After watching a couple of videos regarding Light Adjustable Lens, I opted to have this done. I opted to do monovision so that I would never have to wear glasses again. I would hopefully see both far and near. I had the surgeries done about a month apart at his surgical clinic. The surgical clinic's staff creates a very clean and comfortable environment and I felt at ease. After the surgeries and after "locking in" the final adjustment, I now have 20/15 vision in my dominant eye, and I can read the smallest print on Dr. Kootman's card. It is amazing. I highly recommend the Light Adjustable Lens. Dr. Kootman is very exact, very professional and very caring. I would highly recommend Dr. Kootman and would also highly recommend the Light Adjustable Lens.

Immediately after the…read moreprocedure, I could see that the incision above my left eyelid was visibly higher than the incision on my right eyelid, causing my left eyelid to appear larger than my right eyelid. At my one-week appointment, Dr. Justo's assistant, Mandi, removed my stitches. I was hoping to speak with Dr. Justo at that time to show him my concerns, such as the uneven incisions and the way my incisions were stitched up, which caused "lumpy, bumpy" scarring, as one his assistants called it. I asked Mandi if I had a follow up appointment with Dr. Justo, and she said "just if one is needed." After waiting for weeks for the swelling to go down (in hopes that with time, the incisions may line up more evenly, and that I would be able to see more of my right eyelid), I finally called Mandi to inquire about a follow up appointment to address my concerns. Mandi asked me to email her a picture, which I immediately did. She called me shortly thereafter stating that my left eyelid was definitely still swollen and to give the swelling at least 2 months before following up. After 2 months had passed and my eyelid incisions were still not even, and my left eyelid looked bigger than my right eyelid, I emailed another picture to Mandi who called me right away ready to schedule a second surgery. I let Mandi know that I first wanted to meet with Dr. Justo to discuss my concerns and to see what my options are before scheduling another surgery. She also let me know that I would have to pay a facility fee for the surgery. I told her that I shouldn't have to pay a fee for something that was not my fault. She then said there is always a facility fee, and then scheduled a follow up appointment with Dr. Justo. At my appointment with Dr. Justo, I first met with Dr. Justo's assistant who asked me what my concerns were. I asked her if she saw the picture that I sent, and she said probably, and again asked me what my concerns were. I showed her the uneven scarring, and then the lumps and bumps around the incisions, and stated that I wasn't sure if the stitches still needed to dissolve. She abruptly said, "They are not dissolvable stitches! Didn't you get the paper with instructions after your stitches were removed?" I told her no, and let her know that I was trying to determine whether the stitches were dissolvable as I could still feel them behind the bumps in my incisions. She told me that I should have been putting hydrocortisone on the lumps and bumps and to do so. She then took pictures of my eyes and then left the room. She returned to the room with Dr. Justo who handed me my before and after pictures, which included the picture she just took. Dr. Justo pulled himself right up to me closely on his stool and asked me what seemed to be the problem. I closed my eyes and started to explain that the incision on my left eyelid was higher than on my right eyelid, and I was abruptly cut off with him stating that he "respectfully disagrees". I again started trying to explain to him what I was seeing, and was cut off again with him again stating that he "respectfully disagrees". He then showed me the pictures and said that my eyes looked vibrant and that he would use my picture to advertise. I asked him if he looked at the pictures that I had sent, and he said he probably did but that didn't matter because he was looking at my eyes right now. I then tried to continue to explain, but was cut off stating that he wouldn't remove anymore skin from my right eye as it would only cause more problems. I let him know that I didn't want to have another surgery if that wouldn't help and just wanted to let him see what happened and to see what my options were. He gave me a large smile, shook my hand and left the room. Very passive aggressive and not willing to listen to patient concerns. After my experience, I would not refer anyone to Dr. Justo.

I went to ,y February appt. to be told it was rescheduled to March 12th. Today I went to my 1:00 appointment to be told it was canceled. I stated that I confirmed the appointment thru an email that I received. The girl kept looking at her computer. I started never mind, I'll find a new provider! After I got home, I received a phone call from AZ Eye Institute stating that my spouse canceled my appointment, but they would try to fit me in. I stated that I would be looking for a new provider. 5 minutes later, my spouse received a text and stated that they would be happy to reschedule. In January, I changed all of my contact information. If AZ Eye Institutes front office can't correct my information correctly, I don't feel the deserve my business!

It was worth the wait. I had also scheduled with 2 other doctors for second, and third opinions. The opinions turned out to be exactly what Dr Justo explained to me. I chose to schedule my surgery for Ptosis with Dr Justo. I had Ptosis surgery on both eyes. It was a very short procedure, and my healing was amazing. I am so excited with my results. When I say excited, I mean EXCITED. The staff thru the entire process exceeded my expectations 1000% ! I reached out to Sage Hospice, Primary & Palliative Care hoping to receive the comfort-focused, home-based support they publicly claim to provide. Instead, I was met with deceit, slander from the medical director, and a cruel emotional bait-and-switch -- not once, but twice. My first interaction with the Sage location in Tucson involved a care specialist who seemed to express genuine compassion. When I explained that I live in Sells, AZ -- a rural tribal area -- he assured me we could work around the geographic issue if I had a Tucson address for appointments. I provided one, a relative's address, and he acted as though I was a good candidate for palliative care. But behind the scenes, their medical director -- who had never evaluated me, never spoken to me, and had not even reviewed my case thoroughly -- labeled me a "drug seeker," claimed I was "seeking for high doses of opiates," and refused to provide care. This reckless accusation came without a single clinical conversation. I have extensive medical documentation confirming the severity of my condition. I was seeking palliative care, not addiction services. Furthermore, I have NO history of addiction, it does NOT run in my family, and I've never abused pain meds. I was and am seeking RELIEF for my uncontrolled severe pain, pain that has utterly debilitated me and destroyed my life. But instead of Sage helping me, I was cast aside and stigmatized. That alone would have been enough to earn this review. But then, several weeks later, Sage reached out to me again, asking if I still wanted services. Desperate for help, I said yes -- because I'm suffering every day with no relief and no one else will help me. They gave me hope again. Only to betray me again. After this second round of false hope, I was finally told the truth: Sage "refers out" for pain management -- meaning they don't actually offer palliative care at all. For crying out loud, the verb "palliate" means TO RELIEVE!! Pain management is the cornerstone of palliative care. I used to be a hospice nurse in the past, so I am very well-versed on palliative care, both end-of-life palliative care (hospice) and palliative care for a serious, life-altering disease (which is what I need badly). To claim to offer palliative care but refuse to actually treat pain is misleading at best and fraud at worst. Refer out to WHERE? In southern Arizona, pain clinics are almost exclusively interventional-only, which means they reject patients with complex, high-level chronic pain unless they agree to invasive procedures or stop taking all pain medication. I am far too sick to endure their hoops and unethical demands. What Sage is really saying is: "You're on your own." Sage Hospice, Primary & Palliative Care falsely advertises compassionate care and symptom relief, but they do not treat severe chronic pain and they will stigmatize you behind your back. They will waste your time, raise your hopes, and disappear when you need them most. If you're suffering and seeking real palliative support, avoid this company. Their actions left me more isolated, demoralized, and abandoned than ever -- and I am still in excruciating pain.

Danielle was especially attentive to our family's needs. The private room was comfortable, clean, and climate controlled, with access to the sunlight outside and a chair comfortable to sleep in. Best of all there were no restrictions. We could visit any time. We even brought in my family member's derpy cat that doesn't blink. He's on the catism spectrum. The lobby is a bit small and dated with a strangely-placed TV, but at least it's cozy, with wifi, plus they have free coffee, tea, hot chocolate, and water. No waffle iron yet. If you need anything else, a giant market is just steps away. I highly recommend this location, even though they don't have returning clients.

The nurse freaking out left and decided an hour later to just walk into my house without calling or knocking. Bad move! You don't ever just walk into someone's home without them knowing and surely after you said you were leaving. You could have been seriously hurt! Common sense! She set the tone that was my entire experience with this company. My first nurse was all clinical. Sorry that doesn't work for me. You have to have compassion, you have to listen to the caretakers. Your unsolicited "advice" about me needing to rearrange my place was none of your business and out of line. How dare you think you're entitled to tell me how to arrange MY home. Just keep your voice down for the 5 minutes you see the patient. It's not that hard. After she and a cna broke two pieces of furniture and I asked for them to be replaced (denied), she wasn't welcome anymore. My furniture WAS replaced after I escalated it to people who weren't blowing smoke up my butt (Norma Jean ). Terrible team "leader". After the nurse change, I was chastised by the social worker who I used to really like. She asked why I wasn't comfortable coming to her all while defending the nurse I let go. Hmmm, I wonder why I wasn't comfortable talking to you........ You all were so concerned on protecting a nurse I didn't want you forgot you had a client to serve. My second nurse wasn't better and when I caught her in a lie and called her on it, the relationship was irreparable. Her fake attitude when calling just made visits dreadful. I hated every part of the experience with this company. Resources available were brushed off or denied. They made things so much worse when their job is to make them easier. I've had prior experiences with hospice and this was by far the worst. Stay far away! I CANNOT recommend them.

I had cataract surgery two days ago performed by Dr. Perkins in the Sun City office ... I was pretty much blind in my right eye due to the cataract and the results of being shot in the eye with a BB more than 50 years ago ... The results have been fantastic and my eyesight is vastly improved in just two days ... I know it is far from being completely healed but with a little time, I'm confident I will have 20/20 vision in the subject eye ... It tested at 20/25 less than 24 hours after the surgery ... My eyesight improves by the hour, literally! ... The entire staff is amazing and all very professional ... They are kind, courteous and very effective in their respective jobs ... In all, I was helped by probably 15 or more and each was excellent! ... The actual surgery took something like 5 minutes or less and was totally painless and stress free ... Follow-up has been great and each person on the staff really conveys a caring and concern ... Was there a downside? ... Sure, I didn't look good in the blue surgical hairnet!!! I have been going to Barnet Dulaney Perkins Eye Center for several years and have been a patient of Dr. Alam during that entire time. Not only is Dr. Alam a specialist in his field of eye care, but he also possesses a professional, caring, polite, friendly, calm demeanor, and those are only a few adjectives that describe Dr. Alam. He certainly puts a patient as ease. Every time I go for my eye appointment, in the waiting area, all the patients, "sing his praises," they think very highly of Dr. Alam. He is the BEST - I Highly Recommend him!! Additionally, Dr. Alam has a wonderful, caring and friendly assistant, Yobanna, and other assistants whose names I don't all know. Further, all the other staff members are very nice as well. I chose Barnet Dulaney Perkins Eye Center-Sun City, because it was close and a "one stop" location. The physical address is 14820 N. Del Webb Blvd., Sun City, AZ 85351. All examinations, diagnosis, surgeries and new glasses were performed at the same location. I was given a complete evaluation of my visual needs and all exams and surgery were explained fully. The surgeries were virtually pain free with the nursing staff, anesthesiologist and surgeon very friendly, supportive and professional. All options and costs were fully explained with absolutely no "hard sell" tactics. Just for additional information, I was a standard medicare customer. I don't believe that I have ever before dealt with an entire office staff with a better attitude toward me as the customer. There is always someone on call who is available for any questions before and after the procedure. I would highly recommend them. read more

Obviously this place deserves zero stars!!! This is not a business, it's a collection of people who find loopholes to get money from insurance companies while penalizing patients (many of whom are elderly). You can call all you want to no avail because they just say they will escalate it and you'll receive a call back...but no one calls. If they bother to call you back, they are incapable of righting their wrongs and taking responsibility. I recommend anyone that struggles to review their case with the DOJ, file a complaint with the BBB, Arizona Board of Opthalmology, American Vision Partners, and the Arizona Attorney General Consumer Protection Division. Had cataract surgery six months ago, when at home in Iowa. But, while wintering in Arizona I developed blurred vision in one eye. Barnet-Dulalney-Perkins got me in right away for exam by Dr. Kopp, and I was diagnosed with two issues: (1) cyst on retina; and (2) secondary cataract. I was referred to a retinal specialist who I was able to see in 3 weeks. Further tests by Dr. Alman determined not really a cyst but, rather, a pooling of fluid fromleaking blood vessels. Really good news, with eye drop treatment to reduce mild inflamation. Prognosis is vision should return to 20/20. Nothing but good to say about yhe pracyice & its doctors. read more

I've been a patient that uses the facilities and staff at the BDP eye center for about 14 years. So far so good with the exception of my right eye is mostly for decorative purposes now. One thing that puzzles me is over the that time I had a series of different doctors provide treatment for my condition. The first one I saw once per year. The second one I was instructed to schedule appts every three months. The third doctor instructions included an exam every six months. Dr. Golish has been outstanding in her assessment of my glaucoma condition. She first tried various combinations of drops. When none reduced the pressure, she recommended inserting a micro-tube to 'bleed' the pressure off. She then carried out the very delicate surgery with great expertise. Over the last eight weeks she has monitored my progress, reducing the Durezol drops from 8 times per day to the current 3 per day. The surgery was a complete success, and the pressure in that eye has remained in the normal range. We are now tapering off the drops gradually. I cannot recommend Dr. Golish too highly! read more

By far the most professional and best ophthalmologist I've seen throughout my years and I've been to many when I lived in the Midwest and now here in Arizona where I've resided for the past 20 years. Dr. Rabinowtiz is so extremely professional, knowledgeable, courteous, takes time with his patient(s), explains their condition and answers all questions. Dr Alam and his crack team are professional and very kind. Because they work with many elderly people, they need to be patient and kind when their schedule is delayed or hindered due to the slowness of these clients. I am one of those slow clients, and I appreciate the kindness and the patience more than I can say. It is not common to see this in facilities that deal primarily with elderly people. I have been a patient at Barnet, Dulaney and Perkins Eye Center for several years and I am very impressed with the patient care I have recurved. My provider has been very informative to share with me a good plan of action to best care for me. Most of all I like the fact that I am able to share what my symptoms and concerns are and the provider always listen to me. Had an appointment with Dr. Brian Horsman of this firm at M&M Eye Care in Prescott. Dr. Horsman was absolute horrible. Refused to explain the need for further eye injections; insulted fellow doctors; told me to "shut up". Was absolutely horrible!. This man needs to retire! Rude, belligerent, hostile and told my wife & I WE had bad attitudes! Excuse me??? Who is paying who for treatment! Rude??? For asking questions??? What is wrong with these people!! BEWARE!! Very good experienced I received from the Barney. Dulaney. Perkins Eye Center in Sun City Arizona. They made me feel comfortable which I was very nervous before seeing them. Thank you very much to the first doctor I saw Dr Brussel..and to LeeAnn very nice lady and to other ladies that assisted me in that office.... Dr. Mahmood El Gasim is one terrific doctor. I have always been very weary of getting something in my eyes. But having to get shots in my left eye due to having macular degeneration, Dr. El Gasim is so good at administering the shot that I never feel pain during the procedure. I trust him completely with my eye care. The staff there is also very good, professional and courteous. Dr. El Gasim always answers any questions I have to my satisfaction. I was dianoniised with mascular degeneration 2 years ago, been going to Barnett, Delananey & Perkins in Sun City, AZ since then. My doctor is Brian Horseman, best eye doctor ever, been very happy with him and would not go to anyone else. He's funny, bluntly honest, yet always reassuring and his team are very pleasant as well. I have been going to Barnet Dulaney Perkins Eye Center for over 20 years. I have had retina surgery on both eyes and also cataract surgery on both eyes. I called yesterday morning because I was having a scratchy right eye and they got me in to see a doctor in the afternoon. I have always been pleased with their service. My dear friend worked for the corp office in accounting and was asked by the owner to make copies of bills they could see, The CEO and CFO found out she was copying what she was told to copy and fired her, She is now losing her house because she was honest and had integrity. I have a family history of macular degeneration, and watched my mother become legally blind. At that time there were no treatments. I now have m.d., but Dr. Horsman has been treating me with laser and shots and we are apparently keeping it from getting worse. The scheduling and procedures work like clockwork, and there is very little waiting. I recommend Dr. Horsman, and the Barnet-Dulaney-Perkins eye center. read more

I was also in a car accident. The doctor did not think there was any relationship to the inflammation in the eye. They ended up getting a check out of my settlement for $1000 and we've been going back-and-forth. Insurance paid for everything. I informed the doctor I had a workplace injury. He did not take it serious even though he had all the doctors notes and passed notes from previous doctors with no other issues except for my underline glaucoma. He was subpoenaed when they asked him about the scarring in the eye he said there was none and then I reminded him, and then he did admit there was scarring. I went to another doctor showed her all the doctors notes and she sat down, took several days and put all the different information together instead of my Doctor putting all the notes together and going over everything with me. He keeps telling me I did not have any issues related to my injury, especially after another doctor stated this because he does not want to deal with Workers Comp in injuries. He also knows the doctors that treated me in the past and since they all know each other, this is a big problem! The Billing office and they tried saying the payment for $1000 was part of his council to the ICA. I informed them this was not $1000 fee and it was something that should not have went to the doctor it should have came back to me for my accident I had in my settlement. Dr. Reiss nothing to do with my settlement nor they should not have $1000 and being dishonest about it after my health insurance paid for my treatment and I paid my co-pays. I do not know what they're trying to pull, but they are not working in good faith.

When we arrived we waited for 2 1/2 hours he then proceeded with the laser not checking whether I was still numb enough-I was NOT. I screamed with pain & told them to stop. He then proceed with a regular cataract removal without consulting me. No apology what's so ever. Ever since that time I have problems with my right eye. I asked to see Dr. Patel but they shoved me off to a young Optometrist. Never saw the Dr. again. I would be very careful using this Doctor for anything, he has an Independent surgery Center with an Assembly line.

They charge extra fees for services they don't even properly provide and will nickel-and-dime you. I went in for a contact lens exam and was charged a contact lens evaluation fee, which felt completely unjustified because Holt didn't actually evaluate anything. After paying the exam copay, evaluation fee, and refraction fee, I noticed the prescription they gave me was only changed by 0.5 from my previous prescription. What bothered me most was that Holt never mentioned the prescription change, never explained why it was changed, and never offered trial lenses or asked me to test the new power to see if it worked better. None of that happened. When I got home and noticed the change, I called the office. They told me that if Holt didn't discuss it during the appointment, I would need to schedule another appointment just to ask about it. That's completely unacceptable. We were charged extra for services they claimed to provide but clearly did not. I would not recommend going there. If you do decide to make an appointment, call ahead and ask for a full breakdown of every possible charge, otherwise they'll hit you with unexpected fees.
