I highly recommend Stonebridge Assisted Living and Memory Care. After my elderly mother's sudden illness and diagnosis of dementia, she required multiple hospitalizations. Suffering a rapid decline, I was finally told by hospital doctors that she would not recover her ability to walk or care for herself in any meaningful way. She was refusing food due to swallowing issues and would only sometimes take water. The hospital's recommendation was to move her to hospice and allow her to decline and pass. Devastating news. Fortunately, I had already secured a placement for my mom at Stonebridge, and after consulting with their multidisciplinary staff, they encouraged me to transfer Mom back to their care, so she could be further evaluated. What happened next was nothing short of a miracle. My mother, who at that point was unable to safely swallow, speak, or move her body, had given up her will to live. But Stonebridge did not give up on her. In concert, the facility's director, their skilled nurses, med techs, PAs, PTs, Speech pathologists -- the entire community -- rallied around my mother in an attempt to offer her another chance at life. A thorough re-evaluation of her medications was performed. The nutritionist determined a diet that she could take onboard as she healed. My mom is also very spiritual, and I was overwhelmed by the number of staff members who came just to sit and pray with her when she was unable to speak. Staff even came to visit her on their days off! Gradually the interventions began to work! Over the course of a couple of months, and as a result of this dedicated and compassionate care, my mother has been able to recover her ability to walk, talk, eat solid foods, and is now able to attend onsite church services, social activities and enjoys visits with family. She has picked up weight and regularly engages in physical therapy, which has greatly improved her mobility and reduced her physical pain. I'm so thankful I didn't listen to the doctors at the hospital -- and took Stonebridge's advice to bring her back to them. At the risk of leaving someone out, special recognition is owed to Betty, Brenda, Sara, Moira, Renee, Rebecca, Barbara, Karen, Flo, DJ, and Fabiola. At a time when healthcare and senior services are stretched so thin, and our social services are overwhelmed, it's refreshing to see a care team that truly values their residents as their own family, by treating the entire person -- physically, mentally, socially and spiritually. I worked at Woodbury for 4 years, Woodbury is a place you would leave your parents if you don't give a damn about them. They need to fire the entire management team from the top all the way down, because they don't even come to work for the care of the residents, they come to work for their social clicks and friends. The building is ROACH and BEDBUG infested, which is cause residents to get eaten alive while they are sleeping. Abuse was reported multiple times to the Director of Nursing KYNA ROSE and because the employee was one of her favorites it was swept under the rug allowing them to continue to work there, with dementia residents on the 900 hall that can NOT speak up or defend themselves. When reported the DON Kyna Rose called me a liar. But it's OK because I believe in we reap what we sow, and kyna if you see this you got one hell of a reaping season in store for you, because of the pain you inflicted to the residents by not protecting them and to the staff members you have tormented just because you didn't like them for NO reason. There are no words to describe this place. A lot of the issues here are due to management. Staff here is overworked, unappreciated and underpaid. The DON speaks to them as if they are her property, and the new ADON is slowly taking after her ways. If mistakes are made, you're not corrected..you're humiliated and targeted by management. Paychecks are "held live" at the DON's discretion. (this is their way of saying withholding your paycheck, to make it "legal"). If you call off during the weekend, your paycheck is automatically held. You are expected to make Woodbury your priority- family must come second in their eyes. Management is extremely unprofessional as well, openly cursing at staff in front of anyone, spreading rumors and talking about staff members with others ... yes I'm talking about grown adults here. There are roaches and bed bugs everywhere in this facility... including all over the residents personal belongings and rooms. On a bright side, the scheduler stepped down from his position....So staff is no longer required to deal with him cursing them out, or his sexual harassment. On top of all of this, there have been many abuse cases that have been swept under the rug to avoid state involvement. Nurses are diverting narcotics left and right at this facility, and the DON turns a blind eye to it. Staff is extremely burnt out. They are all there for their residents, in which management is not. True change will never happen at this facility until all of management is removed and replaced. Please note that half of the 5 star comments are from staff members themselves... who have been asked to leave a good review on this page. The major problems here are not the staff members, it's the management. So please, if you don't want to fear your license everyday, don't work here. And if you're a family member considering this facility for your loved one.. please reconsider. The facility looks extremely nice, and clean.. but behind closed doors, the story is completely different. read more

I would not recommend sending anyone you care about to Woodbury. My father was blind with early dementia, and heart and liver failure when he went to live there. He struggled with moving to a nursing home as an independent and cantankerous man, and the dementia made him worse specifically at night. I understand my father gave the staff a hard time as he was often combative and mean. But that should not have had any bearing on his care, and I believe it did. I also believe that post-Covid and with terrible management and high turnover some of the staff at Woodbury were incompetent when it came to his care. Throughout my father's tenure at Woodbury, there were issues with staff doing Hospice-only procedures, hospice finding my father in pee-soaked beds or diapers, my father's call button being ignored for long periods, and many more. I saw staff bringing my BLIND father trays of food, leaving them across the room with no conversation with him about how his meal had arrived and where it was leaving him unable to even reach his food, or know what it was. When we moved him in we set up his room with pictures on the wall, things from his apartment, and organized his clothes and belongings. A week went by and my father had to be sent back to the ER and all of his belongings were packed up; since he is on Medicaid his room could not be held for him. When he returned many of his belongings went missing, including his hand-carved walking cane, a lamp, art we hung on the wall, a new pair of headphones, some cash, and other heirlooms that were left to help him adjust. When I called several employees from nurses to the heads of administration and environmental services to help me try to find these items, I was met with no empathy whatsoever and even a hint of contempt. These items were never returned. A few weeks later, when I called my father he sounded horrible. He said that he had a fever and his body hurt all over and he felt extremely sick. I called to speak with his nurse, who was the former hall manager and expressed concern that my father might have COVID. The nurse told me that my dad was "just being your dad" and that he had not had any of his gabapentin in two days because they "ran out and the nurses on shift were too lazy to go and refill it". At this point, I explained that it was unbelievable that they had not tested my dad for COVID, and were shrugging off a man who was very sick. The nurse said that they could not test my dad because the tests were locked up and no one had access that was currently on shift. After screaming my way through 3 nurses I finally got them to test my dad for covid and he tested positive. My father passed away recently and in his final days, I witnessed some nurses who were incredible, caring, and helpful to both me and him. I also witnessed nurses and CNAs who barely checked on my father, left him in soiled diapers, and attempted to give him pills like he could swallow them even though he was catatonic and on morphine for comfort. After hospice adjusted his medication, I had to tell a nurse twice to check the medication order because she still was trying to give him all of his pills. All the while the hall manager, whom I had spoken with on the phone for months about my father's care, came in and out of my father's rooms, passed me in the halls, and didn't as much as introduce herself or give any condolences to me or my family. After my father's passing, Woodbury attempted to charge my father's bank account for his Medicaid PML for the next month. Not only were they explicitly told not to charge us without consent to make sure the SSI payment would be in the account, but they were also attempting to charge for a month after my father's death. When I called to tell them how unbelievable it was that they would charge us, especially for a dead man, they swore that the charge was not attempted - even though I have proof from the bank that it was. If you can avoid Woodbury, I would. It's a nice-looking facility on the surface, but for us, it was a terrible place to live and work with. read more

Can I give 0 stars? Former employee and this place is a tragedy. The DON KYNA ROSE is an absolute tyrant with no heart. She never steps up to help and talks to employees like servants, she is more concerned with documentation versus actual patient care. I refused to take care of 18 super sick residents ALONE during the pandemic and was FIRED and she reduced my RN pay to MINIMUM wage ($7.50!) Aside from that, the nurses there constantly chart meds they don't give and residents are treated inhumanely. There are like 2 employees there that actually care about the patients. I tried to be one of them but I also have a nursing license to protect and could not safely care for the amount of patients KYNA expected nurses to care for. Dirty water, practically no linens, short supplies, short staffed, and the diet for patients all basically comes from cans. Patients are lucky to get ONE SHOWER A WEEK!!! Sad sad sad, I reported to the board, both the facility and the DON, KYNA Rose, who should not have a nursing license herself. There's also a CNA running around bullying and harassing staff and has been reported being rough with staff by a hospice nurse. I tried to speak with the actual owners and got the run around, so they don't care either. The Physical Therapy staff are great but unless you can sit up there and help your loved ones use the restroom, eat , shower and take meds, not worth it. Lesson here is, don't put your love ones in nursing homes, talk about death and let people pass peacefully when that time comes. Get home health, home physical therapy, Don't send them to a place like this where they are mistreated by strangers who aren't held accountable. read more

Just saw this place listed & brought back memories of my husband being in there several years ago, for a severe broken ankle. The place was terrible then. I saw the lady across the hall mishandled one afternoon , because she made them mad, on a visit to see my husband. But she was okay, when I left. My husband said she had come to his room happy & talking. The next day she was dead. My husband got out of there by calling an ambulance ( he had a phone) to take him to the hospital in wilmington & I took him home from there. Sure hope it's better now! read more

This place is a joke, for staff and residents. They work their nurses like dogs and treat them they like they are robots. They can't keep good staff that they do get because of the way the DON kyna runs the place. Nurses quitting all the time, people coming in for orientation and then never coming back. Residents don't get the care they deserve because there is never enough staff. In my opinion when they are short staffed the DON should be stepping up and passing meds or help the aides instead of barking orders and swearing at staff like a drill Sargent. Also, the building has leaks and it took a whole day for the DON to get maintenance to come in and fix water pouring out of a light socket. Real safe for the staff and residents. I would not send my worse enemy to work there or to live there. Be smart and stay away!!! read more

totally understaffed-4 days and no physical therapy-asked to be released after wife had to wait an hour for her bell to be answered so she could go to bathroom after her roommate used her walker to go to door and scream for someone to help. read more

Rehab for total knee replacement. Plz do not send anyone you love here for orthopedic rehab. Staff and facility not trained or equipped. No ice packs available and medications not delivered in a timely fashion. Furniture not handicap friendly. Physical therapy second to none. Only saving grace. Reason I went there in the first place. Physical therapy is one of the best. PT staff is caring, patient and kind. read more
