12/30/2021- My father was transferred here after contracting covid 19 to quarantine. He arrived on…read morea Thursday afternoon. It wasn't until 18 hours later that I was able to get in touch with him. The staff never gave him his personal cell phone, nor did they answer their phones at the facility. Trying to get in touch with anyone there was impossible after MANY MANY attempts. When I did talk to someone I was told "everyone was off due to the holiday". It wasn't until receiving a text the following Monday 1/3/2022 asking to sign admission paperwork for the transfer, I did actually speak to someone for longer than 1 minute. At that time I asked for a update and strongly voice my frustrations. My reply in text was enough to actually receive a phone call from someone. At that time is where I voiced my concerns about the lack of administration, communication, and it being impossible to reach anyone there. Heaven forbid there be an emergency on my end, or on his. I'm almost certain I wouldn't be able to get through, or even receive a call if something happened to him. I will say after voicing my concerns things got slightly better. But that was still after multiple attempts. Today my dad returned back to his initial facility. Upon his arrival the nurses, and wound care did a full evaluation, and documentation on my dad. He has bruising all over both arms (possibly due to blood work, yet he has a picc line), a fungal infection under his arm pits, and groin area, they noticed what looked to be bed sores on his back, bottom, and heels of his feet from not transitioning him, or offloading his weight during his stay. He literally was in bed and the same position for 10 days straight from the sounds of the report I got this evening 1/10/2022. The wound care he was receiving from an amputation was larger upon his return, than it was when he left only ten days earlier. Due to being a covid patient, I was unable to visit, and relied on him answering his phone to get any sort of updates. If your loved one doesn't have a cell phone, do NOT plan on any communication! Make sure it's plugged in and kept charging. AT ALL TIMES! PLEASE if you send a family member here, make sure to be there as much as you possibly can be!!! I'm sure if something did happen you'd never hear about it from anyone. From what I've experienced these last 10 days I wouldn't recommend having anyone stay at Legend Oaks North Austin. Maybe things would have been different if I was able to be there daily. But given him being covid positive no one was able to enter the 300 hall staff included, nor send me to the nurses station phone "because they don't have one in that hall". Keep researching!!!

Our community service organization volunteers every month at Sedona Trace. The administrators and nurses were helpful in sharing any resident restrictions and locating the birthday folks for the month. The facility provides comprehensive and care and treatment plans for its residents. They have full-time In-House therapy and specialized skilled nursing.

I know they only take people who are able to feed themselves (she is), so the kitchen staff may not think about it when they drop off their trays. I've mentioned it to the nurse, so hopefully that will be remedied. Mom has made so much progress...it's really amazing for someone her age who has been through hell. I'm very impressed with this place generally...everyone is very kind. If I had it to do over I would probably hire a sitter to help with mealtimes, but the confusion is wearing off and she'll be home soon...just a few more days. Thank you, Rapid Discovery!

My sister, fortunately, was referred to stay at Gracey Wood II a little over two years ago for hospice care. I have experienced a lot of different care centers for family members over my 77 + years but their facility is BY FAR the best and most consistent quality care center I have ever seen. Every time I would visit I had nothing but the best experience with their entire staff, protocols and cleanliness. They have been such a relief to me and my other family members to know that our sister was treated so well, humanely and professionally. She passed away two days ago and we will always be so thankful for the way they took such great care of her! Thank you to Gracey Woos II and their entire staff and administration! I cannot recommend this facility to anyone with a loved one who needs skilled rehabilitation or…read morehigh-acuity care. My family experienced repeated problems with severe understaffing that appeared to affect nearly every aspect of patient care. At times, it seemed there was only one nurse or aide responsible for as many as 17 patients. Whether that was the official staffing ratio or not, the level of care reflected a facility stretched far beyond its limits. The most upsetting experience was waiting well over an hour after using the call light for help with toileting. By the time anyone responded, my family member had already soiled herself. We ended up providing the care ourselves most of the time because we could no longer depend on timely assistance. On another occasion, a broken bathroom call light left her stranded for approximately 45 minutes, forcing her to attempt an unsafe transfer on her own and creating a serious fall risk. Unfortunately, these were not isolated incidents. During visits, I regularly saw multiple call lights going unanswered, families wandering the halls searching for help, and patients waiting far too long for basic care. At one point, while patients were waiting for assistance, several staff members were gathered in a back room eating together, leaving the floor seemingly unattended. Employees need to stagger breaks. The food was another major disappointment. This facility promotes itself as a high-end rehabilitation center, yet meals were consistently poor quality and lacked the fresh, nutritious foods patients need to heal. My family often felt compelled to bring outside meals, yet there wasn't even a microwave available for reheating food. I was also concerned by several operational issues, including the use of a shared open ice cooler instead of a more sanitary dispensing system. Delays in laboratory testing that could impact dialysis patients, where timely lab results are critical for safe treatment. There was no onsite lab and lab results were not posted or available for 2 days. What saddened me most was seeing vulnerable patients--including veterans and retired healthcare professionals--left waiting for basic needs such as toileting, hygiene, and assistance. No one recovering from surgery, illness, or dialysis should have to worry whether help will come in time. I understand that healthcare facilities everywhere face staffing challenges, but patients and their families deserve safe, compassionate, and timely care. My hope is that management addresses the chronic understaffing and operational shortcomings so future residents receive the level of care this facility promises. PS: The chief of culture Jim White owns casinos. This man has no clue about how healthcare is run or works.

It's 6 hours a day of education, great food, beds are very comfortable and the property doesn't make you feel confined. You can keep your cell and laptop, just not turned on in classes. You are set up with aftercare so you have a sustainable plan moving forward along with the tools to succeed where most don't. With over 40% of our nation addicted to some sort of substance and 60% of our country prescribed an Anxiety or Depression R/X, you don't need to feel bad about getting help. Scientists have proven addiction to alcohol is a destructive disease & there is treatment that can set you up for your new life. If it was Diabetes, or Cancer, you would get help immediately. Addiction is no different and doesn't care how old you are, the color of your skin, nor does it matter if you are a Doctor or Lawyer. I am surrounded by people of all sorts and you won't want to leave after 4 weeks, the path forward is clear by then but this will be something you have to work on for the rest of your life. I was admitted to St. David's Rehab Hospital's neuro floor in April 2025. I could not have asked…read morefor a better place to recover. I had been in ICU with a Guillain Barre diagnosis and was suffering from a large loss of sensitivity in my feet and hands, weakness everywhere, I had a trach in place to help with my breathing (had been on a ventilator for two weeks prior to that placement), and I was unable to lift my dominant arm making it difficult to do much of anything--getting dressed, brushing teeth, cleaning myself, etc. They took wonderful care of me and even made special accommodations for me to be able to bathe daily--difficult with a tracheostomy and generalized weakness. The SLP, Raya, was so helpful. She helped me learn to swallow again and speak better. PT was aggressive but I don't regret being pushed. In fact, they told me to rest more! OT, well let's just say, they were over the top helpful. They took special precautions to help me get to a shower--which I hadn't had in weeks (being immobile I did receive bed baths but I'm a REAL bath girl, LOL). I give them all the credit for helping me recover almost completely. I'm so grateful they accepted me directly from acute care. I highly recommend their services.

After several days inpatient at Ascension Seton during which I had large amounts of spinal fluid leaking through my bandages I was eager to get to St David's to begin my recovery. I had been to St David's Rehabilitation Hospital in 2022 with excellent recovery after cervical spine surgery and two weeks of physical therapy and delicious meals three times a day. I spent four to five hours a day in physical and occupational therapy. I wanted to recover with them after my difficult procedure. I was once again very happy with my rehabilitation. My doctors, nurses, physical therapists, and other staff couldn't have been more skillful, caring, and kind. Go here, if you want great care and excellent rehabilitation. You'll be very happy during and after your rehabilitation.

At the prices I'll paying to stay here I'd expect a little more decorum. For instance: I had to call over 5 times to have room service make up my bed and do generic room maintenance. The employee who finally showed up changed the sheets but didn't even look in the bathroom to see if it needed service, then got all surly because I asked what took so long. This is not how to run a service facility. No other complaints with this facility.

The entire team was warm, caring, and deeply invested--daily huddles ensured perfect coordination, and everyone genuinely enjoyed helping patients. Mom came home stronger physically and mentally than before the injury, with renewed mobility, confidence, and even new friends among this talented, youthful staff.
