I was transferred here from the hospital for rehabilitation after suffering multiple pelvic…read morefractures. They had sent a salesperson to me to market for them who made false statements about how great they are here. Unfortunately, my experience since arriving has been extremely concerning. I arrived at approximately 5:20 PM. Shortly after arrival staff asked if I needed anything and I requested water. The employee later returned and apologized, saying she had forgotten and would go get it. The next time I saw her she was with two other staff members and they again asked if I needed anything. I told them I still needed water and looked at the original employee, who avoided eye contact. It then took another 30 minutes before I finally received water. I also repeatedly asked staff about my scheduled pain medication after arriving. For hours I was told variations of "I'll ask" with no clear answers. Despite arriving in significant pain with multiple pelvic fractures, I did not receive any pain medication until around 9:45 PM, more than four hours after arrival and at thay time is when they done admission paperwork aswell. Another serious issue involved my PureWick catheter system. At one point, a nurse appeared amused by how full my PureWick container was but made no move to empty it. Later, when I urinated again, I learned the hard way that once it becomes too full it will shut off. Because it had not been emptied, it shut off and I ended up urinating in the bed. For a patient with major mobility limitations, that situation is humiliating and should have been prevented. Patients recovering from serious injuries rely entirely on staff for basic needs such as pain control, hydration, and assistance with medical equipment. Based on my experience so far, those basic needs have not been handled with the level of care they should be. Because of these issues, I am seriously considering pursuing a transfer to another facility where patient care and communication are handled more responsibly.

UPDATE: We just received their letter indicating the…read moreoutcome of their 'so-called" investigation regarding my husband's lack of care in their facility. NOTE: I was there at their facility visiting my husband every day, sometimes, twice a day, for a considerable length of time. So, I DID see what was going on. 1. Their claim: "They went in to bathe him four times, but he refused two times." I KNOW this is false because I asked my husband and staff at the time about cleaning him, clean bed linens, and a clean hospital gown. He was bathed only twice in 7 days. (at the time I wrote my review, he had been bathed only once and the sheets had not been changed nor was his hospital gown changed, but because I complained, they finally gave him a sponge bath and changed his gown and the sheets at 6:30 am the next morning). 2. Their claim: "Our investigation revealed that the required therapy minutes by the (missing word?) provided by all disciplines." The following was his schedule: (I have the original schedules of every day except Monday, Tuesday, Saturday and Sunday of days where no sessions were given. Nor do I have a Monday schedule, because it was not planned for his day of release) a. Monday & Tuesday: no sessions b. Wednesday: one therapy session c. Thursday: four sessions (right shoulder was dislocated) d. Friday: One OT session and one PT session & social/exercise group e. Saturday & Sunday: No sessions f. Monday: One PT session g. Tuesday: One session before releasing him If the staff believes that the above scheduling is advantageous, perhaps they might consider having outside professionals experienced in physical therapy advise their staff members. Please realize that if the staff is questioned, they are going to be on the defensive and perhaps bend the 'real truth' Although I have a doctorate, two counseling degrees, and a degree in psychology, it doesn't take ANY degree to figure out that defensiveness, being deceptive, and perhaps changing records is only human nature, when their jobs are on the line. I've even suggested that they consider having someone who is not recognized by the staff do an incognito investigation as to what is really going on here.... with no success. In summary, because my husband was not allowed to leave his bed (loud alarm would go off due to his high risk of falling), he was worse when I took him home than he was when he came in. Therefore, it appears to me that Ms. Harding, Director of Quality Management and Ms. Walker, Chief Executive Officer did not take my concerns seriously but only did CYA form of gestures that were required to keep 'their' jobs.

There were so many things about my mother's experience in early January 2026 to complain about…read more First, in my interactions with the social worker, their only concern was her leaving the facility after Medicare quit paying 100% for my mother's stay without any regard whatsoever as to her condition/ability to return home. They did not want to her stay beyond the 10 days that Medicare apparently deemed a reasonable stay upon her being admitted. They refused to keep her longer despite my mother having excellent supplemental insurance and her remaining on fall alert her entire stay. When I contacted their billing office in Plano Texas, they told me that they accepted her supplemental insurance provider and I would have to discuss the issue with the local admission office. Secondly, they told my sister and I that we would have to care for my mother in one of our homes or stay with her when she was discharged. They never once mentioned the possibility of sending her to another rehab facility until after we requested an immediate transfer to another facility. Thirdly, and most egregiously, someone at the PAM facility advised my mother than she could hire an attorney to prevent us from moving her into a long term care facility or nursing home. They told her we couldn't move her into such a facility without her permission. Lastly, apparently someone at PAM greatly assisted or wrote a glowing 5 star review for themselves on Google the day before she was discharged. While they may have provided good care while my mother was in the facility, she was not in any shape to return to her home to resume living alone. PAM Rehab Tulsa has some major communication issues to resolve and I would NOT recommend anyone checking in here unless it was an absolute last resort.
