I sought an appointment with Dr. Duke not because I expected surgery, but because I wanted an expert review of my MRI findings and an opinion on potential causes of my ongoing symptoms. I was already aware that my imaging showed little of significance and that surgery was unlikely. The purpose of the consultation was to obtain diagnostic insight and discuss possible next steps. *Instead*, the entire discussion seemed to revolve around the fact that there was no surgically correctable structural abnormality. Multiple times I was told there was "nothing to do medically." and "you're at the end of the line". I found this statement both inaccurate and dismissive. Medicine is not limited to surgery, and the absence of a structural finding on MRI does not mean there are no medical explanations for pain or no treatments worth considering. Nerve irritation, chronic pain conditions, headache disorders, autonomic dysfunction, inflammatory conditions, and many other causes of pain can exist without obvious MRI findings. When I discussed my SCM pain, the conversation quickly drifted toward psychosomatic explanations, and I was left with the impression that seeking pain relief was itself unproductive and even that this might be "in my head" (psychological in nature). This was particularly frustrating given that I use pain medication extremely conservatively, typically only a few times per month and often at reduced doses.. my pain is abnormal, but what I didn't need was this air of dismissive disinterest. I'm well aware of the potential risks of pain medication, and while I don't think he assumed I was drug-seeking his quick "oh I wouldn't do that" to pain meds *and* trigger point treatments felt like someone who might suggest using them as a failure of one's constitution or even possibly worthy of rebuke because the imaging is patent. My impression was that Dr. Duke viewed the absence of a surgical target as the end of the discussion. He may be a capable spine surgeon, but this consultation demonstrated a narrow and reductionist view of medicine in which conditions that fall outside his specialty were largely dismissed rather than meaningfully addressed. A quick administrative complaint as well: I received instructions to pick up CD media for my appointment. So I grabbed two discs and brought them with and I was told "these aren't needed we have them".. so I felt I wasted my time grabbing those. Maybe as a "just in case", but I spent two hours procuring these which ended up being pointless additional effort. Maybe they should attempt to grab all imaging and confirming with you if they have a copy before you make this effort. Overall this felt like a waste of my time, I felt belittled and frustrated, and the commentary suggested this wasn't a "medical problem", despite other specialists with contrary opinions and the sheer fact "medicine" isn't purely a system of resolving 'only' structural issues. If I had seen this doctor earlier on with the issues I had and his desire to not treat with medication I have no doubt that I would not be around to have had this discussion. This is a review of his frame of mind, and lack of ability to navigate issues outside his area of expertise. I'm glad I got help from OTHER specialists and even when those doctors didn't have things diagnostically useful they still pushed for me to advocate for myself and if possible seeking treatment options and finding solutions that were still relatively conservative (including physical therapy). Experiences like this is what ruins people's confidence in doctors and healthcare specialists, he had plenty of opportunities to reassure me or express that maybe someone else would be better-suited, instead he framed it as a purely psychological issue (which he has no expertise in and is extremely presumptuous, he didn't offer a single alternative idea or thought beyond 'psychosomatic - which is a useful panacea when a doctor has no other ideas). - Further, he cannot reasonably rule out physical issues that have not been identified purely because one MRI wasn't useful. (Seronegative tests for example are a thing, but generally "the absence of evidence is not evidence of absence", and in this case doesn't mean it's a psychological phenomenon/issue to be vaguely gestured at.) If you have a nice MRI picture with clear evidence of a specific issue or condition, I'm sure he's great. But if you're wading through the uncertainty of a difficult diagnostic situation and seeking help, I would not recommend him. Purely because this is the kind of doctor who's going to chalk it up to an umbrella term and see you out with zero reflection or help. read more

If you like long waits and staff that do not respect your time, this is your place. Waiting 1.5 hours is an absurd amount of time. Does any doctor appreciate or respect that their patients are taking time off work to be there? If the doctor is running behind due to an "emergency", they should cancel or reschedule appointments they can't get to on time. read more

I love Dr. Duke and Dan and had spacers put in that was very successful. But , I would never recommend anyone getting a laminectomy, far worse after this surgery. You will have trouble standing, walking, lifting, putting anything over your head and will have a drop foot that may or may not ever be corrected. read more

While the wait time for appointments & then waiting well beyond your appointment time are the norm for this practice, I am there for the skill of Dr. Forage and the results! Personality wise, it's hit or miss with him. He is more focused on what going on with your body & treatment options than chit-chatting which is okay with me. His surgery scheduler has worked a miracle for me to have my upcoming spine surgery at my pain management Dr's outpatient surgery center where all my doctors will be present and assisting. I feel blessed. Consider yourself "warned" that you will wait for your appointment & then wait some more to be seen, but if you need surgery, you are in the hands of 1 of the best in Las Vegas. Before my consultation I was asked to do another MRI so they could compare it with the previous MRI (I did) yesterday for the consultation the doctor looked at the imagine all the while I was trying to explain the symptoms I have experienced. He was trans fixed on the image of my brain scan. Never looked back or acknowledged my concerns and said that said it was non cancerous and the symptoms I've described don't match his supposed finding. He never once looked the report which stated that I had edema in skull nor the fact the symptoms could be related to the tumor. The consultation in itself was very quick. I had my mom with me and she was dismissed. Before we knew it we were in his associates office and like the consultation was rushed out. I maybe going to the ER because the swelling is getting worse. read more

Dr Duke maybe amazing but I would know because the staff member that called me to make an appointment was not very friendly at all. That's a direct reflection of the doctor, it's sad how people treat people calling for help. I actually told the staff member she was not friendly and she got upset and apologized like it was my fault she was so nasty. I haven't even yet got a call back for a scheduled date & time, but the Receptionist I called that I mentioned Dr. Hanna Hurley requested me to ask Specifically for the Spine & Brain Center in Henderson close to where I live & go to my Cancer Center. The answer I got back said there is no one there at the Henderson Spine & Brain Center Office & I'd have to come to the one on Sunset. She was not very nice about it either with me! I waited an entire month for this appointment and as I was on my way there, 20 mins before my appointment, they called to cancel because the doctor wouldn't be in the office. As I am very understanding at the fact that on call doctors have emergencies there should be other options in place. I have children and other very important things as well, I had to plan weeks ago for today to make this appointment. The next appointment is not even until next month. Being "a busy doctor" shouldn't constitute in failing your patients and treating them like they're not important enough to understand that they are busy too and they have to rearrange their entire life pretty much for doctor's appointments. Just as an example- my OBGYN was on call many times during my pregnancy appointment. Guess what happened? My husband was referred to Dr. Forage after having severe sciatica and an MRI. First of all, we had to wait 1 1/2 hours to see the doctor and there were many patients waiting. We finally saw Dr. Forage and could tell immediately he had an attitude. He was not welcoming and rude. He didn't give us much eye contact and was dismissive and unhelpful. He spent a total of 10 minutes with us. My husband has had years of low back problems and his MRI showed bulging discs in his low back. Dr. Forage told him he thought the issues are in his neck, not his low back, but my husband has not had any pain in his neck. It didn't make any sense. My husband is getting a second opinion and won't be back at this clinic. Very disappointing experience! read more

The sunset office has a Dr Efrem Cox who is very unprofessional. I drove my ma (83yrs old) to the appointment...got there 15min early, as told ...then waited 2 hours to be seen. When she was seen and asked how we were doing , I said "tired of waiting" and then instead of apologizing for the long delay,( which I would have been fine with) he proceeded to threatened to have her go find another doctor instead. That's sad. I was just the driver and she didn't say anything. And yes, sitting in the waiting room 2 hours past the appointment time....I was tired of waiting. read more

Dr Duke is amazing! One of the best! He truly listens to your concerns, pays attention to detail, and takes excellent care of his patients. He's not a doctor who will just operate on anyone. He investigates, figures out what the problem is and explains everything in detail so you truly understand what the issues are, the options available and lets you choose the best method for you. If MRI's or tests are needed, him and his staff are on the ball to get it approved with insurance and scheduled with the facility of your choice ASAP. It's easy to get appts scheduled quickly and wait time is minimal unless its a super busy day, then wait times can be awhile. He makes sure all your questions or concerns are addressed prior to surgery and you will always meet with his PA Dan prior as well and he takes you through every single part of the surgery, everything that is going to happen the day of, the day after and everything that you need to have a successful and knowledgeable operation and recovery. They can make arrangements to have a home nurse/physical therapist come by your place to show you little exercises you can do to help with recovery. They can help make arrangements for anything else you might need for recovery. Dr. Duke has won doctor of the Year many years in a row along with other awards. He's very knowledgable, successful, smart and talented in his field. He was a total life saver for me after everything I have been through and currently suffer from and he has been by my side since helping me. If you want the best spine surgeon, he's the one you want helping/operating. I did my research prior to selecting a spine surgeon as I only want the best and Dr Duke is the best! I'd rather have a blind dog operate on me than Dr. Forage, or as I call him, Doctor Dumb. I waited 7 weeks for an appointment and took the day off of work. I got called 10 minutes before I was about to leave and was told the Doctor's car broke down, he didn't know when he was coming in, and he had ELEVEN people waiting to see him. Now I'm no Neurosurgeon but even a half-wit knows to get your car towed, call an Uber and get your lazy, entitled ass to your job. I was rescheduled for Monday at 11, promised I would be the first person he would see. Had to take another day off of work. I got there at 10:45. At 11:30 I asked how long until I was seen and was told there were 4 people ahead of me. So a lie. Dr. Dumb saw me for a whole 6 minutes. I brought a disc of my x-rays and MRI's, which he never touched. Turns out he never looked at my x-rays at all, and that's what showed the problem. How STUPID do you have to be to not look at the x-rays??? He basically shoved me out of his office. I was told by my referring Doctor to call and hound him to look at the x-rays. Once he did he told me to make an appointment, I need surgery. Thanks anyhow, but I would prefer someone a little less inept, a little less DUMB! I know Dr Duke in a professional capacity so when I wanted a 4th opinion on surgical recommendations I'd received from 3 orthos I knew who to see. I knew there was a very good chance he'd say I wasn't surgical; I wasn't prepared for him to question if I had MS due to my symptoms & MRI findings. Workup/referrals are thorough. He is direct & to the point which I appreciate as I'm the same way. I appreciate his listening to my concerns & answering my questions. Let me start by saying if I could give 0 stars I would. This facility is the worst. If you have an appointment there, run as fast as you can and suggest you look for another provider. My husband had an appt on 12/20 with Dr. Froage. We checked in 15 minutes early as requested for his appointment When we arrived the waiting room was packed. We were in the waiting for about 1 hour and noticed no one was being called. We then approached the desk asking how much longer before he would be seen. The gal said, there was 6 people in front of you so about 3 hours. WTF are you serious! He had another appt that afternoon and we could not wait, so we opted to reschedule. We had to wait a month for his rescheduled appointment. Fast forward to his rescheduled appointment, we arrived 20 minutes early. The waiting room was empty. We waited about 15 minutes before we were called into the room to be seen by Dr. Forage. While waiting we could hear the Dr on a personal call in an office down the hall with his door open and was on speaker. He was talking to his contractor about a home he is building or renovating. Discussions about the hallway and how far it is to the kitchen, bathrooms, his kids rooms and so on. This went on for about 30 minutes and are now 45 minutes past our confirmed appointment. Everyone outside of our waiting room could hear every word of his conversation. So I approached a woman sitting behind the desk in the adjoining office and asked when would he be seen. She snapped back and said, he just got in and if we can't wait to reschedule??? I was floored. So we decided to wait a few more minutes. We could not take it. He continued his conversation about the renovations, then his kids and how he is so blessed and has great kids, then went on to talk about how he has 8 embryos frozen. I again got up to the other end of the office and demanded something be done. The gal at the desk just apologized and asked us to reschedule? That's not the point, you have a patient in excruciating pain at a level of 8-9 who has waited over a month to see you. Even the staff would not interrupt him to tell him he's got a patient in the waiting room. It's now an hour and we've had enough. We got up, logged a formal complaint with the front desk staff and left. I have also logged complaints with the VA community care Veteran center will also be contacting the board of medical examiners on this doctors behavior towards his patients. We have found a new provider and can say we'll never be back. This Dr is clearly abusing the system and his patients and needs to be called to the carpet about his unacceptable behavior. read more

I waited months for my appointment to meet Dr. Duke and Dan, his PA. The day I went in for my consult and to go over my recent MRI - he explained to me what I needed and sent me straight to ER for emergency surgery. I waited a bit too long but so happy I got the right doctor in town after searching and waiting a bit. I couldn't walk for 4+ weeks in my home in bed rest and I am walking, slowly again, I'm so happy! Very happy he knew exactly what I needed and it was needed asap what I needed. I had no idea and reception told me dr is not back from surgery at 2:30. My appointment was at 1:15. There are 7 clients in front of me! Seriously?? I needed a muscle biopsy that can only be done by a neurosurgeon in Las Vegas. I inquired about Dr. Palmer and Dr Forage. I was informed by the Henderson office that Dr.Forage no longer did muscle biopsies. I was told by Kim, Dr. Palmer's MA, that she does do muscle biopsies. They needed my medical records. I drove completely across town and gave them my records to make copies, and five minutes later, while I'm in my car driving back, they call and tell me she can't do the muscle biopsy. Apparently she has only done a few. They told me that Dr. Forage in fact, does the biopsies. So I drove across town once again with my medical records, and went to the Henderson office, where they told me, Dr.Forage no longer does muscle biopsies.. I got a complete runaround. They could've told me over the phone that Dr. Palmer was not going to do it. I'm in a lot of pain with my diagnosis and I need to know if I'm going to live or die. basically, this is terrible treatment for me and I'm in horrible pain going back-and-forth in and out of cars. No consideration at all. When asked if they could recommend a doctor that did them, they had no answer for me. I've called so many doctors it's ridiculous. I shouldn't have to go through this. I know Dr. forage because I worked with him in the hospital. I'm very disappointed in the way they treat people. I had to vent! read more

Went to Dr. Duke for a 3rd opinion on my back. I had been told by 2 doctors that I had to have a L4-5 fusion to fix my back. I spoke to several friends that were medical professionals and they all recommended Dr. Duke. Took a couple months to get in, but Dr. Duke was very nice and professional. He said that a spinal decompression and foraminotomy would solve my issues and I wouldn't have to do a fusion at this time. I was happy to hear this and I scheduled the surgery. I'm 4 months out now and I'm a new person. 99% of my nerve pain is completely gone. My life has completely changed and I'm very grateful for Dr. Duke and his skills. His office can be slow, but they've always been very nice and professional. read more

No disrespect but this happened to be the worst office I have ever visited here in Vegas . I'm a Veteran that was referred by the VA to Dr. Farage's location . When I walked in a patient told me that Dr. Farage overbooks and usually has patients waiting over three hours . I ignored the gripes because I wanted to have an open mind . My appointment was at 1:15pm and by 3:15pm I still wasn't seen . Red Flag at that point if he couldn't respect me as a Patient and my time as well as the other patients there complaining I can't give him the opportunity to even consider him addressing my spinal issue. At least have the decency to tell your patient your running behind. Just a small courtesy. When I told the receptionist I was leaving because I had to get back to work she shrugged and said ok . She didn't even try and say hold on let me talk to the Doctor and get an status update. I called VA and said they should never recommend a Veteran there again. I will make sure my local Congress person knows As well. I rather fly to New York and have real Doctors that value their patients time address this serious issue. Very unprofessional . read more

This is the worst doctors office I've ever been to. after waiting 6 weeks for an appointment, went in and the waiting area was packed. I asked the receptionist how long the wait was. She told me there were 5 people ahead of me. After a two hour wait, I checked again and she said there were 4 people ahead of me. 1 patient in 2 hours...really. The average age of the people waiting was well over 65. Some on walkers, some in wheel chairs, some in neck braces and to wait for hours and hours for an appointment is just wrong. I looked at the reviews for this doctors group and there is a pattern here of 3+ hour wait times. What is wrong with doctors... people are hurting and crippled and they don't think twice about making them wait for hours and hours to be seen by a doctor. l told them I was cancelling my appointment and they asked if I wanted to reschedule. I said no thank you since the wait times were the norm for this practice. Doctors need to quit being so greedy and schedule patients in a way they can be seen without compromising their health and their time. This is ridiculous. read more

My daughter was diagnosed with Chriari Malformation, it is nerve wracking because she will need a brain surgery to have this fixed. Dr. Forage is a blessing to our family. During the day of the surgery, we were on pins and needles, praying for a successful outcome. Our prayers were answered, Dr. Forage is the best thing that ever happened to us. My daughter was up and walking in 3 days, her recovery was the fastest ever. Today was her post op check-up, the doctor couldn't even find her stitches. That's how amazing Dr. Forage is.
