Staff admitted they couldn't properly heat the rehab hall and told me to bring a heavy blanket. Despite my efforts, my mom remained miserably cold until she was finally moved to the nursing care hall, which had working thermostats. During my daily visits, I observed dark, urine-smelling rooms, patients slumped over food trays, and others calling out for help with no response. Some were wheeled from showers half-naked, wet, and shivering. I left repeated messages for the doctor and staff requesting updates on my mother's care but never received a single return call. Although Hilltop is supposed to be smoke-free, many staff members reeked of cigarette smoke. My mother, who relies on oxygen, often became ill from the odor and struggled to breathe when around staff. The food was cold, bland, and difficult to chew. She was never seen by a dietitian, despite being diabetic and having neuropathy in her hands. Staff routinely dropped off her tray and left, knowing she couldn't open packets or cut food. I began bringing homemade meals, Ensure, juice, and fruit. Even so, she became visibly thin and weak due to lack of proper nutrition. Her therapy was minimal, and she grew increasingly depressed. Housekeeping was abysmal--sheets unchanged, floors dirty, and overflowing trash cans. I provided towels and cleaned her room myself. One aide entered smelling strongly of feces and contaminated my mom's walker and drinking cup. I had to disinfect everything, and the next day Mom became nauseous--clear evidence of cross-contamination and an OSHA violation. She consented to only one shower because the facility was cold and lacked privacy. To preserve her dignity, I began bathing her at my home, despite recovering from major back surgery myself. On 2/28/25, Mom called me late at night, crying that the smoke smell made her sick and that staff ignored her repeated calls for help. After waiting over an hour for Tylenol, she went to the bathroom alone while struggling to breathe. I called Hilltop, but no one ever answered--their main phone didn't work the entire three weeks of her stay. Staff warned me that removing her would be considered an "AMA" (Against Medical Advice) discharge. I left several messages for Hilltop's doctor, Dr. Wusterhausen, asking him to sign release forms so my mother could transfer to her assisted living facility. Calls went unanswered for nearly a week. When I asked for help in person, a staff member took the form "for Dr. Kris." On 3/3/25, she barged into my mother's room, angrily thrust the paper at me, and accused me of "complaining" about her. She was Ms. Jackson, Hilltop's social worker. Her behavior was loud, hostile, and physically intimidating--I had to step back to protect myself, still wearing a back brace from surgery. I reported her to Administrator Ward, who said he'd "speak to her," but I never heard back. That same day, Dr. Wusterhausen finally signed the proper release--not an AMA--and we moved my mother out. She immediately improved under our care at home and later transferred safely to Clear Fork Assisted Living in Willow Park on 3/10/25. On 3/17/25, a representative from Adult Protective Services (APS), named Lacey, arrived unannounced to investigate an anonymous complaint that my mother was being neglected and that our home lacked electricity or running water. After separate interviews, she determined the report was false and closed the case as invalid. It was obvious the malicious report came from Ms Jackson in retaliation for my complaints. This misuse of APS is an abuse of power, wasting taxpayer resources and further traumatizing patients and families. Such retaliation is unethical and should be investigated; Ms. Jackson should not be allowed to continue in her role. On 3/26/25, I called Hilltop requesting the corporate CEO's contact to file a formal complaint. I was transferred again to Mr. Ward, who questioned my reasons and seemed unaware of APS's involvement. I recounted the facility's poor conditions, the staff neglect, and the retaliation. My mother's three-week stay at Hilltop Park Rehabilitation was a nightmare of cold, filth, neglect, and administrative failure. The staff ignored patient needs, the doctor was inaccessible, and the social worker's vindictive actions crossed professional and ethical boundaries. My mother entered Hilltop weak but hopeful for recovery; she left frail, traumatized, and terrified. If you love your family member -- avoid Hilltop Park Rehabilitation and Care.

I wouldn't feed this food to a dog (see photos). The amount of money paid for full time care is outrageous and these residents deserve better. They should be embarrassed to serve this food to our elders day in and day out. Complaints fall on deaf ears. They have many staffing issues using help from "agency" to fill in. Much of the "agency" help has a bad habit of not really caring about their job because they will be off to some other facility tomorrow. I could go on. I think you get the point. There were definitely some growing pains throughout the transition, but one thing that never changed was the care and compassion shown by the regular staff. As my mother's health declined, she spent time at a local hospital. While there, I was told that patients coming from Willow Park were typically in better condition and better cared for than patients from other neighboring facilities. That comment stayed with me. I won't say this facility is perfect. Honestly, the thought of needing to live in any type of care facility someday terrifies me. But unfortunately, there came a point when my mother needed a level of care that I simply could not provide on my own. My mother ultimately passed away, but the care and compassion I witnessed from the staff during her time there is something I will never forget. They knew her. They cared about her. They helped her through an incredibly difficult phase of her life and they helped me through one of the most difficult phases of mine. While I was not always impressed with the agency staff who occasionally worked at the facility, I am incredibly grateful for the management, nurses, aides, laundry staff, kitchen staff, and the many regular employees who cared for my mother. In her final days, I witnessed firsthand just how deeply some of them cared for her. Their compassion was genuine, and it meant more to me than I can adequately put into words. My mother was able to receive care that I was not able to provide myself, and I will always be grateful to the people who cared for her, comforted her, and treated her as someone they truly knew--not simply as a resident.

They lie telling you that you can use the phone when in-fact your phone is taken away and your only allowed to call for 15 min on Sunday and they listen to your conversation the whole time and if you say anything, they don't like they hang up the phone on you. They also call your family and lie to them. When you get mail, you don't get it until a week or so later, they hold onto that without giving it to you when it first gets there. They also put a lady in the middle of the circle of other ladies and be rate her and put her down until she's crying. One lady was not allowed to talk for days because something I'm not sure but supposedly she talked too much or something so they would not allow her to speak for days after that. I was in there because I was taking prescription medication's for pain for 20 years and I needed someplace to be able to get off of these medication's. I was very ill and they refused to take me to the hospital. I asked to leave they said if I left, they would not give him my phone and I couldn't call anybody. I would have to go to the they would take me and drive me to the outside gate of their property and I'd basically be out in the country on my own. Finally, I was able to go to the hospital and when I did go, I had inflammation around my heart and different other health issues because I have health problems also so at that point I was admitted to the medical hospital for a week then I checked out of Stonegate and didn't go back. My legs and feet were so swollen. I couldn't even walk. They also overcharged me for my initial payment that my husband had to pay that they said was my out-of-pocket when I got a hold of my insurance I really only had to pay a lot less than what they charged me, which I never got that money back. They lock you out of your rooms during meal times and you have to ask to go to the bathroom. They unlock the door for you to go to the bathroom and then lock it when you come out. You're also not allowed to sit on the furniture unless they're doing their nightly AA and na meeting. You also have to fill out papers at night to say what you've done wrong in different things like that. You can't say that you've done anything right so it's pretty much putting yourself down all the time I would advise against going here. It's a very unsafe for your health and for your mental physical and emotional and spiritual well-being maybe it's changed now, but it sure wasn't a few years ago. Don't go here they are very mean and deceptive people who worked there a few years ago. Like I said not sure how it is now but id never send anyone there

The staff doesn't care, call buttons going off at the nurses station, and staff is just hanging out chit chatting & laughing. The nurses are annoyed when you ask for help, or when you ask questions about the care. We were lied to, blatantly. My husband works for a hospital corporation, and he is spreading the word to the directors of Inpatient Rehab, and Orthopedists to NOT send your patients there!!

We were quickly introduced to Diane who helped us through the process of having mom transferred from another facility that was not a desirable place to be. When we arrived day of transfer we were greeted by Tess, assistant business manager, who graciously assisted with all intake paperwork. She explained everything well and asked questions to our mom in a gentle and compassionate manner. All the staff has been kind and friendly and placed a priority on generous care and recovery for mom. What more could we ask for? Additionally the facilities are modern and clean, and it is one patient to a room also for privacy. After extremely poor experiences in two other facilities the last two years, we would wholeheartedly recommend Advanced. Who you trust with the care and recovery of those you love is an important decision. With Advanced we have felt the right decision was finally made.
