Maybe they have turned over a new leaf ... I was surprised at the amount of negative reviews when I decided to leave this very positive one. My 87 year old mother was there as a short term resident (approximately 6 weeks) after a hospital stay. The nursing staff on the 4th floor was responsive and the nursing supervisor Chris, in particular, was so kind to my mom. Other staff on the 4th floor were also kind, which I observed during my visits with my mom. The volunteers, the admissions staff (in particular, Cory) and Ms. Park, the head of Social Services were also really great. I did not have any issues with being able to visit my mom. My mom left there in much better shape than when she entered and I'm so thankful for that result. I can't give 5 stars because I learned after my mom left the facility that they wake up residents at 4am to take blood or other samples if they need lab tests. Apparently the reason for that is to gather samples for when the lab opens. That was the only negative thing my mom had to say about Union Plaza. read more

I am writing this review with a heavy heart and a deep sense of urgency to warn other families. My grandmother has been a resident since April 2025, and our experience has deteriorated into what I can only describe as a nightmare. For months, we have raised concerns about her care. She has repeatedly expressed feeling mistreated by staff. We would often find her with a completely full diaper and swollen legs. When we pointed out small, unexplained marks on her body, staff routinely dismissed them as "old skin that bruises easily," showing no curiosity or concern. Most alarmingly, my grandmother has stated multiple times that she is scared of one particular nurse. My mother witnessed this nurse handling her roughly and responding to simple requests for assistance with a clear and unprofessional attitude. Our concerns about this individual were clearly not taken seriously. The situation reached a critical point this week. We last visited on Wednesday, December 3rd, 2025, and she was fine. When we saw her today, December 6th, she was covered in injuries: a black eye, a busted lip, scratches on her neck and arm, a bump on her head, and widespread bruises. She is complaining of head and wrist pain. This did not happen gradually. The facility's response was utterly unacceptable. No one had reported any incident to us. The on-site manager, Mr. Kumar, claimed to know nothing about it, despite being present over the preceding days. He appeared careless and dismissive of the severity of the situation. We had to forcefully press the issue to even get the name of the Certified Nursing Assistant (CNA) assigned to her during this period--the same CNA my grandmother fears, identified as Kimberly "Monique" (last name illegible, as Mr. Kumar only scribbled it). When we insisted on immediate medical attention, the facility prioritized their internal paperwork over my grandmother's well-being, causing a dangerous delay. We ultimately had to call 911 ourselves to get her to the hospital. This is not just "bad care." This represents a systemic failure in supervision, accountability, and basic human dignity. Clear signs of potential neglect and a resident's expressed fear of a staff member were ignored, culminating in a shocking and unexplained physical trauma. BEWARE of this facility. Until they demonstrate a complete overhaul in management, staff training, transparency, and accountability, I cannot in good conscience recommend them to anyone. Our family is now pursuing all available avenues to ensure this is investigated fully. We will update this review as we learn more from medical professionals and authorities. read more

My mom was there temporarily. Was treated very badly by the 3pm-11pm shift and 11pm-7am shift. Even when she asked nicely for a glass of water they had attitude and never got her the water. Worse experience I've ever dealt with. Horrible. read more

Nurse Stella is my husband's nurse at Union Plaza Care Center. She takes very good care of him. She is a very dedicated worker and is always so busy but still finds the time to tend to my husband. She always explain anything to me if I don't understand. I am really so happy to have her here. read more

My Mother had a stroke and was admitted for only ONE DAY and fell off the bed injuring her head and was sent to another hospital! My Sister in law had told a staff member that my mother needs a bed with railings to which the staff member replied "Everyone are all in the same beds!" STAFF DOES NOT CARE! I had called the front desk and was told she was sent to Queens Hospital in fFushing but in reality it was Presbyterian Hospital of Queens! Total incompetence! back to say after my grandmother died in the hands of the employees at union care plaza and LARISSA, they tossed her belongings in TRASH BAGS for us to pick up!! just goes to show how much they actually care about you. when there are SO many negative reviews and a bunch of fake 5 stars that were written by the owners themselves. was only allowed to videochat my 90 something year old grandma who has ABSOLUTELY NO understanding on videochatting. her aid was also impossible to reach and honestly barely spoke english so it was VERY difficult to communicate with her. whenever i spoke to the nurses they were always talking as if theyre in a hurry. i 100% get that but when ur coworkers are FORBIDDING family to visit their loved ones and the only means of communication was through a impossibly difficult videochat, you should be more patient and understand where their concern is coming from. it's clear that no one here wants to deal with you and only want $$$$$ from you and from your insurance/ the government. the physical therapist and nutritionist were nice and great communicators who took the time to answer any questions, keep me posted, and were always reachable. i applaud them and wish everyone was like them. unfortunately that is most definitely not the case. PLEASE GO ELSEWHERE THIS PLACE IS NOT WORTH IT FOR THE SAKE OF UR SANITY. Went to have a tour. The admissions person was rude, unhelpful and mean. If this is how they treat you when you just wish to have a tour... No thank you. I don't know how the rest of the staff are but I'd skip sending anyone here given this person's rude behavior. Maybe she was having a bad day, but don't take that out on me. Yes, the HEAD nurse had the audacity, in less than an hour, to come up to one of my family member with a binder full of people waiting to get in here and tell us to get our grandfather out (when he had passed away a few hours ago) so they can put a new patient in. We just lost someone and you're seriously going to open up a binder in front of us to show us there are patients waiting to get in. This place is completely unfit to take care of anyone. Don't believe any positive reviews on Google or here on Yelp. They are all fake. My father was admitted to this place and was treated like dirt. They withheld nutrition from him, ignored all the hospital discharge information, and was rude to his loved ones who were trying to advocate for him. Was given a slew of excuses for not providing the right amount of nutrition or providing the right equipment for my father. They did not follow procedures on his well-being but chose to follow procedures on other things, like not allowing him to simply walk down a hall to exercise. We were hoping a place where there were more Asian speaking nurses would be helpful to my father but instead they were rude and uncaring. How can anyone leave their loved one here? It's straight from a horror movie. The place smelled of stale feces. I would like to give credit to one nurse supervisor who did try to accommodate the needs of my father after a verbal back and forth. But everyone else there is broken and sick, they do not care about the patients and they don't have the socioemotional well-being to be respectful to humans. read more

My father has been living in Union Plaza Rehab and Nursing Home for about half year. He had strokes and unfortunately unable to return home due to a lot of complications from strokes and he needs medical supports everyday. He stays on second floor where the supporting staff members, clinicians and the doctors all are very kind and helpful. I would have to say that this is a safe place where the residents are being taking care by good people and the family members don't need to worry about day by day for the patients how being treated. Although my father has swallowing issue, they help to feed my father carefully. My father has a walking problem that he often stays on his bed. But the supporting staff has no hesitation moves my father back and forth from bed to the wheelchair for eating or taking some exercises out from the bed. The doctor is reachable and very responsible. The Nurses are lovely lady who give me details about my father. The facility had asked me to participate several phone conferences in addressing some safety concerns for my father. And I am glad that they care and telling me how my father is doing in the facility. The clinicians learnt from me for more about my father so then they take initiatives to help him, and to protect him. Dr. Jaglall, Dr. Lin, Nurse Ms. Jasmine, Nurse Ms. Ding, the Speech Therapist Ms. Abigail, the Nutritionist Ms. Teresa, the Social Worker Ms. Jane, the Risk Management Staff Member Mr. Joe and the supporting staff members on the floor, all of them are amazing people and they are the good team helping my father. The team is very professional. And in this pandemic, the facility uses staff members in supporting facetime, helping my family members to see my father. Without facetime, honestly we would be very frustrated and don't know how he is doing. May God watch over the residents there. Thank you Union Plaza. read more

DO NOT SEND YOUR LOVED ONES HERE. My father in law got discharged from the hospital and sent here. Huge mistake. Just on the first evening, the staff ignored the hospital's discharge instructions and would not give him the proper nutrition or the medical devices necessary for his healing (which was explicitly stated in the hospital's discharge instructions). When we requested these things, they were incredibly rude and fought us on everything, initially claiming they didn't have the equipment, then bringing equipment that didn't work, then finally bringing working equipment, but couldn't figure out how to turn it on. And they could not reach the dietician who had decided to ignore the hospital's instructions and provide her own. The facility, especially the room smelled like excrement. The nurse would not allow him to tube feed himself as he had done at the hospital, told us that we couldn't even be present when he was being fed, and refused to feed him if we were there. The same nurse said that he was not allowed to walk the halls for exercise as his hospital discharge instructions explicitly stated, until a physical therapist saw him the next day. When we asked what would happen if he did walk around, the nurse said that he could ignore the nurse's request for him to go back to his room. In short: Rude incompetent staff who will ignore or fight hospital instructions and family requests. And disgusting facilities. read more

Great care to my grandfather. Everytime I visit there is a lot of care being given to him. During meal time there's people always checking up on him making sure he's eating and isn't hungry whatsoever. I don't understand what the other reviews are talking about but I love this place and the staff there. Highly recommended to go here. read more

Ralph H. In a perfect world, we would not opt for a nursing home for our loved ones. We've all heard the horror stories. The facilities were fine and well kept. But the staff was truly exceptional! At all levels, including building maintenance/service, medical/nursing, and social work/administrative: my mom was well cared for and spent her final years in a safe and loving environment. This is the result of having decent, caring empathetic staff. I will be eternally grateful for the kindness given to my mom and would readily recommend Union Plaza for those in need of such care! read more

Not sure what the others are saying but my grandmother was admitted to this nursing home and the staff members are great! The speech therapist there is highly educated and informative with her work. The OT/PT department are excellent with their job and will work hard to have their patients recover. The social workers were very helpful with having my grandmother obtain a HHA and CHHA services at home. The nurses and CNAs treat their patients as if they were their own family member. If I was old, I would beg my children to admit me to this nursing home. Thank you Union Plaza! Please do not read the other reviews, they have no idea what they're talking about. read more

My mother had stayed at Union Plaza for a couple of years. She was well cared for by dedicated professionals until the last day of her life. We are so fortunate to have found Union Plaza; it's clean, safe and the staffs are friendly and kind. We never had to worry about my mother's well-being during her stay as we were confident that she was getting the best care. One thing we love about Union Plaza is that we were kept informed of everything that we needed to know and were involved in every decision-making. Nurse Stella and Nurse Marilyn are amazing professionals! They always went above and beyond their job descriptions to provide loving and warm care to my mother. My mother looked happy every time we visited her. We can't thank them enough for their dedication! We highly recommend Union Plaza! My mother was interned there before her passing some 10+ years ago. We thought it was OK. Then, recently my uncle (her younger brother) was entrusted to them. My uncle seemed to be OK during his stay. But, one incident I experienced was totally ridiculous for a caring place for the weak and helpless. During much of the day, most residents are gathered in an open lounge area. One morning, a month or so ago, there was this 1 female resident complaining of the need to relieve herself in the bathroom. So, I started asking the attendants on the floor. There were 3 women there. 1 of them I thought may be of the Caribbean extract. I kind of moved this resident up to her, saying that the patient needed to go to the bathroom. This attendant just showed me indifference. She didn't respond, and didn't move. So, I started to raise my voice, as all 3 of them were present there. STILL, none of them did a damn thing. A senior would not be able to hold his/her urine easily. These attendants just showed indifference. Pathetic, which was what came to mind. I subsequently talked to 1 of the floor managers (the subsequent Saturday morning), named Stella, she told me. She said she would've like me to look for 1 of them to present my case..... But, I said I looked all over the complex, I didn't find a responsible person to discuss the situation. We were on the 3rd floor, and I even went to the main floor, to the reception area, only to be told that there's no manager at hand..... That's our real experience of the place. read more

Attempted for nearly a month to find placement for a Long Term Hospice Male patient. Deal with THE only Admissions person: Frances Detres, who on a good day, recalls details, but on a bad day, doesn't know which case you're referring to. Multiple follow-up calls, endless faxes with no results. Finally took matters to Administration and within an hour, Frances calls back saying "the nursing home does not have an available spot...we're just not equipped to provide the care needed." Total BS...because nursing homes contract with Hospice Agencies, who then go into nursing homes and provide all care needed, all nursing homes do for Long Term Hospice patients is provide room & board. This nursing home may have potential, but the Admissions office needs to hire new blood. read more
