I would NEVER want to be here as a patient, nor would I ever bring someone that I love in here. The DSD "Sona" is so incredibly rude and unprofessional, lets the staff treats the residents like trash with zero consequences. The staff is so mean to the residents, treats them like they are nobody, feeds them awful food that doesn't even look edible, force feeds them down their throats, etc. I simply can't believe how people work here and are okay with the way these residents are being treated. My heart aches for the residents and family who are being "cared" for in here. If you don't believe me look up all the law suits, they even changed their facility name. I am disgusted and saddened that this place hasn't been shut down. I will definitely be reposting to the state.

Thanks to the entire staff's exceptional care, I am walking again! The PT and OT folks (Nikki, Deandre and Akashi) patiently gave me the support and confidence to not only walk, but to climb stairs! I have never had PT before and was amazed how their simple instructions and encouragement helped heal my body so that I could return home. There are too many other names to mention here, but the following are a few. I'd like to give a shout out to the nursing staff, particularly Dahlia. The nurses were so kind and patient, helping me with every need, including cleansing. I hope that I was kind and respectful, but I noticed that some patients could be quite difficult. The nurses and CNAs consistently maintained a calm, caring and professional demeanor. They catered to every need and clearly cared about the patients' well-being. Claire, the Activities Director delivered daily menus and a list of scheduled activities. What a sweet individual, dropping by each morning with a smile and a little conversation. Nicole of the Housekeeping Staff was lovely. I engaged her in conversation at every opportunity, as she made sure that each room was clean and in proper order for each patient. My case worker, Mary, was instrumental in working with doctors and staff to secure my release date. She also recommended an appt. with my PCP and referred me to out-patient PT care. Another shout out is to the kitchen staff. The meals were healthy and delicious and tailored to each patient's needs. Mistakes were sometimes made and were quickly rectified. One small concern is that breakfasts always arrived cold. Having worked in catering, I know it is difficult to serve a hot meal to 50 people at once, but it can be done. Icing on the cake: Greta, an optometrist visited some of the patients for eye exams and glaucoma tests. I did not expect this extra service and appreciate it so much! In additional, the facility has followed up with me to check on my healing progress. I hope none of you require such intense care, but if you do, I highly recommend PH Post-Acute Care to administer to both physical and emotional needs.

I stayed there for three months, and during that time I experienced nothing but care and support from the staff. The nurses and CNAs are attentive, friendly, and truly caring. I especially want to thank Marth and Arryan--they are so sweet and genuinely care about their patients. The night nurses were excellent as well. I'd also like to thank the CNAs Melissa, Beth, and of course Dalia. They were helpful, kind, and attentive. They took such great care of me and made me feel safe during my time away from home. Physical therapy and occupational therapy were also great throughout my recovery. They helped me from the very beginning to regain my mobility and strength. The therapists Jordan and Justin were fun, funny, and kind throughout my sessions. They made each visit engaging and always took the time to listen to my needs. They consistently found ways to challenge me while keeping the sessions enjoyable, and they helped me grow stronger every day. I truly appreciated their support and encouragement. My case manager, Bianca, was amazing throughout the whole process. She was always available whenever I had questions or needed someone to talk to. I googled Hospice and came across MD Choice that had some good reviews. I had no idea who I was calling but there had to have been some divine intervention pushing me to this call and this company. After a few questions I received a call from Dr De Leon who was the most compassionate and caring doctor I have come across. He sensed my immediate need, came straight out to the house, assessed my mother and went himself to the pharmacy to pick up the needed medications. He knew the urgency and took it upon himself to do all the footwork. He obtained the needed comfort medication, stabilized my mother and allowed my mother to pass peacefully all within a matter of hours from making that initial call. When she did pass that evening, he again, arranged for everything from the pronouncing to the calls according to the pre-arrangements. Without MD Choice and the connection with Dr De Leon, I don't know how I would have been able to handle the situation. His immediate response on a Sunday, his Taking control of the situation and quick actions made such a difference in the final few hours my mother had. If you find yourself in a situation like this and needing someone to help in this time, I highly recommend MD Choice and Dr De Leon. I am forever grateful to MD Choice and Dr De Leon for acting so quickly and with such caring and love in this terrible time of need. Not only were they able to provide significantly more daily visits from their team of nurses and aids than the other options we considered, but they were incredibly responsive, efficient, and helpful with every question or request we had--and that includes their 24/7 on-call team. Cheryl, the RN Case Manager, in particular, was a godsend. It was a huge relief to be able to rely on them so thoroughly from day one. They delivered everything we needed within hours of getting the paperwork done; and both Cheryl and an aide were there for the last few hours my brother was alive--providing support and care. I was sent to Legacy for nursing and physical therapy services, following Spinal Surgery. This…read morefacility was clean. The frontline staff was professional, kind, and engaging. However, I experienced a lack of preparation, an absence of communication, and a failure to reorder necessary medications --on multiple occasions. These negative aspects of my time at the Legacy stemmed from the Administration's systemic failure. Administration provided no orientation or introduction to the facility. Thus, I had no knowledge of visiting hours, food menus, or any patient rules. I did not even know there was an onsite Social Worker until the last week of my 5 week stay. I had to track her down to find out if administration had a discharge plan for me. I was initially provided a wheel chair with no foot rests and a bed with no arm rests. This was at a time when I was non ambulatory. Eventually, these medical items were provided. Still., when the facility ran out of my two heart medications (needed 2x a day) I wasn't told til the time I was to receive them that they ran out and had not yet reordered. It took 24 hours to get these medicines. All in all, my review of the Legacy is mixed.. I think if the Administrators strengthened themselves into a team that promoted clarity and transparency to their patients/clients this would be a very good facility. Patricia McGuire Lopas

Since this was my first time staying at a rehabilitation center, I was very anxious at first and worried that the facilities might not measure up to a hospital. When I was first transferred here after my surgery, the facilities were understandably a bit different from a hospital setting, which kept me somewhat concerned initially. However, the staff at La Casa Via were exceptionally kind and accommodated my needs to match their system to me as much as possible, helping me adapt in no time. Before my surgery, my condition was severe enough that I had to use crutches, and after the procedure, even walking was painful and difficult. Thanks to the dedicated rehabilitation therapy team at La Casa Via, I was able to make noticeable progress every single day. The physical therapy and the wide variety of daily programs kept my routine engaging and never monotonous. Even when I made requests that I felt might be burdensome or tedious, the staff always took care of them with warm, smiling faces, making my stay remarkably comfortable. When I was in pain and had trouble moving, I needed frequent assistance with things that even family members would find challenging to manage continuously, but they cared for me with utmost kindness and patience. Thanks to their incredible support, after going through two major surgeries, I was able to walk out on my own two feet upon discharge. If you are facing major surgery, I wholeheartedly and strongly recommend La Casa Via. At first, I questioned whether entering a rehabilitation center was even necessary, but after my experience here, I realized how critical it was for my recovery--I honestly don't know what I would have done without them.

But I made a promise to the staff at La Casa Via that I would share our experience, and I wanted to make sure I followed through. My parent was recovering from a broken hip and was in a very vulnerable, emotional state throughout the stay. Between dealing with pain, a language barrier, feeling tearful and overwhelmed, and having a lot of hesitation around medications, it was a stressful situation. The staff showed exceptional patience. The wound care nurse was remarkably skilled, the attending doctor was attentive, and the CNAs ( specially Ling) and nursing team handled the emotional stress and hesitancy with real care rather than frustration. Cam in administration was also very understanding and helpful with the logistical side of things whenever I had questions. The rehab industry as a whole has plenty of room for improvement and faces major systemic challenges, but the five stars here go to the staff on the ground. Despite the inherent difficulties of the system, this team worked through every obstacle with us and provided very good care. I am grateful for their support and would recommend them to anyone needing post-surgery rehabilitation.
