The place is clean. The nurses, CNAs, & therapist took such great care of him. If there's an issue they address it immediately. My husband is a very picky eater. Unfortunately we live in San Diego & Cottonwood is in El Cajon, so it was difficult for me to get there every day. The receptionists Arecelli & Ally made sure he received any food I ordered. Everyone is friendly & helpful. Alexandra was a great help making appointments & setting up transportation for my husband's doctors appointments. I definitely recommend Cottonwood. Thank you everyone. My husband just told me they called to check up on how he's doing. I miss this facility.

Substandard healthcare with equally deplorable living conditions. Electrical sockets not properly…read morecovered, and probably not tamper resistant, despite the patient population being overwhelmingly elderly with severe conditions. There is a serious pest control issue (ants indoors and in patient rooms). There is no dignity neither privacy, and nursing staff takes at minimum 30 minutes to attend a patient's most petty and serious matters. I see elderly patients on wheelchairs just strewn about all over the lobby and at times obstructing the hallways, making you wonder whether they brought themselves out there, or if any of the healthcare workers simply left them there. I was here to visit my eldest brother, who recently is on the mend from a serious and rare necrotic infection. He was sent here from a UCSD hospital, who did not seem to offer him much of a choice at all. While he is lucky to be young enough to put up with a relatively shitty recovery and assistance, being soaked in urine for two hours and not given clean linens and clothing with "meal time" as an excuse for obvious negligence and healthcare abuse for someone who already has a hard time fending for himself is not grounds for recovery. The situation as it is is already hostile to neurodivergent patients, as my brother is one, too. The only time he got faster care was because we had an able-bodied person with us at the time. If this and any other healthcare facilities like this one were your choice, and you were already getting better healthcare where you are, fight for your right to stay, even if it may cost money later on, because this is not worth saving money over. The only positives that exist are the facility is air conditioned and the staff appears to be nice. The food is reported to be mediocre.

Each morning, my mom was thoughtfully dressed and cared for with dignity, which meant so much to our family. The staff showed remarkable patience and kindness, especially in helping her with frequent toileting and ensuring she was always clean and comfortable. When she first arrived after her fall, she was in significant pain--so much so that even turning in bed or transferring was extremely difficult. The team handled her care with such gentleness and understanding during that challenging time. Thanks to the dedication and support from the rehab team, she made steady progress and, within just three weeks, was able to leave and walk again using her walker. We would also like to extend special thanks to the Director of Nursing, Alice Famador, who consistently went above and beyond--taking the time to personally address any concerns, even outside of regular working hours. My dad has been a resident here for quite a while, and I want to say this clearly: the nursing and…read moredirect care staff are excellent. He is genuinely well cared for. Susie, Olga, and Natalia have gone above and beyond for him, and we're incredibly grateful. None of what follows is a reflection on them. What I can't get past is how quickly things changed after his Medicare coverage ended and we switched to private pay. Almost overnight, the business office stopped responding. We're still waiting for a callback about arranging additional therapy. A test his doctor ordered more than a month ago was never scheduled. And my mom has had multiple interactions with someone named Nina in the business office who has been rude and dismissive. During one phone call, Nina hung up on my mom while she was simply trying to get basic information. That is unacceptable. Then my dad ended up back in the hospital, and we discovered his bed had not been held for him. No one ever discussed or obtained our agreement for that. We have documentation showing we understood there would be bed hold fees and were fully prepared to pay them out of pocket. Despite that, we were told his bed was no longer being held. It is heartbreaking to think a medically fragile person could lose his place despite our willingness to pay the required fees. To be fair, after we escalated our concerns, another staff member began working with my mom and has taken the situation seriously. They have been following up on the Medicare coverage gap, our pending Medi-Cal application, and our concerns about how my mom was treated. I appreciate their efforts. It shouldn't have taken things reaching this point to get help, but I'm grateful someone is finally addressing these issues. I used to recommend Jacob without hesitation, and I still would when it comes to the quality of care provided by the nurses and caregivers. They have been wonderful. But there is a significant disconnect between the people caring for residents and the business office. If your loved one is transitioning off Medicare, be prepared to advocate for them, because our experience changed dramatically once that coverage ended. I sincerely hope these issues are resolved in a way that works for everyone. If they are, I'll gladly come back and update this review.

I liked Coast Care for my 101‑year‑old mother -- mentally sharp, on hospice, with mobility issues…read more-- who is also seen by hospice staff, a nurse, a doctor, and an independent caregiver I hired. Mom bruises easily, but a Coast Care representative nonetheless reported "suspicious bruises" to hospice. Not true.I trusted this person. No other worker observed anything concerning, yet hospice was obligated to notify Adult Protective Services. Now my siblings and I are under investigation. And once we're in "the system" we have multiple family conferences, care, plans, etc. Hospice has been supportive and agrees the report was unfounded. Elder‑abuse is real and needs to be taken seriously. But this mistaken report has needlessly disrupted our family, and upset my mother. Dumb. Coast Care needs to train their people better.

We had the best skilled nursing facility(SNF) experience here for our beloved grandmother. After…read moreover a year of making calls to try to move her to a closer facility for our family visits, Claudia was finally able to make it happen for us. She and I had been in touch weekly for over a year and she never lost her patience with me or got annoyed when I called the way other places did. Instead she was apologetic when there wasn't an available bed. The admissions staff was very caring, detailed and informative from the beginning till end. We stayed in close touch with Crystal through the care conferences, when my grandmother had to be sent to the hospital and back, along with when she had to go on hospice. Crystal is kind and considerate. The staff took care of her through her occupational and physical therapy sessions with Alicia and Melody. She was able to self feed and sit up right in a wheelchair again. They got her moving more than she did at her previous SNF. Her dieticians Zakirah and Yesica provided a great diet for her that she enjoyed until she couldn't anymore. I let them know she loves her sweets and they helped make it happen. Her last bites before she passed were pudding and Ensure fed to her by her nurse supervisor, Vivian, who took extra special care during the last days. They built a connection because Vivian spoke Vietnamese to her so naturally our grandma took a liking and was more responsive. Vivian even took the time to stay a little later so she could feed breakfast to her. Dan (charge nurse maybe?), was very detailed, gentle, and graceful in his responses when I called to ask questions about her status. And when he had to break the news, I felt the empathy in his voice over the phone. Her other RN's and CNA's (I'm sorry I don't remember everyone's names) including Adona, have all been a lovely presence in her final stages. My grandma always appeared very clean and fresh and lovely at every visit. The janitorial team is fantastic at what they do too, keeping the facility clean and spotless. Protective PPE is always organized and right outside the door for easy use. It doesn't smell like a SNF. Her last SNF didn't smell clean. Highly recommend this facility and their staff! Thank you for all you have done to take care of her with care, respect, and dignity till her last breath.

I am in fear of being mistreated for the remainder of my stay. I will say though, some nurses have been great and they know who they are. Since the day I have arrived I have documented everything about my experience, the malpractice, the neglect, the work ethic of this staff, the lack of advocacy from the CASE WORKERS, ABSOLUTELY EVERYTHING. For the entire month of May I have been bedridden. I was given the option to sit here for a month until I am approved to start PT again, or sit at home and come back. I definitely made the wrong choice because this entire month has been absolute hell and I regret choosing the benefit of the doubt over my own health. I also assumed I would be in the best of care rather than putting myself at risk of falling. I know now that I should have made efforts to find another facility, but my main focus should be MY RECOVERY and this place seemed to be the best rated one that my insurance allows. My heart breaks for these other residents that don't feel cared for or feel any progress with PT and the activities dept. Activities feel like they are limited to basically a brain-dead type of person because theres is absolutely no stimulation or REAL activities. The part that hurts me the most, especially after seeing another similar review, I'm having to do my own physical therapy while in bed. They send one person thats working pro bono to stretch my limbs 3 days a week. Even though its short & brief she is a Godsend and the only humane person in this place. I have been put on and off of working with PT to help progress my recovery, but no one has given ANY clear straight answer as to why. I requested to speak to a case manager 5 days ago and for the last 5 days have repeatedly asked...nothing. This may not bring any closure or change, but people need to know for the ones that are unable to advocate for themselves. Be cautious when considering short or long term care at these facilities. I guess when it comes to this one in particular, insurance plays a big role into how you are treated as a HUMAN BEING and it is completely barbaric. This is my first yelp review ever in my life and its the most horrific one. If anyone has any pointers on any more avenues I can go down to have my issues addressed please let me know!! To give even more perspective on how this facility has been run: From my notes: 5/21/26 4:30AM staff found my roommate COMPLETELY ON THE FLOOR, not knowing how long she had been there. That concerns me because they usually change her DAILY starting at 3am. Plus, shouldn't the overnight staff be checking on residents throughout the night? According to my dad they did but 5 mins of PT a day and none on weekends. They forgot my father in the toilet 20 mins. They didn't shower him. They didn't check on him. They came in to feed and give pain meds and that's all. The halls on the long term side smelled putrid. The call button in the room was broken and they didn't notice, my dad told them. My father ended up doing his own PT. I brought my dad home with extremely odor and bedsores. While going in to visit signing in wasn't not mandatory. The customer service is very low, asking questions and making sure my loved one is taken care of means I have attitude. Speaking in a regular tone but saying comments or asking questions they don't agree with means I'm getting loud. It gives of couldn't care less facility. I care for my loved one and their care wasn't there and that's extremely unacceptable. If I could give you any advice is look for a better facility. The admin director was puzzled when I mentioned this when taking my dad, it's hard to believe they are unaware their standards are low. Mine are not. My dad is now comfortable, clean, and happy at home.

Their professionalism, compassion, and teamwork were evident throughout my visit. The residents appeared happy, well cared for, and engaged, which speaks volumes about the quality of care provided. They got him on a proper food, medication, showered, cleaned up, and exercising regimen. They got him stronger and walking again. Even if it's a few steps at a time. I cannot thank them enough.

This continued until I called the state again and she finally stopped after 8 months of being there and 3 phone calls to the county! They had a cat there that risked the hallways and threw up in the rooms and was in and out regardless of the fact that I'm severely allergic and I let the. Know this! Is wake up to the cat sitting in my window seal in my room starring at me. Again. I had to call the state to have that addressed because they did nothing about it until they were penalized for it. I like the fact that they have ice water available all the time and staff will come around and ask if you would like ice refill and even a water refill. My mom had dementia, fractured her knee and had to be taken there because I couldn't care for her…read more24/7 and don't have the means to have a nurse at home. -My mom's clothes got "lost" day 1. I dropped them off, name on each piece, including a small blanket. I visit the next day and she's wearing someone else's clothes. Her blanket no where to be found. When I went to pick up her clothes after her passing, they tried to give me a bunch of stuff that wants hers. And I only got back one shirt and 1 pant. Out of 4 tops and 3 pants I had bought her. -The place stinks -Nurses were good while we visited, other than that they didn't help her. -She got mrsa, within a week of her being there. No one noticed it until they had to call 911 to take her to the hospital, almost a week later. -Lost like 15 pounds in those 2 weeks. She wasn't eating and they didn't tried to get her to eat. -I asked all the questions, they "tried" to communicate, asked me different things she liked to eat to get her to eat, or liked to do so she can have some distractions. But nothing went any further than that. -Half of the time I visited I found her soiled. I visited every other day, as I was taking turns with my brother. All of this because we don't have the means for the good places. If your loved one doesn't have a voice, avoid this place. Because they won't take care of them at all. It was the worst experience for all of us. *The people who can speak up for themselves, were ok. This was mid-January 2026
