My case worker immediately told me not to look at yelp reviews, instead look at their score on the Medicare site. I wish I had not taken her advice. I chose this place based on its Medicare score and regretted it the moment I arrived. Before I go into all that is wrong with this place, let me say that every one of the staff are extremely nice. The night staff are a little loud, but still very friendly. Unfortunately, that is the only good thing here. Now, the bad. Food - food is the worst I've ever eaten. I mean, the hospital food was really disgusting; this is worse. I've spent a lot of money on door dash the week I was here. Cleanliness and appearance - the place is rundown, the carpet is stained and things are falling apart everywhere. The thermostat in my room seemed to only have two settings - arctic frost or Sahara hot. Nothing in between. Facilities - The showers in the bathrooms do not work. They have one shower area for the entire wing, thus you are only allowed two showers per week. There is only one small outdoor area to sit, it is on the resident side. It is small and rundown. The only "common area" on the short stay side is behind the nurses desk and consists of two tables, usually occupied by staff. There is a larger dining area on the resident side, as well as a very small seating area. There is a good sized activity room with a tv and dvd player, some books, puzzles and two board games. The books are random and old, the art supplies are old and not kept up. OT - the tech was very nice but the OT program is poor. They had me do a few arm exercises and then had me sort a tray full of washers, bolts and beads into little plastic cups. Seriously. Oh they told me a story about this woman Lois who was a successful stock broker who married a really good looking guy. They had 3 children and she decided to stay home and raise the kids until they were teenagers before she went back to work. Then they asked a bunch of questions like what was her name. I was preoccupied though. I wanted to know how Lois and the really good looking guy paid their bills for the 16 years or so she wasn't working. PT - again, poor. My PT consisted of getting in and out of bed, walking with the walker and getting in and out of a wheelchair. I was supposed to learn how to use crutches to go up and down stairs because I have six stairs to go up to get to my front door, but they only had three crutches in the whole place; two were for the jolly green giant and one was for Tom Thumb. So the skill I needed to learn most couldn't be taught. Summary - I'd like to say don't come here it's horrible, but I'm sure there are a lot of places worse. If you end up here, you're looking at laying around in bed 23.5 hours a day. If you do get out of bed, you're pretty much on your own. Activities consist of bingo twice a week, movie night once a week and snow cones once a week. This is clearly a nursing home for people in the latest stages of life; the rehab was probably added as an afterthought to generate more revenue. I wonder where they send younger people who need in-patient pt after a hospital stay. Because I was probably the youngest person there - and I'm 65!

My wife spent three weeks here in March 2026. The quality of care was excellent. The food was -…read morewell the food you would find in a facility like this, repetitive and rather bland. My wife required a high level of assistance and the staff was always attentive and response. Being located in a smaller City, the staff were representative of the small town, "we care" attitude. The litmus test is, How Did She Do, Once I Brought Her Home? I was instructed on all of the medical gear that I should have to ensure she could transfer successfully. With all of the new gear that we used, she got stronger and stronger. As a side note: During her three weeks in Forest Grove, I had an opportunity to sample a lot of the local restaurants and that is a plus for friends and families of residents. Some great places to order food to go and take it back to the facility. In fact, we are still going to some of the restaurants I found. As an overall experience- I give them a GOLD STAR.

I've been here…read moresince August 2019, and have come to truly appreciate how sincere each and every employee is in their care for our residents! My coworkers are an inspiration, and I regularly receive compliments from residents and their families regarding the care we are providing. Before coming to work here, Avamere had been a customer of the company where I previously worked. I had the opportunity to work in Care Communities (assisted living, skilled nursing and rehabilitation centers) all over Oregon and Washington. I worked in communities and facilities which were run by Avamere, as well as their "competitors". From my perspective, you could feel a difference in Avamere versus the others. The staff were genuine, and offered sincere compassion for their residents and patients. The administration seemed to be connected with the staff and resident and patients, as well. It was the many opportunities I had to work with the different companies, and in so many locations, which led me to want to work for Avamere. Unfortunately, I had signed a non-compete clause with my former company, and therefore had to be separated from them for 2 years before I could be employed with Avamere. So, I did my time! Now, I'm here, at Avamere (Avamere At Hillsboro) with intention! I love this company and couldn't be happier with the decision I made!! Just want to warn everyone to keep they're loved ones away from this place. I am a former employee…read morefor Avamere. I worked in the dementia unit for residents that need extra care with day to day activities such as going to the bathroom, getting dressed, etc. I only worked in the dementia unit so I cannot speak for the assisted living section. I treated our residents with the dignity and respect they needed and deserved. After working there for almost a year I noticed the manager, Executive, care staff and nurse turn over rate was brutal. Not one person could keep a job there because they kept getting fired, I didn't know why until about a month before I was let go as well. Everyone who tried to report avamere was fired. residents were getting neglected in one way or another from nurses giving expired medications (like insulin for our diabetic residents) to the care staff laughing at residents and giving them terrible nicknames that correlated with disabilities that they had. I reported this kind of behavior to my managers and executives and they made me believe they were doing all they could to prevent it. After months of this vulgar behavior i felt that I could not stand by and watch these residents that I had become so close to be neglected and abused. I called the regional nurse and told her my concerns for the residents because my direct supervisors were taking no course of action after 5 months of this behavior from the staff. She pretended to hear my concerns and as soon as I was done talking to her she called my direct nurse and I was brought into her office and given a warning about "going over her head". A couple months go by and the behavior of the staff seemed to be getting worse. One day my co-worker and I were on break at the same time and I expressed my concerns to her and told her I wanted to get in contact with the state board of nursing and report Avamere at Hillsboro as a whole. I grew very close to most of the residents I took care of and hated the thought of them not getting the best care they could if the managers would just do they're jobs... well my co-worker went to my direct supervisor and told her that I wanted to report Avamere to the state for elder abuse. Less than a week later I was pulled into my direct supervisors office and was told they would have to let me go because I was "not getting along with my co-workers". A report to the state would seem a lot less believable from a disgruntled employee... Please don't put your loved ones here, at least not in the dementia unit. The staff feel like they can get away with whatever they want because residents with dementia cannot speak for themselves in most cases and if they can most people will not believe them. Avamere does not care about its residents or the residents families. They are only here for the money. This truly broke my heart.

I had a Friend who went here for Therapy after getting into an accident and the THERAPY team was a…read moredream, they motivated him to do his best, even when he didn't always feel like it and todays he's walking again when we didn't know if he would. The CNAs were always attentive and polite even when he was not. The nurses helped with his wound care and kept his pain under control. The care managers made sure he got to his appointments even tho dealing with insurance was a headache but it really helped take the pressure off his wife so she could focus on his healing. The administrator is so kind and you can really tell she cares about her team. The doctors and his team made a difference in my friend's care. Best Part is when I came to visit, it was clean and didn't smell like a Nursing home, if you know what I mean!

Support staff from all the Nurses, CNAs , kitchen staff and housekeeping were very thoughtful and kind. they truly made our stay there so nice and inviting. Highly recommend this facility!
