The doctor. Everytime I bitedown on my bottom teeth I remember what. an inept dentist he is. He filed down four newly put in veneers for no apparent reason. I have suffered with TMJ for many years. In 1081 I had a silastic implant placed in my jaw."Bob" the doctor had some new technique to help with continued pain , limited jaw opening and consistent sensitivity just to have teeth cleaned because he filed down my veneers. That was just the first part of his two part special treatment. We said we would not pay or have his second part. Enormously expensive and nothing help with two pieces of plastic on my back teeth. He had never worked on anybody with an implant so I was left with sensitive also expensive filed down veneers. It's funny he only lists his five star reviews. When he saw my review on Yelp he called and asked to give me half my money back if I took down review. I said yes took his money as he had ruined my bottom teeth, didn't take down post and contacted Yelp. I have not even mentioned his nam and my new dentists.. both say back tome.. " oh he's crazy, I know exactly who he is and would never refer him. After having so many specialists and ethical dentists since 1980 ,I know who likes to tell the " in thing he does and a good dentist. A couple of years ago, I started waking up with a horrible pain in my jaw that would cause headaches. I went to multiple dentists and even after getting a sleep guard for clenching, I was still in pain. I was told to "see an orthodontist" with a shrug. Then I came to see Dr. Perkins. Dr. Perkins is a genius; I am so happy I found him. I had braces/rubber bands/headgear as a teenager and terrible TMJ pain off and on for the next two decades. Even though this office does beautiful cosmetic dentistry, I was interested in a functional change. Dr. Perkins's knowledge and creativity won my trust and now I have a comfortable, roomy mouth, no grinding or crowding, just way more comfortable. I met Dr. Perkins when my dentist in NYC recommended him to assess my teeth and bite, guessing that might be causing my migraine headaches. I was immediately impressed by Dr. Perkins' explanations of how and why a bite that was out of alignment could cause migraines. I wished I had found him years earlier! I was amazed to find I was migraine-free in the first week we tested the theory with temporary acrylic on my teeth to correct my bite. I then underwent treatment with crowns and braces to get everything into a better position permanently. Dr. Perkins was always easy-going and laidback, which put me at ease in the dentist's chair (quite a feat). He was also incredibly sensitive to any hint of migraines that popped up here and there and rushed to correct my bite to take care of them. I was thrilled to finally have a more long-term solution to my migraine pain (I get almost zero now, and the ones I do are from other causes) -- and loved my new smile from the orthodontic work as well. Dr. Perkins is not the dentist to see for TMJ issues. I think he's probably a great cosmetic dentist--but his "Phase I" treatment (which cost me $2500!!) was terribly ineffective and actually exacerbated my TMJ and clenching issues. During my first appointment I was so excited as Dr. Perkins claimed he could treat 98% of my symptoms in just one visit; of course this is worth any amount of money (if you've suffered from TMJ, you know); so I shelled out the $2500 and was in turn given some acrylic on my teeth which caused me to lisp...the good news is though that my back teeth were "touching" so I could chew in the back again. My front teeth did not touch but he said he could always lengthen them down the line for cosmetic reasons (I've never heard of this, but based on other Yelp reviews it looks like it's a common thing......) After a few days the acrylic popped off my teeth. I had to go back to the office and have it reapplied. I caution anyone who is not an expert on their bite to NOT GO HERE...as he uses no medical means to measure bite position. No CT Scan, no actual medical equipment....he uses his eye to see how which position your jaw "looks" better in and then adds the acrylic. I was having a hard time finding the proper bite position and could tell it was still off, to which he said, "well try it for a few days and if it doesn't work then come back, I can't spend any more time with you today." (To his defense I had been there for nearly an hour.) I have to admit that I was completely sold when I went to my first visit. Dr. Perkins was charismatic, and seemed to be very compassionate and understanding...but I fear now this was a tactic to get me to pay $2500 for Phase I. I asked the office to send me the proposal for Phase II to save me a trip to Malibu, and it took almost a week of them reaching out to make another appointment before they finally emailed me...the Proposal was over $12,000..... I explained several times that the clenching had gotten worse with the acrylic and that I had a lisp that I wasn't comfortable with and I felt those worries were dismissed by claiming .we'd take care of that in Phase II. I just couldn't take that chance when I saw no longterm improvement. I've been back to see my previous TMJ doctor, Dr. Shirazi and am hopeful that I will find relief. Unfortunately I would not recommend this TMJ treatment, I wish my review was different and I had found relief but that was not my experience. read more

I fell hard for the TMJ scam that Dr. Perkins and some other dentists push. I saw the positive reviews on here which I'm now assuming not to be authentic and went for it after being in pain for years with my jaw. I wish I had never came across his practice. His technique is refuted by so many in more legimate fields such as oral/maxilo surgeons and prosthodontists so I encourage you to do your research. His entire philosophy is based off aesthetics and mandibular advancement is a huge red flag/money making scam that should be avoided. He gave me a whole shpiel about sleep apnea despite me not ever having any issues with breatheing and pulled out a scrapbook of celebrities who he said have the perfect bite. I should've bailed right then and there. His orthotic shifted my entire bite and put me in intense pain. I was traveling and was unable to have it removed for 3 weeks which at that point the damage was done. There was a loud pop when he took it off (from my jaw) and I've been in constant pain since. My kids even notice the change in my mood as I'm always rubbing the side of my cheek. The cherry on top is that I paid 2 thousand dollars for this. I'd pay another 2 or more if I could undo the damage this did but I'll be sticking with real doctors from here on out. read more

I've been dealing with TMJ issues my entire life, having constant tightness in my jaw, neck pain, headaches, jaw clicking, and ear pressure. Last year I started treatment with a "famous" TMJ doctor that modified my lower bite by pushing it backward, which ended up worsening my symptoms. Before making any permanent changes to my teeth which would cost an insane amount of money with this previous doctor, I decided to look for a second opinion and that was when I find Dr. Perkins. I'm extremely glad I followed my gut and looked for another TMJ expert! Dr. Perkins is attentive, knowledgeable, and truly willing to help his patients. He was able to give me a proper diagnosis, he realized my jaw was too far back and needed to come forward. My treatment had 2 phases. Initially, he made a temporary change with a fixed orthotic (acrylic that is bonded to the lower back teeth in such a way that creates a new "bite" position). Phase two started after 1 month, and it was a permanent change on the teeth, but he emphasized he would only make this permanent modification once I was rid of my symptoms. One of the biggest differentiators for me was his holistic approach. Dr. Perkins understands the strong correlation between TMJ and BREATHING. He not only fixed my jaw but he also educated me about the importance of striving to become a nose breather in order to avoid TMJ and sleep apnea issues. I'm a chronic mouth breather due to my year-round allergies, and that caused my bite (and jaws) to be more retruded over the years. Dr. Perkins also gave me a sleep appliance that helps me sleep without overworking my jaw muscles and clenching/grinding my teeth during the night. We also discussed other ways to open the upper airway such as using mouth strips and nose strips when I sleep, flonase nasal spray, air purifiers and he recommended other professionals to assist with the airway issues. Treatment is not cheap, but it's worth it. I sleep much better which ends up giving me more energy during the day, my jaw is balanced, and my symptoms have been alleviated thanks to Dr. Perkins! read more

If you live in NYC and thinking of getting treatment for TMD by Dr.Perkins, I would take some precaution.Maybe he is good with other things but TMD he does seem a bit over confident. Another poster mention that too. I have TMD and he made a permanent appliance for my bite. His approach is to make composite made out or acrylic to fix your bite but this is not a removable splint. The problem is if it's not the right fit it can cause discomfort to your TMD and bite. And also you can't go the next day to get it adjusted. He comes into NYC every 3-4 weeks or so. According to Dr. Perkins he said any average Dentist should be able to remove this. NOT TRUE! I went to my general dentist and he has no training in TMD or bite balance and told me he couldn't remove it. So that left me to go find a new TMD dentist . Since I don't have dental insurance I already paid $1500 to Dr. Perkins for this failed treatment. I asked Dr. Perkins if the NYC office he works out can remove this composite in my mouth since I already spent $1500.00 for this. There would be new extra charges for this. I think that's bad business. I figure there should be at least some sort of contingency plan in case the patient is in discomfort and someone at the NYC office should be able to remove it. There is none. Your better off finding a new TMD dentist which what I did. Which is sad cause I think as a person Dr. Perkins seems nice and friendly and the Julies who works out of west coast answering the phone is nice too. But I don't think it's fair to the patient that if the composite doesn't fit the patient has to pay extra to get it removed from the office out of NYC he works out of it. Plus he flies into NYC ever 3 or 4 weeks so if your in discomfort...those 3 -4 weeks will feel like eternity having this in your mouth. From practical sense if you live in NYC I would highly think about this if your thinking of getting TMD treatment.
