My family experienced repeated problems with severe understaffing that appeared to affect nearly every aspect of patient care. At times, it seemed there was only one nurse or aide responsible for as many as 17 patients. Whether that was the official staffing ratio or not, the level of care reflected a facility stretched far beyond its limits. The most upsetting experience was waiting well over an hour after using the call light for help with toileting. By the time anyone responded, my family member had already soiled herself. We ended up providing the care ourselves most of the time because we could no longer depend on timely assistance. On another occasion, a broken bathroom call light left her stranded for approximately 45 minutes, forcing her to attempt an unsafe transfer on her own and creating a serious fall risk. Unfortunately, these were not isolated incidents. During visits, I regularly saw multiple call lights going unanswered, families wandering the halls searching for help, and patients waiting far too long for basic care. At one point, while patients were waiting for assistance, several staff members were gathered in a back room eating together, leaving the floor seemingly unattended. Employees need to stagger breaks. The food was another major disappointment. This facility promotes itself as a high-end rehabilitation center, yet meals were consistently poor quality and lacked the fresh, nutritious foods patients need to heal. My family often felt compelled to bring outside meals, yet there wasn't even a microwave available for reheating food. I was also concerned by several operational issues, including the use of a shared open ice cooler instead of a more sanitary dispensing system. Delays in laboratory testing that could impact dialysis patients, where timely lab results are critical for safe treatment. There was no onsite lab and lab results were not posted or available for 2 days. What saddened me most was seeing vulnerable patients--including veterans and retired healthcare professionals--left waiting for basic needs such as toileting, hygiene, and assistance. No one recovering from surgery, illness, or dialysis should have to worry whether help will come in time. I understand that healthcare facilities everywhere face staffing challenges, but patients and their families deserve safe, compassionate, and timely care. My hope is that management addresses the chronic understaffing and operational shortcomings so future residents receive the level of care this facility promises. PS: The chief of culture Jim White owns casinos. This man has no clue about how healthcare is run or works.

If you're checking into a rehab center with kind and professional staff, this is the place to go. They really follow through on your requests. All rooms are private with private bathrooms and are disability accessible with good TVs. The beds are comfy with regular and air mattresses available. The food is excellent and if I needed anything, they brought it to me quickly. They have an all day menu available which is ideal if the daily menu items are not to your liking. The food is nutritious and you can ask for special diets like cardiac, diabetic and vegan. The PTs and OTs, like Linsey are all encouraging and really cheered me on. The equipment and rec room stand out. The staff all recommend exercises during your off time. The staff addresses your health goals. I really need to give a shout out to the excellent staff. Everyone quickly gets you what you need including the front desk receptioness . The RNs and LVNs who are outstanding and stand out for their professional and personal qualities are Danielle, Joanna, Robyn, and Ingris. The exceptional CNAs who really stand out for their care and hard work include Jasmine, Nicole, Rice, and Ashley. The nurse practitioners that particularly stand out for their care, listening skills, and explanations are Jenn, Lauren, and Sue. I was medically and physically taken well care of. I highly recommend you choosing The Center at Parmer for your skilled nursing and physical rehab needs.

When a company fights harder for $45 than for doing the right thing, that tells you everything. They had me download an app for at-home exercises, never disclosed it would cost money. Then I get billed, plus a same-day cancellation fee tacked on. I called in February 2026 and was clearly told the charge would be removed and I could ignore the bill. Then another bill came. I called again, same reassurance. Then another bill. When I finally reached a supervisor, she was determined to collect the $45, citing "policy," despite their own team repeatedly telling me it would be resolved. I even offered to split it...denied without hesitation. Now they're threatening collections over a charge they told me multiple times to ignore. No transparency. No accountability. Just billing games. Avoid Endeavor Physical Therapy. Transparency and trust shouldn't be optional in healthcare, but apparently it is here.

When she first started, she was still dealing with swelling, stiffness, and pain that made even basic movement tough. The team took the time to assess her injury and build a personalized recovery plan that progressed as she healed. The sessions were a mix of strengthening, balance work, stretching, and hands-on therapy. I appreciated how they explained the "why" behind each exercise, which helped her stay motivated and stick with it even on the hard days. Within a couple weeks, she saw noticeable improvement in her range of motion and stability. If you're recovering from an ankle injury and wondering if PT is worth it--absolutely yes. So we were able to get an appointment at a different location which turned out to be a mistake. Transferred back to this location and made an appointment with Nicole. When my daughter got there she was told she'd be working with a newly hired PT that Nicole was training. The new PT was doing a terrible intake and my daughter did not feel confident she knew what she was doing. She asked not to be assigned to the new person and saw Nicole for 6 weeks and did help my daughter, therefore the star rating is mixed. Sadly Nicole left Endeavor and so we have not gone back. I would not recommend them. Interesting enough that new hire was removed from the website most likely due to other complaints.

    12/30/2021- My father was transferred here after contracting covid 19 to quarantine. He arrived on…read morea Thursday afternoon. It wasn't until 18 hours later that I was able to get in touch with him. The staff never gave him his personal cell phone, nor did they answer their phones at the facility. Trying to get in touch with anyone there was impossible after MANY MANY attempts. When I did talk to someone I was told "everyone was off due to the holiday". It wasn't until receiving a text the following Monday 1/3/2022 asking to sign admission paperwork for the transfer, I did actually speak to someone for longer than 1 minute. At that time I asked for a update and strongly voice my frustrations. My reply in text was enough to actually receive a phone call from someone. At that time is where I voiced my concerns about the lack of administration, communication, and it being impossible to reach anyone there. Heaven forbid there be an emergency on my end, or on his. I'm almost certain I wouldn't be able to get through, or even receive a call if something happened to him. I will say after voicing my concerns things got slightly better. But that was still after multiple attempts. Today my dad returned back to his initial facility. Upon his arrival the nurses, and wound care did a full evaluation, and documentation on my dad. He has bruising all over both arms (possibly due to blood work, yet he has a picc line), a fungal infection under his arm pits, and groin area, they noticed what looked to be bed sores on his back, bottom, and heels of his feet from not transitioning him, or offloading his weight during his stay. He literally was in bed and the same position for 10 days straight from the sounds of the report I got this evening 1/10/2022. The wound care he was receiving from an amputation was larger upon his return, than it was when he left only ten days earlier. Due to being a covid patient, I was unable to visit, and relied on him answering his phone to get any sort of updates. If your loved one doesn't have a cell phone, do NOT plan on any communication! Make sure it's plugged in and kept charging. AT ALL TIMES! PLEASE if you send a family member here, make sure to be there as much as you possibly can be!!! I'm sure if something did happen you'd never hear about it from anyone. From what I've experienced these last 10 days I wouldn't recommend having anyone stay at Legend Oaks North Austin. Maybe things would have been different if I was able to be there daily. But given him being covid positive no one was able to enter the 300 hall staff included, nor send me to the nurses station phone "because they don't have one in that hall". Keep researching!!!
