My Father was sent here for rehabilitation now he has dementia. He was sent here to get a little stronger. He was to do rehab everyday however, it was not done. They told us my dad had behavioral problems on one occasion, we found out later that was all a lie. One good thing is he had no fall while at the facility. Everytime we came my father was soiled a few times we ourselves showed him changed linen and cleaned room. He wasn't giving water when we wasn't there, we know this because day of discharge he was hospitalized for dehydration he had a seizure which the dehydration triggered this. If you send your loved one make sure you to make your presents known and stay on top of the staff. Off the experience I would not let my loved one go here again. read more

The only reason I'm giving a five star is because of Ms. Jeanette Davis and staff. Without ms Davis being a social worker that place would be lost. For example I had an issue today it's been an on going issue. I spoke with other staff and it just wasn't getting handled to the necessary standards. Not on her watch tho because that woman goes above and beyond to fix the issue asap. She takes her time to listen and helps accordingly. The staff is really nice and try their best to accommodate. People come here to get rehab & expect the food to be of a decent quality...NOT so at all. I don't know how the "chef" had the title. 5 of 7 days breakfast usually scrambled eggs of some sort, lunches and dinners were not much better. I'd easily say 98% of the time the food was cold!! No food means no protein you need for healing. If you have a good CNA they would heat it up but that didn't mean it tasted good. So often it was not what it was supposed to be and tasted horrible so visitors brought food. The wound care team was awesome, thoughtful and answered questions!! PT/OT were awesome as well! The CNA's & nurses were usually very attentive; always 1or 2 were apparently there just for the pay. Surprisingly (or not), CNA's didn't care they weren't supposed to be on the phone, it was no surprise to a couple ladies walk in talking to their earbud but it happened often on the 2nd shift. As they walked in the room I could hear them say hang on and then ask what I needed then say "I'm back" as they walked out the door. HIPPA violation....very possible. I'd agree on other reviews on the parking lot! Often the handicap spots were taken by day shift employees and the parking lot is a weird design. No wonder they offer "free" valet parking. Activities workers were nice but not enough activities offered; If a resident has morning therapy they miss the activities and NONE offered after 4 pm...then boredom. BE CAREFUL with Jeanette from Social service...likes to not give you all the information at once, she has too much on her plate, the other two women are easier to talk with and explain things much better. Dr. Singh, Dr. Robby, & the NP for each do what they say they will. Stay away from the "hospitalist" (a tiny little woman) that only shows up in the evening once a week. Definite SHOUT OUT to Guest Services team....always friendly, asking if I needed anything. Reception team was friendly. 2 of 3 social workers were fine; 3rd I'd not communicate with again. Damien in Administration was helpful. Overall, the so called chefs need classes on eating with your eyes & TASTE! If you plan to have a loved one placed at Warren Bar, considered yourself warned. read more

After 3 days of calling trying to get a hold of Social Services we finally were able to reach someone. The receptionist just kept transferring us to ringing lines and voicemails with no introduction other then Director. We certainly haven't been impressed with this facility. read more

If I could give a negative star, I would. Where are the nice rooms that are advertised? I saw a glue mouse trap behind my mother's old raggedy dresser type thing. The toilet seat riser has fecal material on it and there are no garbage bags in the garbage can. Last week when I came the floor was so dirty that I swept it. Where are the towel racks? All I see are towel holders. The pictures show nice window treatments. My mother's room had these raggedy old ass wrinkled curtains. And why after they were told that family would do the laundry did staff keep removing my mother's clothes. My mother deserves better. She didn't raise me in this type of environment and I don't want to see her in it. She's lived a long life and has treated others with kindness. I'll be bringing her home with me to continue her recuperating. read more

Every part of care and rehabilitation helped me to walk again after spine surgery. The doctors, nurses, therapists, aides, etc. were very professional and caring in all departments. I am extremely pleased with the care my father-in-law is receiving at this facility. The nurses and CNAs are doing a good job making sure he get his medications on time, keeping him clean and moving as much as possible. When he got there several days ago, he would only lay in bed and sleep. Now he is eating more, setting up and watching TV and talking to family. Please please please don't take your loved one to this place, my father was there for few days for rehab, they don't answer call light, they left him soil for a long time, food is cold, no showers nor bed baths meds are not given properly or missing meds, no privacy at all, every time I ask someone a question the answer would be I don't know Im from an agency, so basically temps agency employees run this place.when you first enter you will notice the loud employees chitchatting and laughing ignoring the call lights from the patients in the rooms. I went to wash my hands and found what you see in the picture. My father got staff infection the 6th day and had to transported to the hospital for an IV antibiotic. Its your call weather you want anyone you love to go to this place. I recently spent rehab at this facility. It was a wonderful experience. When I had intake the first day, I was able to walk twelve steps with a walker. Five days later, I walked from my room to the car outside. When I got home, I felt so much stronger!!! The help was professional and friendly. It's not a hospital where you have two nurses assigned. I had to use the bathroom often because I retained fluid. Always someone to help. Shared the room with a lady who had a problem with controlling her bowels. She was always cleaned up and the bedding removed. I give this center and it's staff the highest rating. read more

We admitted my father in law here and it was a sad horrible environment. He started at rehab here which did no help at all , they had to take him by ambulance to hospital for oxygen low in his blood . They called and told us they were taking him we showed at hospital and he wasn't there . They forgot to inform us they decided on a different hospital . After a few weeks stay at hospital he need to be put on hospice and went back to Warren Barr for that , he was there 9 days and not ONE nurse or worker came in to check on him or make sure he was alive or anything . Our hospice team and our family did all the care . A few times his oxygen tank was beeping and no one knew and it took a hour to get someone in the room to help . We asked for some ice cream for him (which one of the few things he asked for ) and the RUDE nurse said no , she said if the family wants to give him it they can bring it up there and feed it to him themselves! The place smells like urine , the smoke alarm sound is always going off , the nursing staff is very busy chatting and looking at their phones . It's for sure a place I would never recommend or send anyone there again !! Don't be fooled by the sales pitch !! read more

Do not go to this place! My Dad came here after a back procedure for rehab. From the 24 hour help number that was not answered when called to there being no active nurse in the facility, the care provided is non-existent. On the second day of his stay, I found him hugging the side of his bed, screaming in pain, door closed, his call button on the floor out of reach and nobody responding to his screams. When I pressed the call button, the CNA looked surprised to see me. She didn't know what was going on, said the morning nurse had left and her replacement wasn't there yet, didn't know if my Dad was given his meds and couldn't tell me anything. They dragged their feet getting him transported out of there to the hospital when I asked. I threatened to do it myself and only then did they oblige. Please don't leave your loved one there. My mother in law was placed here thru hospice. The CNA's and a few of the nurses were really great, but nurses like April and Maria, who had to be repeatedly asked to give the medication that was prescribed made a difficult situation unbearable. My mother in law was literally coming off the bed in pain, and these 2 nurses would take their sweet time, typically over an hour after the doses were due. When dealing with someone who is 'actively dying' and who is only getting morphine it's hard to justify making them wait. Also in one week they lost the doctors orders 4 times. The hospice nurses would watch them put the orders in the system and still they would get it wrong. All in all the administration or department of health needs to look into this facility before more people suffer. read more

Good morning I only give 5 stars because of staff on the first floor. Everyone is always helpful, polite and kind always willing to help. There is a in particular actually there's a few but the one that I'm gonna speak about in this her name is Brianna Perkins Nurse Perkins is the type of nurse that any CNA would want. She's dedicated, hard-working, determined and very vigilant. She's compassionate. when you have an issue with your residence, she's right there with the solution. You don't say it twice. She's very happy with the residence. She respects all of them. I've never heard her. Say anything out of pocket to a family member or a resident when she greets has the biggest smile on her face. She's always laughing and joking and making you feel good about yourself. She even does that with her coworkers. It's not just with the residence so it's safe to say that that is just her general nature, she knows her stuff and she gets it done without complaint. I am very grateful for Miss Brianna Perkins. The loud talking about residents in the hallways, the call light wait time is over an hour, the carelessness of what appears to be the big bosses. The administrator arrogant attitude and her partner cocky body language it is clear money is the reason these people work there defiantly not about the care and safety of residents. there is an awful odor in the building when asked they laughed and said oh it's the sewer and kept walking. A very professional and knowledgeable person about the facility and a person to talk to about how to obtain the things you need to make your stay comfortable there. I feel the nursing staff did the best they could but could have stepped up to advocate for the pt. When my dad arrived from the hospital for rehab he was placed in a room and told to wait. After waiting awhile he walked around to see the place. The desk person at the door stated that my dad was going out for a smoke, MY DAD DOES NOT SMOKE, he was just walking around to see what the place was about. After that he was taken to a room, was asked to lift his leg and they placed an alarm on his ankle and immediately moved him to the third floor, no explanation given. My dad was not even fully evaluated upon admission, had admissions done this upon his arrival this all could have been avoided. The next morning after my sister came to Warren Barr, my father was moved back downstairs. Also, my dad was not given all his educations on his med rec, an ointment he had was left up on the third floor, but no one bothered to follow up. Another medication was never given for 3 days, this is just unacceptable when one trusts a healthcare facility and Medicare is covering it!! Lafette was not very understanding or compassionate to my fathers needs or well being, he just had the need to show his authority. Then my father was placed in a room that smelled/reeked of urine and feces from the other resident they placed him with to share a room. There was not much we could do on a Sunday, but on Monday we spoke to an administrator, who listened to our concerns and request to discharge my father, then was offered to move my dad to another room, even a private they had available to keep my dad there. If this was available, then why place him in a room that was unsanitary, reeked of urine and feces, that we had to call the staff to clean the the other resident that my dad shared a room with? Nursing staff and admission administrators are aware of the residents. My dad is 92 and still lives independently and takes care of basic ADLs. Upon discharge the nurse came the room we were waiting in (approximately 2 hrs later), asked me to sign his papers, nurse seemed very annoyed because I wanted to read them, kept pushing me to sign, after reading I request her to talk to my dad as he can still make decisions and sign for himself. One more thing is that a personal belongings record was never done, as when I asked no one had a record off it, nor verify if dad had everything. Fortunately, we had everything of his accounted for. I feel bad for those who have expensive hearing aids, dentures, glasses, etc that are lost because a personal belongings rec was not done! read more

My grandma was a resident here for rehab due to a knee replacement and the staff did a amazing job. The cleanliness and wholesomeness of the facility made my grandmother (nana) feel right at home. Shoutout to Jean the DON , Ryan, Crissy , Lafayette and all the nurses and CNA's that helped us out when we really needed it. I wish it gave me the option for zero. Do not send your loved one here. My grandmother stayed here and me with 12 years' experience in healthcare, my grandmother who was the patient with over 30 years' experience and the patient daughter with over 30 years' experience in healthcare. They lie, they falsify charts, they never return your calls. When you do call, they transfer you to different people then what you ask. When you confront them about the issues, nothing is done, or they say let see the documents, that were false. The CNAs left my grandmother in wet diapers or on the bedpan for an hour. The first night there, the nurse told her she can wait till morning to be change or to use the bathroom. The best was some of the comments, one nurse looked shocked when I said I was going to bathe my grandmother and then asked me why I would want to do that. The infection doctor arguing the fact she had Cdiff, and when I told her she didn't, please show my records, the doctor left. She was there for rehab, and physical therapy saw her maybe 3 times. Then physical therapy lied and stated they been taking her to the gym, which in fact was a lie. If it wasn't for my mother and I, my grandmother would have died at Warren Barr, or ended up back in the hospital with sepsis again. With all are years of experience and Warren Barr knowing that we work in healthcare, you would think they would have treated her better, but no, from physical therapy lying, to leaving her in urine stain sheet, wet diapers, and then leaving SHIT in her bathroom sink, this place should be shut down. Please if you are the type of person that look at reviews, please read this, because with my healthcare experience, I would not want to see someone else's loved one be treated so poorly, like my grandmother was. read more

this place was horrible. The food was terrible, they are very short staffed and all the CNAs are so backed up it takes forever to get them to respond when you press the call button. My mother was here for rehab and thankfully, is mentally quite sharp. Otherwise, I wouldn't have known that dirty diapers were left on her bathroom floor, she was not given her diabetes injection for two days in a row, call lights were not being answered, food was cold, etc. I was assured that she would be in a "private" room. She was, but only for the first night. So don't base your choice on lovely rooms displayed on the website. I will say that the administrator was responsive, but only after I let her (and corporate) know that I was planning on contacting IDPH. Would not recommend unless you're able to watch your loved one like a hawk while there. That said, the therapy agency that they use was quite good. read more

My father was a resident there and he had a lot of trouble not being able to use the bathroom and when he would hit the call button to get cleaned up after ising the restroom on himself. Sometimes it took over a hour before someone came to change him. When he complained to the supervisor about his care they claimed that they would talk to the staff about his health and his cleanliness. The staff still did not clean him up. He spent a lot of time calling his daughter to come up there or to call up there to get him clean at all times of the day and night. I also have spoke to the supervisors myself and have had to change the way there staff handled him so I could stop coming up there every hour or two even while I was at work I have to call off a couple of times just to go handle the issue he was having. Towards the middle of his care to the end I asked the staff to give him eyedrops and the staff and doctors ending prescribing the medicine but never gave it to him. I had to go track a nurse down and ask her to come give it to him. The nurses would call me to come or call and talk to him about his behavior while he was bedridden and couldn't do anything. I will say tho that the wound care team did do a great job healing his wounds. They were completely open and by the time he left in two weeks they were completely closed. The physical therapy team also did a good job, due to he was not walking and bedridden, but after two weeks the staff had him up and walking using a walker before he was discharged. I personally would not recommend this facility due to the experience that me and my family member went through. read more

Lexington Health care has Covid-19 back in the building but the management staff is not coming forth with information. They are not telling employees who has it and it starting to spread. They have taken Dementia patient off of their safe contain floor and put them on skill and total care floor. There are over 70 or more patients on the floor no social distance wondering patients going into other room no infection control. Dementia patient are wondering around getting into the garbage, touching soiled linens and then touching food. All of this is being done so that they do not have to pay hazardous pay as far as we know state does not know we have Covid-19. Someone call state in on other issue but before they could be aware the patient with covid-19 was removed but then another patient test positive. read more

I would stay away from this place, it's all about the money. Tried to get my mom in there, it was an absolute disaster. First of all you can't get anyone to call you back, and Michael Shorter is the worst. The last thing I knew it was ok for my mom to go there so she was transferred from ManorCare. When she got there she was admitted and all settled in. I left her and 1 hour later got a call from a manager saying that she was back at ManorCare as we speak. They never even told me they were sending her back, no sure if that is even legal. When I finally talk with Nikki one of the administrators I was told it was pretty much ManorCare that set it up and she was never approved to go there. How does another place set up something at another facility. Lies, they just lied. Never really told me why she can't go there. they obviously had a bed for her so I think there is more to this. I think they just didn't want another medicaid person. It's all about the money. We don't treat animals like this. If Michael had just called me back and told me she was not approved this whole mess would not have happened. He should hold the position of administrator, he obviously cant do the job. Worst place to stay, especially if you cannot take care of yourself. Nurses and CNA's do not care. You could wait for hours for someone to answer your call light. No healthy food served whatsoever. Getting ice-water was a chore for the workers. rooms are disgustingly dirty. Medical equipment is old and rusty. The rehab was the only thing acceptable. They were caring and friendly. read more

If you are going to this facility and have prescription medication or any health issues, do not go. My mom went for physical therapy and they stopped giving her some of her medications which caused her to become very ill. Aids and therapist kept my mom clean and were very gentle with her, but the nurses were rude and understaffed. We were told twice "I'm busy" from a nurse. Two higher up staff members told me confidentially to get my mom out of there. She became so ill that we called an ambulance. The nurses speak to the director of nursing in a very disrespectful tone, in front of family member. When my concerns were brought to Joseph's attention, who is the director of nursing, there were answers after researching but the answers always showed error on the nursing staff and or miscommunication. He has no follow-up or follow through with his staff. This happened multiple times. It takes 20 to 30 minutes for someone to reply to a nurse call. I feel sorry for people that are patients there that do not get regular visitors. Who knows what they are doing with their medicine! You have been warned. Do not use this facility. read more

Please, please don't send your loved one to this facility. Very unfortunately, I did. My husband was released from Palos/Northwestern and I had to somewhat blindly select a nursing/rehab facility. For convenience and location, I selected this facility from the list Palos Hospital provided me. He was primarily admitted for malnutrition and dehydration. At Palos, he was put on what was supposedly told to me was a nighttime sleep medication named Seroquil because of one night not sleeping. Warren Barr continued administering that medication. He slept most of the day and became difficult to arouse when I visited. I came to the facility every day and often found his breakfast and lunch food sitting untouched. I began to smell a strong odor of urine and noticed his hair was not washed. I asked his roommate if he knew when he last had a shower. I was told he had not received a shower in the three weeks he was there even though he had asked four times for one. His roommate said he was always sleeping when normal showers occured and they just ignored him. When I asked staff when he had last received a shower, I was merely told "everyone gets a shower every Tuesday and Friday." When she went and pulled her book of records, she merely left while I waited 20 minutes for her to return which she never did nor ever reached out to contact me. Finally, I visited one day where he was incoherent and non-sensical and realized his lips were peeling. After contacting a nurse, (which took quite a bit of time), she contacted the doctor who said he wanted my husband sent to Palos for an MRI. He had several open wounds on his body from a fall while he was initially admitted to Palos prior to going to Warren Barr. Once taken back to Palos from Warren Barr, the ER nurse began removing his bandages from his prior fall and was absolutely appalled that some of the bandages had not been changed for many days or even weeks. He was re-admitted to Palos for dehydration and malnutrition once again. After several days once again in the hospital, he was returned to Warren Barr. I began contacting the care team, where they told me orders had been placed for assistance with eating and if this had not occurred, I needed to contact the head of nursing. I was extremely frustrated as to why I should have to contact anyone since this should have been an internal problem that they should resolve. I finally had enough and transferred him to Lemont Nursing and Rehabilitation Center. I am so incredibly pleased with the advancements he has made. He was taken off of Seroquil, receives 6 to 7 days of physical therapy and has gained five pounds. All of this occurred in less time than he had spent at Warren Barr. read more

Her belongings went missing, she fell multiple times, her dentures were literally broken out of her mouth, her prescription glasses went missing, She had such terrible UTIs she was hospitalized twice and that's not even the worst of it. Complete neglect and with COVID-19 we were unable to even walk in the door to check on her. The one and only nurse that would ever help us or follow through on what she said she would check on was Ashley on the second floor and we appreciate her and her kindness more than she'll ever know. The rest, you are at their mercy and they are terrible. I hope their administrators see this and understand how awful this facility is and think about how they would feel if this was their mother. My mother-in-law was placed at Lexington on the third floor. I went to visit her many times. They put people on the third floor to make sure they do not wander off but at this facility it wouldn't matter because there is not enough staff to care for patients appropriately. All nurses at the nursing station look very stressed out and never look up or greet you. The staff would just put my mother-in-law in a wheelchair in the dining room where she would sit all day until she had visitors. She was not taken care of or watched because she fell twice and broke her leg. Her daughter got her out of there. Pronto! There is hardly any parking space either. Terrible place! read more

No communication between staff. The director of nurse's never returns a call. Lost all of my moms things with only a few to be recovered. Her chart documented that she had wandering behaviors but they didnt think she was at risk for leaving the floor. My Mom escaped off the third floor and got down to the first floor where they found her later reading a newpaper. When my mom was sick I had to tell the nurse she was and to call the doctor 2 times before my mom got medications and breathing treatments. Horrible place.
