This care facility is amazing. I have had 2 loved ones stay in the hospice here and both experiences were exactly what you want for your loved one. The place is peaceful and clean and the staff are very compassionate. You can visit loved ones in the hospice 24 hours per day and it is a safe facility with locked doors. Nurses are kind and well trained and the rooms are cozy and comfortable. read more

Somehow I got put on a list to receive mailers from them, and after three calls to get removed, I'm not confident that's going to happen. I do NOT qualify by age or distance to live there...not by a long shot...so I don't know how I got put on their list, or why their marketing department is not skilled enough to screen in advance and remove people for good; even the people I've spoken with over the phone don't seem very competent, so I can't imagine what it's like to live there, but good luck everybody else! read more

Appalling visit. There is no consistent security measure to make sure residents on the campus are safe. Hotels and apartment buildings do better. Here, doors were propped open or unlocked. Anyone could walk straight in from the parking lot and access your loved one's apartment and outdoor sitting area. Just totally unacceptable risk. And management shrugs it off with "well, we've never had an issue." So, it appears that very obvious shortfalls aren't going to be addressed anytime soon. It's such a shame because the location is lovely. Still, AVOID. read more

Just had a tour and everyone was incredibly warm and caring. The place looks fantastic. I could envision myself living there.

If I could, I wouldn't even give this rehab facility a 1 star. I would make it a negative star!! My father was released from the hospital to Hacienda to be able to REHAB him as a bridge before safely going home. This was NOT the case! In a matter of 5 days, he had regressed and declined so severely he almost died! First off, he was isolated to his room and only saw a physical therapist at the most 30 minutes or less per day. He was not strong enough to eat or walk on his own. He depended on staff to help him however, the staff was not helpful at all. The charge nurse Pam would give my family attitude and the nurses are not very knowledgeable. I myself am a RN and FNP so I can confidently say, most of the RN's have NOT a CLUE how to properly assess, provide appropriate care or assess for life threatening situations. I arrived from out of state on Tuesday to find my dad lethargic, and very thin and dehydrated. He was not eating and had fallen 3 times within 12 hours the previous Sunday WHILE UNDER THEIR CARE!!! The one time I rang the emergency light because we needed help in the bathroom as he was too weak to get up and about to pass out... it took 20 minutes for someone to come. The nurse I asked for help said "I'm at lunch"!!! I happened to notice his leg was swollen starting at the knee. I inquired and the nurse Electreeia (Long weave with blue in it) stated she would notify the MD. I then asked if she could change his PICC line dressing as it was 2 weeks overdue. I asked she bring adhesive remover as his skin is so fragile it tears. She came back an hour later with NO NEWS about the swollen leg concern and proceeded to use a skin barrier to remove his dressing without telling me it WAS NOT adhesive remover. She became very defensive and did a horrendous job at the dressing change. I notified the charge nurse PAM and she said that "this is how we do it". The last thing my dad needed was a blood stream infection!! 3 hours later another nurse ANDREA came to change dressing. She gave me a lecture on how she went to nursing school and wouldn't be there if she didn't know what she was doing!!!! I was floored at the way she behaved and how she thought her "sterile field" was still clean after her glasses fell on it contaminating area! I raised concern again about his leg as it was doubling in size as the hours passed. By 1145 PM my Angel Nurse NINA came to my rescue and listened to my concerns about my father possibly having a clot in his leg. She attempted to call doctor as a Doppler study was scheduled for the next day. I did not want to wait! She called the ambulance for me and we made it to the ER at 0200 A.M. He indeed had a clot in his leg... it was so big it traveled up his groin and he even had some pulmonary embolisms!! He was on deaths door! I am happy to report my dad has made it through and is on the mend. We will NEVER go back to that place and I URGE you to do the same! This facility is incompetent and if you love your family member, you will protect them from Hacienda at the River Rehab. To Nurse Nina on the night shift... I THANK YOU from the bottom of my heart for listening to my concerns and getting him the help he needed! He is alive because you helped me. You listened to a concerned family member. The entire staff is committed to the safety and well-being of their residents. I have been around healthcare for 50+ years, and Hacienda at the River is at the top of the list. After researching other properties, I am looking to have my mother move in, and Hacienda at the River is my only choice for her. I was able to do a virtual tour before visiting the property, which was a very good representation of the property and services Hacienda at the River provides. Ellen's review is far from the truth; she must have had a bad day. Ellen's attitude is one of entitlement; she must be an Arizonian Transplant living in Scottsdale or Prescott. I'm not going to waste my time to point out all the things that went wrong during my father's stay, some could have been life threatening to say the least. I have made a long itemized list with names, times, and events that occurred and it will be reported to the long term care ombudsman of Arizona (Arizona Department of Economic Security). I advise other reviewers who have posted to also take their concerns here. It is disgusting what we had to deal with, from incompetent "nurses" and meal staff, to the actual assigned doctor. You know there is a HUGE problem when families of patients are all vocalizing the same concerns. If you care for your loved ones, do NOT bring them here. I do not need a comment from Brenda Ritter - we spoke early on, nothing was done and it was obvious nothing would change which is why I'm escalating this. read more

"Skilled Nursing ... Without Enough Nurses" This review only covers the skilled nursing services at this facility in August 2019. Physically this is a beautiful place that's kept in good physical condition. It is part of the Watermark network of facilities, and that generally means good care and a pretty nice place appearance wise. We looked at other skilled-nursing facilities in the northwest of Tucson, but they seemed rundown and shabby, with a bit of a "rough looking" crowd of patients. My experience at The Hacienda is with my mother being there for 2.5 weeks in skilled nursing so she could receive physical/occupational/speech therapy 6 days/week after a hospital stay. There were not enough nurses in skilled nursing, and for extended periods of time. Those who were there were incredibly busy. They were too busy, in fact. Short staffed, I heard, repeatedly (which is aggravating). The care was good when the nurses were there, but getting them to my mother was hard and very frustrating, especially the first week. If you are considering having a loved one here, call and speak with senior management and ask what the actual ratio of nurses to patients in skilled nursing will be. Ask what it will actually be on the day that your loved one will arrive there, and the next day, and next. If it's more than 10 patients to 1 nurse, then get ready for long waits for your loved one, or look elsewhere if more attentive skilled nursing care is needed. It took 2 hours to get a nurse one evening due to another patient being admitted and that taking 2 hours of the nurse's time to do it. Normally it was 15 to 30 minutes to see a nurse. Management tells me they're hiring more nurses, even ones specifically to do new admissions, so patients will have better care. Verify that this will happen or has happened, or expect quite long waits. I want to be clear that the problem is with a shortage of nurses, not the skill or work ethic of the nurses who are there (they worked hard and were always busy). Management simply needs to hire more nurses. This nurse shortage was a constant and aggravating problem, even up to the day of discharge. That being said, the nurses and care givers (called "nayas") were very good when they were present to care for mom. A big thank you to nurses Jose (he's awesome!), Norma and Mary. Also, a huge thank you to Nayas Thanes, Jenna, Celese and D. Physical/occupation/speech therapists really shined here. They were the most consistently terrific service providers in our whole experience, and helped my mother quite a lot. Thank you to Kaliko, Jenn and Todd! In summary, they need to fix the nursing shortage. Call and speak with senior management to see if they have a 10 (or less) patient to 1 nurse ratio in skilled nursing on the schedule for when your loved one will be there. Beautiful surroundings aside, it is imperative that they fix that. When they do, The Hacienda at The River will be the top notch skilled-nursing facility in Tucson. read more

Do NOT use this facility for rehab especially if the patient has any memory/Dementia issues. They literally tell you that it's the patient's right to fall out of bed. My father fell his first night and had to get stitches on his arm which was entirely black from the bruising - this after a head injury. He was found naked on the floor in his own excrement AFTER that. They are woefully understaffed - but the nursing staff and aids that are here work very hard and professionally to manage a difficult situation created by management. Likewise PT staff are professional and caring but don't have enough on staff to manage the number of patients. Do your research before you choose this place which put more money into its building than into its staff. read more

DO NOT USE THIS FACILITY FOR REHAB PATIENTS. While the facility is beautiful, it is poorly run and almost impossible to get any one with authority to help you when you really need it. When I tried to get the discharge process started, I met with the Executive Director and got no help. Her comment: Tell your wife to "relax and maximize her Medicare Benefits" Which apparently meant, the longer I keep your wife the more I can bill Medicare. When their supervisors go on vacation, they do not pass on any of their tasks to others to make sure nothing drops by the wayside. That is what happened to us. It was impossible to find anyone that took the supervisor's place. The only way I got my wife discharged, was to call the Watermark headquarters, which is in Tucson, and asked for help from their executives. Within 2 hours, an executive personally called me back and got the process started for us. Without his help our discharge would have been greatly delayed. read more

My siblings and I moved our 93-year old mother from Minnesota to The Hacienda at the River 10 months ago, and we have nothing but the highest praise for the 24/7 care she receives, the beautiful, home-like, buildings and atmosphere, and the huge selection of activities they offer to the residents! My mother is in assisted living, and the Nayas, as well as the resident care administrators, are the top-notch, professional, and show so much care and concern toward my mother and all the residents that I know she is in good hands always. The response time to her call-button calls is very quick, especially since the facility is small enough that the response time is managed well. In addition to the horses coming in 3x/week for the residents, they plan outings, like going to.the Tucson Museum of Art, art galleries, a ballet, glass blowing studio, sight-seeing trip around Tucson, the zoo, paint your own ceramics at the mall. My gosh, what other care facilities provide stimulating and interesting activities like that? Most other places offer BINGO, bridge, and sing-alongs. Yes, the Hacienda offers those, but so much more. The.staff greet my mother with a warm "Hello" and a warm smile. They are patient with her, and take great care in helping her transfer from her wheelchair to the bed or chair. I cannot speak highly enough about the great job the Nayas do, as well as the Health & Welllness Director and the Assoc. Executive Director. It's a great team, and I am so grateful my mother is in a safe, warm, friendly, environment. The food is delicious, and there are comfortable places to sit outside, as well as the various living rooms and front entryway. It was definitely worth moving my mother 2,000 miles so that she could be in a place like The Hacienda. Kudos to everyone there. read more

This will be a two-part review, with the first section addressing the Skilled Nursing area of Hacienda at the River. I start with Skilled Nursing even though my mother's stay here was short because I have read some reviews of Skilled Nursing on Yelp that leave me baffled. Our experience was terrific from start to finish. I recommend the Skilled Nursing area at Hacienda with no reservations. Hacienda at the River Skilled Nursing Story: My mother, who turned 100 this year, spent a week rehabbing in the Skilled Nursing area when she came down with pneumonia early in 2018. Mom was transferred from her apartment (in the Assisted Living building of Hacienda) by the competent staff, including an RN to oversee the move. A sharp-eyed nurse noticed that Mom looked chilly, so they brought her an extra blanket to keep her warm. I visited Mom daily during her week-long stay in Skilled Nursing and was always impressed at the level of care Mom got. It mattered to the staff that Mom enjoy her meals, and I noticed they made sure she got food she likes. And the staff cared enough about Mom to check on her frequently throughout the day. In fact, a couple of times I walked in to find a covey of staff in her room chatting and enjoying Mom's company. I saw this happen with other patients as well. The rooms are spacious and spotless. The care Mom received was so good in Skilled Nursing that they transferred her back home in only one week. Originally, they thought her stay might be longer. So that's my experience. Hacienda at the River gets top marks from me AND Mom. Hacienda at the River Assisted Living Story: Mom has resided in the Assisted Living building at Hacienda at the River for about a year and a half. I cannot find enough words to compliment the awesome staff. The Nayas are caring people who greet us so nicely whenever we pass in the halls. The Executive staff walks the extra mile to be sure Mom is comfortable and happy. But it's more than that they are nice. The staff LISTENS to residents and family and tries to meet needs and wishes. Executive staff meets privately with each resident and family every quarter to be sure that individual needs are not overlooked. We like that a lot. The rooms are smaller than I would like, but I suspect that is part of a plan to encourage residents to mingle. They have a stellar equine program, with horses on the property three days each week. There are plenty of activities available, including exercise and yoga, movies, classes, and outings. Personally, I am pushing for a Qigong program, and maybe that will happen, too. Meals are nutritious and tasty, although Mom wishes for a couple of less-healthy options now and then. You need to understand first that Hacienda at the River emphasizes community spirit and wellness at every level. They do so much more than feed and medicate the residents. We are all (including the families) part of the community, and I feel that warmth whenever I walk in. Everyone smiles in a genuine way, and they all come up to talk to me. I feel welcome there, and Mom does, too. I looked all over Tucson for a homey, caring Assisted Living place for Mom. We found it at Hacienda at the River. read more

Hacienda at the River is a great place to work. Our Dining Service Directors are the best bosses I have ever had, and I have been in the workforce for over 50 years. They truly care about our team and let us know, constantly, how much we are appreciated. Our HR Director is THE BEST OF THE BEST!!! Our Mom has been here at The Hacienda at the River a few months and absolutely loves it! Number one is the horses. She has bonded with one of them and it is very special. She has made many human friends as well amongst the other residents and the nayas, caregivers who are all patient and caring people. Excellent food, especially the desserts. Ive been going to visit friends at memory care at this fscility for quite some time now. While the grounds and common areas are beautiful, the cleaning of the individual bathrooms and bedrooms is awful. It seems there is no designated staff to clean on a regular basis. For the amount of money paid for memory care, it is just unacceptsble to see this oversight and important basic task being so seriously neglected. read more

I was at this facility 09/17/19 for about 2 weeks. I contacted pneumonia while at this facility. 2 weeks later still had it and was sent home. non of the staff seems to be on the same page. I waited as much as an hour for a pain med.the social worker Glo is a plain liar. I waited several times for her to furnish information.....still waiting. she never showed up on the day I was to be discharged. I asked the staff where she was because I saw her walk by my room they said she just asked several times if I had left. I finally called my friend and said JUST COME AND GET ME. I have 2 more surgeries that will require skill nursing. I will definitely will seek another facility read more

We experienced great care and caring professionals when my Mother in Law was at Hacienda at the River. The staff was so attentive to her and the facilities were clean and beautiful. I just removed my husband from this place. He was supposed to be there for three weekd, if he had stayed that long he would have died from neglect. He was dehydrated and had fallen twice in two days. Not enough people to take care of patients. If your loved one has dementia do not put them there. Horrible!! read more

Beautiful and clean facility. Excellent food and friendly staff. My room was cleaned daily. I had to ask for my sheets to be changed on days I took a shower. I arrived around 8 PM Wednesday, October 2nd. I was informed on 10/7 I would be meeting with members of the facility regarding my care on 10/17 and that I could invite my family to that meeting. I immediately started to order a walker, shower stool and commode to ensure I had them in place when I was discharged. My first PT consisted of walking outside my room making a u turn and then back into my room. My 2nd PT session was a repeat of my first PT session. My 3rd PT session consisted of walking to the PT room and back to my room. My 4th PT session consisted of walking to the movie theatre and then back to my room. Because I was able to walk with assistance to the movie theatre it was determined by Hacienda at the River and my insurance company that I could be released 5 days later. I didn't even have 1 hr of PT for each session, they did not ensure I could walk up and down stairs (PT 101) or could get into and out of a car without assistance prior to deciding I could be released. A week into my stay no patient advocate or case worker was introduced to me. I was told by the PT on Wednesday, 10/9 that I was being released on Sat, 10/12. I requested to speak with the case worker. Apparently the previous case worker left for maternity leave on 10/3 yet her replacement was not in place till 10/7. A meeting apparently was held on Tuesday, 10/8 with the facility and insurance to decide who was to be released. In that meeting it was determined I was going to be released. The facility and insurance had no idea I took it upon myself to order the equipment I needed to keep me safe once I got home. The facility and insurance were willing to release me without the appropriate equipment, without home service being in place and appointments scheduled prior to my release, without discussing it with me, without my being able to walk up and down stairs, without having at least 1 hr of PT per day while I was there and without working on equipment in the PT room. I will also mention I live alone. I insisted on Wednesday, 10/9 that I speak with my case worker. Later in the afternoon I met Denise the replacement case worker. I voiced my concerns about being release without the appropriate PT, without discussing it with me beforehand, ensuring my home was safe for me to go home to, ensure they ordered the equipment I needed to go home with and ensured that equipment was in place prior to my release. Only then did Denise tell PT to work with me on going up and down stairs. On Thursday, 10/10 I went to the PT room and practiced once going up and down the stairs. If the PT didn't have a belt around my waist I would have fallen. No 30 mins let alone 1 hr of PT. The facility should have know the previous case worker was going out on maternity leave and ensured the appropriate coverage and turn over occurred prior to her leaving. The home was pointing fingers at the insurance company and the insurance company was pointing fingers at the home. Both failed me and left me in a vulnerable state while at the home and when I was discharged. PT was non existent. Communications very poor between staff and PT, between case worker and patient. I was woken up one morning at 5 AM to have blood taken. The dr was in the previous morning and never mentioned blood being ordered. I had no fever or symptoms that would require blood being drawn. A few days later the dr came into my room while the nurse was there and I asked why he didn't mention to me he ordered blood to be drawn. The dr informed me that he didn't order the blood work. He stated the facility (the nurse) ordered the blood to be drawn and does so with every new patient. I then asked for a copy of my blood results. Only then was I given my blood test results. Once again no communications with the patient. A lot of assumptions made by the facility insurance company and staff doing what they wanted to do without discussing things with me beforehand. Hopefully in the future I will not need short term rehab. But if I do I would not go back to Hacienda at the River for short term rehab read more

I moved my husband to Hacienda at the River at the beginning of 2018. He had been in a number If facilities in NY and his health continued to deteriorate. We decided to move him to a warmer client and as soon as I saw Hacienda and had the opportunity to meet all the staff I immediately had a feeling that Hacienda was the right environment. In the 9 month's that my husband has been there my experience exceeded all my expectations. Are my expectations were very high. I had moved my husband a number of times es from very nice facilities because the level of care he need was considered to be skilled nursing in NY. Everything from the Naya's Care. They are attentive, friendly, caring, responsive and answer when called very quickly. My husband is very fragile and very fastidious about his appearance and he his always cleaned, groomed and dressed to his satisfaction My husband has Parkinson's and severe arthritis. He needed specialist doctors in Tucson as well as physical therapy. Hacienda managed all the details and he is with excellent doctors. His physical therapy has also been excellent. I contact the staff regularly and they are always responsive. I must say I am pretty shocked to see any ratings less than a five. I feel truly blessed to have found Hacienda and all the wonderful employees he deals with. I know he is well cared for and there is nothing more important read more

Buyer Beware.  Our review is focused on our experience at the skilled nursing facility within Hacienda at the River.  Our initial experience was positive-the facility is beautiful, the rooms are spacious and we came in contact with RNs and patient care staff that were kind and helpful (with some exceptions-mainly negative attitudes and poor training) However, if your loved one is a complicated patient, please carefully consider the medical personnel that you are entrusting with their care.  Hacienda's website states that they provide "unprecedented access to cutting-edge, university based clinical care".  This was our expectation and definitely NOT our experience.  To quote our loved one, the Osteopathic Physician "breezed in and out", did not stay long enough to anticipate or recognize medical issues, was difficult to reach, and slow to respond.   This resulted in our loved one's readmission to hospital with serious complications and interventions that were too little too late.  In addition, the contracted pharmacy was extremely slow to deliver needed medications and IV products.  Finally, some of the staff at Hacienda should undergo training in how to engage and speak with patients and families going through difficult situations.  We do appreciate the dedicated, qualified staff we came in contact with.  They should be better served by their management, as should patients and families. read more

I just moved my Aunt (who has dementia) into the Memory Care unit at The Hacienda. She was moving from another facility that specialized in Alzheimers. The difference in Hacienda and the other place is night and day. The other place was very institutional and they seemed to have problems keeping good help. Hacienda is great! The facility is absolutely beautiful and a wonderful place to live. What's more is the staff have been fantastic! They have been so helpful for both me and my aunt. My aunt has one caregiver that she works with (the previous facility had many) and this caregiver has provided her with a lot of individual care and help. For me they have helped me with finding the right health care and managing her prescriptions and a lot of other things. Also, the food is terrific! My aunt was losing weight at the previous facility and has been eating well since she moved to Hacienda! I would highly recommend this facility! read more

Don't let the beautiful package fool you!!! Unfortunately, we transferred my Dad out within 1 week because he was not safe there. Even though it was crystal clear by his past medical history & diagnoses that he came in with, they said they did not know that he was a fall risk. That in itself was very concerning to us right off the bat. If they had been aware of it, what would they have done differently? Turned him away? We think not. They had many empty beds in the Skilled Nursing Facility while we were there. We quickly knew we had to staff family at the bedside 24 hours a day for his safety, as they didn't have bed alarms & really no side rails (only a small vertical side rail towards the head). Not only had my father been a significant fall risk for the last 5 1/2 months after brain surgery & radiation, but he was confused, had recent onset of seizures & medication added to prevent them, which also makes him more sedated. Whatsmore, he intermittently became agitated & was often trying to get out of bed even though he wasn't even strong enough to stand because he had just come from a hospital where he didn't get out of bed for 8 days due to the seizures (but he was walking with a walker & caring for himself independently before this hospital stay)! His first full day, when I explained to the oncoming nurse my concerns for his safety, she said to me "If he wants to fall then he has a right to fall"!!! That's when it was clear we were in trouble. From there I reached out to the Liason of the Skilled Facility. I was encouraged when she initially seemed empathetic to our safety concerns & what the nurse had SO inappropriately said to me. She assured me that she would get with the Director of Nursing & that someone would get back to me to see what they could do to make this work. No call EVER came. 4 days later I called her back & shockingly she seemed to wonder why. I informed her no one had ever reached out to me, she said the Director of Nursing was supposed to & then proceeded to say, "But I was told you were right there at the bedside when the nurse informed you that we don't use bed alarms or have side rails, so you have already been made aware of this". It was then that I knew we had to make other arrangements ASAP. Before we had him transferred to Health South RIT, he did end up getting out of bed & onto the floor once, thank God he wasn't injured. The following night I basically insisted that his bed be taken out of the room & his mattress placed on the floor, which they did agree to after getting permission from the DON. It would have made a big difference had management acted even slightly concerned about my Father & the safety concern at hand. Even telling us that this may not be the best place for him in light of the fact that they said "They didn't get an accurate report on him from the transferring facility, therefore the didn't know he was a fall risk"... then initiate a smooth transition out would have been great. We had to initiate the evaluation from Jennifer at Health South RIT & she was incredible & made the rest easy. What is even more ironic. The new facility told us that they got in the report from Hacienda at the River that "He had not required close supervision in the last 48 hours"!!! Wow, how crazy that they forgot to mention that it was because his family never left the bedside & his mattress was on the floor! I can't stress enough that there was some incredible staff there (Naya's: Jennifer, Joseph. Nurses: Renee, Lori, Terry. Therapists: Renee, Ace, Otis) and there are more that we can't remember which makes me sad we can't shout them out.... But, this very much felt like a management problem. The bottom line, I have been a nurse for many years & my husband and I have a business where we specialize in Medicare & Long Term Care Insurance. We were looking forward to spreading the great news to our clients about this beautiful facility, but unfortunately, that is not the case. read more

My father was at Hacienda at the River from October 2017 through December 2017 after a fall which resulted in a broken hip as well as gastric bleeding and dementia. The staff was amazing. There were issues with dementia which were unexpected and the staff was quick to help me find solutions to minimize the effects. Physical therapy helped him start on the path to recovery and he did well. The food was very good and if I had any complaint it is the food is not exactly what elderly people want, more healthy than comfort food. The staff is very quick to respond to suggestions and concerns. I was very lucky to be able to spend a lot of time at the Hacienda and saw the interaction with other patients as well as their families.
