than a week, until the day before he was leaving. We did not get the results. Our home nurse had to call the center to get the results, which were in fact positive for a UTI. Thanks for not getting back to us on that, North Ridge. Ffs... The food was edible but always cold, the coffee was weak but free. They have a common room with a refrigerator to store food. We brought a pie for his grand daughter's birthday and when we went to retrieve it half of it was gone. The OT assigned to Dad was great and went out of her way to help. The PT and ST were there... not great but not bad. People at the front desk had no interest in greeting people or answering questions about the facility. There was one dark haired lady that really offended me with her piss poor attitude. She left us standing there for several minutes while she continued a conversation with a staff member. It takes NO effort to acknowledge someone's existence and say "I'll be right with you", but nope. They need more handicap stalls. All in all I was not impressed in spite of my high expectations from reviews I read. If you plan on sending a loved one here they NEED an advocate to fight on their behalf. BEWARE!!!! I would not recommend placing a loved one in this facility, perhaps someone you don't like... read more

I recently spent 2 weeks at the Center at Northridge. I almost don't have words enough to express what a wonderful experience I had there. The nursing staff, doctors and housekeeping folks were so efficient, loving and caring. I was on the third floor and experience staff hugs every day. The food was also very good. The executive director, Troy, really cares about providing good care. The PT staff was amazing and also very caring. read more

Dr. Gus Ghuneum was insulting and rude to my father while in this facility. He and his nurse, Sherri Stuart, kept telling us there was nothing wrong with my dad... That's keeping him in the facility longer I'm sure for more billable days... When we finally removed my father from the facility after they said there was nothing wrong with him and took him over to the geriatric emergency room at University of Colorado Hospital they diagnosed pneumonia within 20 minutes. I have never ever Rude and pompous doctor. He is the worst Excuse for a doctor ..uncaring, unkind to my dad, telling us in my fathers room that "maybe he's just old". i spent over two months at the center due to a severely broken arm. I am 84 years old and wheelchair bound. I was quite needy, able to do little for myself but eat. The personnel at the Center took good care of me and the meals were exceptional. Physical surroundings are clean and pleasant. I would recommend them to anyone needing physical rehab. EMG read more

This is the worst rehabilitation center. It is a beautiful facility but do not be fooled. The staff on the 3rd floor from the head nurse to the cleaning staff are just horrible. The head nurse rolled her eyes at me when I told her that they gave my mom 3 different broken recliners. I asked nicely for them to just get her one that works. Before even that another nurse pushed my mom on to a wall on purpose. They are late on meds and the food is horrible. My mom arrived on a Friday and they told her she can't get any help or take a shower till Tuesday because that is her day. When you push the nurse call button they take 30 minutes to even come. The staff when we passed the in the hall was arguing with eachother on who was going to do what and that was so unprofessional. read more

I spent 8 days at this facility recovering from bi-lateral knee replacement surgery. I cannot say enough about the excellent care I received during my stay. The physical therapy, nursing, and administrative staff were all top notch - giving the the proper care and treatment needed to ensure a successful recovery from my surgery. Another plus is that the facility is brand new and modern. I would highly recommend The Center at Northridge to anyone requiring extended care following surgery. read more

She constantly is complaining about a specific nurse named Keisha who seems like she is miserable at her job and very unkind to many patient which saddens me for my aunt and all other residents here. read more

Top notch? if the care at Northridge is considered top notch. Sorry you might have not been exposed to much other care. Northridge showering policy for a loved one shower twice a week. It's comical on the welcoming package they hand out it states patients should have a staff member who speaks thier language. Wow what good does a staff member who speaks Spanish English or any other language do for our loved one. The language of a lazy staff is universal. Like I had posted they are sure to get the menu from the patient on Monday for Tuesday,Wednesday. That day shift Cna she so proud of herself for that big accomplishment. All the while my mother wanted to get a shower Monday We have been very lucky at Rose medical Center on the 6 th floor. Northridge Cnas need to take lessons from these ladys. At Rose the Cnas actually come into the room to ask if you would like a shower or water or to go walking. Rose is a hospital the 6th floor is a surgical floor these ladys get it done. Northridge is Rehabilitation it's not a acute center. The difference is Rose expects more from its staff for their patients. I'm going to look into trying to upload my mother's lab work on this site. So you can see the top notch labs that were horrible. When I asked the female nurse meg I think her name is she said your don't need her iron. I explained to her half my mother's stomach is gone she had stomach cancer she doesn't absorb iron. I told meg my mother has needed blood transfusions because she becomes anemic. Meg responded we looked at the labs they are fine she doesn't need iron. I told her look at the prescription on the box it's a oncology doctor. Meg still didn't care. My mother's oncologist did her labs 3 days after she left Northridge. The labs were bad the oncologist asked us about the iron I told him Northridge refused to give it to her. He said her iron level is extremely low. I made some calls and brought up several issues and was directed to a couple places to file complaints. There was 2 good Cnas I don't know there names. One is a female who works night shift curly hair and tattoos. She was very kind. The other Cna was male with long blond hair very nice mack up. When lazy Jackie didn't want to shower our mother he came. My mother and I really liked him I was told once you can't fake compassion. He really did his job well and enjoyed. My brother and I were careful to review centers before sending our mother to a facility. We took that part of the process seriously. We reviewed the Medicare site with their thorough reviews as well as searched the internet, called facilities, talked with therapists ...we thought we made a good decision but time proved us wrong. Where to begin???? 1. Call buttons ARE NOT responded to timely. Many times my mother was waiting 30+ minutes. No lie...we were on the phone with her one time and nobody was responding to the button. She saw someone walk by in the hallway and I told her to start yelling "help!!". She did and only then did someone come into her room. This particular issue I've illustrated she waited 35 minutes for a response to a call button. We reported this to the Director who said he would look into it. We also reported this issue to the Case Manager. Our opinion, they simply are not staffed to respond timely to call buttons because this issue continued on her floor throughout her stay. My mother also said she could hear other patients calling out for help from time to time. When a patient next to my mother's room was checking out of the facility he ripped whoever he was talking to apart about the call button issue - told that person he would never step foot in this facility again...ever. Yes, he was quite vocal and loud so my mother heard the conversation. 2. Some, not all, of the floor teams were impatient and rude to my mother. My mother had a serious issue with her back and they would ignore the notes in her file and move her bed without letting her know - which caused her significant pain. This happened multiple times and again we complained to the Director. FINALLY they put a note on her door so staff would not ignore the notes in her file. 3. For 2 weeks my mother was in severe pain in her abdomen. After a few days of talking to her and having her describe what she was feeling I called the Charge Nurse and asked if anyone had ordered an X-ray because it sounded like my mother's digestive system might have a blockage. They had not and only when I told them to get one ordered did they move to that step. My mother was in pain and nothing had been done to check things out. They had been throwing meds at her but not listening to what she was describing. Furthermore, and to our extreme shock, my mother had been there 15 days and NOT ONCE did the M.D. examined her during this time of severe pain. Needless to say we chewed them out and demanded an M.D. examine my mother - an M.D. was there every day at the facility but that person never once went from the 1st floor to the 3rd floor to examine my mother. You would think when a patient has abdomen pain on a scale of 8 - 10, is not eating and not having bowel movements that a doctor would be involved. NOPE...didn't happen until I chewed them out and told them it was GROSS NEGLIGANCE on their behalf and that if anything happened to my mother they would be held accountable to the full extent. I chewed the Doctor out and asked the doctor why she had not examined my mother. Her response "it was a busy time and some days were a little crazy". I told her she was paid to take care of patients - that was her job and if she couldn't do her job she had no business being a doctor. I also put it bluntly to the person who oversaw the doctor telling him they were lucky this time but next time they might land themselves knee deep in a lawsuit and inside a courtroom. 4. And the final thing - my mother was discharged without me being told she would need a hospital bed because she still could not lay flat without extreme discomfort. How the hell does staff forget to relay that?!? Had I known that I would have had a bed lined up and already delivered to my house. Anyway, I got her home that evening and she said - "I can't lay flat, didn't they tell you?". Talk about GROSS OVERSIGHT on their part. So we had to prop her up with some pillows which was not comfortable for her at all and then I spent the next few days going through the process of getting a hospital bed through Medicare. In short, my mother will never go back to this facility...never, ever, never. Nor will we ever recommend this facility to anyone we know. If you want to consider this facility then you need to question them intently on all aspects of care. If your loved one will need specific types of care dive into with this team. Make sure staff can AND WILL meet your expectations. Be blunt about your expectations and tell them to answer your questions honestly. Don't be afraid to advocate for your loved one - remember you are paying them a lot for care so expect the best. And listen to what your loved one tells you - pay attention to their comments - empower them to be honest with you about what they are experiencing as a patient. Remember, your are their advocate and their voice. read more

My wife was transferred to The Center for physical therapy. For lunch her first day they gave her a sandwich: two slices of white bread and one piece of sliced turkey. Oh, and a bag of chips. read more

First off, the PT/OT staff was excellent! And Connie a CNA was good as well. The Medical staffing was pretty much non existent. How are you supposed to rehab when any kind of exertion leads to a diarrhea outbreak? I informed the medical staff I came to the facility with prior medical issues, please address them. NOPE they would not listen. I was greeted very warmly by a CNA, Elizabeth, I thought gee she will be a joy to work with. NOPE it was all a ruse. First off THEY NEVER ANSWER THE CALL LIGHT! One night I was nauseous & hit with diarrhea , she handed me a vomit bag & left me on the toilet. When I rang her back, her comment was "Oh it's 9:00. I think I'll take my break!" The diarrhea was so bad the first Sunday I (needed assistance to the bathroom) called no one came. By the time Bri came there was a huge mess all over my bed. I was humiliated Bri was pissed. She did offer to"hose me down" in the shower but she was "backed up" on her rounds so it would have to be quick. Finally left that facility for the hospital because someone FINALLY realized I was having BLOODY diarrhea & ended up with a GI bleed! I went HOME from hospital will arrange in home PT/OT. Former employee said owner changed and service not as good. Retired nurse who was pt last year said non nurse staff overworked not great. I was in there 3 days Did not get sufficient Phy Ther, non nurse staff not always well trained One brought in ice water in sipper and did not secure lid, another was asked to fill machine that pumps cold around my knee with ice and she did not know how to put ice in it, close lid and turn it on. During late night i requested a pain pill. Over an hour later someone opened my door, turned light on and said in a VERY mean manner, "what do you want" and i suggested she start with a better attitude. Just wasn't what i expected and left. After leaving and checking myself out they arbitrarily contacted another company and charged additional meds by sending 12 prescriptions to my pharmacy. I requested a reimbursement and no answer Just not worth the money and some poor training. Some there acted like they were irritated at you because they had to work. Should have listened to others. read more
