    Please do not ever bring your loved one here. The pictures are outdated just like the facility…read more It's downright awful and the administrative staff there have absolutely no business running this establishment. The smell is atrocious. Stifling even if you visit after 5 pm. Administration stays in their office so they don't have to inhale it. If you bring attention to it, they will turn around and say that you're raging or are being aggressive with them because you're wanting and needing and begging for care for your loved one or begging them to make sure your loved one has a safe space that is not littered by wheelchair parts in their room and random people's underwear thrown in her closet Or even worse when they put someone else's underwear on my mother. Anything in this facility that is not bolted down has the ability to grow legs and disappear and I do mean anything. They have known thieves that are residence in the facility yet nothing is done about it. The other residents suffer for this. Please note there are few nurses that are gold to the core!! I've given them reviews and accolades to other admin there and kudos but it stops there. Morale here is down for the entire facility bottom to top, admin to clinical, including the residents. This reflects clearly that there is a DIRECTOR problem, and it is evident that this place is not being directed in a very good direction. I have seen nothing but decline since my mother has been there at this facility under current management. It's just nasty. Residents have lost all morale and just rock back-and-forth in their doorways. There's no activities director so the biggest thing they do is play bingo for a soda. If you do not do anything else ever, please understand that this facility has violent offenders in this facility and there have been physical altercations while my mother has been there. Please , If you were having to make this hard decision for your loved one, I can tell you with a calm, caring voice to please do your research do your visits, do not listen to the sales pitches their marketing throws out, do not trust ANY pictures posted!! You have to dig deep in that place just to find a chair that is safe to sit in. The entire facility is just dilapidated and that is the truest word I can think of to describe this place. My mother went in there, clean and mobile. Now we do not bring family to the facility because we're embarrassed of the stench that meets us at the door, my mother is always in dirty clothes that does not belong to her and smell smells like a sewage plant. We have to pick her up from the facility, take her to our home, bathe her ourselves, clean up the chronic fungus that she has from the facility that will not treat her Medically for it consistently, Bandage everything up, make sure we have new clothes for her before we can even sit down and enjoy a dinner and relax,. This is not a dramatization or an exaggeration. This is what our life is with a loved one at this facility. I have posted what 30 months ago looks like vs 18 months in this facility. We were told that my mother would get all kinds of cognitive therapy and physical therapy, but again, the residents are just there to breathe and the evening shift are all too happy to hurry up and get their medicine and food and make them get in their rooms because they don't want to deal with them. This Facility has more issues than Cosmo magazine. Do not put your loved one here I beg you!! We'll see how long this post stays up over on Google since the executive director likes to block reviews so the fake ones that are posted from the facility's family members and friends with fake accounts make the facility look better . Great directing here ! They want absolutely no transparency here!!!

    I have now lost 6 brand new pair of diabetic socks maked with pink thread on cuff of every single…read moreone. And my eye glasses, plus 4 pair of slipper socks and cash money while living here. Glasses and socks all lost this week. Glad I am going to move. Brand new socks were never worn, marked them with pink thread and was saving them until I got a pair of shoes. Now have shoes, but socks are gone. Was kept in a drawer of a night stand next to door.

I was reminded of Parkwood while talking with my mom's home health nurse. She has a new patient in Parkwood. I was interested in reading reviews to see if anything had changed. Doesn't look like it. Whatever you do, do NOT place your loved one in their memory care. Take a look at it and you'll know why. Four years ago, we were all still dealing with problems resulting from Covid. I tried to explain the issues we had at Parkwood on that. Primarily, the caregivers were very inattentive. My parents were in Parkwood for six weeks rehab. One day I found my mother lying across the bed with her pants down below her hips. An aide had taken her to the bathroom and left her like that when she mentioned that at home her husband normally helped her with that. The aide said ok and left her. I was more than happy to get my parents out when their rehab was over. I don't know if the director is the same person, but the director at that time was totally indifferent and did nothing about the issues I brought to his attention. Only unpleasant memories of Parkwood. I do not recommend it.

    I will start with the facility. Yes, it is as old as dirt…read more.. My mom is there after suffering a massive stroke while I was at work. She was admitted into the Rehab. section (back area of building) for 30ish days. That area of the facility 100% excellent. All staff members are A+. Physical therapy is also excellent. They have rehabilitated my mom to some degree, but there is still a long way to go. She is blind in one eye and paralyzed on right side of body. She cannot speak, walk, or feed herself. She is also suffering with two blood clots in brain, has heart and kidney failure. She has high blood pressure, dementia and copd (even tho never smoked in her life). I have a very sick mother... She is now in the long-term care section (front). The problem is there are a lot more patients. Most of the staff really do care about my mom. If I see something then I address it with kindness because I know they care. It is hard to care for an elderly person, I know that. She lived with me until the massive stroke, but I can sleep well knowing she is being watched 24/7. My piece of advice to anyone who wants it from someone who is experiencing this. Do not just leave your love one there without frequent visit, input, and assistance. One thing I do is I choose to wash my mother's laundry so it does not get lost. Also, while visiting if I see she needs her diaper changed then I do it. If I see she has spilled something then I clean it. It is not because they will not it is because they have not gotten to her yet or she just did it. I decorated her area in the room. I brought in a tv and wall mount and hung it on her wall. I know when meals are served. If I can come and help feed her I do. She fell out of her bed. I didn't blame the staff, I know my mom. She was trying to get out of the bed. I have her on hospice and I simply ordered a bolster for her bed to avoid that happening again. It arrived in one day The other day I heard the nurse say to my mom, we are not going to give you your blood pressure medicine today because your blood pressure is really low. My mom didn't understand them, but I did. That is something I would have never known to do at home. I'm there as often as I can to ensure everything is well with her...That is my job...but they do their job just fine. It's a lot and no one is perfect and I don't expect them to be either. Also, I now understand why patients are up front in wheelchairs. They are watching them to ensure they don't fall or they are avoiding the possibility of bed soars, etc... I'm learning as I watch...I give input when my mom is up there. For example, make sure she has her cell phone, so you has something to do and yes, she can be there a little while, but eventually she needs to be put back in her bed. She is old and sick. Again, that is my input and guidance that I provide to them. They need that information, they don't know her like I do. I read the bad reviews and I feel for them, but that has not been my experience. Yes, I have had my moments when I want to take her home, but only because of guilt, nothing to do with the care they are providing. I hope and plan to take my mom home one day, but she just needs more time to get better. For now, I am okay where she is, but I will never leave her alone, I am there every single day. I made sure my mother is close to my home. I can be there in 5 minutes. I am her voice... It is a shame how many forgotten residents I often see without any visitors. I will never leave my mother alone. Overall, I'm happy with the facility. I gave it 4 stars only because the facility is old and can use some improvements, like new furniture in rooms and maybe a few more staff members to help with patients who need feedings, returned to room, etc. It is a thankless job and most of them do a good job and do care. One last thing, I give two thumbs up to the cleaning staff (Terry) and Laundry lady. They work hard to maintain the facility. I don't know everyone's name but they sure know mine. They all know my moms name.

We need more like her she has been a true blessing even though my mom don't understand her helping her out she has alot of patience and cares alot for all the patients she has to care for.     My wife suffered a brain stroke April 18th 2026. This was a devastating situation for our daughters…read moreand I, as she is a healthy and active person. She was two week in intensive care, a week in a regular room and the released to start her treatment at a rehab. I had no idea which rehab to use, and asked for a recommendation, I was recommend Encompass Health, out of many options. My daughter read the reviews and there were a variety of mixed comments, this is mine! From the moment she was moved from the hospital to Encompass, treatment as a patient was very human and very respectful. Once in the rehab, the treatment was nothing short of perfect! The nurses and techs were all very very nice people who obviously take their profession and responsibilities very serious. The director for Encompass himself came to introduce himself and dropped his card in case we needed to contact him for any question or discrepancy, which we never had! The rooms were very nice, clean and kept that way. The therapist were just great, my wife's recovery has been nothing short of miraculous, God in place and the work done the Encompass team and therapist was the best combination to her recovery. The Doctors were constantly visiting and giving updates of my wife's condition. She was offered daily a menu for next days breakfast, lunch and dinner, and the food was just great! She entered the rehab April 6th and was discharged Sunday April 19th, walking, talking and on her way to being independent as she was before the stroke! It is very difficult to see someone you love and have know all your life, suddenly helpless and wondering what the future hold for that person and the family as well! Bear in mind that in this rehab there and many many patients, some in very delicate conditions, so we where not expecting that in a case of some need they staff could react immediately, they did the best at hand and that is accounted for!     Our family's experience with Encompass Health of Mid-Cities was appalling. Based on what we…read morewitnessed, I would strongly urge families with medically vulnerable loved ones to look elsewhere. My father-in-law arrived after major surgery with bilateral C5 palsy, a rare condition that left him unable to use either arm. He could not feed, bathe, toilet, shave, reposition himself, or perform basic daily tasks without assistance. Despite detailed discharge notes from his surgeon, during his first week we repeatedly had to explain his condition to nurses because they appeared unaware of the extent of his disability. Families should not have to educate medical professionals about a serious diagnosis already documented in a patient's records. On his second night, while in significant pain, he asked a nurse to reposition him. According to my father-in-law, the nurse said he had back problems and could not help. A man who physically could not reposition himself was left suffering for hours. That level of callousness toward a completely dependent patient is indefensible. We complained immediately, and Encompass did remove the nurse from his care. Our experience with Case Manager Debra Otness was especially infuriating. During our initial family meeting, discharge home was already being discussed despite the fact that my father-in-law could not use either arm or independently perform basic activities of daily living. We were repeatedly promised advocacy and additional therapy that failed to materialize. Instead of feeling that his case manager was fighting for a vulnerable patient, our family felt we had to do the fighting ourselves. We also have serious concerns about Dr. Muppidi's management of his care. Our experience left us deeply concerned that the surgeon's detailed discharge information and the severity of his bilateral C5 palsy were not adequately understood or incorporated into his treatment and discharge planning. The fact that sending home a man who could not use his arms was considered reasonable was astonishing. I am calling on Mindy Hong, CEO, and Prathusha Maduri, Medical Director, to investigate how this happened. This was not simply poor customer service. It represented, in our experience, a profound failure of preparation, compassion, communication, and patient advocacy. Encompass was entrusted with a man who literally could not care for himself. He deserved dignity, attentive care, and people willing to advocate for his recovery. Our family is disgusted by much of what we witnessed. If your loved one is medically complex, physically dependent, or unable to advocate for themselves, I would think very carefully before choosing Encompass Health of Mid-Cities. Based on our experience, I cannot recommend this facility and would strongly encourage families to explore every reasonable alternative.

The roach I initially discovered, and the nurse and I tried to hunt down was not the roach that I stomped that crawled from the other side of the room. My family member was also not provided a bed railing which if it weren't for the roommate, they would have collapsed at night and experienced a hard fall. Lastly, if the above was not insult to injury, the bed sheets were not clean and my family member was neither cleaned the following day nor provided breakfast. Suffice to say, we discharged our member and brought them to a better facility. I would strongly advise anyone considering to bring elderly to this rehab center to check the facility rooms, swollen baseboards, closets and bathrooms and not go off the "4+stars" it currently has so you are not left with disappointment and anger.

    PLEASE do not put your loved one here. The staff (mostly) were not very attentive. AS with every…read moreplace she did have a couple that treated her with kindness and respect.. When mom pushed the button needing help, they turned it off and said they'd be back later. It was often 30 + minutes before they came back. Once they refused to leave the trash can by her.. ( not the cleaning people the one gentleman that I met was very polite and seemed kind). Mom told that person she has been feeling sick, and needed it by the bed.. That nurse was very aggressive and refused to leave it there.. She ended up puking over the side of the bed. One came in and put a paper towel over it, until another one came back in to clean it. She saw people taking her tray, and eating on it when they were walking out. NO RESPECT , talking loudly on the phone or right outside the door all hours, day and night. They tried to keep her from going to Baylor Hospital, after she began having severe pain. ,She had been seen by their "DR" and she was supposed to be fine. She got to Baylor, went into emergency surgery for gall bladder removal. When she went to Baylor, Bedford Wellness sent NOTHING with her. Every time she has been moved to a facility from hospital or visa versa, ALL her things were sent with her. A nurse or someone asked if they needed to take anything with her in transport. That question was answered with :" no she'll be coming back". She DID NOT. Once leaving from Baylor she came to a facility MUCH closer than moved in with me. I called COUNTLESS times to pick up her things. Left messages AND spoke with her "social worker". NO return calls after the initial conversations. All of her clothes had her name or room number written in them. So it would not be difficult to round them up. All the things I had brought her to snack on, anything I could bring to make her stay easier, I brought.STRAIGHT UP STOLEN. There was always a language barrier with almost every staff member there. This facility was recommended by JPS hospital FTW "social worker" saying that was the closest to me..( again apparently a language barrier , or was it?). Being told that was the closest to Granbury THAT was a lie .Bedford is Over an hour away from me.. THEN when she was going to Baylor for pain. Bedford Wellness sent her to Baylor in Grapevine.. EVEN FARTHER!!!. There were several facilities to choose from closer to me, and Baylor FTW would have been closer as well . After getting her itemized statement from her insurance, there were medicines that she was charged for and didn't get daily as it stated in the statement. I was there at least 3 days a week, and with my job it was always just before bedtime, and mostly on weekends.
