I've been to a few nursing facilities over the years for my dad, and this one is by far the cleanest. It was the one place out of the few weve toured that didn't smell like a hospital, which says a lot. The staff always greeted us with a smile, and you can tell they genuinely care about the residents. The therapy department here is outstanding. My dad came in needing a lot of assistance and left much more independent than we expected. The therapists were patient, encouraging, and always celebrated his progress. We're so grateful for their hard work. read more

It's never easy to see a loved one sent to the hospital, then rehab. You never know what you're going to get. This facility has been really helpful and I feel like they took great care of my family member. But I want to point out Caroline, who answered the phone. I had several questions regarding discharge, when the medical equipment will be delivered and transportation. Because today is a holiday I'm guessing that's why the social worker wasn't in but Caroline took my information and answered every question I had and was able to explain in detail that I have nothing to worry about. I'm not sure if that's a part of her job but I'm very thankful she did that. I live in Los Angeles and it hasn't been easy. Thank you guys for caring for our loved ones read more

If I could add a 0, I would. This place is the worst. Never return phone calls, get put on hold for 20 minutes. A few nice people, but the majority could care less. My mom has been in there for almost 3 weeks and is in worse shape. I can go on and on. I don't recommend this place at all read more

Claremont Care Center - This is what happened to her under their care. - Rehabilitation Center Near Me - Pomona, California

The CNAs are amazing but the social service staff is horrible. They don't care about you or your loved ones needs, the release people when they are unsafe to be at home. They didn't change patient diapers for days, the food served was always cold and I saw a patient slide right out of her bed onto the floor. I have a lot more to say, just don't put your loved one here if you want them to be cared for. I definitely would put negative stars of if I could read more

My medical condition necessitated me to be at Claremont Care Center for more than 90 days. I just was discharged. While at CCC I was treated with the best care I could have expected. The nursing and CNA staff were so kind to me. I was well taken care of. Thank you! read more

beware of nurses who take advantage of seniors! we had this experience with our father while he was there. elder abuse! after my father left facility, a nurse continued to come to his home even when we asked her not to. we expressed our concern and asked her to at least let us (his kids) know when she was visiting. she ignored our requests. she was sneaky and took money from him Multiple times. her lack of ethics should get her license removed. a nurse should not visit a persons home when asked not to and take money from a patient. read more

Removed my loved one from this facility . Care is far from adequate. My loved one declined in a matter of days.

As my dad was being discharged from Pomona Valley Hospital I had brought his belongings up about a half hour before them sending him over by ambulance to this place... I had talked to the front desk and they told me he would be in a room one by the doorway by the front entrance. As I went to go and put his belongings on his room I noticed the TV tray that holds your meal wobbled about three to four inches from side to side and the bolts were loose that hold it to the wheels and frame to the floor. I thought this was a big problem as I brought it up to the front desk and told him my experience with Pomona Valley Hospital of maintenance with their place. I told the lady there I hope I don't have problems here with this place but apparently I did. I wasn't even there for 10 minutes and I found two violations one was the tray that holds your food on it. And then I want to go check the toilet room and that room and the toilet was all clogged up wouldn't even flush I tried flushing it two or three times as it was clogged up I brought this up to a nurse's attention and this is not even your toilet here even works I said I don't think I want my Dad staying here when I find two violations in 10 minutes I'll also be reporting us to the health department.. I definitely do not recommend this place if you're taking your family there you might want to check your room for just like you do at a hotel... May want to lift the mattress up you never know what you might find.. I was quite surprised to find these two violations of a TV tray that's wobbly and a clogged toilet for being there only for 10 minutes I had left the place and I told him I am not having my dad stay here... read more

Computer misinformation caused denial of BP meds. Fixed in four days. Response time to call light minimum 15 minutes, at times 1 hour. CNAs when available were uniformly good. Food OK except for artificial eggs. read more

This place lost all my mom's belongings. Lost her phone and when I reported to the state suddenly they found it. Her other stuff no where to be found. They don't take logs of the inventory so, no track of what was taken or lost. Most times I went to visit my mom, she was half dressed, no blankets. Something was always wrong. When I asked to nurses they would say they'd be back and never would. I wouldn't recommend this place to my worst enemy. read more

My mom was placed in this facility after having bypass surgery. She was very well taken care of (even though I know she can be very difficult). The staff was very kind and patient with her. I'm very surprised with some of the negative reviews on this site. I did not experience any of that. They went out of their way to help my mom through her rehab and got her stronger and healthier. My husband is about to be discharged from the Claremont Care Center following an exceptional experience of their caregiving. Each and every aspect of his four to five week stay was carried out with compassion and expertise. The comprehensive care has him returning home well and with tremendously improved strength from the PT and OT which was the primary purpose of his stay. My mother went to Claremont Care following a hip surgery. She was alert in talking before arriving at Claremont Care. She was sent there for rehab rehabilitation after her surgery. She got no rehab rehabilitation. She developed a fungal infection in her mouth and a pulmonary embolism in her lungs. She is currently still at the hospital and may not recover. This is my second review of this facility. I would avoid this facility at all cost. read more

They failed to do their job and my father was rushed to Pomona valley hospital into ICU he passed 2 days later from sepsis due to lack of care . My mom was at Claremont Care Center for over 3 years. In the end, she was in hospice and I spent all day, every day with her for almost 2 weeks. I personally saw how much the staff truly cares for their patients. When she had to go to the hospital, 4 nurses were hugging her and crying. When I told them she was on hospice, everyone from nurses, to CNA's to the maintenance staff to the cleaning crew were all very helpful and caring. Darcy, knowing she only had a few days left, did her nails for her because she always loved having her nails done. Nurse Arlene was always my moms favorite nurse. She truly listens and cares to all of her patients. I heard nurses up and down the hall helping everyone the same way. Weather it was needing help opening their milk, or needing pain medication, they would always reply and let them know they would be right there. Everyone seemed to want to do everything they could to help. I'm glad my mom was there with such a caring staff. read more

My 84-year-old mother is ending her 11-day stay at Claremont Care Center today following partial hip replacement surgery. First, the positive elements of her stay. The staff was very responsive, present in her room or the hallway, and available to answer questions. My mom was kept safe from falls and made progress with PT and OT. The facility is kept very clean. Some of the longer-term residents were extremely friendly to my mom and I whenever we ventured into the common areas. The negative side made my mom unhappy for the duration of her stay. The facility is very loud at all times. It was very difficult for her to sleep. Also, the rooms are in major need of modernization. Everything from the TV's to the furniture, from the bedding to the window/wall coverings is in need of an overhaul. Being sick or recovering from surgery is difficult enough without being in a dingy environment. Keep you loved ones healthy and well cared for so they don't have to face this level of care. read more

This is the worst place. I got Covid there. I asked if they are notifying visitors that they have patients with Covid. The infection department said yes. I asked if they have masks out for visitors. They said yes. When I was discharged I saw my roommates grandson checking in. I asked if he was told anything about patients having Covid. He said he knew I had it because his grandma told him about me. I asked him about any masks being given out at the desk where he was standing. There weren't any. They lied so much. I couldn't wait to get out of there read more

My mom was at Claremont Care for nearly five years. The staff and the nurses were amazing and took such good care of her. She was always clean, properly groomed, and dressed. The nurses who attended to her needs genuinely cared for her and went above and beyond to make sure she was comfortable. I agree with a previous post in that it is important to visit often and get to know the staff and communicate with them as much as possible. read more

Horrible place for any love one: My dad had suffered a stroke and went to this location to recover and have therapy. He arrived to this facility 9/22/2022 and on 9/26/2022 he complained to the nurse that he wasn't feeling well, his arm was numb, and he had dry mouth to the point where he couldn't talk anymore. The nurse told him to go to bed and that he couldn't have anything to drink since the nurse prior to this one left a note saying that he couldn't have anything to eat or drink due to him coughing so much. Keep in mind my dad was given dinner and water when I arrived to visit him around 6pm by the same nurse. In the beginning when he was admitted we provided my dad a cellphone just for communication. The nurse originally told us that it wasn't necessary since they had 24 hour access. We are glad we gave him the phone because that night my father's neighbor called us and on his end we heard screaming from my father saying he needed help and was begging my mother to come pick him up because something was wrong. I was fortunate that I only lived 5 minutes away so I was able to go to the facility at 10:30pm. When I arrived the nurse was on the phone with her supervisor trying to justify herself on her side instead of checking on my father. I had to personally ask her if she checked his vitals since he had a stroke before and because my father normally would never act like that, so to us something was wrong. Bottom line it took her about 40 minutes after asking me for advice even though I have no experience in the medical field, when she went into the room she came out with a panic look saying she needed to call 911. Once he arrived to San Antonio Hospital after this facility took a little more than an hour to figure things out feeling like a lifetime to have him suffering it turned out that my father suffered another stroke and because of the lack of experience at Claremont Care Center no one had the experience on knowing what a stroke looked like. If anyone is reading this and knows anyone who has also had problems with the same facility, knows someone we can talk to so we can file a complaint or who knows how to handle a situation such as this one please reach out to us so this never happens again. read more

Don't take your family here if you love them! This is a terrible place. They neglected my mother while here, she was here to get stronger and left worst than she came. They don't communicate at all and if you have to leave the facility for dialysis, they will make arrangements with the worst transportation companies. On five occasions that I'm aware of they left my mom waiting at here dialysis center for over 2 hours after she finished treatment, it probably happened more than I'm aware of. Please don't support this place, I strongly believe my mom would still be here if she's didn't go to this place after surgery. read more

Perfect for the no contact parent not in your life. Send them here if you want then neglected, malnourished and stolen from. It wasn't until my father paid out of pocket that he actually got "cared for" but realistically it was the 7-10 hours a day I spent there making sure he was actually taken care of. read more

What a horrible experience, the head nurse was rude when we explained our issues even rolled her eyes when leaving the room. The neglect of my father was disgusting and lack of care was obvious. I am in the process of making a formal complaint to the the administration and relocating my father. They are short of nurses and cleanliness is questionable. Then, soak up the multiple benefits of treatment at CCC I am 45, and I was in search of nearby skilled nursing To complete a 2 week course of around the clock IV antibiotics. I didn't meet the 65 year minimum age requirement of other facilities. and CCC Resulted from my search due to its misleading name. We decided upon CCC based on the stellar reviews OF its PT. Fortunately, My experience at ccc was positive once my initial concerns about an isolated but insidious nursing trio were addressed. Firstly though, services at CCC I would rate 5s include: 1. the physical therapy team who held high expectations for successful, timely progress. And who skillfully developed regiments With laser-like focus on unbeatable TREATMENT RESULTS. Right away, I felt stronger and better healed. And though I was weaker than I had ever been in my life, I didn't fear injury on the watch of this competent team. 2. Though exhausted from efforts healing, i was still capable of teaching night school online. With no obligation, the facility offered me a room providing uninterrupted hours of teaching time 4 days a week. Talk about Over the top accommodation. 3. The large nursing staff showed obvious caring, skill, and an eagerness to help me heal. 4. The kitchen staff went out of its way to ask my preferences, then deliver an individualized menu. Even after changing my mind some, I was again interviewed from bed, and my wishes updated. 5. Of everything, the most impressive takeaway from CCC was showcased by the facility's response after I reported a highly unsettling, multiple day experience endured by My 80 year old roommate and I while under the care of one atrocious graveyard nursing trio. Before Finding a solution, my roommate and I were subject to neglect and disrespect at the hands of 3 sinister employees. The poor treatment became predictable and CONSISTED OF HOUR LONG WAITS before nurses ARRIVED TO DELIVER pain medication, assistance getting to the TOILET even WHEN The need felt URGENT, and a downright shameful failure to deliver the most fundamental care. We were victim to this rogue nursing team for days too long after initially trusting Too much in a few insincere Nursing supervisors whose empty promises Produced nothing, and which denied us precious comfort and any expectation of treatment with dignity or even a stay providing any sense of well-being through long nighttime hours. Later, once I had been tipped off as to the best supervisor for lodging complaints, my roommate and I were swiftly removed from the offending team's schedule, and supervisors followed up with a broad inquiry into the impressions of other patients who had ever been subjected to the abusive treatment delivered by this trio. My experience was not flawless, but CCC's success at setting things right, earn it my enthusiastic support. Many services provided were great. One service was abysmal. But the the response was so respectable and sincere, I would refer my mother here today. . read more

DO NOT BRING YOUR LOVED ONE HERE! their lack of communication and organization made my loved one suffer. They made him wait in the lobby for transport after the first two attempts for transport to pick him up were unsuccessful due to them not having my dad ready. When we got to the facility to transport our loved one ourselves, they had him sitting in the lobby alone, soiled and wanting to urinate. It took them ten minutes to bring the urinal to him and he had to urinate in the lobby, stripping him of his dignity. They admittedly said they would not want their loved one treated this way but they did to mine. Horrible place. They do not even deserve one star rating read more

I have concerns about Claremont Care Center's decision to discharge my Mother, who is currently receiving rehab treatment at their facility, and who is not physically able or ready to be discharged. In fact, she is in worse condition than when she entered their facility. Her regression was caused by their negligence and now they refuse to provide the treatment she needs to overcome that regression, and she was also denied treatment, for two weeks, for a foot injury, which also caused a delay in her physical and occupational therapy treatments. She had improved before all this happened to her. She had a Care Plan meeting, and was getting ready to be discharged the week of January 17, 2019, until she went into a coma, caused by seizures, due to an overdose or reaction to, I still don't know which, the pain medication she was receiving at Claremont Care Center. She is even refusing to take pain medication now because she is afraid it will happen again. Then, after returning from her coma in the hospital she suffered a foot injury, which further complicated her therapy progress, and that injury was left untreated for two weeks. Now, Claremont Care Center wants to discharge her, claiming that she has regressed since she got there. That regression was brought on by them! It was brought on by the treatment, or there lack of treatment, she received at Claremont Care Center. She is in no condition to go home. She needs more uninterrupted therapy. She is unable to walk on her own. She cannot even get out of bed. They even have a bed alarm attached to her bed because she fell out of bed a couple of weeks ago. She needs to be re-evaluated by a different occupational therapist. She needs more therapy! I think I should get an attorney. read more

We've been here twice. The first time was tolerable. This time was a mess. From the in house physician sending my dad to the ER to almost forcing us into home health upon discharge. I tried to put in a formal complaint about the in house physician Dr. Kadhium, he took away a medication and his reasoning was that he would have to be discharged from a hospital with orders. Complete BS. Which of course, landed him in the ER! Upon discharge, the hospital did clarify that he MUST be on that medication and he was put back on. My concern is how can a PCP remove medications that he claims a specialist must prescribe, it should ultimately work both ways; a specialist should prescribe or remove any medication having to do with any specialty related condition, not him, a PCP. Later on he did another look over of his meds and tried to remove yet another crucial medication but we strongly advised against it because we had already tried to and it didn't work. We are currently working on discharge, the social worker insisted that he HAD to have home health in order to have the oxygen that the care center claimed he needed. Upon speaking to both home health and his actual primary care physician, they both stated that the care center could discharge him with the needed oxygen. She finally agreed to find a vendor for us. As stated before, the first time was tolerable. This time was unacceptable. They need to address their issues and not just shove them under the carpet. They're going to end up in serious trouble. So if you must send your loved ones here, be cautious of everything that takes place, most especially their medications and the PCP's medical choices. Best of luck ! read more

Wait can I say about this place... IT IS THE BEST REHAB FACILITY THAT I HAVE EVER SET FOOT INTO! Today was a bitter sweet day for my grandma as she said fair well to the FINE AND OH SO FRIENDLY STAFF. She spent the last month rehabilitating from 3 fractures in her lower back and let me tell you that she is doing absolutely phenomenal because of the staffs hard work and dedication to her. I don't have one bad thing to say about Claremont Care Center. They are all angels in disguise with hearts of gold. Thank so much for your professional and friendly staff from the front desk, to the nurses, to the rehab team and the Human Resources. YOU ALL ARE AWESOME! read more

This is the second time my mom is here in this facility. The staffs are very caring. You can tell that they really care about their staff. I have to commend one of the CNA. I was there to visit my mom and she saw me standing, without asking, she came back with a chair and offer me the chair. My personal experience at this center was good. Even though my Dad had liver cancer, he refused to give up on life. He decided to stayed at this center for rehabilitation instead of going to any hospice care. For over a month, he would get up early each morning, have breakfast, and go to the gym. I have never seen my Dad happier before. He seemed content, pleased, and full of life. Although, he passed away a few weeks ago, I am so relieved to know that he spent the rest of his days in a place where he felt loved, cared, and appreciated. And for that I will be eternally grateful. read more

My mom, who had a stroke, was there barely a week and the CNA gave her a shower, took her back to her room and stated she needed to go get towels. She left my mom in the shower chair naked and wet for a long time and my poor mom was freezing, so she tried to get back into bed and the wheelchair flew out from beneath her and she fell and hit her head on the nightstand. When I found out I questioned the nurse and he stated she didn't fall and asked the CNA if she fell and she said no, she wasn't even there! They never examined her until I made them, which was just a visual. They lied and covered for each other. It took a week to find another facility. I would NEVER EVER have a family member stay there. read more
