Throughout my stay there I encountered catcalls so much that one therapy work said "Hey that not nice" I felt helpless a stroke victim after I told a therapy worker in the 7th grade on my first day in a new school I had to fight about 20, I punched first. He called a draw and the 20 in his crew backed down. But a stroke victim can barely vocalize in a wheelchair can do nothing except take the punch. Race wars exist in Oak Pavilion. STOLEN STUFF Its the weak vs strong. Every law protects the weak wheelchair age 62, over Oak Pavilion Nursing Center's bullies. I have been relocated since August 13. It has been 7 days still nothing, no return call to head nurse to me, no return emails. Get ready for all your stuff stolen systematically under the direction of the manager and all those conspired under him. The most important item is the Hyperbaric Oxygen Chamber which has five boxes, and its worth high four figures. I sold a house to be able to buy this. A manual wheelchair. Two bottles of turmeric it's for my tinnitus. Two suitcases, three boxes of clothes. Power tools, 6-foot staff. This is the second time things were stolen from me since my stroke - and I am tired of it. THERAPY My hairdryer was confiscated, it makes deep muscles warm, what do they know about aches and pain. Hyperbaric oxygen therapy was confiscated, WHY therapy was this confiscated - because it worked and would put them out of business? DO NOT MOVE HERE! Do not ever move to a place that will destroy you. From day one I felt this was a wrong choice the social worker put me in. When you destroyed your life becomes a nightmare, don't tell me I didn't warn. The floors weren't clean in my room or any rooms. Compare it to Miamisburg it looks like they got on hands and knees to get it clean. The building is Oak Pavilion Nursing Center - think about it. What does the name say NURSING CENTER - it supposed be about the cleanliness. It has roaches, bad damages from water leakages, an old fan that made much noise the 4 story building shakes. The air conditioner did not work, never mind the knobs were missing. The entire building stayed half empty. I have been here for 10 months - for 8 months they did nothing. After Sandra Mingua finally admit she screwed up, I just wasted 8 months waiting for nothing. After the fan was replaced with the same noisy fan, I have been after Alex to get me out of here almost daily... GET ME OUT OF HERE! The mean manager and a crooked management company, all that combine together is a really bad situation. The building and the people are really sick and disgusting. FOOD The numerous Epicurious websites, you would think the cooks would taste what they serve. I have a salad (thinking the cook couldn't possibly mess it up). I replaced all kinds of chicken, boiled meats, fake bacon, fake eggs don't look yellow. EVICT NO REASON. Four weeks after I said get me out of here, I'm fed up for put up the damn noisy fan. But he'll do about anything he can to evict patients/tenants for no particular reasons. PRIVACY They show no dignity for privacy and respect. They think they can walk in on you without no notice, taking illegal pictures, leave the doors wide open, even though I kept it shut. I literally was worried about my safety. The smells were some of the most toxic I have ever experienced. My only hope is they shut it down. People are drinking, doing unpredcribed drugs, and living in filth. Very disturbed at what I saw. The owners must play cards with the devil.

Good Sam hospital staff in the Emergency department are rude especially that male intake nurse on…read morestaff at 2am Tuesday morning. The staff does more socializing than attending to patients. I once thought this to be a great place for care but in recent years they have gone down hill. I find them to be un professional and lack bedside decorum. The emergency waiting room is nasty and never clean. Not only are they un professional they play favorites. Especially if you are a certain hue. Goodsam you emergency room suck and whom ever is in charge needs to reconsider how that place is run because it is poorly managed.

My brother was at this facility for almost a year and I wouldn't recommend this place to anyone!…read moreThe majority of his stay was during Covid so we weren't allowed to visit. My mom paid out of pocket to have a phone line installed in his room that nobody would ever answer. He was supposed to have a "babysitter" in his room at all times to keep an eye on him that should also answer his phone when it rings. We called the front desk several times with no answer and also spoke to managers that claimed they would fix the issue and never did. My brother was sent out of there to UC when he contacted Covid and also had a bad case of pneumonia. He ended up in the ICU in bad shape. He had also lost a ton of weight and later was shown to be so drugged up that he could barely even speak. They had tweaked his meds so much, without my moms permission, that he was basically just a body in a bed which is probably why we could never talk to him on the phone. Awful! Besides all of this they are thieves! My brothers things were packed up and half of it was missing. We received a couple bags of someone else's things. I have text messages between myself and Paige, a manager, that she was looking for my brothers things and that they must've been mixed up with a resident that had recently passed away because that's whose items we received. Several messages later they never found my brothers things and never got the other residents things back. Lastly, before my brother got transferred out of there I had taken up a donation on my social media account to raise money to buy the 58 residents there a small Christmas gift. We were told that most of the people here were homeless and didn't have family and never got visitors (against hippa rules) that pulled at my heartstrings. I ended up raising over $1000 and was able to get each resident a really nice blanket, a hat, gloves, chapstick, deck of cards, coloring book, crayons, pens, and lots of assorted foods like fruit cups, granola bars, pudding cups, and several other items. During this time was when my brother was sent out. Myself and my kids still took all the Christmas bags down there because it wasn't the residents faults that we had a bad experience. I coordinated all of this with Paige. If you look at the Facebook page you will see that they have SEVERAL Christmas posts about donations and events they held during this time. I asked Paige if they would put up pics of the residents receiving these gifts as well and she said yes. I didn't ask this for recognition but simply because I wanted to make sure that the residents got the bags and that they didn't walk out the door with the employees. I have several messages between myself and Paige with excuses about why the residents haven't gotten the bags. Finally she said that they handed them out but there were no pics yet because corporate had to approve them first. The pics were never posted, messages haven't been returned and my brothers items have never been found. My brother is now at UC and is doing a lot better, gained 20lbs in less than a month and can actually hold a conversation now. Bottom line is, if you want your relative cared for and you don't want them stolen from then DO NOT GO HERE!!! It's sad and pathetic what happened to my brother and our family. I can't imagine how the residents that don't have any family are treated.
