The answer stayed the same -- a phone call first. I asked three times which rule required the call and never got an answer to that question. No records were provided. The letter we received said one amount. The claim filed shortly afterward said another. Neither came with an itemization, and there still isn't one. That's the entire thing I was asking for, and it's the one thing that never arrived.
