I took care of my father and mother before they passed so I know nursing homes ALL OVER PHOENIX AREA including Tempe, Chandler, Mesa, P.V., I know how scary and terrifying some of these places are! That is not the case for Phoenix Mountain on Tatum. The nurses like nurse Barry and nurse. Tristan are amazing. I can't say enough great things. The rooms were clean. The food was fantastic. OK there was a couple days a couple meals are weird, however, 90% of the time the food was phenomenal! When I was there, they had a fantastic chef! The physical therapy team outstanding, the nurses aids or the CNA's down to the housekeeping crew we're incredible. They kept that place immaculate. Although the nurses are overworked, they need a raise and they need to have a one nurse per hallway!!!!! then it will run so much smoother for the nurses. The nurses worked their buns off! and I am so appreciative of that and I am so grateful! I cannot tell you how pleasantly surprised I was of how great this place was!, the people that lived there and that were in my same situation were awesome. My room was lovely. I overlooked a fountain and had lots of sunlight and I had my own bathroom. My roommate was incredible Patricia. She's a 93-year-old blind woman who is the most interesting person. She was awesome, again the place was fantastically clean! It looks a little old older, but it makes up for everything of how great the nurses are and the care is, it was great. The food was great. They even have an area outside where they had a big barbecue and we had hamburgers and hotdog for everyone and they have tons and tons of activities. If that's what your people are into, they had games they had workouts. It was incredible. I can't say enough good things and I'm only in my 50s and I was so surprised but so grateful! I also did have a loved one who stayed there and she had a good experience there too!, so take it from me, I was just there the last day of February 2026 for an entire month until April and I cannot say enough good things about this place! As a healthcare professional, it's a shameful disgrace the way staff are treating human beings. Our facility just received a new resident from there and let me tell you how awful he arrived. He appears to be neglect; he had no pants on and was literally full off crap, hair oily, face dirty, clothes were dirty. Overall if you want the bare minimal care this is the place for you. But if you want the care that you or your loved deserves look elsewhere.

I can truly tell that she cares so much about what she does. She worked with Henry for his first few visits, and even noticed some other things he needed help with that our doctor didn't notice! She is amazing. When he first started here, he was barely speaking and also had some issues with his muscle tone. He speaks so much now and is so much stronger! All of the therapists here are wonderful. I can't say enough good things about them. He usually sees Miss Malia and Miss Cass, and he always is so happy to go play with them. They always tell me what they did with them, and what I can do at home with him. I am so glad our doc recommended them. I highly recommend them!

He was frequently called the cutest old man and told he had a great smile from multiple staff. There were wait times for certain treatments, but the staff was busy. The treatment was great, but what impressed me more was how my mom was treated. The job of the staff is to take care of my dad. But they were so incredibly caring towards my mom and family. Big shout out to Alexia. She comforted my mom all the time with words and hugs. When he passed, so many staff went out their way to hug my mom. Not generic "sorry for loss," but with love and tears in their eyes too. They took a horrible time for our family, made it just a little bit easier knowing we could trust them with my precious dad's care.

I would not recommend this facility. The response time for the call button for anything my mom needed was between 10-25 minutes, IF anyone ever came to help. The facility left a wheelchair in her room and allowed my mom to use it to wheel herself, unassisted, into the bathroom when she needed assistance with a walker to get strength in her leg after a hip surgery. I requested it be removed so my mom would have to use the walker to strengthen her leg and the wheelchair was never got removed. My mother could have been seriously injured. She said she needed to do it herself because no one ever came after she would click the assistance button. This is unacceptable for a "rehabilitation facility" and I use that term loosely. My mother is diabetic and I told 3 different people this information, including the social services dept., her direct nurse and the liaison before we arrived. She was given sugary food the entire time and high carb meals. We had to beg for a shower for her when she arrived and the employee that cared at all to ever help my mom was Susie, an aide. The nurses never smiled and spent more time joking and laughing at the nurses station than they did assisting patients. On any given visit when I arrived, there would be several (more than 5 lights lit up) patients needing assistance. Additionally, they were NEVER quiet at the station, even late into the night. The overall morale from the good employees was obviously very bad due to work load. We were told by one of these good employees that showers for patients are put off until she arrives. Upon leaving, I had to ask several times to get help getting her to the car, when Susie (the best employee there) offered to help. She was awesome. The other staff - AWFUL. In fact, when I left, paperwork with HIPPA info was inadvertently included in my mom's discharge papers. Wow. Do NOT allow your loved ones to be left alone at this facility. One reviewer said an investigation needs to be done on this place and I AGREE 100%.

When I tell you, it was the worst facility that I've ever experienced. From the CNA's To the Nursing Staff. The smell of urine hit you from entrance to room access, I witnessed consistent call lights going off, horrible food, and the worst patient care ever!!!! I wouldn't put any family member in this place period!!! I worked diligently with his case manager, Pam, who was the only one who cared enough to help me get my Father out of that hell hole. He has been transferred to Banner Rehabilitation Hospital in Phoenix located on 3rd Street and Indian School. On entrance, the staff greets you with a smile. The level of cleanliness is everything. The private room that my Father was provided has given him a level of comfort. I see the light back in his eyes, which has encouraged him to push himself during his seven days a week, rehabilitation program. There has not been one time that I've visited or called to check on my Father where he's not being tended to by staff with tender loving care. On every visit, I walk in feeling warm and fuzzy, knowing my Father is somewhere people are going to treat him with love, kindness, integrity, and respect. At Desert Blossom he developed bed sores, including a major one on his foot. The new level of care at Banner Rehabilitation Hospital is helping Father to rapidly heal, emotionally and physically. I truly want them to know how grateful I am for the level of care. He is currently receiving. If I could put 30 stars, I surely would. I forgot to mention, they have a tablet that you use to check in- you enter your phone number and then it asks if you're seeing your person. It takes your picture, then it prints out a label. You have to use it to check out too, security is important. I went to their Thanksgiving buffet, everyone went to the first floor and got their food. The whole dinning room was open, so family could come visit their loved ones.
