The physical presence is cheery and well appointed. The antithesis of cool and sterile. The whole is compassionate, benevolent and unparalleled in skill and education. We have chosen this facility twice after a family member required care post surgery. The patient care is admirable and the results are unparalleled.

Great Care, Great People, Very good facility - wish they took Medicaid…read more My father, through the last 6 years of his life, had to be in assisted living/skilled facility. We only were at 5 places, but this was one of the two places he felt like he could call home. The people, from the administration, to the caregivers, to the skilled nurses treated him with respect, and care. Despite his decline while there in ability, due to MS, they rarely made him feel like a burden (except when occasionally dealing with turnover). Dad truly felt seen there and cared for... which was wonderful. I have nothing bad to say about he caregivers and the administration other than that they made the experience more affordable (charging 5k a month less than others!). They gave him dignity, in a time in his life where it was in short supply. This facility was the best facility for him, that he had ever had. Sadly, when he was out of money, like many he had to go onto Medicaid. They do not take medicaid, so we had to move on. However his care here was exceptional. This would be a 5 star review, except for their care the last 4 months that my mother for, who was staying with him, and the new nurse they hired said that she also needed to be charged for care and not rent, despite them charging 7,500 a month already. Unfortunately, Mom did have some issues, but after seeing a doctor for them, is doing better. Lesson learned tho, if you're going to stay with your spouse and live with them in the facility and pay rent - you better leave the facility a lot and take care of yourself well as to not be labeled a patient. I have to only deduct one star here though because, overall some of the best memories of the last years of my father's exceptional life, were here. Thank you.

During…read moremy loved ones stay they were ignored, neglected , multiple occasions I had to report and call the state on them more times than I can count . They never cleaned my loved ones room, the food was not fit to give to rats. Never did therapy, I had to provide towels, cleaning supplies, everything just to make it tolerable! There is a HANDFUL and I do mean handful of staff or prior staff that did look out for my loved one and warn me about how bad it truly was there. Those individuals were amazing but quickly ended up quitting or being let go because they didn't "fall in line" with the cliques and bullies. More issues: I had to buy and hide all hygienic products for my loved one bc what they supplied was watered down nothing. Staff would warn me that other staff would steal out of patients rooms at night , on MULTIPLE occasions other residents with severe dementia who were supposed to have constant supervision would wander down to another wing of the building and into other patients rooms which I can understand you can't have eyes on someone 24/7 but please explain how someone can do that , or why some older man is in my loved ones room at 2am and unable to get help bc NO ONE ANSWERS THE CALL BUTTON, or how I walked into the building with 7 police cars bc a patient crawled out their window and starting walking home and no one noticed for 3 HOURS!!! On another occasion the police were there bc someone was sneaking feteynal into the facility and giving it to a resident. Multiple occasions where staff bullied my loved one and wouldn't respond to their call light, so much so they became suicidal and bc Blossom doesn't want to lose their funding they sent a twenty something year old man into their room to "watch" them all night but did not alert the family of this procedure or protocol and my loved one was absolutely terrified. My loved one is handicap and at the time wheelchair bound, and was no way a danger to themselves but they were just absolutley miserable in this hell hole and would have rather died than stay, this is all just a snip bit of the laundry list of reasons why this place should be SHUT DOWN the elder abuse is DEPLORABLE and to Tashi and all her many alias she uses to write fake refuses because clearly her boss Laura asks her to do so, or maybe not but it's pretty obvious. Here's some advice, find another career path bc you were one of the many staff there that were and continue to be a disgrace to medical professionals. Maybe something that doesn't involve the public or any human contact at all bc that is not something you excel in, it's not your passion and it doesn't bring you joy clearly. And Laura you are truly despicable, I've never encountered someone who treats people , especially elders like you do. I've never met anyone in person who can lie and manipulate systems the way you do. Our interactions have shown me something even more troubling, that people like you really do exist. I honestly don't know how you sleep at night. It's disgusting. The residents there deserve help, they need to feel safe and cared for and not just pray for death because it would be better than continuing their life there. I can only hope other potential clients receive better treatment.

After being in two other facilities, she found her home in Hudson Springs Nursing and Rehab. The care that she was given and the companionship and compassion shown to her was remarkable. Our friend communicated via a device on which she typed with her eyes. This was a lengthy procedure and yet the staff was patient, friendly, and respectful as they waited for her to finish the conversation. The administration worked with us to meet all her needs. Our requests and suggestions were respected and addressed. Unfortunately, our friend passed away. We were with her for the last days of her life as we supported her transition. The staff went way beyond the call of duty supporting us yet giving us the space we needed at this delicate time. Even after her passing, they still showed her the same respect while they prepared her for her last journey. The knowledge that she ended her life in a place surrounded by staff who considered themselves part of her family and friends is a comfort to us.

This facility is extremely disorganized regarding medication and pain relief. When my mother…read morearrived at this facility they told us that she could not keep any of her prescriptions in her room so they took all of them. When it was time to check out they did not return any of the prescription medicines to us. I did not realize until we got home. I went to set up my mothers medications that we were never given them. One of the main reasons I didn't realize was I was completely distracted by the fact that they were going to send her home with no pain medicine. The day before she was supposed to get out I asked The nurse about pain relief and would they be sending her home with it or did I need to get it from the local pharmacy and would they be calling it in to the pharmacy. I was assured that they would be calling it into the pharmacy. She was sent home on a Saturday. No one had called the prescription into the pharmacy and they had no intentions of giving us the pain medicine to go home with. They only told me to call her doctor and see if they can call in a prescription. Her doctor is closed on Saturday and Sunday. It took us an extra hour of talking to a lot of different people to be able to take home the pain medicine that they had there at the facility. Now I had to call the facility about getting her prescription medications back because they failed to return those to us. I had to go out of my way back to the facility to pick them up. The following morning my mom said she needed a particular over-the-counter medication and it wasn't in the bag. I had purchased this medication brand new while she was in there. I handed it to a nurse instead of leaving it in my mothers room and asked her if she could give her one when she got out of physical therapy. I was told she would be given one. When I visited the next day my mother said she had never been given one but luckily didn't need it anymore. I had to make a third trip back to get this medication that still was never opened brand new (photo attached) with her name on it. Even though my mother is 95 her mind is crystal clear. She can remember very well. She said one evening they completely forgot to give her her medications. One of those medications includes her blood pressure medicine which is very important. The people keeping track of these medications are not keeping very good track at all. There were many times when my mother who could not get out of bed herself had to wait more than a half an hour after requesting help to the bathroom. When you're 95 1/2 years old it's difficult to wait more than a half an hour after you've requested help to get to the bathroom. As far as the rehab at this facility it's five star. The rehab is amazing here. The people that run the rehab are fantastic. I can't say enough good things about the rehab at this facility. They are wonderful at helping people get back on their feet. But the rest of this facility is one star.

The patient received supreme care from the aides and nurses . Often the activity coordinator would check in on her and offer to take her to the activities room . She even took her time to hunt down chapstick for my patient . Not only is it a beautiful clean building , the workers are all willing to help and make sure the patients needs are met .

My grandmother is a current resident and has been for years. I had an opportunity to visit her several times during a recent visit. The moment you walk through the door your immediately greeted by the receptionist. If you're lucky you came on a day that she made freshly popped popcorn. Walking through the home all the nurses are lively and greet you during your passing. I had the pleasure of meeting Loretta who was one of the staff nurses on site who was entertaining the residents. Her bubbly personality warmed my heart. I can tell she was well liked amongst the elderly. Rarely do you find an individual who enjoys her work. You can tell she very happy and passionate about making a difference. This meant a lot to me as I confirmed my grandmother is very well taken care of. The facility is very clean and organized. There is always something to do or a form of entertainment for the residents. I saw a performer sing to the staff in one room and another room crafts being made. I appreciate how they handled my grandmother during her time of care. One person who truly stood out to me was Steve I saw him interact with the staff. My grandmother mentioned he takes really good care of her. I find this home a safe place to leave your love one whom may be in need of 24 hour care.

All meals are served in to-go containers. Every meal was cold and very unappetizing. The powdered cream would not melt in a cup of coffee. One night, she dropped her pencil. When I went to pick it up behind the night stand, I found her lipstick. It turned out that it was not hers. Leads me to believe the night stand had never been moved before she was admitted to that room. So apparently, cleaning is at a bear minimum. Another unfortunate event happened the night before she was to be discharged. She fell in her room, somewhere between 10:00 and 11:00 PM. She broke her hip, shoulder, and pelvic bone and had a huge gash on her leg. After spending the night with her in the emergency room and then moved to the ICU, my sister and I went to Heather Knoll to pack up her belongings. (We were scheduled to be there around 1:00 PM, because she was supposed to go home.) Her room had been emptied. The kind folks at Heather Knoll had already thrown all of her belongings into boxes and those boxes had been placed in storage. This happened within 16 hours of her fall. We had no idea where her glasses were and if everything she had at the facility was actually in those boxes. We had to unpack all them. And to top it off, they had taken her walker (even though it had her name on it) to their PT department. The worst part - not one staff member asked us how our mother was! This staff is as cold as they come! We would not recommend this place to anyone.

He is under VA disability and they will pay for his care here but is really sad. Had issues with AIDS lack of communication. Some of the workers are very rude. They they emphasize on how they want for residence to feel like at home that is a big joke. The place is so outdated. The smell is horrible. They talk about the patients having dignity, you see patients in the hallways hanging halfway out of the wheelchair they never changed my father's clothes only on shower day which is two times a week which I think is horrible . I have everything labeled his room to make it convenient for AIDS, but doesn't seem to matter, he tries to do what he can by himself but his short-term memory. It's very difficult for him. At least once or twice a day there is a family member of the there , I change his sheets & do you extra cleanup. I do see them clean rooms, but I never smell any clean. Smell always smells like urine notice the mop the cleaning person was using was filthy dirty , it's very disappointing and heart, wrenching to see people live in that odor, especially when a big percentage of them. This is considered their last home my grandkids do not even like going there because of the odor it makes me very sad. Why can't these companies do better. I feel like the owners of these companies have no idea and I can say 100% they would not live in those conditions themselves. All they are worried about is what kind of money they are pocketing.
