The RN evaluator is also caring, friendly and very capable. If family is involved and communicates they are willing to work with you for the care of the patient. The housekeeping staff is friendly and kind and always want to help. My mom needed doctor appointments and transportation and this was all arranged by the hardworking social worker. She handles the care meetings and coordinates care as if she was a case manager. She handles the hair salon and even additional nursing attention as needed. She walks around and checks in on patients. At discharge she handled all the needed items for home care and worked hard to advocate for my mom. Admission staff was awesome. They were very welcoming and made sure she had coverage with her insurance. They are great to work with. The doctor that oversees the facility was not easy to communicate to in part due to her availability. She is a full time doctor seeing patients in her office during the day. She made an effort to call back after hours but my own schedule also got in the way. It would be best if the doc attended the care meetings upon admission so goals and care can be discussed clearly. It was difficult to pinpoint care plans. Staff would send message to doc that family want to discuss care and it was hard to receive info back quickly or at all. Maybe if the doc was present at limited but fixed hours on site it would help. PT/OT was good and made an honest and persistent effort to assist recovery. It helps if family is involved and asks questions regularly. My mom was independent prior to her medical issue so the goal was set to return her as close to that as possible. You have to make it known to them where she was before very clearly. Otherwise she may be released too early and becomes a fall risk. The food staff is very nice and concerned. They made an effort to give her Greek yogurt often for her digestion. The food is otherwise not very tasty or presentable and comes from the main facility. I'm sure the quality in Claremjnt Manor assisted living is better than in the rehab unit. My mom ate mostly in her room and so could not ask for extra items from the kitchen as those in the dining room. The dining room was filled with behabing persons that need assistance eating and this was hard to see for her to see while eating. She is a social person otherwise. I would definitely recommend this facility. They have helped her recover through 4 events in the past year and a half. No place is perfect but Claremont manor care center does a great job. They want to help and truly care. We are lucky to have them in Claremont.

My family had a very difficult experience with Pomona Vista Rehabilitation Center. My mother was…read moreplaced there following hospitalization and required very specific medical care, including strict dietary restrictions related to a feeding tube and recent surgery. Unfortunately, there were serious communication and care concerns during her stay. Despite repeated efforts to inform staff about her medical restrictions, she was given food that she was not supposed to have. This led to complications, including her feeding tube becoming dislodged and requiring an emergency hospital visit. Even after the hospital provided updated instructions, it felt like important information was still not being clearly documented or followed. In addition to the medical concerns, it was very difficult to get timely responses from staff, and we often felt that our concerns were not taken seriously. This created a great deal of stress and worry for our family during an already challenging time. We ultimately requested a transfer to another facility closer to home so we could better advocate for her care. I strongly encourage families to stay very involved, ask detailed questions, and closely monitor care plans if their loved one is placed here. I am sharing this experience so that other families can be informed and prepared.

We had hospice with us for 1 whole month (until my father passed away). I will say that the staff…read moreas a whole was 5 star (amazing; respectful, kind, courteous). However, we had 1 staff member who came and decided she needed to take pictures of my incoherent fathers private parts. We did not sign any paperwork giving consent for that to be done either. My wife walked into my fathers room to the sight and sound of their staff member taking pictures on her personal phone of his naked private parts. My wife immediately told her to delete the pictures and to never do it again. I messaged the management about it and they apologized profusely & promised me the problem had been rectified and that it would never happen again. We never saw that staff member again either. Outside of that specific issue we had no other issues with any of the staff who cared for my father. The rest were amazing & caring. My other issue was when my father passed. We called and had to wait like 6+ hours until someone came to pick up his body. The real kicker was the staff member who came out to make sure my father had really passed. She was disturbingly more focused on making sure she got a count of all my fathers prescriptions that they had issued him. He's laying dead on the bed and she wants us to run Around the house and track down his medications. She could have cared less about the 20 other medications that he was issued by other doctors; she only cared about the meds that they issued (many of which are simple over the counter meds). She then proceeded to take forever individually counting all the medications and then she poured them into cat litter & mixed with water. Then she left the bag for us to dispose of. Biggest distasteful professionalism in my life that I've seen. My father just passed and I got to sit at a table while this staff member counts medications. Like for real! Horrible method of assessing when something is appropriate to do; timing! Outside of these 2 horrendous issues (of which, I have not sued them over for illegally taking pictures of an incapacitated mans private parts) - I will say that the Visiting Nurses who come to care for the Hospice Patient - they were 5 stars Hands down!!!

Losing a loved one is already hard enough, but GHC hospice took a really hard situation and made it as painless as possible for my family. I will be forever grateful for GHC hospice staff! I searched and visited many facilities prior to me choosing Mountain View Center. I visited them a couple times and was able to meet the Owner, Coordinators, the caretakers, the cook, all the helpers I just knew that they stood out far beyond any others I went to. First thing that stood out was how immaculate everything was and it smelled so fresh and clean. This was very important for me if my mom going to live here. They gave me a tour of facility and the living quarters where my mom would stay. She has her own bedroom whom she shares with a wonderful roommate. They were very careful about matching mom's roommate, so they carefully introduced my mom to a few ladies and we found the perfect one. My mom has been her for 3 years and she loves it. They even have a program where a bus picks up the residents that are able, to have fun filled days dancing, singing, doing crafts, then the bus brings them back. They are very involved with many onsite activities. They have lots of parties, holiday parties too, they bring in musicians/bands so they can dance and enjoy, bbqs, dress up..etc. I just had to share my experience with anyone looking for a place for mom or dad. Dementia again has impacted our family, my dear Auntie is suffering with this horrible disease now. I am not a disgruntled employee and won't be discredited by the owner of this facility. 1) This facility is an absolute death trap. You do not send your family member here to live and thrive you send them here to suffer andie. This facilities owner only cares about the financial bottom line and does not provide the care promised when you drop off your loved one 2) residents are routinely neglected and their personal hygiene is non existent ! They are not groomed daily, have their teeth brushed daily or bathed daily. Although they claim they are ! 3) the staff here has a true heart for the residents. They truly try to give the best care to them but when the owner and management does NOT provide the appropriate tools and necessities needed to provide the care expected even the best intentions fall short. 4) the building is old, dirty and unkempt. Mold, feces, bed bugs and trash riddles the floors of this facility. 5) the management and owner keep the truth from both authorities and families here. They give the run around for any question you might have of your loved ones and of any reports of negligence, abuse or violence in the facility. They arranged for all the necessary equipment and medicines to be delivered to the senior care facility she was at. The lady my mom was in contact with told my mother to call her anytime and that she was available 24/7. This held true when a couple days later, my mom contacted her as my Grandma's condition had deteriorated and they sent out a nurse to be with her around the clock. We had the pleasure of working with Emma and Rosalyn, who were two of the most caring and dedicated nurses you'll ever meet. Their care for my Grandmother was gentle and thorough. They were there around the clock with our family and saw us through to the very end, making my Grandmother's transition and passing as painless and comfortable as possible. These two ladies are truly called to be in this profession as their interactions with the patient and family show their love for people and what amazing hearts they have. We are so blessed to have met them!
