Completely horrible experience from the first day to the day we pulled my mother out of memory care, which was only 3 days later. Staff was virtually absent. Getting in and out of the unit took sometimes as long as 10 minutes or more. On a Saturday we observed the only staff member available to open the door was the same only staff member serving lunch to the zombies they have for patients. Upon entering the lunchroom there were children's cartoons playing on TV, green looking hot dogs with some type of sauce that looked like it came out of a bathroom, and my mother could barely speak to us, as her voice was hoarse, her hair was filthy and matted to the back of her head, mismatched dirty clothes, and when I tracked down the same nurse I had met 3 days earlier who was handling the same vials that were later given to me as "haloperidol", she did not know who I was, rudely stated her name, and shrugged me off when I said I was taking my mother out of the lunch room immediately. My father who would visit my mother from independent living (we also removed him and all his belongings 3 days later when we learned the entire apartment was infested with various types of roaches when we visited after movers brought his items in 3 days earlier) would try and help her eat and take her back to her room and would be told he could not take her out for sometimes as much as two hours while the other zombie residents (by all appearances every resident had been given some type of sedative, my mothers was Halperidol, I learned later when I pulled her meds upon taking her out of the facility. NEVER SANCTIONED IN FACT TOLD HER HOSPICE NURSE NOT TO GIVE THIS TO HER EVER AGAIN PRIOR TO ENTRY INTO FACILITY as side effects are " potent antipsychotic medication with common side effects including sedation, dizziness, dry mouth, constipation, and blurred vision. It carries a high risk of extrapyramidal symptoms (EPS)--such as tremors, muscle stiffness, and involuntary movements--as well as potential for serious conditions like tardive dyskinesia, QT prolongation, and neuroleptic malignant syndrome (NMS)." When I told the staff I was taking her out for at least the weekend (only way to get her out of there quickly was to state she was coming to visit with me at my home) I was given an entire bag of 30 or more doses of this Halperidol drug she was NOT PRESCRIBED prior to admission by the 'nurse on duty' to bring home with me. WHAT? My father observed patients with involuntary movements throughout the 3 days she was there and again, my mother could barely speak or raise her voice above a whisper and reported "voices in her head" and clicking, roaring, and thumping in her ear. AFTER 3 DAYS. DO NOT BRING YOUR LOVED ONE TO THIS FACILITY OR ANYWHERE NEAR ANY PART OF THE PROPERTY FOR ANY REASON. read more

It is very degradering to treat the older people the way they do, the girls in physical therapy treat the patients like animals I saw it with my eyes. this place needs to close doors, I would never be there before I died, terrible place. read more

My Dad has been at The Citadel for just over a month. First, in independent living and now in assisted living, he needed a little more help that I originally anticipated. My Dad can be negative at times. I was worried that he wouldn't stay and accept the help he needed after his stroke in late April. His best friend for more than 40 years called me last week and said my Dad sounds happier than he can remember. He's made friends. My Dad called me today and couldn't stop raving about how nice everyone is. He said he hasn't met one grouchy staff member. He said everyone has been kind and helpful. He said the lady who took them on an outing at Walmart today couldn't have been better; she helped him (and everyone else) get their groceries onto the bus and helped him buckle his seat belt. I stop by for meals sometimes and he's socializing with others. The Citadel has helped my Dad and our family during one of the most difficult transitions he has had to face. It's not perfect. I'm an RN with over 20 years of clinical experience. Perfect doesn't exist. But he's happy and I'm beyond grateful for everything they are doing at The Citadel for my Dad and all of the other residents. Thank you read more

BEDBUGS! The whole facility! 7 years straight! They cant get rid of them after trying almost everything. Notice the new carpet inside. That was the last attempt to get rid of the bugs. My dad and his room mate were crawling with them. In their hair laying eggs, in the bedding, clothes, covered! Your loved one will be bagged up and carelessly thrown into another room for 7 days while they kill the bugs in their own room. Over and over. Rude staff that leaves people hollering in their rooms for hours...they dont answer the emergency buttons and friends of my dad died alone calling for help that never came. Poor food quality, rental hikes sometimes double without any advance notice, meds not given to most of them on time, countless excuses, zero entertainment and the place is filthy and stinks of urine everywhere. Horrible! Even if you hate your parents, this place is too much!! Last year the laundry room caught fire and the entire place had to be evacuated into the street since noone ever cleaned the lint traps. fun fun. Myself and the nurses that came in to work with my dad said over and over this place needs to be shut down. The front desk ladies are trained to have great big smiles.....its all a load of crap. Don't fall for this place. All the residents can only complain with nothing ever getting better. Pure hell hole! Sickens me! read more

Is this where nurse rejects go? The lack of care or attention for proper protocols is ridiculous. The amount of times people came into my father's room and never washed their hands or even put on gloves after coming from other patients rooms.. my dad contracted covid, pneumonia, and c. diff; all at the same time when he had to go to this place after a knee replacement. No one could ever answer simple questions or requests. Absolutely horrific place to go for after care. read more

Trying to get showers was difficult. There were times when the food was cold, and the dining room times meant that sometimes my dad would miss meals. The people were generally friendly, but it wasn't a pleasant experience overall. read more

They didn't know what a vegetarian meal was, frequently had meat on my tray, even after I requested multiple times that I am a veterinarian. My family had to bring me water because they only brought me a 4 ounce cup to take my medicine with. When I was on the outside patio, a man got locked out because the button to get inside wouldn't work. No one check on us and we only got inside because some other women showed us that you had to hit it with a stick. When I was discharged, I ended up back in the hospital within a week. I was unable to answer their phone calls so they callEd my son complaining that I didn't pay my copay yet. This was less than 2 weeks after I was discharged from there. They also cram you in a room that was designed for 1 person. This is where people go to die, not heal. read more

I have been a resident here for almost 2 yrs and I absolutely adore this place! The staff is amazing they really care and you can feel it! Any concerns I may have are addressed in a timely manner! When I got here I could not even help myself, now I'm up again taking care of myself completely! I'm positive I can return to independent living! The physical therapy staff goes above and beyond concerning your treatment plan! The director and senior staff do more than enough to make you have everything you need and more! I've been in a lot of places and this is the best! I feel safe and at home I LOVE IT HERE! read more

This place should be shut down by the state. It is dirty, fecal matter on bathroom floors, and for extra fun how about a 4 inch long cockroach to top things off. Care is sub par. Call themselves post acute, really. I was sent there after full knee replacement an they don't even have ice machine or ice sleeves for securing leg from swelling. They come out with ice cubes in a plastic bag, really I could do that at home. I suffered post surgical shock and was placed there for rest and recuperation.....ya think?. They place me in a room with a hostile Dementia patient . She verbally started attacking me saying I couldn't share her room.No one has the time or cares about my severe health issues but mollycoddled the old woman. 3 days of complaint and bathroom remains dirty. I may leave here with a complete nervous breakdown. Don't be fooled by nice entrance area. I am a retired 13 year Advanced Senior Service and Hospice nurse. I know care and this is a living nightmare. It's...No Place for Mom!!! read more

It's really hit or miss. My dad has been there for several years. First on the Independent Living side, now on the Assisted Living side. Turnover is too high. The good caregivers are under appreciated by the staff so they leave. The food is abysmal. It's the running joke of my dad and his friends. When I've eaten there with him on a visit, it's taken an hour to get his food. Communication is rough. Management and the caregivers often don't communicate well. What that means for me is when I provide direction from my dads doctor, I have to tell the staff at Citadel several times over sometimes several weeks to make sure they do it. I'm often repeating myself. (The high turnover aids to the problem). All that said, some of the people there are amazing. But given the poor experiences (like how a food spill has not been cleaned up since Feb (pictures attached)(time of this review is June - and yes, I have told the manager, shown him the pictures, and still nothing has been done), my dad and I are looking for other places stat. read more

I can't begin to say enough about the Citadel facility and their staff. My dad passed away on January 26, 2016 and spent the last 2 months of his life there. The accomodations were just amazing. My parents stayed in one of the efficiency apartments and it was so nice that they felt like they were at home. Because of my fathers dementia he couldn't really go to the cafeteria so my mother was able to order meals and have them delivered and the food was wonderful. Paula, my parents favorite CNA, and the rest of the medical team was so patient with my father and, I feel, truly loved him. Even the janitorial staff was wonderful. I can't thank the CItadel staff enough for making my dads tranistion much easier and being so warm and kind to my parents. I just can't imagine a better place to recommend. XOXO read more

When my mother first moved in to the Citadel just a few months ago I absolutely loved it! My mother has Alzheimer's, and it is progressing. If you are familiar with Alzheimer's you realize that there are many stages of Alzheimer's one of those stages is behavioral issues. In my opinion any facility that has a memory care unit and deals with Alzheimer's patients should be well trained in how to handle a patient with Alzheimer's at any stage. This I quickly learned is not the case at the Citadel. Although I have personally not witnessed a single incident of any behavioral issues I am told that they are happening. I found this out when I was told that they put my mother on a certain medication. I had never heard of that medication so I looked it up. The warnings on this medication scared me. It actually stated that it should not be given to an elderly patient with Alzheimer's. I called The Citadel and I asked them to stop that medication. I told them that I would find something else and work with her neurologist. Realizing that you can't always trust what you read on the internet I actually called the FDA and I spoke to their pharmacist. The FDA told me do not give that medication to my mother. I then called my mother's neurologist they also stated that they do not prescribed that medication due to the dangers and the warnings. Now, back to the Citadel. Knowing full well that I am in the process of working with my mother's neurologist and having tests done and have ongoing appointments they actually issued me a 14-day eviction notice. Apparently the Citadel doesn't care that I am in the process of getting my mother medication. They don't care that there is covid-19 going on which makes it extremely difficult to move. You can't just walk into a facility and ask to see it. Some facilities aren't taking people because of covid-19 regardless of whether or not they have a bed available. I have also asked, since I have actually not witnessed any behavioral issues, that they call me when this is happening and I will come and sit with my mother. I have not received a single phone call to do so. In order to get her into a new place I have to have all of the paperwork from the Citadel, which includes all the caregiver notes. I have been told that my mother hits caregivers and other residents and that is why she is being evicted. However when I received all of the notes and paperwork from the Citadel there was not a single mention of her hitting anyone. I was also told that my mother is the only one in that entire facility that has any behavioral issues. I find this difficult to believe in a memory care unit full of dementia patients. Now mind you, at the beginning of this statement I stated that I really liked the Citadel when my mother first moved in there. If your loved one does not have dementia I would say that this is a good place to move them in. However, if your loved one has dementia and or needs the memory care unit I would think twice. I am still not convinced as to the true reason my mother is being evicted as I have asked for phone calls during her outbursts and have not received them. Is she not having them or they just not calling me? Why was there no mention in her paperwork? Is the paperwork wrong or was I lied to? It seems logical that it has to be one or the other. Or is this all stemming from the fact that I do not want to give my mother a medication that is truly dangerous for her to take? Wouldn't you think that if one of their memory care patients was actively working with a neurologist to get a safe medication that they would work with you? I believe a facility with the appropriate training and qualifications would. But the Citadel finds it easier just to issue an eviction notice. Additionally if you are aware of issues with Alzheimer's, you are aware that changes in routine, settings, people, Etc can further exasperate the mind of an Alzheimer's patient. This is what's being done to my mother. Shame on them. read more

I don't even know where to start. This place seemed amazing when we toured it, but after our loved one being in here for a couple of months, we have nothing good to say. We have been trying to schedule a meeting with Kathryn, to discuss our concerns, and she will not make time for us. Today was the breaking point, one of the women there (Conchita) yelled at my grandmother, because she said her coffee was cold and she did not get everything she ordered for breakfast. Conchita then proceeded to heat her coffee too hot for her drink. The physical therapy here is a joke, they had my grandma walk across the room and back, checked her blood pressure, and that was it. My grandmother is visually impaired, and they keep trying to get her to sign documents she can't see. We are paying extra for them to escort her to their events, and they don't bother telling her there is even events going on. We bought her a taking clock, because one nurse told her it was 8:30, when it was actually 9:30 (they were 1.5 hours late bringing her meds, and didn't want her to know). When you are paying over $3,000 a month for care, you expect much higher quality than is being provided in this place. I would NOT recommend bringing a loved one to this facility. read more

We can't thank the entire staff at the Citadel enough! We just moved in my father and his wife and the staff made the transition for them seamless! They are so caring and the personal attention is so appreciated. The apartment is large and bright very comfortable for them. The food and amenities are terrific and much appreciated. The property is so clean and upgraded they are very happy with their new home. The care givers watch over them and are available for their every need.
