Approximately six months ago, I visited this office for a routine preventive cleaning and was charged $50 for a "deductible" and $75 for irrigation. I questioned these charges at the time but paid because the services had already been completed. I was told by staff that I would not be charged these fees again at future visits. During my most recent visit for another routine cleaning, I was told I would need to pay $95, which was described as a deductible. This immediately raised concerns for me, and I questioned the charge while still in the office. I also clearly stated that I did not want any additional services, including irrigation. Despite this, irrigation was performed toward the end of my appointment without my clear consent or acknowledgment. While still in the office, I questioned why I was being charged $95 and asked what their standard cost for a cleaning without insurance would be. I was told it would be $125, which further raised concerns about the accuracy of the charges being presented to me under my insurance plan. After leaving, I contacted my insurance provider and confirmed that my preventive cleaning is covered at 100% with no deductible or copayment when performed by an in-network provider. This directly contradicts what I was told and charged by the office. Additionally, I was asked to sign a document during my visit. Upon reviewing it more closely afterward, I noticed it indicated that I had periodontal disease. This was never explained to me, and when I directly asked the hygienist if everything was okay, I was told that everything looked "perfect" and that any issues would have been communicated to me. This creates a serious discrepancy between what I was told verbally and what I was asked to sign. When I contacted the office after the visit, staff provided inconsistent explanations for the charges, first referring to them as a deductible, then later as a copayment, and also claiming that irrigation is a required part of the cleaning, which contradicts standard practice and the fact that I explicitly declined it. The receipt provided lists the charge as "custom," with no clear or standardized description of the service, further contributing to the lack of transparency
