The kitchen staff does a excellent job creating tasty and nutritious meals. The administration staff walks the fine line of corporate needs and patient needs. it is necessary to advocate for yourself at times, but that is actually a healthy part of your rehabilitation process in my eyes. I was a patient here for over a month, during that time I met some amazing people on their staff! One thing to consider when you're coming from a hospital environment or a critical care unit, this isn't that. You won't get a 2 second response on the call light, and your needs may be met in order received. be patient be understanding, be courteous to the staff and it will return to you tenfold. be sure to ask questions, advocate for yourself.

Having a loved one in post-acute care can be stressful and emotional, but the compassion and kindness shown by everyone here has truly made a difference for our family. Every staff member we've encountered has been helpful, patient, and genuinely caring. They don't just treat my mom like a patient--they treat her like a person. Over time, so many of the staff have become familiar faces and almost like friends to our family. What has touched us the most is seeing staff members come in, sometimes simply to talk with my mom, check on her, or spend a little time with her. Those moments may seem small, but they mean so much to our family. It shows that they genuinely care about her as a person and not just as another patient on their assignment. Everyone has been so willing to answer our questions, listen to our concerns, and make sure my mom is comfortable and receiving the care she needs. Whether it's the nurses, CNAs, therapists, or other staff, we have consistently felt supported and cared for. We are incredibly grateful for the compassion, patience, and kindness they have shown my mom and our entire family. They took a situation that began with a lot of worry and uncertainty and made us feel welcomed, supported, and reassured. We will always be grateful for the people here who have gone above and beyond--not only in caring for my mom, but in taking the time to make her smile, talk with her, and simply be there. I would absolutely recommend this facility to anyone looking for a place where their loved one will be treated with dignity, compassion, and genuine care.

They haven't brought him a walker. My son is only forty six years old. He took a few steps already, but that's already been a month since. Now they want him to stay in until the fifth of January. They were supposed to let him out on the twenty third of december. So he could be with his family. Really ridiculous. And every month he was supposed to get out on the twentieth, but they keep stalling, they say it's the insurance. The devil's a liar! And his mother and this place

Manuel (Manny) Salguero From the moment I met him, Manny demonstrated extraordinary compassion, professionalism, and dedication. He went way above and beyond in making me feel comfortable and cared for during what could have been a very difficult time for me and my family. What stood out the most was his warmth and attentiveness. He took the time to get to know me, treating me not just as a patient but as a person and a friend. His kindness and humor brightened my days, and he ensured that I always felt at ease. Not once did he make me feel rushed or overlooked, even when the Health Care Facility was clearly busy. Moreover, Manny was incredibly understanding and accommodating with my family. He never hesitated to answer all our questions, kept us informed every step of the way, and always welcomed my family during visits without making them feel like they were intruding. His patience and care made all the difference to me during this challenging time. I truly believe that Manny embodies the highest standards of nursing and patient care. He is a credit to Oak park Healthcare and I hope this letter reaches those who can recognize Manny for his outstanding contributions. Please extend my heartfelt thanks to him on behalf of my entire family. I am I forever grateful for the care he provided and the peace of mind he gave to us all. Sincerely, Patrick J. Romero

I urge you to carefully consider this before contracting with any hospice provider. Unfortunately, our experience with Faith & Hope made an already painful situation even harder. In April 2025, my mom suffered a massive stroke. She was placed under Continuous Care (CC), which meant she required critical, around-the-clock nursing support. We were told her case would be re-evaluated after three days. However, the care was stopped in less than 24 hours and we were given only 4 hours' notice that services would be discontinued. At that point, it was clear our needs no longer aligned with Faith & Hope's philosophy of care. We scrambled to hire private help on extremely short notice. That night, my mom was in significant pain & experienced agitation. She needed her medication adjusted and an increase of the dosage frequency to help manage her symptoms. The next day, we made final attempts to Faith & Hope to reassess mom's eligibility for CC, even though the on-call night nurse, when we asked, did acknowledge that we were free to seek another hospice provider. We waited all day for a response from Faith & Hope, but heard nothing. Faced with another evening without adequate care, we transferred my mom to a different hospice. They gave us the right care without hesitation. Sadly, our mom passed away only 4 days later. The only positive experience we had with Faith & Hope was with Nurse Theresa. She cared for my mom with compassion, dedication and a true commitment to patient advocacy. Theresa's thoughtfulness and support helped guide my brothers and me through an incredibly challenging time, and for that we are deeply grateful.
