I explained that he was the fifth dentist I had consulted regarding concerns about tooth #2, especially now that it is taking 100% of my bite force following the unanticipated extraction of #15. I also explained that I was referred to him specifically for a custom crown evaluation due to an occlusal (bite) mismatch. The referring dentist believed a standard crown would prematurely fail because of misaligned bruxism forces. Despite this back story, he quickly dismissed the purpose of my visit and focused instead on questioning why an implant was not placed at the time of my #15 extraction. I was never informed that immediate implant placement was an option, and his comment felt unnecessarily critical of prior care rather than helpful. It offered no comfort or relevance after the fact. He then assessed tooth #2 visually and by tapping on it with a dental tool. When I clarified that any discomfort was from the cavity itself and not the tooth root, he did not acknowledge this. Instead, he tapped aggressively on the tooth, which caused lingering pain afterward--pain I had not experienced before this appointment. He then stated that, in his professional opinion, I needed a root canal. This was surprising given that tooth #2 had been evaluated just days earlier by three dental professionals using X‑rays and CT imaging: a dentist, an endodontist (who also evaluated #15 before recommending extraction), and the oral surgeon who performed the extraction. None of them expressed concerns about tooth #2 or indicated that a root canal was needed. After his assessment, he immediately referred me to an out‑of‑network endodontist. When I explained that this was the third time that particular endodontist had been recommended to me, but that the cost was prohibitive, he stated that his practice was also out of network and that "you will not get good care in network--just look at what has been happening." This comment felt dismissive and unprofessional, especially given that he himself did not provide an estimate for crowns or implants for #15 and #13, nor did he address my occlusal mismatch, which was the primary reason for my referral. I arrived 10 minutes early and was asked to complete forms on a tablet despite having already completed them online. I then waited 30 minutes past my appointment time before being seen. The consultation felt rushed, and his tone and approach felt dismissive throughout. Overall, I left without the crown evaluation I was referred for, without any assessment of how my occlusal mechanics would impact crown placement, without answers to my actual concerns, and with more discomfort than I arrived with. Based on my experience, I cannot recommend this provider.

On June 11th at 7am, I faced my fear and started my All-on-4 implants journey. 172 baked potatoes, more oatmeal and chili than I care to remember, and--believe it or not--this surgery gave me the confidence to go up on stage when Jared Leto called me up during the 30 Seconds to Mars concert! Then, of course, there was the trip to Disney where I couldn't eat a single good snack. But today, October 30th at 2pm, my beautiful zirconia finals went in--and I ugly cried! Dr. Nahlah and his incredible team made this happen! Thank you for being so patient with me, for believing I could do it even when I didn't, and--most importantly--for the WaWa turkey shortie. However, when I arrived for my next appointment, they claimed I did not have an appointment for the filling. Instead, they had scheduled me for a deep cleaning, which I never agreed to. During the visit, they pressured me into getting the deep cleaning, insisting that it was necessary before they could proceed with the filling. They also threatened to charge a cancellation fee if I refused. When I asked to continue my original treatment and complete the filling as planned, they flatly refused. They made it clear that unless I agreed to the deep cleaning, they would not proceed with my treatment and would impose a cancellation fee. This experience made me feel that they are highly unprofessional and more focused on making money than providing honest, patient-centered care.

After two root canals through crowns, both teeth needed attention from a general practitioner, and Dr. Dee and her staff were fantastic. I was concerned I might have to replace the crowns, but Dr. Dee, after her exam, informed me all that I would need were permanent fillings, and she and her assistant, Ms. Sincere, went right to it. It was easy, painless and very quick....I was out in less than 45 minutes and all has been well since. Both Dr. Dee and Ms. Sincere were friendly, relatable, and very fun to be around. It made for a comfortable engagement. The way they engaged was professional and I never doubted their skill and knowledge. I should also mention Franco and Ashley, both of whom assisted me in the initial contact and arranging treatment. The treatment required coordination with my endodontist, and that went off without a hitch. The new patient forms were easy to access and complete and, having done those, the billing portion was also a breeze. A special nod to Franco, who ensured he made contact with me after several missed attempts. So friendly and kind. To top it off, the practice provides each new patient with a coffee mug filled with goodies and, later, a thank you card that, admittedly, was designed both to thank and promote the practice, but was still a nice touch.

The receptionist and the gentleman who took my X-rays were friendly and professional. The problem started when it came to the cleaning. After I was already there, I was told that I supposedly needed a deep cleaning instead. They spent a significant amount of time explaining why I should get it, showing me pictures, and discussing information that was meant to convince me to pay for a much more expensive treatment, which I was told could cost around $1,400. I questioned why I was being asked to leave without receiving the cleaning that was advertised. Only then did the receptionist check with the hygienist. The hygienist eventually came in, but her attitude was extremely rude and unwelcoming. She clearly did not seem interested in performing the advertised cleaning. What I received was essentially a very brief polishing that lasted less than a minute. I did not receive what I understood to be a dental cleaning. That experience completely changed my opinion of the office. I have no problem with a dentist recommending additional treatment when it is genuinely necessary. What I do have a problem with is advertising a $98 new-patient cleaning, exam, and X-rays, getting the patient into the office, and then trying to replace the advertised cleaning with a much more expensive treatment. Based on my experience with this particular team, I would not return to Reston Family Smiles. I also would not feel comfortable trusting them with future dental or emergency care. The receptionist and X-ray technician were genuinely nice. Unfortunately, the experience with the hygienist and the way the advertised cleaning was handled completely outweighed the positive interactions.

Dr. Cai was very gentle, calm, patient, and NON judgmental about the state my teeth are in. I have some heightened phobias revolving dental work, so needless to say i was very scared to go in. I embarrassingly hadn't seen a dentist since i was a child, so i have some extensive dental issues. Everyone there made sure i was comfortable and assured me that i was in good hands. Due to my crazy anxiety, i was panicking and crying thru most of my appointment. Dr. Cai really went out of his way to make sure i was okay, and he assured me thru the whole process that i was doing a great job. I cant thank him enough for doing his best to put me at ease. The actual facility is beautiful. Very modern with obvious attention to technological advances. Everything seemed high tech and state of the art. It doesn't have that super 'cold' vibe some medical offices have. Very warm and inviting. I've already told several people about Dr. Cai & his dental practice. Highly recommend. I can't speak for every patient as im sure its case by case, but since i don't have any dental insurance, they worked with me so that i could afford the work i desperately needed. I genuinely felt they cared about my health before making a buck. Dr. Cai personally called me himself 4 days after having 2 teeth extracted to check on me and make sure everything was healing as planned. His staff(especially Lisa i think her name is) was also very warm and helpful. I called the office multiple times before and after my procedure to ask questions and she was a delight every time. Never made me feel like one of "those" annoying patients. I think i have found my forever dentist. I really cant say enough nice things about this place. The dentist, Dr. Kourosh Bazarjani, was rude, squeezed my face while performing a root canal, pulled on my hair and pinched my lips and cheeks with his instruments, and was incredibly condescending and dismissive to me. He outwardly showed frustration while working on my mouth, and even let out several loud sighs to let me know that he was having a tough time. Before the procedure, he asked me to raise my hand if I had pain. When I raised my hand several times, he dismissed me and pressed the drill into my tooth even harder. The office manager "had a talk with him", and he came back even more rude and aggressive. I will be reporting this doctor and the practice to the Virginia Dental Board.
