When my mom and I toured The Maplewood the social worker explained that they didn't have outside Hospice agencies come in because they (The Maplewood) provided palliative care and handled end of life care for their residence "We have have a palliative team, doctors, nurses...they take care of everything. " Great we thought! Mom moved in. 4 months later my mom was hospitalized and diagnosed with inoperable cancer. She returned to The Maplewood and we clarified with the social worker and staff- would mom need a hospice nurse? NO. Would she have to move rooms when she got close to dying?NO. "We provide palliative care. That's what we do." They reassured my frightened mom that - "You have a palliative team caring for you. We will make sure you're comfortable." 4/6 I get a call from the day nurse, mom who is battling her second bout of pneumonia in 4 weeks is "having a down day. and by the way she is too weak to hold the phone, but she'd like to talk to you, by the way she is confused and she won't take her pills and her antibiotics aren't working and her pneumonia is worse..." My mom decided in January to be a DNR, and she didn't want to go back to the hospital. I told the nurse I was coming (a 7 hour drive) she said good. It's Friday, my mom is dying I think as I drive. When I arrive at midnight my mom is breathing 35times a minute sounding like a washing machine. Her eyes are rolling in their sockets as she involuntarily opens and closes them. I ask her how she is doing and she says "I ache all over". The night nurse enters the room, I tell her my mom's airway needs suctioned and mom needs pain meds. The nurse says to me (a 25 year ICU RN) "Your mom doesn't need to be suctioned and she can't have any pain meds." I said my mom is dying what do you mean she can't have pain meds? I'll stop right here. It only gets worse. The fact is the day nurse, the evening nurse and the social worker all knew my mom had taken a turn and not a one of them thought to tell the doctor or pa. A doctor or pa could have assessed the situation and recognized she was in multisystem organ failure and dying. They then could have modified her care plan and added medications that would maximize comfort. The nurses and social worker with their educational backgrounds should have done something to make sure her palliative needs would be met! It was Friday, they went home. The Maplewood promised my mom comfort care in the end. Her first appropriate end of life pain med was finally was obtained at 0540. She suffered until God finally took her at 0645. No end of life medications were ordered until 530am despite the day nurse and social worker knowing she was declining. How does this happen? They told her over and over that they would take care of her when she was dying! It was her biggest fear. We believed them. We were happy to pay $20,000 out of pocket a month for such inclusive services! Beware!!! They, the staff, smile a lot and sound so reassuring but YOU don't be fooled. If the end is near insist on seeing the care plan. Insist on seeing the med list. Please please educate yourself about end of life care (palliative and hospice). Pain meds, nausea meds, anxiety meds, meds for excessive secretions should all be ordered and a available as needed for a dying DNR patient. These are all meds that maximize comfort. I assumed they took care of this for mom. Friday night at midnight is no time to be begging for comfort care medications. Heaven help the residents who don't have a voice due to dementia, Alzheimer's etc.or don't have an advocate when they are dying. Update:12/14/2018 change is on the way. After extensive investigation, the New York State Department of Health CITED The Maplewood for NOT providing (my mom) adequate Palliative Care. The "last hours" can be as good as the first seven months!

St. John's has dropped from a 3-star to a 1-star Medicare rating, and my family's experience confirms why. Critical Medical Neglect & Safety Risks Dietary Hazards: Despite being a stroke survivor with aspiration risks, my father was frequently served non-pureed food and thin liquids with no staff present to assist. This was a direct threat to his life. Medical Emergency Failure: When my father was struggling to breathe with low blood pressure, nurses dismissed it. They failed to notify us when he stopped eating or when a failed IV attempt left him untreated. We had to insist on a hospital transfer; St. John's refused to call 911, opting for a private transport that took 2 hours. He arrived at the hospital septic and passed away two days later. Hygiene & Infestations: The facility is infested with mice. I witnessed multiple mice running through the living area. The dining room was filthy and plagued by fruit flies. Management & Staffing Issues Callous Management: When I brought safety concerns to an upper-level manager, his response was: "You haven't shared anything that I don't already know. If you want to move him, I have lots of people who would like his bed." Neglectful Environment: Aides are frequently on their phones, ignoring residents. My father sometimes wore the same clothes for days. Residents would beg me for help because they knew the staff would not respond. Lack of Activity: Memory care residents are often left sitting in front of a TV or alone in rooms. There are virtually no activities on weekends. The "Two-Tier" Reality Do not be fooled by the beautiful courtyard or the locked "Day Program" rooms. Those resources and the higher level of care seem reserved for the private-pay or rehab floors. In the long-term memory care unit, the environment is one of "waiting to die." Summary: While a few select nurses, chaplains, and security guards are kind, they cannot compensate for a systemic culture of indifference and unsafe conditions. The funeral director noted that since COVID, "no one there seems to care." I would not recommend St. John's to anyone who loves their family member. St. John's: Do better.

Most direct care staff are wonderful and doing the best they can for patients. Meals are simply awful. Not much is made in the kitchen. When the menu said "banana bread", I imagined a fragrant slice. What it turned out to be was a commercially prepared, sealed plastic bag with a greasy piece of something inedible. I self discharged after 4 days. I do not recommend this place. My 92-year-old mother was admitted for care, and throughout her stay I have struggled to obtain clear communication, consistent answers, and meaningful involvement in her care despite being her Power of Attorney. I was repeatedly provided documentation from the facility referencing cognitive concerns and her inability to safely return home independently. Despite those concerns, documents continued to be presented directly to my mother for signature after I specifically requested that paperwork requiring signatures be directed to me and/or her attorney. When I raised concerns, I was told the paperwork was "just paperwork stopping PT." I later learned that documents affecting her Social Security direct deposit had also been processed. Months later, I am still seeking complete records and answers regarding what was presented, who presented it, and how decisions were made. I have repeatedly requested participation in care planning discussions and a care plan meeting that includes me. Obtaining information has often required multiple emails, phone calls, and follow-up requests. What concerns me most is not a single mistake, but a pattern of poor communication, lack of transparency, delayed responses, and an apparent disregard for family involvement in important decisions affecting vulnerable residents. I will say that there are individual staff members, therapists, nurses, and aides who have been kind and helpful. Unfortunately, those positive interactions have been overshadowed by administrative issues that remain unresolved. Families deserve clear communication, transparency, and confidence that concerns will be addressed promptly. That has not been my experience.

The prescriptions written by the physician at Fairport nursing and Rehab are not worth the paper they are written on. In fact, Wegmans pharmacy will not fill prescriptions written by their physician because of her questionable prescription practices. The facility should be closed and the management face charges.
