If you care about your loved ones, please take them somewhere else…read more Based on what I witnessed, patients here appear to be heavily medicated to the point that they can barely stay awake or interact with their families. Instead of helping people recover, they often seem completely out of it. Watching this happen was extremely disturbing. Overmedicating patients to keep them quiet raises serious concerns and, in my opinion, borders on elder abuse. Patients deserve attentive care, dignity, and proper treatment while they recover. Families place a lot of trust in facilities like this, and unfortunately this place did not live up to that responsibility.

My mom shared a room with one other person. The roommate was only there for about few days before she had to be transferred out (dementia). When I came to visit sometime after that, I was stopped by one of the personnel and was told I had to don a PPE gown. I questioned this as I wasn't informed I had to do so during my past visits. This is when I was told the former roommate had COVID. I was a bit miffed that this was previously not disclosed to us- lack of protocol. Other improvements needed: -Patients are to receive showers 2-3 times a week. I don't know why some are two and others are three, but my mom was skipped over- TWICE! I asked about it and was told she got one, which was false. -There is a button to push whenever a patient needs help in the room. The response time isn't the greatest. After a good amount of time had passed, I left the room and went to get someone myself when I was there. -I was emailed the admission packet to fill out 11 days after she was admitted and less than a week before she was to be discharged. By then, what's the point? -It was summer with 100+ heat and the room was warm. There were fans blowing throughout the day. I'm guessing there is AC, but it was not strong enough. -The clock in her room stopped working. I thought this would've been an easy fix, but it wasn't fixed until two weeks later. So basically, the majority of her stay there. Despite all of these issues, everyone I encountered was nice. Nice is one thing, but maybe they need more workers.

I needed PT really bad, my initial assessment from PT was several days after I moved in then I never saw anybody else for about two weeks, when I went down there to inquire as to why I was told their records show OT has seen me everyday. I filed a complaint with Medicare for fraud investigation. I was also shocked how little equipment they had, I was told their other facility had all the equipment, so we had to wait in line and there were some severely disabled people there. Absenteeism was a big problem which resulted in over an hour response time minimum, one of my CNAs spoke no English at all I would ask her to do something she would nod, leave and I would never see her again, it got to be where I would yell "don't act like you understand me when you don't!" Finally I went down to the nurse's desk to complain and the whole staff came out to gang up on me, how dare I complain about someone who speaks no English, the nurse finally told me that I am receiving good care and I should be happy to receive the care I'm getting. I complained to the administrator and got an apology, after that when I asked that CNA for something she'd bring in somebody else to interpret (a waste of staff don't you think?). My roommate contracted COVID and they wound up quarantine us both no medicine. I asked the doctor for something like paxlovid any kind of prescription, he turned around and walked out. Was it effective no they wound up quarantine the whole Wing. All the physical progress I made with my PT over the weeks went right out the window and I had to start over again. This used up the time to get covered by Medicare which I felt they should cover as they didn't properly treat me. Speaking of the doctor he played footloose with my medication (they just can't leave it alone), when I showed my PCP what he did, he was livid why go to the trouble of assigning and tweaking medicine only to have somebody change them willy nilly without consulting as to why? I consider that arrogant! I had a talk with the dietitian and was assured everyone was getting their nutritional needs considered only to find out all the patients got cheap sugary Kool aid to drink Food was horrible I lived on sandwiches and take out Good luck getting somebody to answer the phone. No I will not go back.

My…read morebrother has been there for eight long months and from day one he has received the best health care and the best enter-action with staff that I've ever experienced in a medical facility. Even though it's obvious that they are busy; they're never too busy to stop and talk to the patients or their families. Having worked in the medical field I know how important it is to find this characteristic in a facility! I also know that it's very hard to find!!!!! So I appreciate the fact that these qualities are presented so freely every single day. This is the quality that the entire facility gives and if there is a problem of any kind it is taken care of ASAP with care and concern and respect!!!! If that ever get the food to eatable status I myself would freely give this facility and every staff member a (10) ten from the bottom to the top of the employee pool. If you ever find that you have to go to a rehabilitation facility if you have a choice I would by all means try to get into "Bridgewood Post Acute Care"! The shower was broken for the first week I was there and has no grab bars. The CNA's were caring. Our women's room shared a bathroom with a men's bedroom, and I had to clean it every time I wanted to use it. They lost my personal medicine. They didn't give me a release date until I threatened to call the ombudsman. The director of nursing did not return my family's calls. The dietitian worked with me because the food made me vomit. Physical therapy was pretty good. The activities director was great.

My sole reason for being here was to strengthen my legs to walk. Things to be aware of are the alarm system. I believe that the staff uses the emergency door to get to the parking lot and they do not use the code to immediately turn off the alarm. It's deafening. They do not have headsets for the televisions. You are competing with your roommate to hear a program. They do not have phones in the room. Please note that some of the people have mental problems scream into the night. Many are immobile and line the hallways. There are two patios. One is for smokers and the other is not. The non-smoking patio has some landscaping, but it could use some improvement. Mark your loved ones name in their clothing and equipment if you want it cleaned. Otherwise it might not be returned. Bathing is twice a week. I realize that food is prepared blandly and soft, but when it comes to a special diet - beware. I'm a renal patient and my protein for lunch and dinner was either a hamburger patty or chicken. I always was given a scoop of white rice and peaches. Vegetables were either peas, green beans, or carrots and the last two were usually prepared well. I'm sure the food budget was slim. I was fortunate. I was aware of my surroundings and could get to the therapy room to do my exercises. The therapists were friendly and assisted you to do your best. That was the best part of my day. Some of the nurses and CNA's don't always speak English. Many are from various countries. They have too few on staff. A man fell in the shared bathroom and it took over 15 minutes for someone to respond. I found two who were exceptional: LaShawn and Pinky. I'm grateful they were assigned to room 105. I tried to be especially pleasant to all, but it was not always reciprocated. Double Tree Post Acute supposedly is under new management. Come for a visit.     Concerns Regarding Patient Care Standards…read more Observation of Patient Treatment I have become deeply concerned about the overall well-being of the patients. The level of care I have observed falls significantly below acceptable standards. It is disappointing to witness such neglect, and those responsible should feel a sense of shame for their actions. The Role and Responsibilities of Caregivers A caregiver's primary duty is to provide compassionate care from the heart. Unfortunately, some staff members are not fulfilling this essential responsibility. Instead of focusing on patient needs, there have been instances where caregivers are seen standing around and talking about their patients rather than attending to them. Examples of Neglect Patients have been left in unacceptable conditions, including being found in their own urine or bowel movements. When patients request assistance, it is critical that caregivers respond as quickly as possible. If an individual caregiver is unable to help immediately, it is their duty to locate a team member who can provide assistance. Additional issues include trash being left under beds and patients remaining in wet gowns or clothing soiled with bowel movements. Such neglect is unacceptable in any care facility. Accountability and Professionalism It is important to acknowledge that not all caregivers behave in this manner. However, those who do should seriously reconsider their profession. If someone is unable to provide the appropriate level of care, it is clear that this role is not suitable for them.

I was able to witness how teamwork plays a critical role in improving patient outcomes and maintaining continuity of care. The facility was clean, organized, and well-maintained, which contributed to patient safety and comfort.
