This place is dreary and depressing and staff is poorly trained. The food is awful and unhealthy. I was supposed to be on low sodium cardiac diet and was fed hot dogs,processed cheese, soggy cold fries ect. Luckily my family brought me some edible healthy food. As a patient, I would NOT recommend this place if you expect to be unable to advocate for yourself. The condition my son repeatedly found me in was very unclean, and I was left that way for long periods of time. He was working and couldn't be there all the time. I feel they take advantage of helpless patients. We walked out of this facility as soon as I was able. I was there in early 2024 - I can only hope they have improved. I forgot to mention - the food was beyond TERRIBLE. I wasn't on any restricted diet, and the slop I was served turned my stomach. It was beyond tasteless, and was served in 2 to 3 scoops on a plate. Extremely bad experience. My 63 year old developmentally disabled brother was transferred to Windsor El Camino from Sutter Sacramento ER. When he came in he had a broken ankle and torn ligaments in his knee. While at Windsor for one day he developed a bowel obstruction and pneumonia. The day after he was admitted, Windsor called his caregiver about 5:00 PM to say they were transferring him to Mercy Emergency because he was vomiting (due to the bowel obstruction). When we called Mercy Emergency about 7:00 PM, he was not there. Two of us tried calling Windsor several times that evening and letting ithe phone ring 20 or more times. Windsor does not answer their phone at night. We tried all the Emergency rooms in Sacramento and none had a record of him. At 8:00 AM the next morning his caregiver went to Windsor, the person on staff told him he was at Mercy Emergency - WINDSOR HAD LOST HIM! The Caregiver asked them to call the Ambulance Company, and the Ambulance Company's records showed they took him to Sutter Roseville Emergency, not Mercy. When the caregiver asked if Windsor answered the phone at night, the Windsor employee laughed. On top of that that, my disabled brother, who is not stupid and has has great memory, said two members of the staff were mean to him, hurt him by trying to move him, and did not know how to do their jobs or deal with disabled people. I cannot understand how places like this stay in business, or why reputable institutions like Sutter would transfer any human being there. Zero Stars. read more

If Yelp reviews are based on 1 - 5 stars, this review is "00000" -- I almost don't know where to start. We've had a VERY rough end-of-the-year 2019 holiday season. Starting in November, my father in law came to stay with us after he had to downsize (sell) his property in AZ. He has been battling cancer for quite a while, but it's been "under control" and he's been able to thrive. But, his health took a dramatic nosedive during his stay with us and he had to enter the local hospital. He was in the hospital for a few weeks (around Thanksgiving time) and his health insurance was Medi-Cal. With medi-cal, unfortunately options for treatment and care are not always "the best" He was well taken c/o in the hospital, but this review is not for them. -- So, he was moved here for reasons really unknown to the family, but blah blah.... they were trying to see what the next direction of his health was going to be, "would he improve? Hospice? etc".... so he was moved here. -- Upon visiting him at this location, it was HORRIBLE. Just walking to the care facility, you're smacked in the face with the smell of feces. It's very run down looking, stains on the walls, the patients all look zombified (those that linger in the hallways). -- When we found Dad's room, he was literally just draped in a bed, naked, wearing only what appeared to be an adult diaper. No clothes whatsoever. He was a little incoherent due to his health issues and medications, but he was in a lot of obvious pain, and the nurses available kept eye rolling and looking bothered to even check on him! At one point, he complained to us that he felt as if it was, "hard to breathe" at first they checked his oxygen machine and were just like, "no Jeff, everything is fine.... just be calm, blah blah".... during this time, my wife burst into tears just from the treatment and disgusting services of the room, his body, facility, and staff that are employed here. It's a place that can leave one with nightmares of ending up ill or elderly. -- When the "head nurse" finally checked his vitals to find out why he was so anxious, she approaches us and said, "I called 911".... we're like, "what exactly is going on?" to which she says his oxygen levels were low and he needs the immediate care of the ER. Well, no sh**** once we followed the ambulance over. He was JUST above breathing levels that lead to cardiac arrest. Not that it matters, but he did pass away few days later. -- Why is this review given 0 stars? Because it's a living nightmare. Don't forget, literal smell of feces in the air. Staff that don't take their job seriously. -- The only thing this business has done correct is apparently hiring the best photographer in the land because the photos featured on this Yelp company page are definite lies. It looks nothing like the "sunshine and rainbows" appearance that these photos presume. It's the ghetto, care facility edition. AVOID AVOID AVOID. Thank you for reading! read more

I can't say enough about how wonderful this facility has been for my mother. I took care of her until I couldn't anymore and they took over and were amazing. My mom smiled for first time in a while because all her needs were met. My mom was on hospice and they helped her alongside the hospice team. The entire staff but especially the nurses were wonderful. The place sets back on a beautiful property. Well kept, always very clean, the people staying there are happy. They have a family like community going on. They embraced my mother and were sad when we lost her. I thank God this was the place that opened up for us. read more

(1) The doctor came once and never came back despite many voicemails. (2) We did not get a "care conference" until the fifth day when I requested for one. (3) Acquired only two 35-minutes physical therapy. WORST: (1) My mom feared of drinking water because it takes an average of 20-25 minutes for a nurse to come in after you ring/ "press the button" (2) Due to negligence, she FELL on the bathroom floor....and they did not let me know until 3 hrs later after I came to visit her. I can go on and on... taking my mom home has been one of the best decisions we have made. read more

My mom is in pretty bad shape with severe COPD, so when she fell and broke her arm and hip we braced for the worst. She spent about 2 weeks at Windsor, before leaving for a lower level of care at Walnut House. The staff at Windsor were very nice. I think they did a good job with my mom. My grandma came here walking with a walker. And when it was time to go home she couldn't walk anymore. The nurses didnt take care of her sores on her lower back. That main sore really messed up her ability to walk. Staying here got her worser. Every single nurse that helped my grandma Isabel did a very VERY bad job and neglected her to the fullest! The nurses will be loud and rude to the patients and they will be in the hall way not doing there job! My grandmother had to go to the hospital because it got so bad. Long story short my grandmother past away on November 12 2019 because of the neglect she got at this place. Her sores gotten way worse the puss that came out of her sore went in her blood strem and her body couldn't take it anymore. One of the main doctors at the hospital said that place killed her slowly her sore had gotten worse and bigger. Please do not send your love ones here. I wished we would of known before sending my grandma here to get better. Rest in peace grandma. I will begin with the negatives. The place is a dump. Owners must be trying to maximize their profits. The food left much to be desired despite cooks clear desire to offer appealing and healthy food. Her budget must be very constraining. We did not select this place but went there because Mercy San Juan sent my husband there. The staff were excellent...very patient and kind and all seemed able. Physical therapy and occupational therapy and the social worker deserve high marks. My husband was well cared for. Some challenges...communication about changing Covid policy were not consistently known by staff so it was confusing and of concern. My husband was left to stay in bed except for his daily PT/OT sessions. He needed to move around more. I would attribute that to a shortage of CNAs...another profit protection policy by greedy owners. While I would have hesitation about returning there I have not seen other places in the area. Do not trust the pictures on Yelp. Visit the places first. read more

Reposting my google review which was hidden by them I want to kill myself. This is shrugged off by staff and the head of social services has told them to pay no mind because "It's not like she can do anything" In July 2020 I was hospitalized with a gangrenous pelvic infection and sepsis (courtesy of their inadequate incontinence care). Upon my return, little has changed. The quarantaine hall was a little more attentive, after I returned from the hospital a second time. I was moved to a shared room after quarantaine, told that my single room was needed for isolation patients but it's been empty. I am roomed with someone that is extremely vocal at night and wants all the lights on and the door open. It's noisy and and I get little sleep, no privacy and I'm told to get used to it I have to spend the rest of my life here, I don't want to live like this I am 33 years old and quadriplegic. I could not be given appropriate care by family so I have ended up here. Suffice to say, I do not recieve good care here. I have been in this facility for a little over three months and all of my concerns,requests, and complaints have been met with empty promises to do better (surprise, it has gotten worse) Daily Care: I am literally living in my own filth. Showers are twice a week but you pretty much get sprayed down with lukewarm water and a cloth with hand soap is very lightly passed over your limbs. Incontinence care is horrible. You may be stuck with a soiled brief for hours at a time and you are cleaned with washrags. If you are female, they will not address feminine hygiene needs past the urethra. I was told by the director of nursing that it is "sexually inappropriate" even with consent. I have suffered verbal and psychological abuse from some of the CNAs, told that I am too difficult to work with, faking my disability and that nobody likes me and that my call light is being ignored on purpose because I'm so detestable. Meals: Poor in quality and quantity. There are no fresh fruits and vegetables available. Everything is canned or frozen and the absolute bottom of the barrel in quality. Meal portion sizes are tiny and do not meet dietary guidelines. I lost a significant amount of weight and had thoughts of suicide before I was allowed to order delivery. However, that's not a habit I can sustain for long. Soon I will know starvation again. General Medical Care: I have almost no complaints here. Most of my concerns have been addressed and dealt with in a timely manner. The only complaint I have is regarding my pressure sores (stage 4 with exposed tendons) which should not have become so severe and may have an out of pocket cost to treat. Mental Health Care: near nonexistent, I met with a nurse practitioner twice and had some adjustments made to my medication but have gotten worse. I am told there is a therapist here and I've asked to see her but she is always out of the office? Physical Therapy: this is why I was sent to this facility, because my case manager was told they could meet my therapeutic needs. I had one month of subpar occupational/physical therapy that focused on sitting at the edge of the bed or brushing my teeth, both of which I can already do. I now see the RNA a few times a week doing the same thing The facility itself is in sore need of an upgrade to... well, everything. There is a PA system that goes off around the clock and is crackly with a harsh tone that contributes to my anxiety. The patient call system needs to be more than what it is now; flipping on a light over the door. So many times I have been told that a CNA didn't see the light. The carpeted areas are heavily soiled and the building smells of sewage, decay and a heavy citrus deodorizer. I don't want to be here. I cry day and night. Being here makes me wish I hadn't survived the accident that led to here. Do not leave your loved ones here, please. I wish I was dead read more

The staff at Windor El Camino did a wonderful job helping us care for our family member. The admissions and front desk staff were always very polite and thanked us for visiting. The nursing staff was very engaged and responsive whenever we called with questions or updates. When we came in to visit they were also very transparent with information. They mask the smell with febreze, Jan my mother was admitted to this "care facility". Some cna's did there jobs great Alice was one of them, Jessmer only did a good job because we went above his head, and they set him straight, though it had to happen a few times in order for him to do his job right. Shelly was another good cna, she didn't always need my hrlp, she could do the job on her own, though her pants were always falling down, if you don't mind the view of butt crack. Kedesti, she went to bat for my mother when the nurses weren't doing there jobs, she might not have had the strangth to roll my mother but she supported my mother in other ways that really helped her out, she earned our trust. Habtam, abraham, and Richman were 3 of the cna's who half assed their jobs. Nurses Tessa, and Hana were always willing to help, Tessa was one nurse who really took good care of my mom's wound, even more so then the ones in charge of doing her wound care. I for the most of the time was doing there job for them... one cna doe he broke my mother's wheel chair, and lied that he didn't do it, and then he even to my face he claimed he didn't do it... At random times in the day the cna's go on "breaks", more like hiding. Two times in her 3 months the cna's didn't control the hoyer lift and let my mother fall, lucky for her the bed was underneath her. They don't clean the rooms good, the toilet had black mold growing in the toilet bowel. Maxie and Kent are good people, in a facility that seems to care more about filling there pockets then the care they should be focusing on. Wound care, Jomar was stuck up and rude, he did his job how he wanted to do it, not how he was supposed to do it. We found out when he doesn't want to work with a patient he slacks off till the patient asks for him to be removed. The food was nasty, mystery meat, it made elementary school lunches seem like fine dining. T.v. dinners would have been more healthy to eat. We brought up breakfast, lunch, and dinner plus snacks for my mother, everyday. They couldn't make pudding or Jell-O, how can you not know how to make those. They never gave in enough time to set ugh. Misty from p.t. was honest about everything, and really did all she could to help my mother. Valery was good to. Steven ment good but had super bad communication skills, he tried to incurage my mother but treatened her instead. Chickens running around with there heads cut off. Upon departure they didn't give us the 3 days before sending her home, they told her the day before. Also they said that they had to giver her 3 days worth of her wound care supplys, but what they gave her was a supply for 3 wound changes. And when we called them to correct there mistake, they said not to give us anything. Mind you before we left Susanne the assistant director said if we need anything give them a call and they will help us... lies... read more

This place is unprofessional, below standards. From the first person at the main entrance, who didn't have a mask on her face, no knowledge of skills to "Greet" visitors. She had no roster of the residence. No sense of who to connect me with when giver the resident name three (3) times. I sat outside and observed for over one hour. Six (6) uncaring staff personnel were not wearing a mask at all, some had there mask below their chin. When I approached one of the staff member exiting the building in reference to the other staff members, pointing them out, and without masks I was told YES! that they all should be wearing a mask. Not even a week later I received a recorded phone message "That 9 residence and 6 staff personnel had tested positive in the Windsor El Camino Care Center. My sister (the resident) received unprofessional services from the physical therapist to the staff workers. ALL of her new clothing that came directly to the facility from Amazon where stolen by staff,or given to perhaps some other resident, after I was told that all clothing would be tagged before given to her. I've called twice and now being told the laundry person was off sick by someone guy in HR. When my sister was discharged she was wearing a scrubs bottom that was falling off of her and a top. She had $84.00 worth of new clothing and I want to be reimbursed for my expense or give the clothes back. If you can avoid this CENTER at all cost you and your family member would be better elsewhere. read more

If you care about your loved one/family member, RUN from Windsor El Camino! I found out my dad was discharged to this facility and immediately headed out there. I would soon learn that this place was trash. The doctors, nurses and CNAs are understaffed. The staff allowed my dad to STARVE as he lost over 30 pounds in matter of a few months! On top of that, they denied my dad physical therapy services; which was the major reason as to why he was there in the 1st place. My dad's clothes/belongings came up missing several times; regardless of how many times staff documented his stuff on their inventory logs. I would constantly have to remind nursing staff to give my dad his meds, and- you know, do the basic responsibilities of paid nursing staff (dispense meds, clean, feed etc). Worse of all, my dad would have bed sores and open blood wounds on his body from harsh care and/or the lack of nursing care; which caused him additional and unnecessary pain. Of course, I brought these concerns to management's attention, and received nothing but the run around and empty promises...No real productive results as leadership was just looking out for each other, rather than protecting their patients. I really don't know why such a negligent place exists to take advantage of people who are sick, and their families. It's really messed up! Overall, my dad's health, both physically and mentally, completely deteriorated while under the care of Windsor staff. This place is absolutely horrible!!!!! My mom came there from Kaiser and was neglected. She was paralyzed and was unable to walk and had a catheter placed by Kaiser. The staff at Winsor neglected to change it. Which caused my mom to have a sepsis infection which ultimately killed her. Please if you love your family please do not place them here read more

Windsor Carmichael is absolutely one of the most impressive Skilled Nursing Facilities I have ever seen. There are RNs on every shift, physical therapists. physicians present. More than ample staffing lends to the clean and comfortable environment. Patients are up and about or at therapy or in the recreation room. NO ONE is ignored or tied to a chair!! As an RN, I am delighted my mother-in-law was transferred to Windsor. I have seen a lot of places and Windsor honestly feels most like a hospital setting. There is not another SNF in this town that I would rather see mom in; she is actually HAPPY there! My only criticism is I have been hung up on a few times. read more

Stay there at your own risk! Dirty place, pics show the nice parts of the hell hole. I was treated poorly. Once when I was showering I was left alone. I was still at risk for falling. I was slip sliding. Thankfully I didn't fall! I had to peep my head out of the door to find my CNA and she was nowhere to be found. I saw the RN and asked her to send my CNA in to help me. It was a nightmare. My meds were messed up several times. And last but not least I met my case manager the day before discharge. Wow. I was surprised. They discharged me with a list of meds and then apparently the facility canceled them after I was discharged. Now I am out of my meds until I can get into to see my PCP. Best advice tour the place before sticking your loved one in there! Better yet just stay away period. read more

My lovely 85 year old mother was admitted here in late May 2015, as a transfer from Kaiser Roseville, to rehab because this place is contracted with Kaiser. Kaiser needs to rethink this! I will write them next... We were fortunate to be able to get her out alive.. April 10th. I will come back again, later to write ALL that "must and should" be said about this awful, scary, mismanaged place! I hesitate to give one star truly. This is an unsafe place to send human beings to. Please be warned.. I am not exaggerating in this post. My mother could have easily died here, or have had a serious fall. If it were not for the fact that my mother had family with her almost every day (we were even staying in hotels nearby) during her approx. 3 week stay there.. I wonder if she would have gotten out alive. This place is in it for the profit, not for caring for humans. In fact my mother even overheard a conversation outside her door one day where the new management (a doctor she said) was talking about the goal of trying to turn a profit "finally" - someone who was a new owner.. we won't know if that is true.. yet it certainly fits my experience as a visitor. Everyone needs to go to the California Advocates for Nursing Home Reform site.. www.canhr.org to find reputable nursing homes, and look at the homes reviews and ratings there. Do not rely on Yelp only. Luckily she did manage to rehab and overcome pneumonia after staying there.. thanks to family watching over her, showing up almost every day, (family that flew out from another state to help in a tandem effort). I believe this made all the difference for her. The care is very tenuous at best.(they are over-taxed and understaffed and mismanaged).even the Kaiser doctor that was to oversee her care plan was invisible.. as getting a hold of him or even knowing when he was to show up was a mystery!! The nursing desk said he comes in whenever he chooses to, to see patients.. and that was usually in afternoons, and that he reads the binder notes at the nursing station desk before seeing patients. So I left notes there, as instructed and yet no followup to the notes was ever given to me.. and no callback by him either when I left my number.. as I asked for. I will write all the other comments later, too much to say really, yet I wanted to say something for now. Please think twice before placing a loved one in this facility. I am also going to write the State Ombudsman, who oversees nursing homes with my concerns as soon as possible. Take good care of your loved one, and do not place them here if you can find another place. Do not let the hospital push your family member out of the hospital - to any rehab facility - before you feel you have found the best place for them. It is your right to take the time you need to do so ! The CANHR people will help you if needed. Best to you and yours! read more

They need to just shut this whole facility down..it is very dirty and nasty.. they have holes in their roof.. you always hear nurses talking about other patients in the hall ways and talking about patients personal medical issues.. which I believe is a HIPAA violation.. most of the staff is very rude.. the nursing staff rarely help the patients as they should.. I was told the nursing staff hide out in break rooms to avoid having to do work or minimal work.. residents are always yelling for help in the halls for hours on in.. the halls smell of human waste.. it's just disgusting.. the food is disgusting.. I've also been told by nursing staff, the kitchen staff never wears hair nets and have poor hand washing and hygiene as a kitchen.. the kitchen management is rude and insensitive and blame every problem on their staff.. the only friendly people in that building was the housekeeping staff.. please shut this establishment down.. its unsafe and unhealthy and these poor elderly people should not have to endure this kind of treatment and live in a place like this. Im not even sure how there a 5 star facility.. they must know someone who works for the state or something..I would of gave it zero stars if it would of allowed be but I was forced to to put 1 star.. read more

My Aunt Della L. was a patient at Windsor El Camino Care, she was very sick and weak couldn't walk from being in the hospital for over a month in San Franscio where she had back surgery. once we got in touch with Laurie in addmissions she invited me in for a tour. she was very informative an comforting , about my aunt needing rehab to walk, an that made me feel relieved , the care an compassion of the staff , was exactly what my aunt needed... to build her confidence to become independent again .. if we ever have to go back to another rehab facility, Windsor is our choice.. read more
