    My Dad was a short-term stay resident in the rehab facility after a hospital stay. As soon as he…read morearrived there, I was received an email and a phone call to discuss his needs. The email had phone numbers for the front desk, the nurses station, and his social worker. The nurse gave me her cell phone number right away and was texting with me through the first day and evening to be sure everything was set up and to reassure a worried daughter. Everyone I came into contact with at Park Place was kind, helpful and caring. The daily COVID email updates as well as follow up phone calls regarding any cases were very appreciated. All safety protocols were followed and I felt confident that Park Place was doing their best to keep all of the residents safe and healthy. Dad was well taken care of and he was always very happy on the phone and during an outside visit. His therapy went very well and I was informed about his progress weekly. Everyone, including the director, nurse and social worker were very prompt to reply to any email as well as any voicemail. Upon his discharge, I was given detailed discharge information as well as all necessary medications and prescriptions. Park Place also calls the caregiver for a few days after discharge to check on his progress at his new living arrangement. I highly recommend Park Place.

    Do not let this place fool you, when you first walk in it looks nice but the care here is terrible…read more The nurses are neglectful and will leave a call light sitting for hours. My mom was a diabetic who was staying there because she had a fall and broke her hip, she was only 46. My mom would need help changing soiled pants and she would be left there for hours at a time while nurses would sit there and do nothing at the nurses' station. There would be times I would go to the nurse's station, and I would even ask them if they could come to my mom's room. I was even told one time "It's gonna be awhile" and she just sat there on the computer. They have no sense of urgency. They are completely unprepared to have diabetic patients. I walked in to visit my mom to find her in a state of Hypoglycemia, moaning, groaning, and completely out of it. I had to call in a nurse who claimed "She wasn't gone for that long" my mother was a sensitive diabetic and they knew that but left her unattended after my mom had told them her sugar was getting too low. For about an hour they were running around trying to find something to bring her sugar up and they COULD NOT find anything because no one knew where the needed items were, they were frantically running around telling me my mom would be fine, and they handled the situation unprofessionally after they were the ones that caused the situation, they ended up having to call 911. This was only one of 3 or 4 times this has happened. Two of them I was there for. In any bad place there are always good people though, the people in the cafeteria were always so kind to me and my family and the front desk people as well, just be aware that the cons weigh more than the pros with this place.

My family has been sitting in a dark room daily because the ceiling light needs to be replaced. Their response is to say they don't have maintenance to fix it. They "forgot" to order their powerful pain patch, and they are in excruciating pain, probably going through withdrawal. They keep saying their pharmacy is in Indianapolis instead of making arrangements to get the medication locally. The meals are tremendously lacking. One meal was a tuna salad sandwich with nothing to drink. They obviously don't have a dietitian on staff to ensure the patients receive proper nutrition. The call button goes unanswered for a long time. When I walked to the nurses' station, two staff members were sitting at their computers. People were obviously available to respond. It is sad to have a beautiful facility that refuses to provide the most basic requirements.

    My experience with Ignite Medical Resort Dyer was extremely disappointing and concerning. Based on…read morewhat I observed during my loved one's stay, I believe the facility falls far short of the standard of care and professionalism that its name suggests. On the very first day of admission, there was a cloth chair in my loved one's room that appeared to have dried human waste on the armrest. I immediately brought this to the attention of the staff. After investigating, I was assured that the chair would be removed right away. However, three days later, the chair remained in the room. Throughout the stay, the level of care, responsiveness, and bedside manner displayed by many of the nursing and CNA staff was unacceptable. The facility appeared to be understaffed, and there was a noticeable lack of urgency when patients required assistance. On numerous occasions, we used the call button to request help for my loved one. Not once did a nurse or CNA respond to the call button in a timely manner. Each time, a family member had to physically walk to the nurses' station to request assistance before anyone responded. My loved one shared a room with another patient, and I frequently overheard that patient calling out for help. She could often be heard moaning, groaning, and asking for assistance for extended periods. When staff eventually entered the room, I was shocked to hear them scold the patient, telling her that she was "always calling" and that she "didn't want anything." Witnessing this interaction was deeply troubling and demonstrated a lack of compassion and respect for a vulnerable resident. Another concern was the apparent lack of engagement with residents. Many elderly patients spent entire days sitting near the nurses' station with little to no interaction from staff. They appeared isolated, disengaged, and unhappy. The most disturbing issue occurred when I noticed what initially appeared to be clumps of dirt or lint along the wall behind my loved one's bed. Upon closer inspection, I discovered that the area was covered with hundreds, if not thousands, of ants crawling along the wall and throughout the room. I immediately reported the issue to staff. One CNA responded by saying, "Well, I just got here." A nurse then acknowledged that she had already informed the day aide about the ant problem. This indicated that staff were already aware of the infestation, yet no action had been taken to resolve it or relocate the patient. When I expressed my concern and disgust regarding the situation, the nurse rolled her eyes and told me that I should contact maintenance about the ants. I found this response unprofessional and inappropriate, especially given that the issue involved patient health, safety, and sanitation. My final negative interaction occurred during the discharge process. The nurse manager reviewed my loved one's discharge instructions and medications. When I asked questions regarding follow-up medications, I was met with visible irritation and an unwelcoming attitude, as though asking questions about my loved one's care was an inconvenience. I strongly believe the facility should reevaluate its hiring practices, staff training, accountability measures, cleanliness standards, and responsiveness to patient concerns. Patients and families deserve compassionate care, professional communication, and a clean, safe environment. The one positive aspect of our experience was the therapy department. I did observe meaningful improvement in my loved one's therapy progress, and the therapy staff appeared dedicated to their work. They were the bright spot in an otherwise disappointing experience. I was relieved when my loved one was discharged from Ignite Medical Resort Dyer. Based on my experience, I would not choose this facility again. I encourage families with loved ones in nursing homes or rehabilitation facilities to visit frequently and remain actively involved in their care.     Why would anybody rate a nursing home 5 stars? you may ask. I think I can honestly do that. My mom…read morespent four years as a resident in the Alzheimer's unit, and from everything my aunt and I could see, she was safe, protected, and well-cared for. Finding a nursing home for an elderly dementia patient, especially when it's a woman on Medicaid, is not easy. I was literally having anxiety attacks, because so many places turned me down, either because of the Medicaid issue or because beds for women are scarce. When Sebo's said they had space available for my mom and could take her immediately, I did not check nursing home ratings, do an inspection, or anything. It was a bed. It was available. I placed mom there. Mom was in a dementia unit called Daisy Lane, and my review is only about that area and what happened there. I have no knowledge of the rehab side. My aunt, a long-time resident of Hobart, told me later that Sebo's hadn't had a good reputation in the past. Since that time, however, Sebo's has become employee-owned. We believe that made all the difference. You know all those pictures you see of dementia patients left sitting in wheelchairs in hallways, unattended, soiling themselves, all sorts of awful stuff? We never, ever saw that at Sebo's. Instead, the staff works tirelessly to get patients moving, even a little. I saw activity sessions at Sebo's led by an enthusiastic young woman, playing with and encouraging patients to participate, even a little. There is an activity board that lists everything going on, and there is a lot. Have you ever walked into a nursing home or rehab center and been greeted by some awful smell? That never happened at Sebo's. It's always spotless. And we dropped in at all sorts of times. I think my mom was a staff favorite, because she was the happiest nut job you ever saw. She was not paranoid, hostile, or abusive. I did see patients who were very difficult, and I saw the staff respond in appropriate ways. How the staff handled my mom's aversion to the shower demonstrates how clever and compassionate they are. At first, Mom saw the shower sign, and she'd turn around and say, "I already had one." So the staff covered the shower sign with a towel, and they were able to get her in. Sebo's will notify you of the tiniest change. Mom has a bruise on her arm. Mom has a rash. They do call at all hours. I had a message once at 1:14am. Trust me, I'm not answering my phone then. But it shows that nothing gets past them. At one point, Mom had a skin rash. Sebo's called and told me about how they were treating it. They asked me to purchase bath soap for sensitive skin for her, which I did. Apparently the rash was bothering her on the bra line. I got a call from Sebo's: "Miss Frances has informed us she won't be wearing bras any more, so could you get her some t-shirts?" I did, and all was well. One time, Mom fell and actually got a cut on her forehead and a black eye. The nurse, Stephanie, called me, because she was the staffer who responded when Mom's roommate came over and said my mom was laying on the floor for some reason. Stephanie and my mom had become great friends, and she was very worried that mom had been hurt. She called me and gave me a complete run-down of what happened, what she did, what mom said, and how they were responding. I got follow-up calls over the next few days. When mom went into her final decline, they updated me with condition changes, and talked to me about hospice. Each step of the way was handled with professionalism and compassion. Mom was very lucky that Sebo's had a bed available that day, because, frankly, I would've taken anything at that point. And I was lucky, too, because I never had to worry about what was happening there (I live in Illinois). As the head of the Alzheimer's unit helped me carry out mom's few possessions after her death, I said something about, there's probably another old lady waiting for this space. He said, yes, we have a waiting list. I can believe it. I highly recommend Sebo's for dementia care.
