I am writing on behalf of a resident currently in placement, Mark. Mark has been a resident for 3 years. He has had and continues to have GREAT CARE. We are made aware of health issues as they arise. The communication with us, the family members, is impeccable. We are made aware of medical challenges in real time, we are afforded meeting with Social Service staff, (usually Melissa) the Shift Nursing staff to discuss current status, Shift Nursing Supervisors to discuss upcoming medical appointments, (usually Isaac), Karen (#1) to obtain updates of concerns brought up in patient staffing, Respiratory Therapist, Randy, who in our minds is the "Emotional Rock", for Mark, especially during episodes of breathing challenges. We have observed him in action. Faith, remains my anchor in which direction or person that I should address concerns. Christine, Kind, Patient, Compassionate, all one has to do is observe a BINGO. CNA Teresa, whom started with Mark on his first day and has made it a point to check in on him. I cannot conclude this review without thanking ALL of the staff who are assigned to Mark's care. My dad was at this facility and it was the worst thing that could ever happened to him. I honestly think he would still be alive if he didn't go here. He had cancer and had gotten surgery on his tongue. He was non verbal with a trache but he was recovering. This place was disgusting. It hurt me everytime time I left him He would walk us out then became weeker due to their negligence. He ended up getting pneumonia and if it wasn't for his appointment at the hospital we would have never known. This place needs to be shut down. If you have a loved one that needs hospital aftercare do not send them here. A grace will be the first to take any patient and then you will have a hard time getting them out of there. read more

Writing on behalf of resident: I have been a resident of A Grace Sub Acute and Skilled Care for almost 8 months. As someone who is receiving care firsthand, I can say do not believe the reviews as they are a few years old. The care staff is very compassionate and hardworking. I always get my medications on time and do not have to wait too long when I push my call lights. Communication is very good amongst the staff, myself, and my family. The food is good (My favorite is the coffee) and there are a lot of activities for residents to enjoy; Bingo is my favorite as they always have great prizes for the winners. My son has been at A Grace for a little over 2 years. He is there because of severe traumatic brain injury. After 3 months of hospital care, he arrived at A Grace still mostly unconscious, on a ventilator and feeding tube, with a very limited range of motion and with bedsores. The bedsores were treated and gone in a week. With good care, he was off the ventilator in a couple months and they were able to remove the tracheostomy tube not long after that. He started regaining consciousness soon after he arrived. It was a hard process on everyone as he was terrified, defensive, and yelling. Since then, effective medications have helped him be calm. He was able to get off the feeding tube recently, and now enjoys a meal and activities in main room daily. This is not to say there have not been any problems, but the results have been good overall with a difficult patient. What is more, they have a wonderful new administrator who is attentive to everything and is raising the whole level of care. read more

DO NOT!!!! Send your loved one to this place. First of all, they gave me the doctors number but when I called he said I had the wrong number. It took forever for me to finally catch him at the facility and when I did he was super rude. I lived an hour away and went to visit my mom at least once a week. I would be there for 4-5 hours at a time. They never came in to reposition her. Then I noticed one of her legs were stuck in a folded position. And her head ended up being stuck to side. Sad to say, it was only by the grace of God she caught pneumonia and had to go to the hospital because every time I told them I wanted to take her home they would play games. Then at the hospital she had an episode with the thing in her neck. (Treg or whatever it's called) And the doctor I spoke with even said they wasn't caring for her properly. My mom could not talk so she couldn't tell me anything. Every time I asked the nurses anything they would act like they barely spoke English or they would claim they were new and didn't know too much. They never had blanket on her. They never had the TV on for her. She has got a bed sore. When my mom first got there one of the nurses even told me that I should work there because they don't require any experience.... No individual patient records just binders in hallways left unattended. What? You want a shower. Where are the Showers? The employees that did an excellent job helping us are Mandeep and Andrew. My husband was at A Grace for about 5 months. He got excellent care. The nursing staff is compassionate, competent, and very friendly. The head nurse, Edythe, is always so cheerful and helpful. Physical Therapy is especially great. They got my husband up and walking with a walker, helped transfer to the wheel chair and the car. The respiratonal therapists got him off the trach. He is now home and doing great. The food is good and there are activities for the residents regularly. There are some maintenance issues, but don't let that fool you. If I can give this place 0 stars I definitely would. Please believe all the reviews on this page and do NOT let your loved ones come here. My father was at this facility for a total of 4 days until we finally got him out of here. We have a very close family and we would be there opening to close, 11-9 so we never got to see what the morning CNA's would do but from 11-9 NOBODY would come in to help him out. Thank God for a nice LVN by the name of Harnish who would drop everything he was doing to come in and help us move our dad around or show us how to suction his mouth (CNA job). If it weren't for my family being there my dad would not get moved at all, he would not get cleaned or even checked on to see if he was okay. He went those four days without getting the right treatment for pink eye and now can not see from one eye due to all the gunk that accumulated. This facility needs better CNA's who are actually going to do their job and a supervisor who checks on the families and the needs. We requested a doctor twice and nothing was done. Please dont bring your loved one here. The staff are here clocking in and clicking oyt to get paid. They dont work with their heart. I visited my mom Friday and Saturday. Her hands and body got poop every where. My kus ages 5 and 10 have to experience that. They know we were visiting. Still don't care to give my mom diaper change. Imagine when we're not there. I only visit on the weekend. I lived 1 hour away. Single mom with 3 kids. Where I'm at they won't accept her medical. So can't bring her here. The staff their think their in charge of everything even the CA HEALTH INSPECTION. I think the staff is right. They are in charge for miss treating the sick elders. If i could give this place a zero i would, absolute unprofessional, zero accountaility and zero communication. My Sister Stephanie is in this facility, a vegatable. We have decided after 8 months to remove her from the breathing machine. When i tell you all they have given us us more greif and heartache, believe it. Not the Charge Nurse, Cheri, or her assistant or the Dr. Chen can give us any information around the process and setting a plan. We have been waiting on them since November 5th and still; all we get is the run around. Meetings to have meetings to talk about the meetings that no one showed up to. We had a meeting on Friday, that they requested, no one know what we were talking about and the team not there and not ready. They then stated "the team will meet Monday", on Monday, no one showed up to the meeting, again stated they didn't know there was a meeting. Today i called and talked to Dr. Chen, doesn't understand English well at all, rushing me off the phone, could not explain the plan around the situation; only that he referred the case to yet the 2nd Hospice facility and advised me to call them but he didn't have the number nor would give me the number. No one has reached out to use, provided an update, advised on the plan nothing. At this point i'm thinking of sueing this facility; we as a family are already broken from the tragic events from my sister but to add insult the facility will not provide updates and information. We request she be sent to the hospital to have the procedure done but the nurse/Cheri's assistant advised us we can't have my sister sent to the hospital. It's time to talk to a lawyer and involved Public Health & Social Services. Even Dr. Chen stated he didn't know why they have not removed the tubes yet, but he, the Dr. could not do it. WTH! Do not have your family come here. My grandma is currently here because of various reasons. She has a urinary tract infection and bed sores due to terrible service of the nurses and other people who work there. All reviews on Yelp is true. The employees do not care about the patients. They do the bare minimum and let the patients die a miserable death. The urinary tract infection and bed sores are due to neglect of the employees!! I came to visit my grandma and see her cry in pain due to her bed sores and urinary tract infection. Inside is extremely dirty and I cannot believe this place is still running. The parking is behind the building and is very limited. There is no AC. During the summer, they turn on the fans and cover the windows to avoid the heat. IT IS SO HOT IN THERE DURING THE SUMMER. Think about your family who are there 24/7. Please do not have your family come here for treatment. They will definitely die a slow and painful death. read more

Is horrible place to take love ones. Toilet flooded and the patient was blame, abused emotionally, verbally, maintenance staff removed paper towels and left bed wet sheets on the floor for days. Some nurses are great, but some very passive aggressive saying that she will not follow Dr's recommendations. Lazy CN's graveyard shift.. I can go on but.. Do not take your loved one here!-- water in patients room with cords over the water. Another patient was foaming at mouth as we did our walk through and a hour later he was still foaming. Nurses and staffing are not organize on what to do for new admission patient, my son was discharged in Santa Clara Medical Hospital with EMT transported him to A Grace Sub Acute & Skilled Nursing Facility along w his Humira medications on August 20, Doctor from the hospital and I aggree that I will pick up Humira prescription to the Pharmacy as the Doctor said he'll call for refill bec my Son needed the Humira every 2 weeks, since they just give him on August 20 the next shot would be September 4, I'm not in the hospital when they discharged my Son to A Grace and I didn't know that they gave my Son 2 box of Humira and syringe, Facility did not say anything, on Sept 3 the nurse supervisor in the evening asking me about it and I said I will call the hospital Doctor to find out why I haven't received any phone call to pick up the Humira to any Pharmacy, I'm calling back and forth to the hospital and 2 Pharmacy asking why can't they refill it and they keep telling me when my Son was discharged they gave him the Humira on bedside to bring to the facility so the pharmacist asked me to call and ask the facility to check the admission lists, now facility supervisor told me they found the Humira in their refrigerator, the whole day I was in the hospital and the 2 pharmacy and yet they have the Humira on there fridge, since they found it on Sept 4 around 1pm I'm assuming they gonna give my Son the shot, no they didn't not give him a shot they waited on around 12:30am on Sept 6 and said why the Doctor order it to give my Son on Sept 4, I talked to the Director of Nursing Facility about this problem and she said they will adjust the date when to give my Son the next Humira shot, she said instead of Sept 18 it will adjust to Sept 20 and that's what we agreed, but no again they almost gonna make a mistake again, on Sept 13, I called the facility to find out how's my Son doing and I talked to his nurse on duty that's around past 1pm the nurse told me his fine and I'm about to give him a Humira shot and I said who told you to give my Son a shot it's only 1 week that soppose to be 2 weeks nurse said the D.O.N. told her, so that's why I'm saying this facility is un organized and they can kill patients the way they do as far as the medications involved, I didn't hear any apologies from this 2 nurses supervisor, if I have a choice I don't want my Son to be on this facility but their the only facility that can take care of my Son for long term, I want to give this facility a zero star they don't deserve to have any star read more

This place should be shut down immediately, most staffs here are filipino and they are rude and unprofessional. They are careless and don't give a fuck about taking of their patients. They let the patients drooling or coughing until it becomes really bad then they would come and do suctioning for them. I feel really bad and heart broken for those who have to stay at this terrible place. My dad had been staying at A Grace for 6 months and passed away for almost 3 months. His death makes our family feel very confusing and curious because when he rushed to the ER and the doctor said he has heart infection. We went back to ask all the staffs there about what happened to him and nobody can give us an answer. Everyone say they don't know anything. However, staffs at A Grace should realize that one day they will ending of staying at nursing home or rehab like all patients that they are taking care right now. I was thinking to sue this place but my mom she stopped me from taking too far. And we should let it go and pray for my dad to rest in peace and no more suffering from this place and have to deal with these heartless monsters. If you need help don't thinking that social workers at this place can help you with anything. Cause they don't know shit and very impolite. Especially social worker name Merisa or Nerisa something like that. She wouldn't wanna talk or help you with anything if you keep requesting or complaining about stuff. I had experience with her that she would literally talking with you but no eye contact and just slowly step out of the room without finishing her sentence. Some staffs at A Grace feel bad for treating patients like animals so some of them quit their job also base on low pay. I made friends with some good staffs there and most of them said they don't feel happy of working here because of stress and over taking care of patients and also the owner wanna keep the patients to stay longer at A Grace. So that they can keep receiving money from medical. If your loved one is sick and if you can afford to send them to a good rehab or nursing home. Please send them there and take good care of them. I never seen any place like this place. The Service here is horrible. Staff doesn't help the patient. I told one of the nurses here to help set up the OC and they came in and didn't do it. The staff said they would change my dad's diaper at 5 and at 9 they still didn't come to change the diaper. Then my sister ask one of the nurse to change his diaper and they said that since there is family here they wouldn't change my dad's diaper. Even when I asked for a pillow they said they didn't have any. When the trash can in my dad's room was full they didn't come to change the trash can. When my dad felt like he had short breath we pressed the button that alerts the staff to come help my dad they didn't come into the room 1 hour after we pressed the button. Finally when the staff came in they said what did we need and she said that when we pressed it she was on her break so she couldn't help us. read more

I would give this place a zero not desirving not even one star. I went to visit a friend of mine and I was very uncomfortable with the surrounding. the outside was an eye sore and scary, the inside looked clean but I don't let appearances fool me. There are three beds in a room and the nursing or care givers weren't around looking in on my elderly friend when I was visiting. I was very sad when I left this place and was concern with the health and wellbeing of his health and all who are there. I saw medical records on a clip board in sight for any one to see. OSHA should be notified. Care giving goes beyond a clean floor it is the responsibility of all nurses and medical assistant and any one administrating care to be professional ,compassionate in the well being of their patients. To the administrators to make sure that their staff is doing their jobs. A daily walk though should be conducted. I sure hate to send this review to the commissioner of the state and Health Care Dept. This place needs to be reviewed. Privacy is an issue there. read more

This place is deplorable and disgusting. My brother was brought here yesterday. When I arrived I found him sitting in the hallway. He hadn't even been put to his bed. One person approached us and asked who I was. I told the person my brother needed suction immediately. Half hour later the R T shows up auctions him. He then crawled into the bed which was not even cleaned from the previous occupant. When he did this he basically flopped on the bed. The RT staff just walked out of the room leaving my sister and I to make him comfortable in the bed. While in the hallway two "nurses" were arguing of who had to suction him. He did not survive 12 hours. I believe he suffocated due to lack of simple care. I received a voice mail today saying he had passed and that his body had been removed. I had made pre arrangements for this with a different funeral home. Not once while I was there did any personnel approach me for any info nor any type of conversation. They had the nerve to tell me I had nothing set in place. They never spoke to me. This place is horrible. Never leave your loved one here. read more

The first day my brother was admitted to this facility, i went down to sign papers an hour before his arrival by ambulance and my first reaction when i walked into the lobby was "Oh Hell No!" That was my first gut instinct, and they did nothing to improve my opinion of them. I visited my brother almost every day from the day he arrived 6/16/15, to the day he was rushed in an ambulance to a real hospital due to lack of proper care. He was severely dehydrated, 104 temp, stage II bedsores acquired in less than 2 weeks in their care, severe infection, and sepsis...He died 7/19/15...I wouldn't let my worst enemy stay there. If i was faced with having to stay at this UnGraceful & UnSkilled Nursing Facility, I would rather die. read more

I'm pleased to write about my experiences with A Grace Sub Acute Skilled Care Facility. I have previous experience with other nursing homes in the SJ area over the years, and feel like I can fairly judge A Grace by comparing it to the others. First, as the saying goes, You cannot judge a book by it's cover. Along those lines, neither can you judge a skilled nursing facility by it's exterior. The building may be old in it's 'bones,' but it's interior is clean and well maintained. A dear friend is currently under care at A Grace, having suffered a stroke. The professional staff working on his care include: a Nurse, CNA, Respiratory Therapist, Physical Therapist, Speech Therapist, a Dietitian and weekly visits by his MD. Any requests made on my friend's behalf have been met with an immediate and positive response. I'll admit that I don't visit as often as I'd like, but most of these professionals not only recognize me by sight, but they acknowledge me by name! I'm sure they see a variety of visitors on a daily basis, and I'd never expect so many of them to remember me. The initial apprehension I felt on my friend's day of arrival was one of mixed emotion. Having accompanied my friend on his transfer from the hospital ICU, I was unsure of the (age of the) facility, the new staff, the new routines, etc. A Grace has senior staff members assigned to assist in the arrival of new patients. I have had the opportunity during these past few months to see other patients transfer in. The staff obviously work as a team to make the transition a smooth one. My friend's arrival was smooth and within the first day or so, I was comfortable with his placement at this facility. The Nurses and the Cert Nursing Assts, along with the various Therapists have always conducted themselves professionally and willing to assist with any request. A couple times, other Nurses have stepped in and helped me when I my friend's Nurse was busy with another patient. I would like to single out one special staff member who provides excellent care and attention to my friend. He is Ciro, a CNA and a gifted care provider. He has never once faltered when I've made special requests on behalf of my friend's care. Ciro shows respect to his patients and allows each individual their diginity and privacy. He has gone out of his way to make my requests happen. I must admit that I'm not there 24/7 and have only the last four months by which I am judging and comparing A Grace to other similar facilities. A Grace is well staffed, clean and free of the offensive smells I've had the displeasure of experiencing elsewhere. They have responded to my concerns, inquiries and requests within a timely manner and always with a smile. The offices of the Nursing Supervisor, Social Services staff, and the various Therapists are along the hallways near the Nurses Station and close to the patient's rooms. Everyone is accessible with a simple knock on their door. I've never had a problem reaching any staff member, except when the Physical Therapists door was secured during a patient's session. I want to add that the Activity Director maintains daily social activities held in the main dining room. I've had many opportunities to witness patients engaged in a variety of activities, with what appears to be genuine pleasure. I have given A Grace a 4 star rating because I don't feel I have had long enough to truly evaluate all of what the facility has to offer. This is based on the current health status of my friend, the patient. Due to his physical limitations, he is not able to utilize some of the other features and services A Grace has to offer. If he could speak for himself, I feel confident that he would wish that he was on a sunny beach, rather than laying in a hospital bed. However, he's seen the action around him these past few months, and witnessed the responses to my requests made on his behalf. I know he'd give a big thumb's up if he could. read more

I was at A Grace for 5 weeks last summer and I can't say enough about the wonderful nursing staff (Hui Chi & Evita) they were caring and attentive to me and my husband. I love you both. My Physical Therapists were awesome. They never gave up on me and really cared about me and my progress. Love and Hugs to you guys!!!!!! My Respitory Therapists. What can I say? You kept me breathing. Good job all of you. I formed a special friendship with Melissa and I feel our bond will be there a long time. Every RT has excellent skills and we never questioned any of them. Again, love and hugs. The CNA's were great!!! They were friendly and everyone of them were very efficient. Big hugs. The Administrative Staff were very helpful and knowledgeable. When ever we had questions they were there to help. We have been back to visit once and who knows. If you Love your family member or Friend, I'm begging you not to let any hospital or facility send them here. My brother had spinal surgery and was transferred there from Santa Clara Valley Medical Center. From the very beginning he was neglected. He had no call button on his bed to seek assistance from the staff. When he needed to use the bathroom he was barely able to walk and his attending nurse took off and left him alone. Some how he managed to make it to the bathroom, but fell when he got to the toilet. They left him there for a very long time. The other nurse came in and told him he was taking to many pills. To many? They are administrating his medications. Then they gave him an over dose. I spoke to the Supervisor and when I didn't get the answers I wanted, I asked her for her Supervisors name. She refused to give it to Me! My brother almost died and had to be sent back to the hospital and is now in intensive care. This is where people who only have Medicare or limited resources are sent. Our dogs and cats at the SPCA get better care then the patients who are sent here. Please don't send anyone here. They are likely to be killed. read more

This place is one of the worst places that I have ever seen. My dad was placed in there for recovery. Went there to see him the day he was put in there and it was bad. He was laying in a room by himself with the lights out, No TV, No table, no nothing in there. We were told that we had to bring in a tv and pay for cable service. If we wanted him to watch TV. They had him for 6 hours and didn't even get any of his pain meds that his was supposed to be getting every 4 hours. Wound care was ridiculas. For the amount of money they charge their patients the place looks like a ran down jail. The machines looked really old and out of date. What are they spending the money on. ON THEMSELVES. My dad decided to walk out the same day. Think twice before putting your loved ones in there. read more

I completely understand some of the low reviews. However, our experience was not bad. My stepmom was here for several months and I have to say that Respiratory Therapy, Nursing, and Physical Therapy employees were very caring. The staff that we dealt with, particularly RT and PT, were really knowledgeable and took a lot of time to explain things and to push her back to recovery (she needed the push from PT). When the hospital failed to document that she needed a certain type of liquid diet and she showed signs of a blockage, I spoke with the charge nurse and she immediately took me seriously and called the doctor after hours with my concerns. She unfortunately, had to be readmitted, but that wasn't their fault. It could have been much worse if they had delayed responding to my concerns. Again, I understand some of the low reviews, but I've also seen multiple other long term care centers and slow response times, smell, and some of the other items reviewers noted are not uncommon unfortunately (hence this is not a 5-star review). The facility itself is kept clean (we were there every day and the floors were always being sanitized), but it is older looking and the air temp in the building is too warm and that doesn't help with the odors. My strongest complaint is response time for acute events, which we spoke to management about. They absolutely need to have a system for those who have acute events to get a hold of nursing staff quickly. My stepmom would suddenly have her airway blocked and would need suction and there was absolutely no way to indicate an urgent response was needed. They have one type of call button and it can take 30 seconds to 2 hours for a response. Not acceptable for people with breathing issues. For any of these types of facilities it is REALLY important to be present as much as you can and ASK questions. Ultimately, due to the staff in RT and PT, we did choose to come back the second time around and she recovered well. read more

do not! i repeat do not take your loved ones here....ever!! my sister was admitted on a wednesay and by the following wednesday, she was back at the emergency hospital. these people gave her medication and did not document what it was or what time it was given. they gave her the wrong medication and then deny deny deny!! they are liars. push the nurse button? oh no!! that light and buzzer can go off for hours if you dont go up to the nurses station to let them know you need them. took over 20 minutes each time to change my sister!!! place is a dirty mess! sheets are so thin they have holes and runs in them! place smells like poop and the nurses there are so uncompassionate!! director and admissions will tell you what you want to hear. cannot believe this place is still open. they dont even deserve the one star from me but had no choice if i wanted to post. i more than hated it. dont believe me? go see for yourself. read more
