This facility was a nightmare for my mother. From the time she arrived til the time she left. Laticain patches put on my moms upper back when her injury was on her lower back. After bringing it to the nurses attention her response was "oh well I didn't know where it should go". 3 days before it was placed in the proper location after numerous requests for this to be corrected. One day I noticed a rash on my moms lower back as the nurse was removing the laticain patch. I asked my mom how long that was there she said a few days. After I requested no more patches and cream for the rash, they still put one on the next day. An appointment was supposed to be scheduled for my mom to see a Urologist 1 week after she arrived. It wasn't ordered until after that time frame and then the referral was sent to the wrong medical facility which delayed the appointment another week. The end result was my mom getting a UTI from her catheter was not removed or changed out in a timely manner. Laxatives were given to my mother even though she had diarrhea for 3 days. After requesting that it be cancelled they still gave it to her the next day "because it was on the order". My mothers blood pressure tanked and they still gave her high blood pressure medicine. They were putting a cream on her chest for a rash she didn't have. I had to monitor all of the medication that was disbursed to my mom. My mom was on thickened water due to a choking hazard from some medication she was on. Her food also had to be chopped and couldn't be dry. This was "in the orders" yet not followed. Two of the nurses made it very well known that they didn't like me and couldn't wait for my mom to be released. read more

If I could give Folsom Care Center 10 stars, I would. I have never met such an amazing staff, and everyone treats you with such kindness, care and love. Our Mom came in for long term healthcare, and unfortunately, she ended up in hospice. We have been here day and night, and the staff has been the most compassionate people. They are always sweet, helpful, loving and caring. I would not want my Mom to be anywhere else. Special call out to Jackie who got Mom in to the facility. And to nurses Stacy and Sava and to Sonia and Victoria. You have made us feel like family. You gave love to Mom until the very end. My sister and I cannot thank you enough. read more

My mother just completed a 2 month stay at Folsom Care Center. We were EXTREMELY impressed with the staff and facility! Staff was caring, friendly, and knowledgeable. The in-house PT services (provided by Burger Rehab) were excellent, and provided the therapy that Mom needed to get back on her feet. I recently had surgery and was provided skilled nursing and rehabilitation therapy at Folsom Care Center after discharged from the hospital. It was the worst of time and the best of time. I fell and fractured my pelvis. The most painful experience you could ever imagine. The staff was dedicated, patient and remarkable in their care for me. Physical and occupational therapist gently nudged me to start moving and getting me out of bed. There was some swearing in the beginning but with their skill they prevailed and I am forever thankful. My family asked their doctor friends for a recommendation. I can say, magnificent, wonderful, kindness beyound compare. Iwas there for six weeks while recovering from a fall. My upper lumbar was injured and I was in tremendous pain and hardly able to walj. The healing process within starts from the moment one comes to them. I can name names like Ednathe day CNA that, works from Joy in her heart daily to wae me up, get me dressed ato eat breakfast. So when their outstanding Rehab staff of physical therapy n occupational therapy staff arrive, I am ready... Like it or not.. Or feel like I can't.. I go. Remakable each one. Uplifting to the body and SpirThe daily nurse that cared for me everday with remarkable and genuine Loving Souls supported my return to heath, strength and courage. I am, as I said, at the start here deeply grateful for each and Every person I have had the gift of receiving care from are Emensely Outstanding. Genuine, caring. Amazing. Gloria, the recreation director here welcome d eveveryday to the new day over an loud speaker. The, care, healthy carefully planned meals, and snsckes were delivered everyday with amazing people. My dad was sent here following a Mild TIA and a few days in the hospital. Because of COVID he was restricted to his room for 14 days (a good precaution) but they took excellent care of him. We were able to visit with him outside his window and talk on the phone and the staff was wonderful to him and us. We were in the middle of moving my parents to an assisted living place and when they heard where he was they said "that's one of the best care centers" so that was good to hear as well. read more

I've meant to leave this review for months and am finally getting around to it. It was necessary to do so because Folsom Care Center was so kind, loving, and professional to my mom that their care accelerated and made the difference between her getting better. She is elderly, a lovely woman, and needs the kind of attention they give her. Like most facilities, it takes a few days to settle in (get to know each other and the family,) but once that took place, all was good. The staff frequently dropped in, took our feedback, and addressed it. There are always areas for improvement, a few items of clothing were lost on wash days or responsiveness to "call button pushes" took longer than usual, but we felt that it was a relationship in these situations, and FCC did their best to address and follow-up with our concerns. I should have written this review in November but better late than never. My mother spend time here twice. I will say that it may not be the "prettiest" facility, but my mother (& I) had been treated VERY badly at a "pretty" facility. Folsom Care staff treated my mother and myself as personal friends or family. My mother loved the Care staff and they seemed to love her. I noticed that it didn't matter who specifically was assigned to a client, if some thing was needed or requested they did their best to fulfill the request. If a client/resident needed extra attention or redirection, they took turns so no one staff was unable to continue their assignment. We offered suggestions for her menu and they updated her chart. My mother "lost" her dentures just before she was being discharged and the staff mobilized to help locate them. There were 5 of us outside going through the lunch trash! More staff was inside scouring her room. (She had taken them out, wrapped them in a napkin, and thrown it into the bathroom trash) There is an interior courtyard with tables and chair which is a very pleasant change of scenery for people who have been stuck inside for long periods of time. All of the interior rooms have courtyard views. Most exterior rooms have outdoor views. Only a few look at the parking lot. The bottom line is the staff and Folsom Care staff tries hard to be available and helpful and properly trained. I was comforted knowing that they were caring for my mother and that allowed me to function in my own life. read more

Where do I begin. First, I followed in my dear Mothers footsteps and I have done hundred of hours of volunteer works in all sorts of different kinds of care centers ranging from Assisted living through Skilled nursing. More recently having had my Rat Terrier Dog, Kira certified through the Lend a Heart Program in Sacramento to do Animal Assisted Therapy (we are both retired now). My Loved one is 83 Years Old. He was otherwise Healthy ENough to undergo a 4 Level Laparoscopic Back Fusion. The Surgery went off without a hitch, but no where in any of his pre surgical consults did ANYONE Ever mention that people over 65 can suffer serious side effects of memory loss (it apparently is called Surgically Induced Delirium. He went into surgery sharp minded and woke up like he had dementia. It was just awful. I wont go into all the details its just a good thing I trust Sutter - (This is the place to be if you have such a crisis) The only information got for days was from friend and the internet.Then I was faced with 'choosing a care home. WHat to do? I had read the reviews on Folsom Care center, they had a bed, I chose - and we got the lottery! I could not be more pleased. Hands Down Folsom Care Center is the BEST Skilled and Short Term Rehabilitation Center I have ever been in. I have been in Private facilities that don't even come close to comparing the the quality and attention of service that I experienced there. Katherine H, his Nurse, was exceptionally attentive (She is simply AWESOME) as was his entire care team (I will come and add names later, as I am using a friends computer but I wanted to get this review up and running asap). We got the Lottery when we were able to Place my loved one there when an unexpected turn in a surgical procedure requiring to have care and rehabilitation and services of a skilled nursing facility. I have been power of attorney and have been in the role of handing healthcare matters before, but things were complicated because of my personal life matters right now - I myself have been hospitalized for serious health issues of my own twice in the past month, and I am the ONLY relative he has, I am run ragged myself. and this situation was completely unexpected. I have also been 'displaced' from my living situation in the middle of all of this due to a legal separation so my plate is overflowing. What was supposed to be 'house sitting' turned into much much more. Normally, I would have been able to weather much of this (I come from the midwest, we are tough people. But this situation was just 'too much' being sick and all. The staff at Folsom Care center went Above and beyond to take over for me and provide not only the quality of care my loved one needed to get better - but to get me some 'rest' I needed to focus on myself getting well so I can continue to care for him, and get my own issues taken of. They are just amazingly caring. Not only that - the first thing you notice when you walk in is EVERYONE SMILES. WOW. I have never seen this even in expensive high end facilities (remember how I started this - all the volunteer work I did - I DO Have a really good basis to make an accurate comparison) He is home now for a few days. At discharge they reassured me if anthing went amiss, to return to them (I did not even know this was an option instead of the hospital.) I cannot thank them enough for thier caring approach to helping him, but also guiding stressed out over the top me through all of this. THANK YOU THANK YOU THANK YOU to the entire staff and crew. Oh, one more thing, They have an aviary for the Residents. Its just lovely. They keep finches and doves in a clean, lovely enviroment. Its bright and cheery. The birds are even sitting on eggs and raising babies! Its so sweet. Attached is a picture. The residents are well dressed, its a small place with a lovely outdoor courtyard. Everyone that can is encouraged to move about and socalize. The short time he was there, there were many enriching events I witnessed, even a group of young children coming in to visit and sing songs, a birthday song to my loved ones 93 year old Roomate. Its clear they care deeply about the people there - the ladies are dressed nicely even with lipstick and jewerly and the men the same in matching outfits. This is a far cry from some of the places I have visited. IF there were 20 stars it would not be enough! Thanks you Folsom Care Center for being there when We needed you and to all the staff for what you do each and every day to provide a high quality of life for the people there. Five star skilled nursing facility, and they have five stars on their quality measures. This has been a family owned business for 50 years they are kind and generous to the employees and the community. The employees are happy and provide compassionate care. Unfortunately, we feel bad that the team is so awesome, BUT, the doctor is so horrible that it overshadows the great care given. The doctor (if he is one) changes medications without knowing the patient, never speaks to the families and has NO clue how to write a prescription. After 5 times back after our mom was released, we finally got the 'doctor' to write the prescriptions correctly so that we could get them filled. Mom was without heart meds, cholesterol, pain and breathing meds for 5 days. The pharmacy was shocked and kept sending the prescription back with me since they could not reach the doctor! Dr. Gut man needs to go back to medical school!! read more

I was an Air Force medic and civilian CNA for almost 20yrs. The Folsom Care Center is THE best skilled nursing facility I have ever experienced. My Mom has been a patient numerous times over the past few years and is always cared for as if she is a member of the family and thoroughly enjoys her stays. I've noticed that the staff has a low turnover rate which is quite rare for this type of facility, truly shows their dedication and love for what they do. My Mother is a new resident of Folsom Care Center about a month. 30 years ago when I came to USA from VN , I worked as a CNA at many Skilled Nursing Homes including some places for rich and good insurance people , the service at Folsom Care Center is one of the best, the facility is clean and the staffs are very nice and friendly, especially My Linh the physical therapy assistant , DJ the LVN, and many others good CNAs such as Melissa, Elizabeth,etc. I can't say enough how much our family appreciate their kindness toward our mom. My brother even said: " When I am old , I don't mind to be a resident here" . He used to be scared of nursing homes after my father passed away in a nursing home in Orange County. read more

My mother was sent here for a quick stay after her hospitalization in Mercy Folsom. The transition was fair, as I took her personally. However; the follow through with her was terrible,until I complained. My mother was given a tray of COLD food ( should have been hot) that looked like it was canned dog food. She was offered a plain tuna sandwich that she ate quite willingly as she was very hungry. I'm telling you folks there is no real control on the situation with the food. Very lacking. UN edible breakfast also. We checked her out the next morning. I had toured this place and it was clean and did not smell or anything like that and I did not see the seniors warehoused in the activity room which I thought was a good sign. But poor attention to the quality of meals really ticket me off. She is doing quite well at home with help. read more

My mother came here after a hospital stay and was here for 6 weeks. I cannot recommend this facility highly enough! My 96 year old mother spent 4 weeks here after hip surgery. This facility is not 'fancy', but it is clean and the staff here is wonderful. I visited Mom regularly and each time was impressed by the skill, care, concern, and consideration that was shown my mother by each staff member that interacted with her. I was kept informed of my mother's condition and any changes. If my family is ever in need again of a rehab facility I would not hesitate to use Folsom Care again. read more

The Staff here is excellent but the doctor is questionable. Since we are the ones who are responsible for the care and treatment our Parents receive it is our right to question the medication changes and treatment plans identified by the doctor who makes these changes. Dr. Gutman has placed our mother on a DO NOT ACCEPT because I questioned him changing medication when he had no idea what our mother's baseline is or anything about her prior treatment before being admitted to this facility. He doesn't return phone calls to family members inquiring about these changes and has no idea how to properly write a prescription. My sister had to return to the facility on three different occasions because he could not take the time to write the prescription properly. When our parents come to need a facility such as this it is our DUTY as responsible children to question everything that is out of the ordinary and get the answers to those questions. I am also writing a letter to the State regarding this incident as I don't feel anyone else should feel they have no say in what happens to their parents once they are placed in facilities such as this. OUR PARENTS NEED US NOW MORE THAN EVER to ensure they are cared for properly. read more

My rating is 4.5+. Our 96-year dad stayed for over two weeks from early December through 12/22, and we local family members were impressed with the services and attention he received. Sure, the facility is not the most expensive looking place, but we have experienced those other places first hand, and after you get behind the fancy lobbies, the care in other locations was often sub-standard. Folsom Care Center really cared about our patient. It's family owned and the owner is on site; the director in the front office made time to give us a tour when we dropped in. Nurses arranged special meals for his condition, they found a recliner chair to put in his room so that he'd be more comfortable napping, and they kept me as the primary family member apprised of anything we needed to know, often before we knew we needed to know it. Julie, his lead nurse, was the best, and all of the nurses and aides in her wing were great. Physical therapists and occupational therapists were also wonderful. The only downside is that the parking lot is small, and one often has to park on the residential streets near the facility. But that was manageable; we always found a spot, days or evenings or weekends. I would take any family member to Folsom Care Center. read more

My husband has been a resident of Folsom Care Center for a little over 2 years now. For the most part, it is an excellent facility. Staff is very caring and friendly. The facility is clean and fresh most of the time. The only fault I find with this center is that the CNA's need to be checked on more often, to check on how they are doing their jobs, and taking care of their people. Every 3 months they have a meeting to voice any concerns, or questions, which I think is a marvelous idea, but it seems as though I am repeating myself at these meetings too often. Like I said, the staff is wonderful, but I think they could pay closer attention to their charges, and make sure that they are cleaned properly, and made presentable for any visitors who might drop in to see them. I have found my husband with his hair disheveled, shaved, kind of, clothes hiked up his back, slouching in his chair, and crusty eyes. Now I know this isn't much, in the grand scheme of things, but I feel that the CNA's could take a bit more time to make each person presentable. Most of them have lost enough, lets, at least, give them their dignity. read more
