Concerns Regarding Patient Care Standards Observation of Patient Treatment I have become deeply concerned about the overall well-being of the patients. The level of care I have observed falls significantly below acceptable standards. It is disappointing to witness such neglect, and those responsible should feel a sense of shame for their actions. The Role and Responsibilities of Caregivers A caregiver's primary duty is to provide compassionate care from the heart. Unfortunately, some staff members are not fulfilling this essential responsibility. Instead of focusing on patient needs, there have been instances where caregivers are seen standing around and talking about their patients rather than attending to them. Examples of Neglect Patients have been left in unacceptable conditions, including being found in their own urine or bowel movements. When patients request assistance, it is critical that caregivers respond as quickly as possible. If an individual caregiver is unable to help immediately, it is their duty to locate a team member who can provide assistance. Additional issues include trash being left under beds and patients remaining in wet gowns or clothing soiled with bowel movements. Such neglect is unacceptable in any care facility. Accountability and Professionalism It is important to acknowledge that not all caregivers behave in this manner. However, those who do should seriously reconsider their profession. If someone is unable to provide the appropriate level of care, it is clear that this role is not suitable for them. read more

Don't use this facility, they dont have any safety protocol set up for pt safety. They left a lady outside in a wheelchair who is nonverbal by herself with no jacket on in a 45 degree weather. No one check on her or out there with her. If you a have choice go some where else please take your love one somewhere else. This place is a death trap!! read more

3.5 stars. Thank you for the help and work you've shown my parent. Needless to say there are many here who need individual recognition for their capable and great work that had direct contact with my parents and those who were working behind the scenes. However, this place also have folks who should probably rethink about continuing their line of work. Overall, we had a decent to good experience here but this required constant vigilance. I am not sure if the level of care would be the same without constant vigilance from family members. So, my advice is heed other concerns that have been mentioned about this nursing home. I put a caveat to the reason why we rated this place 3.5 stars. I can honestly say you cannot rely on most of the workers to do their work properly or with good bed manners. But also bear in mind that the job is hard. And it's especially harder when the patient's condition is also hard. Family needs to be vigilant and involved. read more

Hey Rusty B. It's appalling that you have the audacity to lie to all of these people who have suffered & watched their loved ones suffer at the hands of your facility & your employees. As my uncle is currently, & I use this term extremely loosely, under your care. Every single concern & complaint in each of these terrible rating are still just as true today as they were then. Before I even knew the name of your facility, my 2 aunts & grandmother visited yesterday for the first time. My uncle has been in the hospital over 8 weeks, between your facility & UCDavis Cancer Center. NOT ONCE, has any of us been able to reach our uncle or been given any treatment plan, status update, no name of attending physicians, NOTHING!!! On the rare occasion, someone actually answers the phone, they are immediately told that they only have 2 cordless phones to share amongst 170+ patients?!?! And they are always busy or not working. Family members drove all the way from 3 counties away, to finally get the first bit of information, which was that they stopped chemo & radiation & transferred to you. Where he was put in a room with someone who had Covid-19! Then for more than 3 weeks, again, couldn't get through on the phone, couldn't talk to my uncle, still in isolation, blah, blah, lies! I have been in the medical field for 33 years, I have had enough! My poor family is suffering & completely in the dark because you take advantage of people that don't have the knowledge to know their patients rights & are helpless & uninformed. My aunt that visited has worked in rehabilitation centers& convalescent homes over 40 years. She immediately called me in shock at the filthy conditions, the constant high levels of noise, then she saw my uncle for the first time, and it was enraging! Stuck alone, they have not gotten him out of bed the entire 8 weeks! No physical therapy, nobody asking him how he's doing, they come in do whatever, change IV bag, but never even bother to acknowledge him, engage with him, allow him to communicate with his family for God's sake! My husband is in this facility currently. They are not giving him the correct medication. They have not changed his clothes. And there are no headsets for the televisions. So it is a cacophony of noise 24/7 there. The staff barely speak English at all and therefore cannot effectively communicate with the patients there I have witnessed this firsthand. I asked someone how to get out to the courtyard five flipping times! Her English was so terrible she couldn't freaking understand me. How affectively can they help and treat patients if they can't speak their damn language? Can you tell I'm infuriated with the staff there and the lack of good care that my husband is getting? Of the choices for rehabilitation, I ended up here. I met some of the head staff, but unfortunately I can't remember their names. My sole reason for being here was to strengthen my legs to walk. Things to be aware of are the alarm system. I believe that the staff uses the emergency door to get to the parking lot and they do not use the code to immediately turn off the alarm. It's deafening. They do not have headsets for the televisions. You are competing with your roommate to hear a program. They do not have phones in the room. Please note that some of the people have mental problems scream into the night. Many are immobile and line the hallways. There are two patios. One is for smokers and the other is not. The non-smoking patio has some landscaping, but it could use some improvement. Mark your loved ones name in their clothing and equipment if you want it cleaned. Otherwise it might not be returned. Bathing is twice a week. I realize that food is prepared blandly and soft, but when it comes to a special diet - beware. I'm a renal patient and my protein for lunch and dinner was either a hamburger patty or chicken. I always was given a scoop of white rice and peaches. Vegetables were either peas, green beans, or carrots and the last two were usually prepared well. I'm sure the food budget was slim. I was fortunate. I was aware of my surroundings and could get to the therapy room to do my exercises. The therapists were friendly and assisted you to do your best. That was the best part of my day. Some of the nurses and CNA's don't always speak English. Many are from various countries. They have too few on staff. A man fell in the shared bathroom and it took over 15 minutes for someone to respond. I found two who were exceptional: LaShawn and Pinky. I'm grateful they were assigned to room 105. I tried to be especially pleasant to all, but it was not always reciprocated. Double Tree Post Acute supposedly is under new management. Come for a visit. Went to visit relative for 1st time July 21, 2024. Staff was wonderful but as I walked down the hall to his room the smell of feces and urine was constant. Hallways were crowded the carts and wheel chairs and rooms are too small for two patients. Looks like place has not been painted inside for years. I looked out window to dead grass...not much of a few. I left feeling sorry for my relative, as he will be there for so long. He kept telling me the place was awful, and I thought he was making it up. Turns out, he was right. Wish there was someplace cleaner and brighter for his remaining time. This "for profit" facility is keeping their profit, and not reinvesting. I have a difficult time giving it even one star.....Again staff is so friendly, and with a care giver shortage, this "for profit" facility needs to make their working conditions much better. This place is a health hazard. My mother in law was brought here after a 45 day stay in the ICU. Her first night, a CNA named Rosemary, told her that she was "too fat to rotate" and that her back went out so she could only rotate her only every 2 hours. My mother in law was on life support and had major surgery, collapsed lung etc and her pain medication order was never sent to them by her doctor so instead of helping adjust her to get her in positions where she'd be a bit more comfortable, she was told by that CNA that she wasn't going to "sugarcoat s**t" and that she should "be thanking God that she's even willing to rotate her every 2 hours". They checked the cameras and confirmed this happened and then brought in the director to come apologize. My MIL agreed to give them another chance with the exception that Rosemary was never her CNA again and they agreed. Her call light was also mysteriously snapped in half so it was not working for over 24 hours and we had to keep asking for a new one. 4 days later, my husband went to check on her and she was very confused and unlike her normal self. The CNA confirmed that she was like that all day, but she ignored it. My husband told me about it an hour later when I got home from work, so I called to check on her and spent an hour calling and no answer. I panicked and called/texted the social service worker's personal cell phone that I found on her business card when we complained about Rosemary and she directly called a staff member who went to check on her and found she was unresponsive. Luckily the social worker directed them to call an ambulance and she made it there in enough time to reverse the toxin level that was causing her "confusion". In the time leading up to this event, we also got absolutely no communication from the staff regarding her care plan. This place is negligent, dirty, worn down, full of uneducated, rude and poorly trained (or just lazy) staff. Only put your family members here if you're ok with them dying. read more

This place is scary. Our father almost died here in December 2015. He was a fall risk but they did not have a bed alarm or pull alarm for him. He got an infection and got disoriented and fell on the way to the bathroom. According to his roommate they yelled for help for 10 minutes before anyone came. The nurses claimed they called us about the fall but we got no voicemails or calls. We showed up the next day to visit and my father was a complete vegetable almost. Non responsive. Unable to walk. And I had to MAKE this facility call an ambulance to transport dad to ER. I filled a grievance but of course never heard back. Do your research. Put your loved one in a good place that will care for him/her properly. Not here. read more

A family friend needed care for his elderly father who was on hospice care. Double Tree's staff took care of his father and the family needs all the way to his death. My father was a resident at DoubleTree for almost 1 year before he passed away in July. My father loved it there at Doubletree it was always clean the staff always friendly and very attentive to my dad and to me. 1 month before my father passed away he had been in the hospital for over one month and I was really afraid that once he got released from the hospital he would not be able to come back to DoubleTree and his bed would be taken ,I was so blessed to have reached out to the director and his bed was still available for my dad to come back to. I am thankful that my father was able to pass away at Doubletree amongst the staff and residents that were his friends. I was so thankful to DoubleTree that after my father passed I came back and brought athank you card and pizzas for all of the hard-working staff, social workers and managers. If you're looking for a skilled nursing facility that is clean, safe, attentive staff, DoubleTree I would highly recommend. read more

I was a patient there after I fell and broke my ankle. It's not a bad place. Most of the CNAs and staff were carrying and helpful. The physical therapy I received there was good and for the most part I felt safe and happy there. It was a lot better than the hospital. read more

This place is horrible. DO NOT place your loved one here, they will not receive good quality care or almost any at all. I had my uncle at Double Tree for a couple months and the care he received was unacceptable. I walked into his room once and found him on the floor after he had fallen, and no one had noticed. He informed me that he had been on the floor for about 10 minutes without any help, or anyone coming to check on him. It was very hard for him to speak so he was unable to ask for help. My uncle had very advanced pneumonia and was never supposed to be lying flat, I found him once lying flat with mucus all on his sides basically choking on his own mucus. The CNAs were not helpful when I informed them that I was upset with the quality of care. I contacted the public health department and they conducted their own investigation. My uncle has passed away, but shortly after public health contacted me to apologize and let me know that they had found the center to be unfit and not providing adequate services. I am writing this review because I do not want other families to go through what we did because of this place. read more

This place is horrible! It's dirty and your loved ones are not receiving any type of care at all. My grandmother stayed there temporarily and they never changed her clothes and her neck is stiff to one side. They gave her some kind of drugs cause she was completely out of it!! STAY CLEAR!! The state needs to come in and shut this place down!! read more

Double Tree Center is one of the most dirty, dirty, dirty place on earth. We put our mom at this center because no one was home. In one month, she developed a disease that killed within a month. She got five different infections and diseases from this center. The doctors had never heard of one disease in the United States. They transferred my mom from Sacramento hospital to UCSF Medical Center as they could not control the disease from spreading. The people over there will come by all the time when you are there once you leave no one will even give patients glass of water. Another lady was visiting her mom and she told us she found my mom on the floor and no one was helping her until she got help. City of Sacramento needs to investigate the individual that gave them the health certificate, as no one in their right mind will ever left them pass the health inspection. City of Sacramento health inspectors are crooks and I testify under oath and proof it in court! If you want to kill your loved ones, then take them to the Double Tree Center. read more

I would not recommend this facility even though it is the place the Veterans Administration NCVHCS has my father in. I have found my father unshaven, sleeping in the hallway, not bathed, stinking of urine, and they even placed him on psychotropic drugs to shut him up. He is a fall risk and they don't take the time to regularly monitor him in the evenings or have the medical devices such as a bed alarm or motion detection systems most facilities incorporate to mitigate or prevent my Father from exiting his bed during the night. My Mother has found it necessary to visit, to prevent it from reoccurring, every day since finding him in this uncared for condition and has to stay on the management to ensure he is cared for with dignity and respect. Both myself and mom have been disrespected and even threatened by the Head Nurse when we have complained about his condition. We have already discussed this situation with the VA Patient Advocate and his VA Social Worker. This place is one step from being reported to the VA Contract Administrator and the VA Office of Patient Affairs if we experience one more flippant, unprofessional and or aggressive behavior by the Head Nurse. SHE NEEDS TO GO OR BE REPLACED WITH SOME ONE MORE PROFESSIONAL. The rest of the staff seem to be competent, diligent, polite and professional and value the input and opinions of the family members and they exhibit a true care and compassion for my fathers well being and recovery, when notified and followed up by the family. But they must be monitored by a close family member to ensure they provide the proper and appropriate care in a timely manor. Its appears they are doing the best they can considering the situation under the management of the disgraceful and arrogant Head Nurse.
