    Excellent food & upscale retirement /memory care. Good location & cannot find a more luxurious setting.

What stands out the most is how vibrant and active the residents are. Every time I'm there, they're engaged in a variety of activities and genuinely having a great time. It's clear that there's a strong sense of community and joy throughout. Everyone I've encountered, both staff and residents, has been incredibly friendly and kind. It's a place that really feels full of life, connection, and care. Highly recommend the Forum!!

    I cannot believe the business manager from Discovery Management said to contact the executive…read moredirector, Lisa Grey. SHE IS A BIG PART OF THE PROBLEM! I strongly encourage the management company to reach out to literally every employee that has either left or been fired since Lisa Grey started. I highly doubt any exit interviews were done by either the previous management company or the previous ownership entity. She yells at employees and absolutely berates them. It is beyond horrible. Residents have moved because of it! Fabulous employees seem to disappear. All of this pales in comparison to the dangerous situations that have happened at assisted living in the past couple of months. On the third floor alone 1) A resident was trapped in their room by another resident outside the door. The aggressor resident has bullied the other person FOR MONTHS. The aggressor resident was then drugged. From my understanding all that was wrong with the aggressor person is a UTI that went untreated for too long. 2) A resident that doesn't leave their room much WENT MISSING for 48 hours. Then a 15 year employee who never even goes to the residents' rooms was blamed and fired for it. I personally adore the fired employee, Gloria. She went above and beyond to help my mother with her medication. I really hope Discovery Management checks the video footage before it somehow disappears. 3) A 92 year old resident was left by the weekend nurse on duty in the hallway. The woman was projectile vomiting, shakey, sweaty, and very ill. I desperately tried to reach the executive director LISA GREY. I got no response. And she never followed up with me AT ALL. An off duty nurse responded within the hour. I ended up reaching out to the elderly woman's son for permission to call an ambulance. She was swiftly taken to the ER by the hospital and treated for dehydration. The off duty nurse who responded late on a Saturday night is another employee who no longer works at the Forum. Her name is Belita. And she was beloved by the residents. Her replacement was brought in with ZERO ONBOARDING. Absolutely zero. Numerous things have fallen through the cracks since she has arrived. For whatever reason, she blames her predecessor, Belita. Blames her to the point that she tells the residents that Belita did not do her job. What???? Belita was on top of everything and ran a tight ship with those below her. I'm positive that a lazy subordinate employee played a role in her leaving the Forum. I hope this candid review forces Discovery Management to take a close look at the Forum. Furthermore, I hope they reach out to the previous employees like I suggested.

    If I could leave 0 stars I would. I was there for a workers comp claim as they are a provider on…read morecareworks. After I submitted my workers comp claim to my job, they sent the necessary paperwork to get the ball rolling then informed me to see a workers comp doctor. I seen dr.Chang 1. I was initially advised that my case and symptoms were too complex for an urgent care setting and that I would need to see a non-urgent care workers' compensation provider or my primary care physician to serve as my treating doctor. 2. After consulting with your medical director, I was informed that your director did not believe my workers' compensation claim would be approved because my symptoms were related to repetitive hand and wrist movements and other job duties(I type and drive for work ) . I explained to him that it does fall under works comp especially because it occurred during the job and that workers comp dept would have never insisted for me to see someone if it was going to be denied. He still was adamant that my chief complaint(carpal tunnel) was not going to be approved for workers comp because I had progressed symptoms (my entire right side) was presenting the same + new symptoms as carpal tunnel. My entire right side is not functioning as my left. 3 /many dx was neuropathy of leg,shoulder,&hand but no referral out, restrictions, or off work while I sought a new doctor? I extensively type and drive for work? I can't feel the right side of my body and it involuntarily twitches/shakes (he saw it) ?? How am I going to work fully duty ? It's a safety hazard?? Not to mention I'm in severe pain! 3. I requested referrals to Orthopedics and Neurology for additional evaluation, including nerve testing and further diagnostic workup. However, I was advised that referrals would not be made because, in your opinion, the claim would likely not be approved. 4. I was released to return to work with no restrictions. However, I was also advised that this did not necessarily mean it was safe for me to return to work and that restrictions were not issued because, according to your director, the claim would likely be denied and placing restrictions would be "a waste of time". He wouldn't even try. aside from the assumption that my workers' compensation claim would not be approved, I was released to full duty without restrictions despite presenting with significant and progressive symptoms, including severe carpal tunnel symptoms affecting my hand, wrist, and fingers, as well as symptoms involving the entire right side of my body, all of which impact my ability to safely perform the essential functions and full duties of my position. Furthermore, I couldn't be put on restrictions or off work while sought out a more qualified/appropriate doctor to further eval. 5. My workers' compensation personnel also expressed concern regarding the statement that restrictions would be "a waste of time" and questioned the rationale for releasing me to full duty given the nature and severity of my symptoms of carpal tunnel and ect-especially because the doctor saw them himself and my dx! If my claim was going to be denied-I would have never been able to start the process... it would have been at the front end. This is why people don't get help. I would never recommend!! Heads up they make workers comp Decisions based on assumptions not the symptoms presenting to them.
