I had driven a 5 hour drive that day and my leg was just not feeling right when I arrived home. Better safe than sorry. This review is about the high points and low points of that visit. Every single one of the medical professionals with whom I came into contact were phenomenal. I am talking the doctors, nurses, u/s technician. The time arriving and departing was appropriate given it is an Emergency Department and I was not in any urgent medical state at that point. I interacted with two registration personnel during my time there. One when I entered the ER and the other at some point during my stay regarding further insurance information. These two employees were the most unprofessional people one could never hope to come into contact with during an emergency visit under any circumstance. It was like they selected the least customer savvy employees they could possibly find to put in critical positions. I was truly agast! Rude, barely able to put a grammatically correct sentence together and not an ounce of compassion nor kindness for the types of patients they were serving. I watched them as I waited demean patient after patient in utter shock. Now... there is no way Mercy Emergency Department management is unaware of these , pathetic, unskilled employees wreaking havoc on sick and, in some cases, frightened patients. There is no world where this would be acceptable behavior. I am certainly glad I am not a Dignity Health plan member. My Health Care Provider would never hire employees like these. What were they thinking? I have read Mercy's responses to some of these Yelp reviews. Embarrassing in my opinion. Better to not respond at all than write pathetic, patronizing dribble. Do better Dignity Health.

    My mom was recently transferred to this facility from our local hospital, where she had been an…read moreinpatient. The facility is approximately two hours away from our local hospital. I am listed as my mom's advanced directive and have been identified on all of her medical records and paperwork. When my mom's transfer to Vibra was being arranged, Sue advised me that I would receive a phone call once the transfer was completed. Unfortunately, that never happened. I had to call the facility myself and speak with a nurse, only to learn that my mom had already been there for approximately an hour and a half. I was extremely concerned and disappointed that I was not notified as promised, especially given my role in my mom's care. The following day, I called the facility again to check on my mom. I was then told that I was not listed on any of her paperwork and that I would need to wait to speak with the case manager. I contacted Sue again because I was frustrated and in disbelief. These were important assurances that had been given to me when we were deciding on this facility, and they have unfortunately not been followed through. Today, the situation became even more concerning. My mom told me that she had been experiencing severe chest pain and had been waiting approximately 40 minutes for a respiratory therapist. I called the facility myself and spoke with a nurse. It was only after my call that my mom was given 25 mg of Tylenol, repositioned, and suctioned. I was also told that her ventilation had to be increased after she had already been experiencing chest pain for that length of time. I am struggling to understand how this level of care is acceptable. If my mom is experiencing severe chest pain and requires respiratory support, why did it take approximately 40 minutes and a phone call from me for someone to respond? Why was her pain not addressed promptly, and why was I not notified about the situation? I am utterly disappointed and deeply concerned about the quality and responsiveness of care my mom is currently receiving. I trusted the information I was given when we selected this facility, and my experience so far has not matched those assurances. I am asking for a clear explanation of what happened, why there have been repeated communication failures regarding my mom's care and my status as her designated representative, and what steps will be taken to ensure that she receives timely and appropriate care going forward. At this point, based on what my family and I have experienced, I cannot in good faith recommend this facility to anyone. My mom deserves better communication, better responsiveness, and, most importantly, safe and attentive care.

If you're considering this location for your friend or loved one, go back to the drawing board and don't bother accepting Vibra in Folsom as a viable option. Based on the professional trust that I instilled in the doctors, coordinator, and medical team, I had incredibly high hopes especially after speaking with the NP. In the past, the location was known to be one of the best, but that has drastically changed perhaps due to change of ownership as you'll notice the success stories published are all from at least five years ago - nothing current. Although the unit clerk provided me an excellent tour and the admission coordinator super sold me on all services and level of care, what we experienced was the exact opposite. Our decision to move my spouse here was certainly proven wrong within the first few hours of admission. Here were the initial red flags: 1. The case manager asked if I was going to visit daily. As the patient's spouse, her intent of the question and tone was incredibly confusing. 2. Upon arrival in the assigned room, the wall clock had a chip and displayed the incorrect time. 3. The patient's board was not updated with the daily care team members. 4. Every time a care team member entered the room, they did not identify themselves or their role. Their badges were all backwards so that they had to physically turn it around to actually be seen. 5. In the general hallway, I found what appears to be an injection tube just sitting there (cleanliness? Infection control?) 6. In the cafeteria, the vending machine dispensed expired soda (this is probably managed by a vendor but brought it to the attention of the cashier). 7. The care team charted notes that were very different from what was observed/experienced. 8. Before admission, they stated there is a neurologist on site. Not true - they're recruiting for the position and only found this out upon escalating concerns once fully admitted. 9. Due to the level of care, there is no ombudsmen (a term the medical staff were apparently unfamiliar with), but they do have a "Patient Care Ambassador." Such a position does not exist - they have a dedicated office but when it was time to request for their support, there was no one to confide in. 10. The charge nurses (identified as Jenny and Harjas) did not know protocol when elevating safety and care issues. In fact, their subordinate had to coach them on the spot and actually took a greater lead role in the situation. Although the insubordinate lacked customer service and clear respect for management, at least she took immediate action. 11. A nurse who goes by "Jenn" but actual name is "Jennifer" did not follow HIPAA requirements. That feeling that you get in your gut that something was truly wrong was eventually proven after observing a student nurse under the guidance of the supposed RN completely infuse the incorrect feeding tube rate leading to life threatening consequences. The worst part was no one wanted to take ownership and a handful of hours later a doctor finally made his way to the bedside. I doubted some of the initial reviews posted previously, but they were fully spot on. If there was anything good, these are area of kudos: - RT (probably the main ones responsible for keeping this place going) - they do their rounds and truly show compassion and care to the patients. They seem to truly enjoy their work and were willing to inform family/visitors. - PT and OT - truly do their due diligence when possible. They were observed making rounds and motivating some of the patients. - The cafeteria offered fresh food for I believe $5. - Plenty of free parking available. I'm not looking for perfection - there's good and bad in everything; however, my family and I lost all trust and confidence in Vibra in Folsom due to the incompetent staff, inconsistency in policy, and simply lack of care.

Staffing appears to be understaffed, with some dedicated caregivers and some who seem to be there to act as prison wardens, or sanitation workers who regret their status in life. Case managers are clipped, and not easy to communicate with, and my spouse was seldom ever offered insights by real nursing staff. She was quickly fitted with a catheter, apparently to save staff from having to help her to the bathroom and back. Physical therapy was hit or miss; in the first two days she was eligible, and was skipped by the staff. One of the physical therapists was probably 100 pounds overweight; and the physical therapy she did receive was minimal at best. The physical therapy department is not open on the weekends, so Saturday or Sunday PT is not a possibility. Food was passable to worse. My spouse was in a shared room with another client, a woman who loved to have her TV on late into the night with the sound at 100 DB - never did Staff come by to suggest the sound be turned down - giving my spouse earplugs which were little help. And result of the substandard nursing environment was that we checked out early, to the chagrin of several on the staff, who said "we were leaving against medical advice". We were pledged a referral for home-based nursing, but it took several phone calls and several days for Staff to file that paperwork. Overall grade, D. We would not recommend this place.

The staff has been incredibly warm, kind, patient, and hardworking, and it gives our family tremendous peace of mind knowing he is in good hands. Billy, Marsha, and Cassie have been especially wonderful about keeping us but the entire care team deserves recognition. They genuinely take the time to get to know the residents and make them feel comfortable and welcome. They even know my dad's favorite breakfast--two eggs over easy, hash browns, bacon or sausage, toast, and fruit! They make it for him every morning. And they have been so warm and welcoming to my dad's dog when she visits. They have a great outside courtyard where dad and his dog Coalee can play and enjoy the comforts of home. The physical therapist assigned to me was very knowlegeable and listened very well to all my aches and pains. Unfortunately, my rheumatologist only specified my knees, so there was where my PT concentrated on. I have been to therapy 3 times already (of an expected 6 treatment span). So far so good! My range of motion has improved, the pain is still nagging me a little, but I know it'll all get better with time! The place is very nice and clean. Inviting. And the staff are all above par. The treatment areas are all well-lit. And the exercise machines and other equipment are amply spaced...a lot of breathing room here. Even on their busy days, there is still a lot of room available for the patients and the staff. If you have the need for physical therapy and you insurance allows, Burger Physical Therapy is a great place to go!
