My parents lived here a few years ago and it's taken this long for me to bring myself to write the review The Forum deserves. If your loved one is mobile and can take advantage of the amenities for Independent Living like restaurant - style service for dinner with jackets, valet, maybe one or two other things, it's nice. But, their Memory Care program and staff should be entirely shut down. The experience we had there with my mother, who had Alzheimer's, was so so awful, it's hard/painful to reflect on it. They put here through a nightmare, and all of us as a result. They had a staff person in charge that was not qualified to run a memory care unit with all its complexities. My mom ended up suffering for that. Horrible horrible experience for Memory Care. We thankfully moved her to a place, Colonial Oaks near the med center, that was experienced for this population and so very kind and loving. She passed there much more peacefully than she would have. I suspect someone will respond to this as I'm seeing they've done for other reviews. Better yet, give me a call and I will tell you all about it. read more

I Love all the Residents and Staff at The Forum. It is a wonderful place to work and Fivestar goes the extra mile for their staff.

They treat their tenants like actual humans. Such a wonderful place for the elderly that require care in a nursing home. Very safe and very nice building! read more

Don't leave your loved ones here! Terri Smith the activity manager did not even evaluate my family member until 3 weeks after arrival!! Horrible!!! Physical therapy team was a joke. They would come in, put our family member in a wheel chair and leave! When Covid happened they said they would provide daily updates since we could not visit. Never happened! Came in on nights and saw no one on the floor, and nurses sleeping in the dining room in the corners out of sight. When I would call to check on our family member, no one would ever answer the phone! Horrible! I would recommend looking elsewhere read more

Considering the current climate with Covid-19, I'm sure glad my parents are at The Forum at Memorial Woods. I know it's tough being quarantined but I'm so thankful they moved fast to protect them. I ate there before the pandemic lock down, and the new chef did a great job with the food. Now food is being delivered to them at their door daily with safety in mind. They tell me it's good considering delivery. I have friends with horror stories at some of the other assisted living places and nursing homes. Very sad for them and my heart goes out for them. I'm thankful their experiences have not been ours with our loved one's at The Forum. I'm an apartment manager so I know how tough it is to run a multi-family operation. I look at their performance overall and I give them 5 stars especially during these challenging times. Keep it up. read more

I am totally not happy with the food at the forum, cold 90% of the time. Meat products are terrible.

I love the safety guidelines and protocol that they have for both staff and residents during this entire pandemic. This is the place to be and have your loved ones! They actually care about the best interest of the residents . I wouldn't choose any other community for my grandmother . read more

My mother died here a couple of weeks ago, they lie and are understaffed. My mother was there over 4 years, and Jessica the director never even called to give condolences. We went to see my Mom everyday to help feed her with lunch especially in the last year. They never had enough staff to feed all the residents there, I complained constantly. The care givers are not trained and over worked, you complain and all Jessica says is yes we will take care of it. I got to know a lot of the staff there, and management is the biggest problem, there is not management on the 1st floor of memory care at all. The care givers work doubles and sometime there has been one care giver for the whole floor. They don't care for the residents and will take whoever in memory care no matter what their illness is. Not enough people working to care for these people. I should have taken my Mom out of there when Jessica took over. My Mom got on hospice 2 years ago so they did her bathing and meds. The care givers do not brush their teeth ever. SO if you don't plan on going everyday to see your loved one this is not the place, my mom also had bruises I complained about the staff did not know how to transfer no training again in that area. When they called to say our Mom passed away a nurse who did not speak very good English called me and said ah I can't find a pulse on your Mom, no vitals, then said nothing! When we got there my Mom had passed hours ago. They all lied, I asked everyday did they have enough workers oh yes they said. They would not let us visit the last few weeks of her life, but said if something happened we would be called. I feel guilty everyday for not doing more. Do not put your loved one here. So sad you pay a lot and trust them to take care of your loved one when you can't. God knows the truth, and that is most important. read more

The grounds are well maintained and it's always festive according to the holiday and seasonal decor. Located in a great location also.

The food at this facility needs a lot of improvement. It is frequently served cold and is sometimes not eatable. Nursing care quality is poor especially on the evening and night shifts. Nursing staff is sometimes consistently working longer hours than is safe. All that being said, the Forum, is probably one of the better facilities in the Houston area. read more

My loved one has been at the Forum, and as a skilled nursing facility, the health care section is exactly as the reviewer Greg K says: Awful. In fact though my loved one was not in the Alzheimer's unit, the experience was about the same as he stated. The nursing staff with only a few exceptions are dictatorial, indifferent , lazy, and ignorant of many nursing basics. There were a couple of the nurses who were openly hostile or antagonistic. I won't name names, I will save that for the reports we are intending to file with Medicare and the regulatory board for skilled nursing facilities. We had the best treatment from the aides, who at least in a couple cases were friendly. There were also several medical errors with the meds, which we are also reporting. We frequently had to intervene in the care, from the administration of meds to the mundane tasks of housekeeping. It was very exhausting. The physical therapy staff are wonderful and do a good job. But they aren't enough to make this place worth it. The administration tries its best , is caring, friendly and has its heart in the right place but it cannot overcome the resentful incompetence of the nursing staff. As Greg says in his review, the holding company has got them in a bind. There wasn't a day when our loved one felt cared for or safe at the Forum. Perhaps if others avoid the health care there, something will move the ownership to invest in real nursing oversight that demands competence and responsibility. read more

This place is horrible let me first start by saying the lady that answered the phone has a very nasty and ugly attitude her name is Dildred Black. I called to ask questions about the property because I was looking for an apartment for my parents, she was not helpful at all. She talked down to me like I was stupid and she would not let me finish my questions before she kept interrupting me. So I decided to go in to see the building In person now I thought I would get someone different but I encountered her again but this time she was talking down to a maintenance worker I thought that was very unprofessional in front of a potential renter so that turned me off from this place and that lets me know that if she can be as rude as she was to her co worker I know she would not treat my parents with respect.... so with that being said this place is a big fat no for me but I do suggest they do some about MS. BLACK because they will lose a lot of business if ppl had to deal with her on a regular basis read more

We (my wife, my puppy, and I) are celebrating our one-year anniversary at the Forum. We have found it to be a great place for independent living. I have no knowledge of the various forms of assisted living available here, except to note I have friends here who have used them, and they generally say good things about it. As for the Indy-living side, great staff and pleasant co-residents make living here an enjoyment. Looking forward to many more years here. read more

First of all, I have some REALLY bad news for anybody who is about to put a loved one in a nursing facility... THEY ALL SUCK. Actually, I'm sure there is some Mom and Pop down-home facility out in the middle of nowhere that's wonderful. The Forum, however, is not that place. Let me clarify. If you just need a nice senior living apartment, this place is really nice and fairly high class. However, if your loved one is sick and needs a skilled nursing or alzheimers facility, this place is just about as bad as the rest. The staff is made up of barely educated, inner-city, minimum wage zombies. I guess the corporation that owns the place doesn't want to spend too much on decent staffing. But they don't seem to mind charging mind boggling fees. We were paying $7000 a month for my father!! That is not a typo. That seven grand got him a dingy, beat-up, old room, three cold meals a day and some "occasional" help going to the bathroom, eating or taking a shower. How they charge that much for such incompetently bad service is really a crime. The government should look into this. We would find food all throughout my Dad's mouth that hadn't been brushed or checked or cleaned in days. The management staff is very friendly and seems to try their best. Unfortunately for them, the holding company that owns the place doesn't give them much to work with. It's a no win situation. I doubt a day goes by where some seriously pissed off family member doesn't ask them, "I'm paying thousands of dollars for this?!!" On a side note: The hospice woman who helped us with my father said the cheaper, lower-end nursing facilities were downright scary. She said she wouldn't keep her dog in one. So... there are your choices. My advice, don't get old or sick. Sorry. read more

We have 2 family members in Independent Living at The Forum; we visit one to three times weekly. There's both good & bad news -- read more. 5-Stars: The good news: The Forum's independent Living is a really NICE facility. The staff consistently address residents by name. "Good morning, Mr. XXXX." The door men / security staff are very well-trained and courteous; the dining hall staff are attentive and polite. All the public spaces -- the lobby, elevator areas, hallways, dining room, activity rooms -- are beautifully furnished. Live orchids are always in the lobby. The food is high-quality and attractively served; the menu is nicely varied. Breakfast and mid-day meals are buffet-style with wait staff providing drinks and assistance; evening meals are ordered restaurant-style, and wine is available. Residents are offered interesting art classes and have the opportunity to participate in Forum-offered trips to cultural events (i.e. the Opera, etc.) or lunches out on the town. Throughout the year, there are fun on-site celebrations -- authentic TX barbecue during the Rodeo, grilled American favorites and live entertainment outside for the 4th of July, and many more too numerous to mention. In the month of their birthdays, residents are invited to a lovely champagne luncheon. One guest is complimentary, and others may attend for a reasonable fee. Happy Hours are offered twice-weekly. There is a phenomenal effort by all staff to provide a really wonderful holiday event in December. Residents are encouraged to invite friends and/or family, again for a reasonable fee. The Forum is decorated beautifully; the food is absolutely top-notch, and the entire first floor is converted into a lovely dining experience. Live entertainment is provided in each dining space; everyone -- staff, residents, guests & tiny tots -- are dressed in festive attire. It's a really nice event, and residents get to meet each other's children and grandchildren. In all these aspects, the Forum is 5-star senior living. 1-Star: The bad news: We noticed both of our loved one's apartments had very high humidity this summer. We called maintenance repeatedly; they never answer the phone!!! We left voice messages but received no responses. I took time to physically track down a maintenance man; he explained that we had the a/c unit fan set incorrectly; we corrected that in both apartments. In one apartment, we were shocked to find mold growing on the upholstered couch and chairs. We called maintenance and left messages again. Weeks passed before we got a response. The head guy had been on vacation. (And NO ONE ELSE can check voice messages?!?!!?) The Forum wanted permission to spray the furniture with MOLDEX. We didn't want a heavy chemical presence in the apartment; we researched the product. It's NOT recommended for use on upholstery!! We -- ourselves -- successfully used tea tree oil and rubbing alcohol to kill the mold. We still needed to fix the humidity issue in both apartments; we left more messages. We noticed mildew on the wood cabinets and furniture in the other loved one's apartment. We left more messages. I showed the mildew to the marketing person who had assisted us with both move-ins; I asked for a plan of action and a timeline. The response, THREE WEEKS LATER, was an invitation to a phone conference with a mold / mildew expert. The remediation has yet to begin. Meanwhile, our loved ones' allergies and asthma have intensified. There is clearly a huge problem with humidity / mold / mildew at The Forum. Check out the huge wet spot on the ceiling in the corner of the lobby! I don't know how many apartments are affected, but 100% of our family's are. read more

My 87 year old mother was recently at the Forum for skilled nursing care and rehab after becoming ill and fainting. Although a "skilled" nursing facility, there was no R.N. on the floor. The lack of human kindness and compassion was a real problem. Although there were some nursing staff that were sweet and attendant, there were too many that not only didn't seem to care but were rude and unkind. I was shocked. For example, when she asked for more ice water, the caretaker looked over at her 1.2 fill cup and stated, "you already have water....", and promptly left the room. My mother was bedridden and one night was "fussed at" because she soiled her bed. And, although physical and occupational therapy was critical and a major reason why she was there, it was not provided consistently. After several days, we had seen enough and requested a transfer to another facility. I thought it was going to take an act of God to get her out of there. The person in charge argued that she had received the therapy that we knew she had NOT received (she had a family member with her 24/7). I was flabbergasted! They are very expensive and are a substandard facility. I would NEVER use them again, even for my worst enemy! read more

My loved one WAS in the Alzheimers Unit and it was really bad. Due to our bad experience we had to move. Any other facility would be a lot better. The charge nurse (LVN/Resident Serv Coordinator) is someone you have to communicate with quite often but after a couple of months will get rude and sarcastic when you ask him anything. He is not very knowledgeable so as a family member you will have to do all the thinking for him and try to figure out how to care for your loved one. His answers to your questions don't always add up, it will just be an answer to get you off his back. Even if you recognize something different about your loved one, he will say they are doing good and he doesn't see anything wrong, lazy. A few years ago he was demoted from Director down to Charge Nurse and now needs to be demoted from Charge Nurse. There is no Alz Director in place, the Director for the Assisted Living is acting Director for the Alz Unit who just takes up for the Charge Nurse and seems to be the reason the Charge Nurse still has a job. I was told by the Exec Director that the last Director was let go (there for 3 mo) because no one could work with her. It was more like the others did not want to work with her because she recognized the problem. She had experienced problems from the Charge Nurse herself and was quickly seeing the problem. Instead of fixing the problem they decide to just get rid of the Director and the Charge Nurse won't have to take orders from the Director. No customer service whatsoever. If your family member gets injured (bruise, skin tear, etc) they never know how it happened (so they say). That is a good answer and the easy way out. Furthermore, they don't really care how they got hurt. A lot of the good CNAs (aids) have left and more keep leaving. So now they are understaffed. One day we were visiting our loved one and an aid started an argument with another aid right in front of us. The aid that started the argument is still there. Any other place, she would have been terminated. No leadership and they can't afford to lose anymore aids. No use in going and talking to the acting Director or the Exec Director because in front of your face they act like they care and will take action but when it is all said and done the family member is the bad guy. All of the upper staff ends up covering for each other so you get no help. OMG all the lies they end up telling you! but their lies aren't very good (not everyone is on the same page) so very easy to figure out they are lying. There is no one to talk to. Not sure what the connection is between the Assisted Living Director (acting Director for Alz) and the Charge Nurse but they definitely protect each other. This is not only my observation and experience. While I was there, other family members shared same/similar concerns. For some other families it just isn't as easy to get up and move their loved one for different reasons. Fortunately everything fell into place for us and we took our $6000 elsewhere. read more

There are no words to describe how wonderful my experience has been with the Forum. It is a first class place with first class staff. My family had an emergency and rather traumatic situation where we needed to find an independent living/apartment for my father within 24 hours. We were able to move him into the Forum where he stayed for several months. The staff are very professional, friendly, and caring. I would recommend the Forum to anyone looking for a loving and nurturing environment for seniors. (*I only have experience with the independent living side.) read more

My Mother was here for a few months during her pallitave care period. I will honestly say in my opinion, this is the best skilled nursing facility in Houston. My Grandmother was at The Hampton at Post Oak and the care between the two is night and day. The staff is attentive, albeit task oriented but, that was my only complaint. Some of the nurses aids cant speak english so you have to talk to management but, their language barrier does not inhibit their duties. Mom was very well attended and the staff really took the time to know her as a person. the admissions director was also quite accessable and timely in his responses and willingness to give tours. The rooms are a bit tired and show wear but, that was more than mitigated with the standard of care. If you ask, things are repaired in a timely manner. Residents are encouraged to decorate their rooms, including hanging pictures. read more

My grandfather was at the forum a few months ago for therapy from a broken hip. All the staff were wonderful to him. They made sure that all his needs we're taken care of. The only thing was that the furniture needed to be replaced it is old and outdated. But the staff outshines any defect that was on the bedside table. read more

The quality of the staff and care at this facility has deteriorated a lot. My mother lived there for some years and I have seen a dramatic change. The staff was lacking in empathy, on their cell phones all the time and things (particularly food and liquor) had a way of disappearing. You pay a hefty fee to move into this place, but don't think that it is a deposit because it is not. They also require 60 days notice to move out which I think is holding people hostage. All in all, I am very disappointed. read more

Had my mom in there paying lots of money a month for her to be not looked after properly and not helped with hygiene care. Found a personal care home in se houston and my mom loves it. She is well looked after and always clean and dry when I go there. Personally you should shop around when looking for a place to keep your loved ones the most expensive is not always the best. I was paying 7500 and now paying 2500 with exceptional care. Please check on your love ones often when in a facility. read more

My 94 year old mother has been a resident of the Memory group at the Forum at Memorial Woods for 1 1/2 years. She has been in two other Assisted Living locations; however, she is definitely much happier here. i can hear it in her voice when i speak to her and she tells me about her day. Every three months i travel from New York to Houston, Texas to visit. I always stay on-site for 6 days. This is a wonderful way to see what really goes on, I am very pleased and relieved by what I see. They have morning and afternoon activities like seated exercise, coloring, current events, movies and other tihings that assist their memory and development. The staff is very caring, and capable. I always feel she is safe when I leave. Medication is given on time. There is a central seating area with music where people can gather and visit with each other. There is always a care giver right there with them. They get individual attention from a staff that knows all the residents. The facility is beautifully maintained. They will do laundry if you want. They have adequate security and a private dining room. And the food is great! read more
