On 9/16/22, I visited Bela Vida Urogynecology for a Pessary for my condition. They charged me $92…read more I was asked to sign an Advance Beneficiary Notice of Non-coverage, with 2 options. I choose option 1 which basically states, I will pay $92 now but I want my insurance billed for an official decision on payment. If my insurance company does pay, Bela Vida Urogynecology will refund any payment I made, less co-pay or deductibles. My insurance company paid Bela Vida Urogynecology and the "Explanation of Benefits" states my total share of the $92 to be $8.07. After waiting for my refund and receiving no refund, I emailed, and sent a letter by US mail to Bela Vida Urogynecology requesting my refund. No response. They now have my payment and my insurance company's payment for the Pessary. However, when I filed a Better Business Bureau report, Bela Vida responded immediately. This is now 5 1/2 months.
