This is my wife's and my favorite place to eat. Great food with phenomenal service. The owners regularly come out and check on the tables as well as engage in fun conversations. The pizza is to die for. I have yet to be there and try something I haven't thoroughly enjoyed. I would recommend them to anybody. To have a restaurant that is serving food that can be found in some of the most trendy high end areas (i.e. Park City, Down Town SLC, etc.) right here in Vernal is truly something special. I have ate here a handful of times. For the most part the food is amazing. Service can be slow. The first time I ordered a pizza. The pizzas are cooked in a wood fire oven. The bottom of my pizza was COVERED in ash! It was nasty, like eating ash out of a fire pit. If I would not have had that experience I would give this place 4.5 stars. read more

Not above average, but serviceable. The problem is the price and the morals of the servers. Oh, that and how incredibly SLOW it is. Don't go if you're hungry. Go an hour and a half BEFORE you get hungry. My wife got the veggie pizza and asked to add extra veggies. The menu said that would be $1.75 extra. No problem. But they charged $1.75 for EACH added veggie, without mentioning that fact. So my bill was like $6 more than it should have been for a couple bits of spinach, tomatoes and peppers. Shady and not worth it. Your options are a bit limited in Vernal, but avoid this place if you don't want to wait forever and don't want to get hoodwinked. read more

Food is very good, if you order takeout don't expect it to be what is on the menu..it's true! They sort of throw it together (missing salad items mentioned on the menu) and get frazzled easily. While Antica Forma has excellent food the service leaves much to be desired. We just tried to eat there. Walked in, air conditioning is broken. Ok, no big deal, it wasn't too hot. But as we are coming in another couple was leaving without being seated. We went in, told them it was a party of four and were told it would be about 5-10 minutes. Here's the thing-the entire back of the restaurant was EMPTY. The front there were two or three empty 6-tops. We hear the hostess ask some tall blonde woman about seating us there. She tells her no. We wait almost 15 minutes. Nobody else has come in. We ask about one of these empty tables. The hostess tells us the blonde lady won't let her seat us there. We asked her if it was worth having 4 people leave over waiting for a 4-too to open up. The poor little hostess was embarrassed and went over to talk to the blonde again who at this point was openly glaring at us. While there's at least 10 open tables in the back and six in the front. Seriously. Never again. They want to cater to the tourists, that's fine. But they need to remember it's the locals that keep them in business in the winter months. read more

The pizza and calzone was excellent. The salads were not your typical small dinner salad. The greens were helping on the plate mixed with home-made cheese and dressing. The menu is full of a variety of personal pizza options. The staff was very helpful with the menu items, and accommodating. Service was very quick, despite how busy they were at the time. We were passing through Vernal to see the Dinosaur Monument and we were super impressed with Antica Forma. It definitely lives up to its Yelp reputation and is way better than what you would expect for a small town or a restaurant that shares part of the structure with a Best Western. Cleanliness - 10/10. Modern, clean. Service - 10/10. Very accommodating of kids (let our kids play with some dough while we waited) and very attentive on drinks, etc. Food - 9/10. Crust on the pizza was fantastic, sauce was good. My only complaint would be that there was a little more cheese on the margherita and kids pepperoni pizzas than we prefer. Still good, but they could probably go a little lighter here as a neopolitan pizza. Fresh, tasty, reasonably priced. This place would be solid in a city of any size but in a place like Vernal it is a gem. read more

The volcano pizza is an amazing pizza. It is a stuffed pizza with more toppings on top. I would definitely come back and grab more pizza. Fresh ingredients make the pizza so much better than any other chain place you go to. Beer and wine selection is to die for. Squatters ipa is a definite go to for a beer. Spent the night in Vernal and the hotel front desk recommend this place. Ended up there as a default (there are NO restaurants here, not even a weak chain place except Golden Corral). Glad I did. It was great pizza! Highly recommended it. However, I sprung for the $8 cherry pie À la mode. For eight bucks it should've been half a pie. The slice they served me was so small that I took two or three bites and it was gone. That was very disappointing. Would've given it five if the desert would have been worth the money. read more

After planning a trip to NE Utah for the family and reading the reviews online for Antica Forma we decided this was going to be our "Nice" dinner while in Vernal. We ordered four different types of pizza (with the understanding we would save roughly half of each for lunch the following day) which were the: Cotto, Salcicci Fugha, Salcicci, and the Hot Italian Sausage (I forget it's Italian name). Along with this we ordered the 4 pc Garlic bread with Fresh Mozzarella as appetizers and split 3 entitely homemade desserts amongst the 4 of us after the main course. We were seated right away and our server took our drink order shortly thereafter (Waters all around, A Squatters IPA for me and a Nutella espresso for my oldest daughter) . So far so good. Then, many minutes later, she came back with only the water and to take our food order. I won't keep the suspense going but I have read many of the reviews here and saw that overwhelmingly the main complaint was the Service level. This would be mine as well but let me say that despite having to check with our server several times on the eta of our various orders and the staggered delivery of most of it there were some delays but certainly not bad ones, in fact, I would rate the service overall as "Good, needs some refining" rather than bad service. And really, the only thing even in the same vicinity as a negative. This is because the pizzas as documented here, were superlative. I am not a Neopolitan pizza fan, preferring mine in the thick crust pan Chicago style with lots of meat and lots of sauce. However, this place turned my head! Everything fresh, perfectly cooked in the wood burning oven...really top notch! Would recommend to anyone to try it out! Vernal is in a beautiful part of the world and with Antica Forma, has made the culinary options similarly beautiful for your taste buds! If they ever got to the "Outstanding timely and attentive service" level from the "Friendly and good" level, we'd be talking about the best pizza restaurant in the state if not the entire West! Undoubtedly the BEST pizza I have ever tasted. As several thousand people descended on this small town for the Utah High School Mountain Bike Races, I have to say I am overwhelmingly impressed with this family owned and operated restaurant. Our first night was the very first time this wonderful restaurant had ever had a wait list to be seated in its history. We all watched as all the Owners and employees work so hard to please everyone and serve everyone in a timely fashion. When in Italy we sniff out the wood smoke to get the wood-fired pizzzas. Unfortunately the fires in Utah and California have wood smoke everywhere. Thanks to Yelp we found Antica Forma, a true gem in Vernal. Our server, Natalie recommended the pistachio pesto pizza. I thought - are you nuts- PISTACHIO? But we took her advice with our addition of extra garlic. Also the pere e gorgonzola salad was Natalie's favorite, so we got it also. The success of this order was on Natalie. The salad was super fresh spring mix with thin slices of pear and the right amount of gorgonzola with a delicate balsamic vinaigrette, super tasty. The 6 slice pizza had crumbled Italian sausage, housemade mozzarella, pecorino romano and basil along with the pistachio pesto and our added ROASTED garlic. The crust was straight from Naples, thin and tasty and the toppings melded into the best pizza we have eaten in the U.S. All this in Dinosaur land! The whole menu looked extremely appetizing, but we only tried the pizza - so far. read more

Who would have imagined you could have fresh mozzarella in Vernal Utah? Loved the burratta appetizer, beware the balsamic vinegar reduction is addicting. Followed with an amazing salad, I could have stopped but I came for the artisans pizza and I was not disappointed. Coming from the Quarry Exhibit Hall, us New Yorkers were famished. I know the pizza takes a minute to arrive to the table but all ingredients are so fresh and delish, these pizzas taste just like the ones in NY. They are melt in your mouth good, and the owner Jody Smith is simply so hospitable. We left there with the best desserts, on the house. Great place for a party or large group of people. The food was okay, but the service was down right comical. We waited for 10-15 minutes to be seated, despite there being at least 10 open tables. The wait staff was running around like crazy and there maybe wasn't a hostess. The staff had no idea what food went to what table and they would just come up and ask "did you order this?". All three tables around us had mistakes with their orders. I'm guessing this was a busy night for them, but it really wasn't that busy. They had plenty of staff, but no direction. Sadly, the pizza was soggy and the crust was a little underdone. This place has potential and we loved the homemade mozzarella and good flavor combos, but it's still a 3 compared to city living. Can't comment on the food since we were seating and left sitting for 20 minuets. Not one person came to give us water, take our order, check on us, nothing. Tried to get several people's attention, they would not even make eye contact. After waiting with no water or service for 20 minuets we left.
