I have been In Shadelands Post Acute two times now. The first time I was here 12/2023 - 4/2024. My most recent stay was from 3/2026 - 4/2026. I love this facility, because it is well kept, clean and the staff members and services are second to none! I love how staff members willingly cover for each other during lunch breaks, busy times, etc.). I never experienced any rudeness or problems from any employee at Shadelands. Shadelands will ALWAYS be my first choice for Post Acute care. read more

I've had my fair share of experience with SNFs--both as a healthcare professional and as a family member of a patient. I can honestly say that Shadelands exceeded our expectations. Not only is the building new and well-maintained, with private rooms and bathrooms for each patient, but the quality of care truly stands out. The Director of Nursing makes a point to round on patients regularly, remaining visible and available for questions, concerns, and feedback from both patients and staff. That leadership presence carries over to the frontline nurses, who consistently treat patients with empathy and compassion--qualities that can be lacking in many other SNFs. The food is also impressive. It may not be a five-star restaurant, but even as a family member, I found the meals enjoyable and something I wouldn't turn away. Like any facility, there are opportunities for improvement. Therapy services at times seemed to be provided at a minimal level, likely reflecting staffing and workload challenges--an issue I recognize is not unique to this SNF. That said, I'm grateful my family member was able to maximize her rehabilitation. Her extended stay following an acute stay helped build her endurance, and she received excellent nursing care before being discharged home. First, and most importantly, everyone gets a private room. I was in the facility for 2 1/2 months and having my own room was the best benefit. The care that you receive is 10 out of 10!! I have to mention the name of one nurse in particular...Trina. She really showed me, everyday, how caring she could be. All of the RN's and CNA's were excellent, but Trina was special! Obviously I do not ever want to need post acute care again, but I would only choose Shadelands again if I did. read more

It's often challenging to get needs met quickly in a convalescent care setting, so I want to give a shout out to Emily and Wendylen, nurses at Walnut Creek's Shadelands Post-Acute. My adult daughter, who has Down syndrome and many health issues, was in residence there recovering after a fall precipitated a long hospitalization. Physical therapy being terribly hard for her, I decided to get an orthopedic consult to assess her risk/benefit. I was offered appointments at their nearby offices a month out, or...only about 90 minutes after my call! I needed the information they could provide asap so I opted to get her there any way I could. That's where Emily and Wendylen came in. They changed her quickly and dressed her for the cool day outside. They modified her wheelchair with elevated leg supports so the blisters on her shins would be less likely to flow. They brought in the equipment to help her stand for transfer to the chair and secured her once she'd moved. All that while being cheerful! No lie, I would've loved a transport van to have been an option, and I'm too old to have pushed her half a mile, but the rapidity with which Emily and Wendylen helped me, no questions asked, made it possible for her to arrive at the physician's office in time, where I got my valuable information. And when we returned, they made sure that we got some lunch! read more

My mom was here for 2 weeks healing from a broken hip. We were blown away at what an overall great experience she had. Caring, professional staff, her own private room with a TV and window that opened, folks coming in regularly to play music for everyone, great rehab, a nice big outside patio, activities, and an overall sense of caring, sweetness, support and freedom. Things to work on: Food wasn't horrible, better than hospital food, but maybe could have some improvements. Better coffee would be a win! Also some folks had long (30-60min) wait times for certain things, like help going to the bathroom. This was not the norm, they are just always very busy. Otherwise, an absolutely excellent experience. We were really pleasantly surprised, especially coming from a small town where acute centers can be downright depressing. They understand care, healing, and proving good communication. I rarely leave reviews but wanted to put a good word out for a nearly perfect place. Beautiful facility, wonderful staff. If I ever have to do acute care, I would pick this place. Thanks to the staff and management for taking such good care of my mom, and allowing us to visit even past hours sometimes, and bringing our Door Dash out to us multiple times, and for the excellent and warm-hearted care!!! read more

This is an outstanding facility. It starts right away with the friendly and helpful receptionists, Marsha and Fey. Fey drug a huge and heavy amazon order to my room and put it away for me. Marsha laughs at my silly jokes. The nurses and nurse assistants respond quickly, listen to your requests and treat you with respect and kindness. Nurse Melanie and nurse assistant Ariel stand out in this area. The food is good and and nutritious. It is seasoned very well. It's obvious the kitchen manager, Princess, wants you to enjoy your meals. The presentation is appetizing even when you don't especially feel like eating. You can always ask for a snack. The activities director, Arcel, provides lots of opportunities for socialization and the activities include live music, bingo, visits from therapy dogs to name just a few. Her assistant Madeline does very good manicures every Wednesday and that is a real treat. Physical therapy is provided 5 times a week and occupational therapy provided 3 times a week. Haley stands out as a therapist who encourages you to do your best, pushes you but not so much that you hurt worse afterwards. Doctor Kooturu is kind, funny, experienced and addresses your concerns immediately. He explains in a way you can understand. I wish he could be my doctor on the outside. The few minor issues have been addressed quickly. The best thing I can say is that I really, really didn't want to be in post acute care and now I'm not in any hurry to get home. I have been in other facilities so I do have something to compare to. All in all, a 5 star experience! read more

My mother in law, Myra, just spent nearly 4 months in this facility recovering from a terrible infection in her shoulder that spread all over. This facility really took care of her. She came home today and was raving about all the nursing staff and physical therapists who really helped her get back on her feet. She will have to go back after her up coming surgery but we know she will be in good hands. Thanks for taking such good care of her! My mother is blind too so care for her is extra necessary when she can't do it herself. Very appreciative. read more

I wish I would've read and looked more into this facility prior to putting our mother into this place! . What I will say is please please do not put your loved one here unless you want them to be neglected physically, emotionally and mentally abused. After my mom passed away at the hands of this facility, we ended up finding more families that have had neglectful care from this place! If you want your family member to wait to go to the bathroom up to sometimes 39-35 min, then by all means put them here. The caregivers are careless and all speak in their own native languages over your family member which is completely rude. They do not follow up with care plans, and Judith, The director of nursing will act like she cares but she could give two cents what you really are saying. A extremely clicky and neglectful facility is what this is ..by all means I hate to put anything negatively down, BUT if there's something I can say to save your family any heartache, do not put your family member here. My mother only came here for knee rehab and ended up dying from sepsis and pneumonia. Something that could've been prevented. I caught them multiple times telling my mom that she could just pee in her adult diaper versus being able to take her to the bathroom! On top of that multiple times, I told the facility if they could please just clean my mom's room a little bit more. There was always just crumbs and dirt everywhere and I understands get dirty but to be told over and over and still, no one took care of it. Things were always misplaced upon my arrival. My mom's hair was never combed, which is a simple humanely task to comb and make someone feel good, something CNAs learn! was too much to ask for here. I spent almost daily here after work and or before. My sister as well. It's despicable here. I'm heartbroken as 1 year of her death arrives tomorrow. Please stay away from here. read more

I have been a patient at Shadelands Post Acute, and my husband has been a patient here as well. I have received superior care for my hip replacement surgery. My husband has received superior care following the emergency fasciotomy surgery, which is life threatening. The Shadelands wound nurse monitored and cleaned the gruesome fasciotomy incisions with perfection. The wound nurse and all of the staff meticulously cared for my husband who also had suffered many strokes and brain hemorrhages. My husband is bed ridden, which requires constant attention. The entire Shadelands Post Acute staff provided superior medical attention. Shadelands Post Acute is the finest long-term nursing facility in every single way. My dad was recently at Shadelands for close to 3 months. I was extremely pleased with the care he received, the staff, and with the facility itself. A few points: 1) The building is so nice! It's modern and cheery. And the courtyard area, which my dad could access from his room, was beautiful and a nice retreat when the sun was out. 2) My dad's room was great too. Lots of natural light, high ceilings, a tv that was easy to work, with all the channels he needed (this may seem small, but the cable situation at another SNF was terrible and my dad was perpetually frustrated). Also, the fact that Shadelands only has single rooms is so nice. 3) I was impressed by the cleanliness of the facility. I often saw the cleaning people when I was there, and I could see that they were doing a fantastic job. 4) Every staff member we met, from the directors, to the front desk receptionist, to cleaning staff, was friendly, caring, and welcoming. I sensed that the staff was happy at Shadelands. 5) Aman and her rehab crew were excellent! I talked to Aman several times - she was always receptive, informative, and helpful. And her staff did a great job with my dad. 6) Cristina, the Social Services director, was also great! I talked to her multiple times about the plans for my dad. She was always so helpful and so friendly. 7) I'm so impressed by the communication from Shadlelands staff. From the introductory meeting to the check in conversations, to the follow up calls I've received after my dad's departure, I can see that communication is a point of emphasis, and one that I very much appreciate. 8) The food was really good, something I didn't think I'd say about a SNF ! And it was great that they offered alternatives for meals (like sandwiches instead of hot meals for lunch or dinner). 9) Special shout out to a few staff members who I saw frequently while I was there: Fay (front desk), Lesley (RN), Jewel (CNA), and Marcella (CNA). My client was here and I came to see her everyday. I must say she loved the CNA's especially Kaitlin and Alyssa and some of the PT's and OT's are wonderful except this PT Mae who did a terrible and awful service. The first time I observed her while doing PT with my client, Mae just left her hanging and felt helpless, after giving my client the first exercise Mae went off talking and eating with her colleague and most of the time on her phone, the second time around was that Mae took 2 patients at a time and supposedly asked them to do exercises and then she took off somewhere doing something else , I guess she's trying to kill 2 birds in one stone to save time but this is terrible service as I've seen PT's doing one patient at a time and focused on one person. Just saying! Hopefully one of your loved ones who's staying here at Shadelands will not get this PT's awful service. read more

March 2, 2026 I highly recommend Shadelands Post Acute Care. If you read the current reviews of Shadelands Post Acute Facility, you'll see a common message: that Shadelands Post Acute truly stands out as one of the best post-acute and skilled nursing facilities in the Bay Area. I could not agree more. I had no experience before with a rehabilitation facility, but a series of injuries to my right leg and hip required my wife and I to decide quickly what facility would be the best for us. I was lucky to be at John Muir Hospital, Walnut Creek for my surgery, where the medical staff recommended Shadelands above any in the Bay area. They were right. I spent 84 days recovering at Shadelands, and I am extremely grateful I ended up there. I had the best care possible, and my needs were always met in a timely manner by the terrific staff. It is a difficult job, but the staff was always upbeat, friendly, and caring. To be specific: 1. Physical and Occupational Therapy: Aman Ahluwalia - Director of Rehab I want to thank Aman for her flexibility in assigning me the PTs and OTs that best met my needs. Then something fantastic happened just a few days before I was discharged. I WALKED (well took a few steps painfully) without any support (no walker, no PT) for the first time since my second surgery, and that gave me the incentive and even stronger positive attitude to work harder to heal completely. I know I have a long way to go, but the best thing that those few steps gave me was REAL hope, and that was an incredible feeling. I want to thank Rachael, Caleb, Gio, Kashvi, Jeff, and the other folks for all their work that made my major progress possible They were fantastic. 2.RNs and CNAs: Judith Gasmin - Director of Nursing (RN) and Mikey Delaney - Director of Staff (CNA) The RN's and CNA's care got me through the most traumatic injuries I ever had in my life. They were fantastic. I am so grateful to them because all their efforts resulted in much faster healing than I ever believed would be possible. It was not just their medical care, but they also provided so much positive encouragement and emotional support that got me through a period feeling no hope of recovery. They succeeded in turning me around and my attitude improved, resulting in significant progress. I am so grateful especially to Leslie, Jerimiah, Nitika, Marcella, DD, Justin, Lydia, Jewel, and others who cared for me during an incredibly long journey and for their positive attitudes (upbeat, friendly, and caring) to help make my first time walking possible. This is a service that is provided at its best at Shadelands and keeps patients from developing any infections. I want to thank the RN named JJ who made himself available every time I needed him to take care of my incisions. He serves a very vital role here that is missing in many other Rehab facilities (I am told by friends), which helped make it a very safe place to recover. 4. Social Services: Cristina Dela Cruz-Social Service Director Many thanks for all the arrangements you made to ease the burden from my wife and me. You have been extremely helpful and we appreciate it. 5. Activities: Arcel Bulatao-Activity Director Thank you for all your work on activities. At first, I did not participate but that changed a few weeks ago with Bingo and "Blackjack." I got a chance to meet other patients whom I would not have met without those activities. To my surprise I enjoyed their company and it helped to ease my mind and keep a positive attitude over my recovery. I want to thank Madeline for her dedication to making all patients here feel good and less lonely. She was always positive, smiling, upbeat, patient, and fun in the activities to the enjoyment of not just me, but all the patients. 6. Medicare: Jerly Camacho-Business Office Manager I want to thank Jerly, for taking care of all the Medicare/Supplemental Ins and making sure everything was kept in order. Your help was much appreciated and took a big burden off my wife and me. 5. My father had a very short stay here, but honestly, I wish it was shorter. I don't care that this is the best and newest facility in the East Bay. Facilities, private rooms with individual bathrooms, fancy extra curriculars and good food do NOT make up for incompetent staff. You will be impressed with that way Shadelands looks when you walk in, but the second you are there as a patient and need something, get ready to wait for at least 15+ minutes. I would push the call light for my dad because he needed a cup for water, and literally waited at least 30 minutes to get help. By that time, I had to go out to the nursing station and ask for a water cup. The charge RNs hang out at the stations, but they act like they don't have to answer call lights. So, I waited over 30 minutes to ask for a water cup, and finally went out and saw the RN at the station. I told her I was waiting over 30 minutes, can I have a water cup for my dad? And her response was, "The CNA is on break." I looked and saw another CNA frantically answering calls and trying to cover for the CNA that was on break, and I'm thinking to myself, you can't help and answer call lights when your staff is on break? And for a water cup no less, not asking for her to change a poop diaper or something! I have worked as an RN for over 10 years, many of those years in acute care in the prestigious hospitals in the Bay Area, and we would help each other out in situations like that. If you are thinking "that's not my problem," not only do the patients suffer, but you and the facility suffer because you look worthless and as if there is no teamwork or accountability. These RNs seemed to be more concerned with staying at their station, looking at their computer screens then actually helping their patients. Even when I would come out and try to ask for things, they would ignore me for at least minutes on end, until I finally got their attention. I swear, they just looked like they were staring at their computer screens, doing nothing. So these are all very important points I am bringing up, but nothing is as important as the time my dad almost died at Shadelands and it all felt completely avoidable. The urine in his Foley had been looking very cloudy and tan for a couple days, and I even brought this to the attention of the even so entitled nurses at the nurses' station. They told me they'd "mention it to the doctor." I have no idea what happened next, but I thought it had been addressed, because, well, I thought they actually did their job there. But no, my dad seemed worse and worse in the coming nights, more confused and disoriented. I mistakenly chopped it up to being in a new place, being tired, etc. One morning, my husband visited him, and he was mumbling to himself, looked terrible, and according to his Nursing Notes his BP was 74/64 (which doesn't even seem like a real blood pressure). Then my husband asked they check him again and it dropped to 60/40! For anyone who knows, this is dangerously low, and they were not moved to call 911 or anything. My husband, who is a firefighter paramedic, insisted and was the one who called 911 for my dad. He ended up being taken code 3 to John Muir in Walnut Creek and was in septic shock! He literally almost died and if my husband hadn't called 911, I swear, I have zero faith that they would have moved quickly enough to get him treated. Luckily, he is now back at home and doing great, but not without being sent to another SNF and you can trust me that I didn't think for a second about sending him back here! I wouldn't trust my worst enemy here if I'm being honest. read more

My father was here in December to receive care and PT for a back issue. He too developed sepsis while here. He was sent back to John Muir and then back to Shadelands. My sister myself and my mother went everyday . When we would go his food trays would be sitting for hours untouched, he was just left in his room day after day with the promise of PT that he never received because it was too difficult for them to help him. With no movement or activity that was supposed to be done daily he became depressed and had a massive stroke and no one noticed or called the family. My mother had him transferred back to John Muir when she saw how bad his condition had changed overnight He died from what should have been simple. When I was there I asked when he was going to get help or PT and they always said later. No one came in his room to check on him I would go to the nurses station and they were all chatting or on their phones and were clearly irritated by being bothered. Please don't take your lived ons here He should have never died the way he did read more

The facility is so nice- new, airy, clean and bright. And no hospital like smells. The staff are excellent! Attentive, caring, responsive and professional. They offer various types of therapy and have numerous activities for those well enough to participate. You can even take your family member on day trips. I've been here since December 11th and it's clean somewhat respectful place I have an issue with the CNAs and the nurses station when you push the button to use the restroom it takes about 45 minutes for them to respond to take you to the to the potty and then one day I waited over an hour and a half to be able to go potty because everybody was on break lunch break that's not my problem I think that could really be addressed and I hear a lot of people complaining about it then you get people trying to go on their own and they're falling short staffed last weekend it was a mess don't get me wrong it's a nice place the food is all right little salty it is from what I hear one of the best ones around I'm 66 just had a total knee replacement so I could not do anything at all at this point this is why I came in to rehab from John Muir I'm starting to get more mobile now so I can get up at least take myself to the potty so that's not too much of an issue no more yesterday morning I got up to take a shower for the first time in 10 days and the boiler was off so I had to take a cold shower my remote control for the TV has been broken for 2 days took them two days to replace it then I'm sitting here watching TV and the fire alarm goes off and you have about 20 of the stuff come running in here and looking around asking if there's any smoke or anything smoking answer no I do not smoke and at this point the management I believe his name was Matt came in and asked if anybody was smoking Vapes and I was really pissed off because I do not smoke at all. And then he opened the window up and left I really think this behavior is not good in this type of environment and it needs to be addressed he needs some communication classes for his I'm concerned I'm here to get myself back together so I can live independent home I'm a senior and I'm here just to get my rehabilitation for my knee only like I said this place needs work do your studying before you send your parents here and keep eye on them because they're really bad on taking to the 40 potty and I feel sorry for some people that have to depend on these people it's really open my eyes up thank you for your time I'm not putting a facility down we need more of them like this but it's definitely got some issues and also they need to have the fire fire alarm company come out and probably recalibrate their fire alarms because obviously they're not working right and I'm going to contact Medicare on Monday and follow a complaint thank you very much that's my review. read more
