I was admitted to this facility for 20 days from a broken knee cap. I was treated very well from everyone on staff from various shifts. I want to thank NuNu, Dillon, Isaac, Delfina, Grace, Eddie, Pam, for all of their assistance and understanding during my stay. The physical therapy department is excellent. They are all caring, understanding in all of the different ailments from all kinds of different patients. I want to thank Jacalyn, Jim, Claudia, Max and Ranon for all of the therapy and helpful ideas to continue my own therapy at home. The facility is very clean and doesn't have any kind of smell to it. I like the fact that they have ice water available all the time and staff will come around and ask if you would like ice refill and even a water refill. I would recommend this place for injury rehab and for your elderly loved ones. read more

My mom had dementia, fractured her knee and had to be taken there because I couldn't care for her 24/7 and don't have the means to have a nurse at home. -My mom's clothes got "lost" day 1. I dropped them off, name on each piece, including a small blanket. I visit the next day and she's wearing someone else's clothes. Her blanket no where to be found. When I went to pick up her clothes after her passing, they tried to give me a bunch of stuff that wants hers. And I only got back one shirt and 1 pant. Out of 4 tops and 3 pants I had bought her. -The place stinks -Nurses were good while we visited, other than that they didn't help her. -She got mrsa, within a week of her being there. No one noticed it until they had to call 911 to take her to the hospital, almost a week later. -Lost like 15 pounds in those 2 weeks. She wasn't eating and they didn't tried to get her to eat. -I asked all the questions, they "tried" to communicate, asked me different things she liked to eat to get her to eat, or liked to do so she can have some distractions. But nothing went any further than that. -Half of the time I visited I found her soiled. I visited every other day, as I was taking turns with my brother. All of this because we don't have the means for the good places. If your loved one doesn't have a voice, avoid this place. Because they won't take care of them at all. I caught Ebola from a dirty debriefing tool near my left ankle Scripts's costal med center cut off my left leg and no one did any thing they kept over drug me in 2020 I died was in a coma on life-support and then on ventilator for another eight months woke up in vegetative state took me four months to start blinking in two years to get better. I'm still not better. I have a little bit of brain damage. I can't type I talk to type so this is the way it is folks be careful out there in these nursing homes, cause a lot of people are being abused in there all these nursing homes so can I get some advice please I need some advice. I don't know what to do. They got a free left leg out of me. you know a pound of flesh for a pound of gold more my my amputation weighs 79 pounds help me anybody out there. Tell me what I can do to figure this out. I'm dying now my butt I have multiple dysfunctional organ failure syndrome, and I have less than a year to live thank you out there please respond if you can Sarah was my social worker. I don't even know if she's there anymore, but they stole my computers. They stole my all my brand new clothes at pilfered everything money was stolen out of my wallet my laptop was taken away brand new shoes all that was under my bed my sister went to go get all the stuff and it was all gone through all the boxes were empty, and my laptop was gone. This place is an absolute dump. My father was discharged from the hospital after a 12 day stay. When presented with a list of possible rehab facilities this one had a 5 star rating from Medicare so I assumed it would be very nice. I was dead ass wrong. As we entered we were met with a nice fragrant urine smell. As we continued down the hall an elderly man was sitting half naked on a stool. His gown had fallen off and no one was helping him. As I got further into the building my anger and disappointment grew. We entered his room and it was worse than ALL of the barracks rooms I lived in, in the Marines over 25 years ago. The furnishings were old and beat down. We were there for quite some time before ANYONE came to speak to us. The were numerous residents with gowns falling off and meandering around. To add to that it was far from presenting a tranquil relaxing environment, that encourages recovery. I felt like I was in a scene from "One Flew Over the Cuckoo's Nest." Absolutely abysmal and depressing. Needless to say I checked him out and took him home. Does not even deserve one star. read more

My wife was in this place only for 3 days. A brand new electric blanket was lost. I tried to contact the manager for days and weeks and she never answered my messages. I wrote her a letter, no answer. I went looking for her, she did not show up. Finally, I showed my disappointment in the waiting room and they send me to talk to a gentleman named Paul. I vented on Paul and he was very nice and professional. He apologized on behalf of the insensitive and rude manager and I left feeling a bit better. All I needed was for the manager to tell me what happened to the electric blanket because my dear wife died shortly after she came out of that place. I dropped the case, I lost $75.00 and the manager only added to my mourning pain. Horrible! read more

Unfortunately at the time of my discharge from hospital this was the only available place for me. I would of course gone home knowing what I was walking into. Below Pros & Cons Pros is the amazing kind staff, except when you try to push your call button & need assistance from the CNA. Seem the more I was vocal about situations the least they wanted to help. There were some angels I would like to acknowledge: Hanif, Daphne, Christina, and Rose my CNA My concerns were listened to apologize made but still continued. Clear & concise information given to me was not what was provided. I admitted on Wednesday November 16. I continued to ask for no male CNA's. Especially one in particular who was rude & brash with me. I was confused how you needed a name bracelet for me but not all Staff wanted to wear name tags. A treatment nurse came in with cream for my feet?? I have no bacterial infection? She never asked my name. My IV line was infiltrated. I was told the doctor approved to use original line from surgery. It was in my arm for 5 days and now I am in incredible pain. They did remove the line and gave me oral Antibiotics so I should be able to go home. Nope required to stay one more night so that they could arrange my discharge. Well my husband was able to get me home safe and sound. I completely understand understaffed, overworked, and underpaid. But apparently a pain doctor whom I never met basically diagnosed me with a fatty liver, so no Tylenol? Also he said I was allergic to Sulfur. This is news to me. I guess because he was bothered on the phone, he was quite angry. He hung up the phone after saying I need to listen to him & he didn't have time for me. Like when I needed help to have my commode closer to my bed. But the CNA said she had to help an ambulance admit and walked away. Boy I was sooooo happy to walk away from this Skilled Nursing Facility. read more

This place is a living NIGHTMARE. After my loved one had a hemorrhagic stroke and needed post-acute care, I informed the staff on many occasions that he was in danger of falling, which could be devastating, as his stroke was from a bleed in the brain. Consequently, after only 3 days in the facility, he fell -- after calling out for help and no one came to help -- and broke his nose, cut his eyebrow, and hit his head, causing a re-bleed in his brain!! They put a mat down and lowered the bed AFTER this happened, but it was too little too late. He only spent a total of about 10 days (including the time he had to be transferred to the ER and admitted). I regret having him "cared" for at Magnolia - I could myself have had him lie in bed all day at home, saved the large copayment, and not have had his recovery set back because of the re-bleed. Highly recommend NOT letting anyone you actually care about stay at this facility. read more

Every day I wake up this place is on my mind. Can't get past what this place didn't do and did with me for over a year! I am worst off from being there , they didn't do a thing for me. They made things even just as bad for when I transferred out. There's no support or anything, there were meds called in I never used to my pharmacy. The insurance wanted to start an investigation , they would just covered up things again. I am truly worst for staying there. It should be closed down , it was a complete rip off in care!!! I'm home now feeling the effects from this place, and what they didn't do there. They even lie about helping with the aftercare, the company they picked sucked just as much! My life was ruined by this place , and the people there working. The good workers quit before I left there, which says a lot. I hope other Families don't take their love ones there ever. read more

The day that I got to Magnolia Post Acute Care as soon as I came in it smelled like urine in the hallways I saw elderly people on wheelchairs humped over sleeping in different places it almost brought me to tears. There's 3 people to a room. Nobody came to greet me or to ask me if there was anything I needed like help using the restroom.... I was there for two weeks and right away you get to know your nurses which ones really care and which ones don't... night shift is HORRIBLE. I was on IV meds and my antibiotic ran for 30 min. I was left hooked on the machine for 3 hours one time before someone came to unhook me I did press for the call light if anyone is wondering, is not like I just sat there and waited. I was unable to get up at that time if not I prob would have gone and looked for someone... read more

This facility is recently under new management, and it is apparent that they care very much about their patients. As a healthcare provider myself, I appreciate the nurses here and the attentiveness they provide to their patients. Facility is clean, and patients appear to be well groomed and cared for. Atmosphere is calm and call lights seem to be answered promptly any time I have visited. Direct care staff and management are readily available for any concerns or needs. read more

I brought my grandmother to magnolia, a little hesitant due to the previous reviews. However, the staff was really friendly and surprisingly smelt really good. I was actually happy to see that the building and the staff are willing to work with patients that have dogs and other animals to make their stay more like home. The service is 100% geared towards invidiuals care and not generated by "just another patient." The therapy team is amazing and I would defiantly refer my grandmother to go again. read more

My mom contracted Covid in their facility from a staff member. She went in because she badly sprained her knee and couldn't put any weight on it and came out with Covid and died in a hospital. The staff are ill equipped to take care of a Covid positive patient. They waited too long to get her to the hospital. My mom had told me that she kept telling them that she couldn't breathe. They FINALLY called 911 when her oxygen level was 80%. My mom arrived at the hospital with severe severe Covid pneumonia, dehydration from uncontrolled diarrhea and blisters. She was given imodium which took care of her diarrhea right away. My mom begged me to pick up her stuff from Magnolia because she was never going back. My mom had shortness of breath at least 5 days before she was rushed to the hospital. She should have been taken to the hospital as soon as she had shortness of breath so she could start Remdesivir. Ultimately, our seniors should have tons better care than what my mom received. Since she was part of the vulnerable population she should have received more to help her fight this deadly disease. Monoclonal antibodies would have been nice for her to receive the minute she was diagnosed. Then remdesivir when she developed symptoms. The minute your loved one contracts Covid, get them out of there and to a hospital where the standard of care is much much higher! read more

Went last month to visit my friend. Sink plugged up and no water. Health code violation. Went yesterday sink still plugged up.

Anytime in a rehab center will have its issues. It is a tough situation and, for the most part, all you want to do is go home. However there is a need to stay at this type of facility until the patient is able to return home. All that being said, my brother spent two weeks there and all things considered, the stay got us to the goal to improve his medical issues and get into a position to return home. While understaffed to a certain extent, especially on weekends, the staff did everything they could to attend to my brothers needs. Ferdi, Jacqui, Vanessa, Ranon, Chelsea, Mary and Marie all were all food to work with and were attentive to my brothers needs. Again I hope that the situation comes up where time in a rehabilitation facility is required but should it be required Magnolia would be a good choice. read more

I spent 2 1/2 months there, the food was horrible. Always sent the wrong items, what was offered as a substitute never available. One resident runs a mini mart out of her room with her roommate. I had a roommate that wore a pull up, got a staph infection and spread it to me. Call lights go unanswered for up to 20+ minutes, and the CNA's though nice, let me in the shower room with dirty barrels several times during my time. As of today 3/4 of the facility has covd-19. read more

This place is a lawsuit waiting to happen. I went there with my friend to pick up her family member and just in the two hours i sat in the parking lot saw so many instances of patient neglect, negligent staff, poor management and what seem to be a facility inadequate for the types of injuries the residents i saw appeared to have. Specifically a main entry door to the facility with no wheel chair access button forcing patients to call for assistance or else have to jam their wheelchair into the door just to get a hand hold. No covered sidewalk to the designated smoking area so residents have to hug the curb through the parking lot. Also no permanent structure or gazebo for residents to utilize to begin with, just a "pop-up" tent. If I could give this place no stars I absolutely would. This place should be shut down immediately. Its a danger to all those staying there. I stayed here after I got out of the hospital when my kidneys failed because I was also dealing with a pretty bad pressure ulcer that none of my family was equipped to help me with. It only got worse and more infected while I was there. When on dialysis youre on a very strict diet because without help from your kidneys, your body cant regulate certain nutrients in your body and those levels can become so high in your blood that it can make you sick and eventually kill you. These people seem to have no idea what a dialysis diet is. Even after explaining to them in detail what I could and couldnt have they continued to give me things Im not supposed to have. Also Im supposed to take meds with every meal to bind the phosphorus in my system. Ive been taking meds for years. I know what I take. Because of a mistake they made they argued with me about the dosage and refused to give me the right ammount until they talked to the doctor. They also let patients who are suffering from dementia wander freely around the facility. Several times I had an older man wander into my room and when I told a nurse about it they knew about it and didnt do anything about it. One of my biggest problems about this place though is that they are crooks. I had $40 stolen right out of my wallet while I was staying there and when I reported it I was told that I shouldnt have had my own money. I also had half my clothes go missing when they switched me to another room. And now Im getting a bill for over $1200 from them for months I wasnt even there even though I informed them I wasnt coming back. This is the most unprofessional "medical" facility Ive ever seen and Im glad I dont ever have to go back. DO NOT GO HERE. read more

My relative was only there a little over a week and it was terrible. They lost parts to the device she needed to wear so she couldn't leave the bed. At one point her call button was either ignored or malfunctioning so she yelled for almost 30 minutes before anyone heard her. Even though she had plenty of clean clothes they never changed her once the entire time she was there. Sorry to say but this is one of those nightmare places you hear about and hope to never encounter. The smell when you walk in just wreaks of old and musty. Patients sitting in wheelchairs facing walls. I was beyond shocked. Living in an alley would be an upgrade!! read more
